Abstract Background: Anxiety and depression are the most common psychological disorders in the aging period. One of the ways to deal with anxiety and depression and increase happiness in the elderly is to train positive thinking. Therefore, the present study aimed to investigate positive thinking skills training on anxiety and happiness in the elderly.Methods: In this quasi-experimental study, 48 elderly people referred to two comprehensive health service centers in Kerman in southeastern Iran were selected by convenience sampling method and randomly assigned into intervention groups (n = 24) and control (n = 24). The data collection tools included the Beck Anxiety Inventory and the Oxford Happiness Inventory. Questionnaires were filled out before the intervention, then positive thinking training was held in eight sessions of 90 minutes. After training program, the was conducted one month after it, the follow-up tools.Results: The results showed that the mean score of anxiety before positive thinking training was (13.58 ± 8.61) and (19.25± 11.67) in the intervention and the control group, respectively. After positive thinking training, the mean scores were (4.50 ±4.07) and (15.54±9.04) in the intervention and control group, respectively. After the intervention, which significantly reduced anxiety the intervention, group (t= 8.10, P<0.001). The mean score of happiness before the test was (26.58±12.40) and (37.91 ± 5.57) in the intervention and control groups, respectively. However, the posttest happiness scores of the intervention and control groups were (62.91± 4.66) and (35.62 ±10.62), respectively. The results showed that implementing positive thinking training increased happiness significantly (t=-4.08, P<0.001).Conclusion: Since positive thinking training effect reducing anxiety and increasing happiness in the elderly. Due to the growing trend of the elderly population, the positive thinking training approach can be used as a suitable, cost-effective, and efficient method in reducing anxiety and increasing happiness in the elderly.

Abstract Background The knowledge and attitude of health care providers are important and influential factors in providing care services to the elderly and need to be considered during the training course. Simulation in geriatric nursing education can be an opportunity for learners to experience the restrictions of the elderly. The present study was conducted to determine the effect of training through simulation on the attitude and knowledge of nursing students in elderly care. Methods This study was quasi-experimental with two experimental and control groups of pre and post-test, which was conducted on 70 nursing students of the 5th semester (two groups of 35 people). For the experimental group, the elderly simulation suit was worn for two hours, which was designed by the researcher and created sensory, physical, and motor restrictions similar to the elderly for students. Before and after the study, Kogan’s attitudes toward older people scale and Palmore’s “facts on aging quiz” were completed by students. The data were analyzed using an independent t-test and paired t-test using SPSS version 16 software. Results The mean scores of students’ knowledge in the experimental and control groups had no significant difference at the beginning of the study (p < 0.05). But the mean scores of knowledge in the experimental group before and after the intervention was (9.2 ± 2.6) and (15.3 ± 3.5), respectively, and in the control group before and after the intervention was (10.4 ± 2.9) and (11.3 ± 2.6), respectively, which had a statistically significant difference (p = 0.0001). The mean scores of students’ attitudes in the experimental and control groups had no significant difference at the beginning of the study (p < 0.05). The mean scores of attitude in the experimental group before and after the intervention was (114.69 ± 8.4) and (157.31 ± 10.7), respectively and in the control group before and after the intervention was (113.34 ± 13.6) and (108.5 ± 16.6), respectively, which was significantly different (p = 0.0001). Conclusions Based on the findings, the experience of aging restrictions through simulation has improved the knowledge and attitude of nursing students towards the elderly. Nursing education requires the growth of attitudinal skills, individuals’ beliefs, and creating empathy among them, so creating simulation opportunities can assist nursing students in the educational processes.

Abstract Background Service satisfaction ratings from clients are a good indicator of service quality. The present study aimed to investigate the impact of communication skills and self-efficacy training for healthcare workers on clients’ satisfaction. Methods A quasi-experimental study was conducted in health centers of Saveh University of Medical Science in Iran. Primary Healthcare (PHC; N = 105) workers and service recipients (N = 364) were randomly assigned to intervention and control groups. The intervention group received four 90-min training sessions consisting of lecture, film screening, role-playing, and discussion group. Before and 3 months after the intervention, a multi-part questionnaire (including demographics, self-efficacy and communication skills in PHC workers; and satisfaction questionnaire in service recipients) was completed by participants in both intervention and control groups. Results PHC worker mean scores of self-efficacy and communication skills after the educational program were increased in the intervention group compared to the control group (p < 0.05). Also, mean satisfaction scores for service recipients of the intervention group (PHC workers) generally significantly increased compared to the control group (p < 0.001). Conclusions The educational program improved the self-efficacy, and communication skills in health workers and improved client satisfaction overall. Our results support the application of self-efficacy and communication skills training for other medical groups who wish to improve clients satisfaction as an important health services outcome.

