This article provides ways to improve the conflict management system in health care based on analyzing the causes of conflicts in a private medical institution. The authors considered the healthcare delivery process in the triad «health worker – patient – society». The study involved analyzing conflict causes in the health care system in the relationship between «health worker – patient» and «health worker – health worker». To determine the conflict causes in private medical institutions and ways to minimize them, the authors used questionnaire data collection, interviewing technique, data analysis, and graphic approach. The study object is medical staff and patients of SILMED Diagnostic Medical Center in Ukraine. The findings identified the main problems in the organization of the conflict management system. Therefore, the essence of conflict situations arising in a medical institution was determined. The obtained results allowed formulating proposals to improve the conflict management system of the private medical institution. Thus, the authors emphasized that the conflict management system in private medicine should minimize the probability of conflict, focus on the treatment quality and the service price. Besides, it should include the development of moral codes for doctors in private medical institutions. The authors noted that to reduce conflict in the relationship «medical worker – medical worker», conflict prevention is more effective than constructive resolution. Thus, conflict prevention requires less effort, resources, and time for a successful clinic operation. To decrease conflicts in the relationship «health worker – patient», it is essential to make a pact between the patient and private medical institution. It establishes the patient’s relationship with the clinic and is publicly available for customers. In the study, the authors concluded that during the consultation, the healthcare professional should persuade the patient to follow all recommendations for maintaining or improving the patient’s health, requiring, if necessary, evidence from the clinic’s patients to follow the treatment protocol.

In modern market economy, among the variety of problems associated with the normal and efficient development of enterprises and organizations, one of the main problem is the personnel management. In the near future, the best development will be achieved by those organizations that focus on human resources and their management. Data shows that strategic personnel management in the current context of health care reform will increase the competitiveness of medical institutions. The paper analyzes the state of staffing in the healthcare sector. It was determined that there is a tendency to reduce medical facilities, and the outflow of highly qualified personnel to other sectors of the economy. Thus, since 1990, the number of hospitals has decreased by 43.5%, from 3.6 thousand to 1.1 thousand. Due to this, the number of hospital beds per 10,000 population decreased at the end of 2017, the number of beds decreased by 53.9%. The analysis revealed that in Ukraine there is a problem of layoffs. It is established that in the period from 1990 to 2017 there was a decrease in the number of paramedics in the health care system by a total of 32.1 people per 10,000 population, which is 27.3% of the total number of paramedics in 1990. Research shows that the movement of health workers depends on the movement of labor migration from districts to large cities, and is common in all countries, but in Ukraine there is a tendency to reduce the average number of medical staff at a faster rate than the reduction of doctors. This is due to difficult economic conditions in the country. The analysis of the personnel potential of health care institutions of Ukraine was carried out, which determined a significant reduction of medical staff, especially in the part of paramedical staff. It decreased by 27.3% of the total number of paramedics in 1990. It is determined that the main factors reducing the human resources of health care institutions of Ukraine are the inadequacy of staff training to modern requirements in the field of health care, insufficient material and social security of staff. An effective system of personnel management of a medical institution has been developed, which is based on the formation of human resources and the effective use of the personnel of the medical institution, which includes an analysis of the labor potential of the medical worker; planning and selection of staff depending on the demand for medical services of the institution; development of rules for hiring staff in accordance with labor legislation; application of methods of adaptation of medical staff; application of a transparent method of personnel certification. The characteristics of the functioning of the health care sector, which influence the formation of the personnel management system of medical institutions, have been identified. Directions for improving the personnel management system of medical institutions have been identified, which will ensure high quality of medical services with sufficient material security of medical institution staff and promote the retention of highly qualified personnel in medical institutions, which in turn will increase the efficiency of the health care system as a whole.

Aim. To assess the importance, role, methods and conditions for creating and sustaining positive motivation of medical personnel in deploying a modern management system based on lean production to optimise the production flow in medical institutions, improve quality, availability and efficiency of public medical care.Materials and methods. The authors employed historical, bibliometric and statistical methods to analyse domestic and foreign scientific archives on lean production, motivation and motivational risks as key factors in implementing lean technologies in medical institutions.Results. Implementation of lean production (LP) as a manufacturing management system is among key conditions for creating a new model of the medical institution. Principles of LP are applied at three interfaces: with patients, personnel and resources. Positive motivation of employees is prerequisite for accomplishing a project, which requires supervisors to know and leverage the basics of motivational management. Creation of a motivating environment for various categories of employees in developing a LP system requires taking into account the type and management culture level of a medical institution.Conclusion. In order to create and sustain motivation for realising creative potential of the team, it is necessary to account for the motivation risks, develop and implement a motivation system differentially by employee categories and project stages. Effective motivational measures are necessarily based on a system of material and non-material incentives with clear and achievable criteria, amounts and forms valued by employees and rooted in medical and economic feedback of implementing the LP principles.

