O ensaio reflexivo aborda as práticas seguras, do/no cuidado de enfermagem, no contexto hospitalar. Para tanto, analisa como o cuidar na enfermagem é ressignificado nas diferentes propositivas técnicas e políticas para a almejada ampliação das práticas seguras. Self-stigma is a salient concept in mental health care. Consequences of self-stigma include increased morbidity and decreased engagement with the health care system. Cultural factors may affect self-stigma and its consequences. This article utilizes the Giger and Davidhizar transcultural assessment model to explore veteran culture and self-stigma. Veterans have their own cultural values that can affect nursing care. Operation Iraqi Freedom and Operation Enduring Freedom veterans are seeking care in community settings more often than within the VA system; therefore, all nurses should be aware of the cultural needs of veterans including the impact of self-stigma.

Caring presence, especially the use of silence, is a difficult concept to teach. The use of case study or story is an excellent method to teach this aspect of nursing care. This case, describing an event in a clinical rotation of an undergraduate student, has served as an exemplar for other students. A pedagogical discussion of caring presence as a nursing dynamic and the impact on the patient, nurse, and the environment of care is presented. This story also serves to illuminate the values of the culture of the health care system and the impact that the action of one nurse can have on the care environment.

ABSTRACTObjective: to evaluate the impact of the human milk bank (HMB) on the nursing care to the newborn and her/his family and highlight the importance of this strategy for the nurse's work. Method: this is an observational research with a qualitative approach, consisting in the experience lived by university students from the undergraduate Nursing course during their 5th semester and the experience of nursing professionals, accomplished through the practice of performing educative activities in the HMB of a university hospital, in 2010, in Rio de Janeiro, Brazil. The research was approved by the Research Ethics Committee of Faculdade de Medicina/Hospital Universitario Antonio Pedro (CEP CMM/HUAP), under the Protocol 129/08. The experiences lived were reported in a field diary and posteriorly analyzed. Results: It was observed that anxiety, along with maternal  fears, collaborate to increase the difficulty in the process of breast feeding (BF). This fact reflects the unqualified prenatal care, the importance of the education process for preventing complications in the BF during the puerperium, and the importance of having a quiet and adequate environment for these practices, which is HMB. Besides, one can notice that the work of HMB with regard to these women provides an increase in the quality of life of the mother, the baby, and the whole community around them. Conclusion: one stresses the need for creating systematic nursing actions that contribute to a better treatment offered to these women, considering the impact that the nursing actions in HMB have on them. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa da Faculdade de Medicina/Hospital Universitário Antônio Pedro (CEP CMM/HUAP), sob o Protocolo n. 129/08. As experiências vivenciadas foram relatadas num diário de campo e posteriormente analisadas. Resultados: observou-se que a ansiedade, unida aos medos maternos, colaboram para o aumento da dificuldade no processo de aleitamento materno (AM). Tal fato reflete o atendimento pré-natal desqualificado, a importância do processo de educação para prevenção de complicações no AM durante o puerpério e a importância de se ter um ambiente tranquilo e adequado para essas práticas, que é o BLH. Además, es posible notar que la actuación del BLH con relación a esas mujeres proporciona el aumento de la calidad de vida de la madre, del bebé y de toda la comunidad en torno de ellos. Conclusión:  resaltase la necesidad de crear acciones sistematizadas de enfermería que contribuyan para un mejor atendimiento ofrecido a esas mujeres, dado el impacto que las acciones de enfermería en el BLH generan sobre estas. Resumo: O presente artigo retrata através de uma revisão bibliográfica as fragilidades das publicações científicas nos dias atuais, bem como o impacto das publicações brasileiras a nível mundial. Tem como objetivo identificar os indicadores de qualidade que as entidades recomendam às publicações científicas em âmbito nacional e internacional como: Qualis, ISI, SciELO, agências de fomento e autores da literatura. O trabalho também ressalta a importância de se avaliar os periódicos científicos on-line como forma de manutenção da qualidade, e recomenda um formulário de avaliação para o mesmo. SCIENTIFIC PRODUCTION: TOOLS AND QUALITY INDICATORS Abstract: This article shows through a literature review the weaknesses of scientific publications today, and the impact of Brazilian publications worldwide. Aims to identify the quality’s indicators that authorities recommend to scientific publications in national and international level as Qualis, ISI, SciELO, fomentation agencies and literature’s authors. The work also observes the importance of on-line scientific journals evaluating with the quality of maintenance of way and recommend an evaluation form for it. An analysis of the impact of explosion hazards on the level of environmental safety of disposal and destruction of ammunition. An analysis of existing technologies of land reclamation that can be used for places of disposal and destruction of ammunition, and identified opportunities and limitations of their use. For the first time, a simulation model of the safety management system for land reclamation and ammunition destruction was created. During the development of the model, it is proposed to consider the parameters of the site of disposal and destruction of ammunition, which determine the parameters of explosion risk, and environmental quality indicators, as responses to the influence of factors of operation of the site of disposal and destruction of ammunition. Safety criteria are determined using a regulatory approach in three areas: current factors, explosion risk parameters and environmental quality indicators. The integrated safety criterion is defined as the highest value of all individual safety criteria.