AbstrakKualitas tidur pada lansia yang mengalami insomnia sangat bervariasi  yaitu ada yang baik dan ada yang buruk.  Salah satu keluhan tidur lansia adalah Insomnia , dimana ini  merupakan suatu proses degenerasi pada lansia menyebabkan waktu tidur yang efektif semakin berkurang, dan menyebabkan tidak tercapainya kualitas tidur yang adekuat.  Intervensi yang diberikan dapat disisipkan dalan kegiatan Posbindu. Salah satu Intervensi yang dapat diberikan adalah terapi beapreasi. Penelitian ini bertujuan untuk mengetahui  ada atau tidaknya pengaruh terapi Beapreasi terhadap kualitas tidur pada lansia dengan insomnia di  Posbindu Kemuning Kelurahan Candirejo Ungaran. Design penelitian yang digunakan adalah quasi experiment pre-test post-test with control group dan dilakukan pada 128 responden yang terbagi kedalam kelompok eksperimen dan kelompok kontrol,  masing-masing kelompok terdiri dari 64 responden . Metode  yang digunakan untuk menentukan responden adalah teknik purposive sampling. Penilaian Kualitas tidur di ukur dengan kuesioner PSQI dan dilakukan 2 kali pengukuran yaitu sebelum dan sesudah diberikan terapi Beapreasi. Penelitian menunjukkan bahwa rerata nilai PSQI pada kelompok eksperimen sebelum dilakukan intervensi sebesar 6.27 dan rerata  nilai PSQI pada kelompok kontrol sebesar 5.15. Selanjutnya rerata nilai PSQI pada kelompok eksperimen setelah  dilakukan intervensi sebesar 3.67, sedangkan  rerata  nilai PSQI kelompok kontrol  sebesar  5,73. Terdapat perbedaan nilai PSQI pada kelompok eksperimen dan kontrol dengan nilai p 0.00, nilai p < 0.05 sehingga ada pengaruh yang bermakna dari pemberian terapi beapreasi terhadap peningkatan  kualitas tidur. Intervensi terapi Beapreasi dapat meningkatkan kualitas tidur pada lansia yang mengalami insomnia . Terapi Beapreasi dapat dijadikan salah satu intervensi mandiri perawat dalam  merawat lansia yang mengalami insomnia dalam upaya meningkatkan kualitas tidur pada lansia.  Kata kunci: Terapi beapreasi , kualitas tidur, PSQI (the pittsburgh sleep quality index), lansia insomnia, posbindu Abstract The Effect of Breapreasi  Therapy (combain for brain Gym and benson relaxation) on Sleep Quality in the Elderly with Insomnia. Sleep quality in the elderly who experience insomnia varies greatly, there are good and some are bad. One of the complaints of elderly sleep is Insomnia, which is a degeneration process in the elderly which causes effective sleep time to decrease, and causes inadequate quality sleep. The intervention provided can be inserted in the activities of Posbindu. One of the interventions that can be given is therapy of beapreasi. This study aims to determine whether or not there is an effect of beapreasi therapy on sleep quality in the elderly with insomnia at Posbindu Kemuning, Candirejo Village, Ungaran. The research design used was quasi experiment pre-test post-test with control group and carried out on 128 respondents divided into experimental groups and control groups, each group consisting of 64 respondents. The method used to determine respondents is purposive sampling technique. Assessment of sleep quality was measured by the PSQI questionnaire and carried out 2 measurements, namely before and after being given therapy Beapreasi. The study showed that the mean PSQI value in the experimental group before intervention was 6.27 and the mean PSQI value in the control group was 5.15. Then the mean PSQI value in the experimental group after intervention was 3.67, while the mean PSQI value of the control group was 5.73. There are differences in the PSQI values in the experimental and control groups with a value of p 0.00, the value of p <0.05 so that there is a significant effect of the provision of beapreasi therapy on improving sleep quality. Beapreasi therapy interventions can improve sleep quality in elderly who experience insomnia. Background: Anxiety and depression are among the most important and common problems in patients admitted to the intensive care unit open-heart (ICU-OH). While the family plays a vital supportive role in decreasing these complications, patients are deprived of this supportive source during the important post-operative days due to visiting restrictions at these wards. Objectives: Therefore, this study aimed to evaluate the role of online video visitations on the anxiety and depression of patients at ICU-OH. Methods: This randomized clinical trial was carried out among 66 patients at ICU-OH of Imam Reza Hospital in Mashhad, Iran. The subjects were selected by the convenience sampling method and were randomly allocated to the intervention and control groups. Data were collected using a demographic characteristics checklist and HADS. In the intervention group, online video visitations with the family were carried out three times in the morning, evening and night on the second and third days of hospitalization in ICU-OH. In the control group, patients received the routine care of the ward and had no visitations with their families. Data were collected before the surgery, as well as 24 and 48 hours after admission to the ICU-OH. In addition, data analysis was performed in SPSS using independent t-test, Mann-Whitney U test, paired t-test, and Chi-square test. Results: No significant difference was observed between the intervention (7.8 ± 2.2) and control (8.3 ± 2.3) groups regarding the mean anxiety score 24 hours after the intervention (P = 0.416). However, the mean anxiety scores 48 hours after the intervention were 6.1 ± 2.3 and 7.7 ± 2.6 in the intervention and control groups, respectively, showing a significant decrease in the intervention group, compared to the control group (P = 0.010). However, the mean depression scores 24 and 48 hours after the intervention were estimated at 8.0 ± 2.0 and 7.1 ± 2.0, respectively, demonstrating no significant difference from the control group (P = 0.933 and P = 0.269). Conclusions: According to the study results, online video visitations decreased anxiety in patients at ICU-OH. Therefore, it is recommended that this method be used as an alternative to in-person meetings of patients at this ward.