Introduction. The formation of a modern model of development of the national health care system presupposes the existence of economically efficient and financially viable medical institutions. The collaboration of economics and medicine is designed to facilitate the transformation of existing hospitals into medically and economically efficient actors in the national health care market in the face of global threats to human health and budget deficits. Methods and tools of economic analysis should be the basis for the formation of a model of profitable medical business. Goal. Substantiation of the algorithm for the formation of cost-effective medical business entities based on economic analysis in terms of reforming the national health care system. Method (methodology). Methods of analysis and synthesis, induction and deduction are used in determining current trends and problems of development of a new format of self-financing of medical institutions in the formation of the national health economy and the formation of an algorithm for profitable medical institutions. Results. It is proposed to apply the criteria for evaluating decisions to achieve financial and economic efficiency of medical institutions: effectiveness - medical efficiency; cost effectiveness - economic efficiency; necessity - necessity; feasibility probability of realization / feasibility. Factors of potential success of medical business and investment attractiveness are singled out. A model of calculating the potential profitability of a medical institution according to the ratio of available resources (value of medical services and medical technologies) and the time required to implement the updated idea (mission) of the medical business is proposed. An algorithm for forming a medical business or improving the existing format of a medical institution in the conditions of the national health economy has been built on the basis of methods and tools of economic analysis.

Introduction. The article is devoted to the problem of development of managerial human capital of national healthcare institutions as a basis for ensuring medical sphere effective reform. The relevance of the research is due to the high degree of implemented and planned changes dependence in the field of medicine on the knowledge, skills, skills and experience of representatives of medical institutions governing bodies. Purpose. The article analyzes the existing requirements and recommendations for the implementation of the processes of providing medical institutions with leading staff of the relevant qualification and the trajectory of its development, which became the subject of research. Attention is drawn to the need to improve approaches to the development of managerial human capital in health care facilities, taking into account the intellectual economy conditions characterized by the dominance of intangible resources. Method (methodology). Usage the approach proposed by the American psychologist E. Giselli and domestic scientists research allowed to form a graphical dependence of medical institution effectiveness on the level of management corps human capital development. In this case, the scenarios of the activity of the healthcare institution are defined and characterized, depending on the chosen trajectory of managers development. Based on the results of the substantiation of the medical institution activity optimistic scenario, a profile of manager competencies is formed in the form of professional competences and personal qualities. Results. The conducted survey of existing and potential managers of medical institutions allowed to establish the degree of importance of the developed characteristics for this category of managers. The results obtained and elaborated according to the developed methodology show the highest degree of importance for such personal characteristics as «human orientation» and «innovative ability». Proposals for improving the development of managerial human capital of medical institutions were offered in form of a list of manager abilities, skills and skills, as well as opportunities for their acquisition in the system of training managers of the medical industry.

This paper considers human resources to be important for the effective functioning and development of the organization in current socio-economic conditions. Corporate management involves the impact on people (corporate staff) primarily. The authors noted that the staff management assessment is a powerful instrument to increase the management process’s effectiveness. The normative documents of the Ministry of Health of Ukraine define the following methods of quality control of medical services: clinical audit and monitoring. The purpose of a clinical audit is to find opportunities to achieve high-quality health care. Continuous tracking of staff provides an opportunity to make effective management decisions and increase productivity. An adequate system for assessing the quality of medical staff should respond not only to the result of medical care but also to prevent defects. Personnel management in health care quality should be carried out to avoid mistakes in the provision of medical care. This concept is designed to create an internal structural quality management system. Therefore, it is essential to understand the assessment process and its interactions at different stages of the management process within the organization. In the paper, the authors determined ways to enhance the medical staff management assessment. The study presents the essence and features of medical staff assessment based on the general principles and methods for assessing the effectiveness of public and private medical institutions. The authors analyzed the medical staff assessment system in medical institutions and identified its features under medical reform. Besides, the study provided the investigation of the current state of the staff assessment of medical institutions. It analyzed the areas of concern in the methods and mechanisms of staff assessment. To investigate the current situation in the medical institution, the structure and movement of medical staff were analyzed. The findings identified the gaps in the current staff assessment system and the reasons for the low rates of nursing staff. The authors highlighted ways to improve the current staff assessment system in the institution through the social satisfaction criterion, interviewing the parturient women and configuration management of quality based on the PDCA cycle. The obtained results showed that the medical institution had different reserves available to increase the parturient women satisfaction with the level of medical services received.