Objetivo: Identificar na literatura indicadora de qualidade na assistência de enfermagem em cuidados paliativos. Método: Revisão integrativa da literatura em três bases de dados, com os descritores Nursing, palliative, indicator, palliative care, nurse care, quality indicator, cuidados paliativos, cuidado de enfermagem, tratamento paliativo, palliative assistence, indicadores de qualidade, hospice care, cuidados de conforto, atencíon de enfermería, health care, indicadores de calidad, com amostra final de 7 artigos. Resultados:  Dentre os artigos encontrados foi evidenciado um foco no direcionamento dos indicadores para os cuidados multiprofissional, porém, a maioria dos indicadores encontrados se aplicavam aos cuidados de enfermagem e se dividiram nos aspectos físico, psicológico, social e espiritual. Conclusão: foi encontrada uma pequena quantidade de referências nas bases de dados em relação aos indicadores em cuidados paliativos necessitando de mais estudos voltados para essa área.DESCRITORES: Cuidado de enfermagem; Cuidado paliativo; indicadores de qualidade. Objective: To identify in the literature quality indicators in nursing care in palliative care. BACKGROUND: There is a paucity of published data regarding the quality of care of inflammatory bowel disease (IBD) in Canada. Clinical quality indicators are quantitative end points used to guide, monitor and improve the quality of patient care. In Canada, where universal health care can vary significantly among provinces, quality indicators can be used to identify potential gaps in the delivery of IBD care and standardize the approach to interprovincial management.METHODS: The Emerging Practice in IBD Collaborative (EPIC) group generated a shortlist of IBD quality indicators based on a comprehensive literature review. An iterative voting process was used to select quality indicators to take forward. In a face-to-face meeting with the EPIC group, available evidence to support each quality indicator was presented by the EPIC member aligned to it, followed by group discussion to agree on the wording of the statements. The selected quality indicators were then ratified in a final vote by all EPIC members.RESULTS: Eleven quality indicators for the management of IBD within the single-payer health care system of Canada were developed. These focus on accurate diagnosis, appropriate and timely management, disease monitoring, and prevention or treatment of complications of IBD or its therapy.CONCLUSIONS: These quality indicators are measurable, reflective of the evidence base and expert opinion, and define a standard of care that is at least a minimum that should be expected for IBD management in Canada. The next steps for the EPIC group involve conducting research to assess current practice across Canada as it pertains to these quality indicators and to measure the impact of each of these indicators on patient outcomes.

Background/Aim. Accreditation is considered to be the oldest and most widespread mechanism of independent external evaluation of health care quality and is implemented in over 70 countries worldwide. Despite numerous studies in this field, there is still no solid evidence about its impact on health care quality and patient safety. The goal of this paper was to investigate if the accreditation process has an effect on the difference in values of health care quality indicators. Methods. The study was conducted in two tertiary level health care hospitals, one accredited, the other non-accredited. Values of seven quality indicators in the period before, during and immediately after the completion of accreditation (from 2007?2015), which measure health care quality, patient safety, the efficiency and productivity of the institution, were compared. Results. Of the seven monitored quality indicators, a positive effect of the accreditation process can be attributed to a shorter length of waiting for the first scheduled health check at the institution, shorter length of waiting for the first scheduled surgical check, lower rate of patients with decubitus as well as a decrease of the rate of hospital days per patient with acute myocardial infarction. No effect of accreditation was found on the mortality rate, mortality rate within the first 48 hours of hospitalization, and the average rate of hospital days per patient at the level of the institution. Conclusion. The process of accreditation undoubtedly intensifies activities that contribute to improving health care quality, which results in better health outcomes. Additional research in this field and new evidence about the relationship between accreditation and quality upgrading in health care institutions are required because this could motivate their managers to decide more easily to enter into this process and implement it, despite the additional efforts and financial investments associated with accreditation.

Abstract Research conducted in 2016 explored the health care experiences of people who use illicit opioids in small Ontario urban and rural communities. Perspectives of participants who used opioids and of nurse participants were interpreted using Friere’s critical social theory framework to explore sociopolitical, economic and ideological influences. Findings describe pervasive experiences of stigma, discrimination and inappropriate care. Exploration of why such negative experiences with nursing care might be so pervasive led to a consideration of the context of health care systems and in particular of the influences of neoliberalism and the impact of the global War on Drugs. Mitigation strategies to support contextualized nursing practice are outlined. Nurses are called upon to actively resist the pressures of these political forces by advocating for policy change including decriminalization.