AbstractBackground To evaluate sulfentanyl combined with dexmedetomidine hydrochloride on postoperative analgesia in patients who received video-assisted thoracic surgery (VATS) and its effects on serum norepinephrine (NE), dopamine (DA), 5-hydroxytryptamine (5-HT), and prostaglandin (PGE2).Material and Methods Ninety-nine non-small cell lung cancer (NSCLC) patients who received VATS were included in the study. All the patients received intravenous inhalation compound anesthesia. Of the 99 cases, 49 subjects (control group) received sulfentanyl for patient controlled intravenous analgesia (PICA) and other 50 cases (experiment group) received sulfentanyl combined with dexmedetomidine hydrochloride for PICA after operation of VATS. The analgesic effects of the two groups were evaluated according to Visual Analogue Scales (VAS) and the Bruggrmann Comfort Scale (BCS). The serum pain mediator of NE, DA, 5-HT, and PGE2 were examined and compared between the two groups in the first 24 h post-surgery.Results The VAS scores for the experiment group were significant lower than that of control group on the time points of 8, 16, and 24 h post-surgery (pall<0.05), and the BCS scores of the experiment group in the time points of 8, 16, and 24 h were significantly higher than that of controls (p<0.05). However, the VAS and BCS scores were not statistical differently in the time point of 1, 2, and 4 h post-surgery (pall>0.05). The mean sulfentanyl dosage was 63.01 ± 5.14 μg and 67.12 ± 6.91 μg for the experiment and control groups respectively with significant statistical difference (p<0.05). The mean analgesic pump pressing times were 4.30 ± 1.31 and 5.31 ± 1.46 for experiment and control groups respectively with significant statistical difference (p<0.05). The serum NE, DA, 5-HT, and PGE2 levels were significantly lower in the experimental group compared to that of control group in the time point of 12 h post-surgery (pall<0.05). The side effects of nausea, vomiting, delirium, rash, and hypotension atrial fibrillation were not statistically different between the two groups (pall>0.05).Conclusion Patient controlled intravenous analgesia of sulfentanyl combined with dexmedetomidine hydrochloride was effective in reducing the VAS score and serum pain mediators in NSCLC patients who received VAST.

Previous research has examined the utilisation of musical cues to improve the performance of cardiopulmonary resuscitation (CPR) delivered in training environments. We postulated a musical cue that is both contemporary and transcends cultures may improve CPR performance. Our aim was to establish whether chest compressions are performed with improved rate and depth if a song of a fixed beat (PinkFong’s ‘Baby Shark’ with a tempo of 115 beats per minute (bpm) and 15 beats in each verse) is played to a healthcare professional immediately before undertaking CPR compared to whale noises (a non-metronomic rhythm). 58 Participants of a paediatric conference (majority doctors) were randomly assigned to listen to a minute of Baby Shark (28) or whale song (30) and then undertake a minute of CPR. There was no significant difference in the mean compression rate between the Baby Shark and control groups, with the groups achieving 121 and 125 bpm, respectively (p=0.18). In relation to compression depth within the target zone, the Baby Shark group had more compressions completed within the target zone (55%) than the control group (39%) although this difference was not significant (p=0.08). Listening to Baby Shark prior to undertaking simulated CPR does not improve overall performance, but there is a potential tendency to improve adequate compression depth which may be beneficial in training exercises.