The purpose of the article is to identify the possibilities of medical institutions development in the conditions of change of health care financing mechanism with taking into account the further orientation on autonomization of medical institutions, which received the status of enterprises.The paper implies active implementation of new planning methods that are alternative to the traditional national approach to expenses accounting and their analysis, which are fundamentals of planned fi- nancial indicators calculation.The transformation of the financial mechanism in the healthcare sector necessitates a financial provision for medical institutions, that includes, first and foremost, a reasonable calculation of cost of medical in- stitutions services.Methodology.The methodology of the study is to use a combina- tion of methods: economic analysis, synthesis, comparison – to estimate the ac- tual values of financial indicators; economic-mathematical modeling and forecast- ing; systematic analysis, typification and comparison of observations, grouping, generalizations – during the process of making final conclusions based on the results of analytical studies.The scientific noveltyof the resultsobtainedconsistsnew approach offered by authors that let medical institutionsolve existing practical and methodological issues of correlation between current actual expenses and, forecast financial needs of medical institution in variable functioning conditions. The au- thors prove that the further development of medical institutions is impossible without reasonable pricing of healthcare services. The existing «Methodology of healthcare services cost calculation»is aimed at standardization and unification of expenses accounting methods, but does not promote getting veracious calculable data, which should be used in financial planning and modelling. Conclusions.The combination of analysis methods presented in this paper enables to make a forecast about the financial needs of the medical institution in the status of the enterprise on the basis of scientifically-based approaches. The use of the CVP-analysis meth- od allows to purposefully manage the main financial indicators that are taken into account in different scenarios, and the correctness of their analytical application at the hospital level is included in a justified approach to a higher level of gener- alization in accordance with the theory of averages.There is a high bankruptcy risk for a medical institution in the status of an enterprise in a case of insufficient level of reimbursement of medical institution expenses within the limits of state medical insurance, as well as within limited possibilities of receiving income from paid medical services. For more effective realization of medical institutions «au- tonomization», it is necessary to improve the legislation in terms of free choice of the list and volume of paid medical services, economic approaches to pricing for paid medical services and a clear policy of the state within a support of health care capital investment programs.Effective, purposeful preparatory work of reason- able financial indicators usage, taking into account current legislation, will enable the medical institutions to be competitive in the healthcare market.

This article deals with the role of mediation in conflict management and especially in the monitoring of conflict factors of education activities. In the article the authors analyse the system of monitoring and its principles and methods. Special attention is paid to the integration of monitoring and mediation. Recently, the mediation institution has been developing quite actively in Russia. Mediation is a negotiation with a third neutral party. It differs from negotiations by the removal of responsibility for carrying out procedures from representatives of the parties. It is one of the ways of conflict prevention and regulation. The article shows different models of embedding of mediation into the conflict management system in high school. Mediation and monitoring are considered as important parts of this management

In the logistics approach, the health care organization is seen as a system that incorporates many interconnected processes, the ultimate goal of which is to provide quality health services. Modern system and process approaches to management have also undergone significant changes in the management of medical institutions, applying new methods and technologies of resource management, which are based on the concept of logistics. The positive aspects of logistics technologies in health care institutions are the following: the ability to influence the strategy and tactics of health care institutions, the formation of new competitive market advantages in this market segment. The use of logistics methods and technologies optimizes the processes of information input on the amount of resources consumed, its receipt and transmission of information, thus improving the quality of medical care. The main idea of the logistics process is to create a certain result from the resources available in medical institutions. It should be noted that today companies in various sectors of the economy are interested in the transition to logistics management. The advantage of the logistical approach in health care is the ability to influence the strategy and tactics of the medical organization, to create new competitive advantages in the market of medical services. The use of logistics accelerates the process of obtaining and transmitting information about the necessary resources as a result, improving the quality of medical services. In this way, logistics coordinates the resource provision of the health care organization as a whole. Conclusion. Implementation and use of the world best practices of the logistics concept "just in time" to optimize the needs and time of supply of medical devices of each structural unit of the medical institution will ensure continuity of processes in the health care facility as medical supplies and property arrive on time, and , at the same time do not accumulate in warehouses unclaimed (which, in turn, reduces the turnover of working capital and as a result negatively affects the financial stability of the organization), which contributes to the rapid restructuring of the supply system to the needs of health care

The relationship between the quality management system in a medical institution and the use of document management information systems are considered. The distinctive features of the document management system, typical for modern Ukrainian institutions, are given. The analysis of ISO standards dedicated to document management is carried out and ways of their fuller use in domestic practice are shown. The consequences of the implementation of an information document management system in a medical institution are investigated. The need for informational support of management changes in a medical institution is shown on the example of improving the process of building a quality management system. It also shows the need for joint consideration of the processes of the quality management system and their support with the help of identification document management systems. The main provisions of the technique are given, which clearly define the importance of documents in the activities of a medical institution, as well as the use of the basic principles of corporate culture in the life of an organization.

The history of a medical institution and those who have worked in it can provide valuable insights into the nature of the health care of a particular historical period. While it is acknowledged that health care is provided both inside and outside medical institutions, the hospital has significantly shaped the nature of health care in general and the role of nurses in particular since the turn of this century (Reverby 1985).