BackgroundWhilst there is evidence to suggest paramedics experience significant stress when working in the ambulance setting little is known about the experiences of first year paramedic students. This research aimed to: (i) identify whether levels of stress, anxiety and depression experienced by first year paramedic students changed after ambulance placement compared to a control group, and (ii) identify the main perceived and actual sources of stress around ambulance placement.MethodsA before-and-after quasi-experimental design was used to compare whether the experience of ambulance placement altered the levels of stress, anxiety and depression in an experimental group that attended an ambulance placement (n = 20) and the control group who did not (n = 10). Online surveys encompassing the Depression, Anxiety and Stress Scale (DASS-21) and qualitative questions about sources of stress were concurrently deployed to both the experimental and control groups before and after the ambulance placement. Participants were first year paramedic students working in Queensland Ambulance Service, Australia.ResultsThere was a significant reduction in levels of stress in participants after undertaking their first ambulance placement (Mdn = -4.00) when compared to a control group (Mdn = 0.00), U = 52.5, p = .035, n2 = 0.15. Responses to survey questions suggest anticipation about experiencing death and dying of patients was the most frequently reported stressor of student paramedics before undertaking ambulance placements, however insecurity about knowledge, competence and fear of failure was the most frequently experienced stressor reported after completing ambulance placements.ConclusionsThe findings from this study suggest that the fear of the unknown may be worse than the reality. Anticipatory stress is the foremost problem for first year paramedic students attending their first ambulance placement. Placement pre-briefing should focus on educational interventions to build knowledge and skills competency to reduce stress levels and fear of failure.

Introduction: Several chemotherapy agents, especially anthracyclines, are associated with the development of non-ischemic cardiomyopathy. When chemotherapy-induced cardiomyopathy (CHIC) is associated with left bundle branch block (QRS >150 ms) and a left ventricular ejection fraction (LVEF) of 35% or lower, cardiac resynchronization therapy (CRT) is often utilized to improve cardiac function and relieve symptoms. Hypothesis: CRT is associated with improvement in left ventricular strain in patients with CHIC. Methods: The study included 22 patients with CHIC and 44 age- and gender-matched controls with other types of NIC who underwent CRT implantation between 2004 and 2017. LVEF, left ventricular global longitudinal strain (GLS), systolic strain rate (SRS), and early diastolic strain rate (SRE) were assessed at baseline and 6-18 months after CRT. CRT response was defined as LVEF improvement by >5% after CRT. Results: CRT responders had a significant improvement in left ventricular GLS as compared to CRT non-responders in the control group, but not in the CHIC group (Table 1). The mean change in LVEF after CRT was similar between the CHIC and control groups (10 ± 10% versus 11 ± 11%, p= 0.67). The mean change in GLS after CRT was also similar between the two groups (-2.15 ± 4.32 versus -3.57 ± 4.02, p= 0.19). The proportion of patients whose LVEF increased by more than 5% was similar between the two groups (59% in the CHIC group versus 68% in the control group, p= 0.46). The mean changes in LV myocardial systolic and early diastolic strain rates were similar between the CHIC and control groups. Mean SRS decreased by 0.13 ± 0.14 in the CHIC group and by 0.34 ± 1.28 in the control group (p=0.45). Mean SRE increased by 0.06 ± 0.18 in the CHIC group and by 0.07 ± 0.22 in the control group (p=0.86). Conclusions: CRT is associated with improvement in left ventricular strain in patients with CHIC. Prospective studies are needed to further evaluate the echocardiographic strain outcomes of CRT in CHIC.

The study aimed to determine 1) the effect of digital eco learning in student worksheet flipbooks on environmental literacy projects, and 2) the effect of digital eco learning in student worksheet flipbooks on student ecological competence. The research is a quasi-experimental study using a Pretest-Posttest Non-equivalent Control Group. The research subjects were students in the Environmental Geography course semester 6 of 2020, divided into the experimental and control groups. The research was conducted at the Geography Education study program, Social Science Faculty, State University of Malang, Indonesia. Data were analyzed using the independent sample t-test in SPSS version 23 for windows. The results found: 1) there is a significant effect of digital eco learning with student worksheet flipbook on environmental project literacy competence, and 2) there is a significant effect of digital eco learning with student worksheet flipbooks on ecological competence.

The aim of this study was to identify the effectiveness of teaching role playing strategy in developing dialogue skills in the immortal language of the intermediate third grade students. To achieve the objectives of the study, the researcher used the semi-experimental method by designing the two equal groups. The sample contains 51 students, 26 students for the control group and 25 students for the experimental group. The researcher used the statistical treatments using the SPSS program, The study has resulted in The results showed that the level of dialogue skills on the student scale among the average third-grade students in the experimental and control groups was medium. The mean of the two groups was on all dialogue skills (2.23). The skill of expressing the opinion is one of the most common dialogue skills, (2.25), while the skill of listening and listening is second, with an average of (2.24), followed by self-esteem in third place with an average of 2.23. Finally, ). In addition, there were statistically significant differences at the level of α 0.05 0.05 in post-performance in all dialogue skills (self-esteem, good listening and listening, respect for others, expression of opinion). In the light of its findings, the researcher made a number of recommendations.
