Objetivo: Avaliar o Mapa de Conversação como estratégia de ensino para o desenvolvimento do autocuidado de pessoas diabéticas. Medtodologia: Estudo descritivo correlacional com abordagem quantitativa. Participaram do programa educativo, 20 usuários. Resultados: O período de execução do programa foi de oito semanas. A avaliação das ações educativas foi testada pela aplicação do Questionário de Atividades para o Autocuidado em Diabetes, cujos escores foram analisados no início e no final do programa. A variável ‘ressecamento’, relativa às características da pele e as variáveis “Alimentação específica (doces)”, “atividade física”, “glicemia capilar” e “cuidado com os pés” apresentaram significância estatística (p<0,05). Conclusão: Programas educativos são essenciais para a capacitação da pessoa diabética e a mudança de seu comportamento em relação ao seu autocuidado. O Mapa de Conversação em Diabetes demonstrou ser adequado para aquisição de novos conhecimentos e autonomia da pessoa no controle glicêmico.Descritores: Diabetes mellitus; Educação em saúde; Autocuidado; Enfermagem, Estratégia de Saúde da Família.CONVERSATION MAP AS A TEACHING STRATEGY FOR THE DEVELOPMENT OF SELF-CARE IN DIABETES MELLITUS ABSTRACTObjective: To evaluate the Conversation Map as a teaching strategy for the development of self-care for diabetic people. Methodology: Descriptive correlational study with quantitative approach. Twenty users participated in the educational program. Results: The period of execution of the program was eight weeks. The assessment of educational actions was tested by the application of the Diabetes Self-Care Activity Questionnaire (DSCAQ), whose scores were analyzed at the beginning and at the end of the program. The “dryness” variable, related to the characteristics of the skin and the variables “Specific diet (sweets)”, “physical activity”, “capillary glycemia” and “foot care” were statistically significant (p <0.05). Conclusion: Educational programs are essential for the empowerment of the diabetic person and the change in their behavior regarding self-care. Conclusión: Los programas educativos son esenciales para la capacitación de la persona diabética y el cambio de su comportamiento en relación al auto cuidado. Objetivo: analisar a frequência de atividades de autocuidado em pessoas com Diabetes Mellitus tipo 2 e sua associação com o controle clínico. Método: trata-se de um estudo quantitativo, analítico, em cinco Estratégias Saúde da Família, com 86 indivíduos. Coletaram-se os dados por meio de um formulário e um  Questionário de Atividades de Autocuidado com o Diabetes. Analisaram-se os dados por meio do programa Statistical Package for the Social Sciences (SPSS) IBM®, versão 20.0. Resultados: observou-se, considerando o autocuidado, resultado satisfatório para a ingestão de doces e uso de medicamentos e insatisfatório para as demais atividades. Encontrou-se baixo percentual de tabagismo. Associou-se a frequência de atividades de autocuidado significativamente com tempo de diagnóstico, tipo de medicação e glicemia capilar. Conclusão: conduziram-se os resultados a um perfil de autocuidado insatisfatório que influencia no mau controle clínico do DM2. Permitiu-se, além disso, avaliar a importância do autocuidado no controle metabólico do DM2, servindo como subsídio para o desenvolvimento de estratégias de enfrentamento. Resultados: se observó, considerando el autocuidado, un resultado satisfactorio para la ingestión de dulces y el uso de medicamentos e insatisfactorio para las otras actividades. Se encontró un bajo porcentaje de tabaquismo. La frecuencia de las actividades de autocuidado se asoció significativamente con el tiempo desde el diagnóstico, el tipo de medicamento y la glucosa en sangre capilar. Conclusión: los resultados condujeron a un perfil de autocuidado insatisfactorio que influye en el control clínico deficiente de la DM2. También se permitió evaluar la importancia del autocuidado en el control metabólico de DM2, sirviendo como un subsidio para el desarrollo de estrategias de afrontamiento. Objective: to detect factors related to negative adherence to self-care in individuals with diabetes mellitus. Methods: observational, cross-sectional, analytical, and exploratory study, conducted with individuals diagnosed with type 2 diabetes mellitus and followed-up by the Family Health Strategy. The sample consisted of 250 participants. The Summary of Diabetes Self-Care Activities Questionnaire was used for self-care assessment. Association tests, prevalence ratio and Poisson regression with robust variance were performed in the analysis. Results: negative attitudes related to self-care increased by 21% for single/divorced/widowed, 20% for retired, 54% for those who did not diet, 28% for those who did not practice physical activity, 24% for hypertensive, 30% for those with dyslipidemia, and 44% for those with retinopathy. Conclusion: socioeconomic factors, behavioral factors, presence of comorbidities and complications related to diabetes mellitus were related to negative adherence to self-care.

Diabetes melitus (DM) merupakan penyakit kronis yang ditandai dengan kenaikan kadar glukosa dalam darah dan dapat menyebabkan komplikasi akut ataupun kronik jika tidak ditangani. Neuropati Perifer merupakan komplikasi kronik yang banyak terjadi pada pasien DM. Perawatan diri (Self Care) yang baik dapat membantu mencegah terjadinya komplikasi tersebut. Penelitian ini bertujuan untuk mengetahui Hubungan Self Care dengan kejadian komplikasi Neuropati Perifer pada pasien DM tipe II. Jenis penelitian ini menggunakan deskriptif korelasi dengan sampel 69 responden, yang diperoleh melalui teknik Accidental sampling. Pengumpulan data menggunakan modifikasi kuesioner Summary Diabetes Self Care Activity (SDSCA) dan kuesioner kejadian komplikasi Neuropati Perifer. Hasil penelitian menggunakanujikorelasi Chi square ini menunjukkan terdapat hubungan antara self care dengan kejadian komplikasi neuropati perifer pada pasien DM tipe II dengan p value 0,010 (p<0,05). Dari 69 responden, 33 responden memiliki self care baik, 24 diantaranya menunjukkan tidak terjadi komplikasi neuropati perifer dan 9 diantaranya menunjukkan terjadi komplikasi neuropati perifer. Sebanyak 36 responden memiliki self care kurang, 14 diantaranya menunjukkan tidak terjadi komplikasi neuropati perifer dan 22 diantaranya menunjukkan terjadi komplikasi neuropati perifer. Peran perawat sebagai edukator sangat penting untuk membekali pasien DM agar memiliki perawatan diri yang baik.

To verify characteristics related to self-care and clinical parameters in patients with type 2 diabetes mellitus. Methods: descriptive and exploratory, cross-sectional study, conducted with 173 patients assisted in 12 Family Health Units in the urban area of a city in the Northeast region of Brazil. Results: most participants (61.3%) were female, aged less than 60 years old. There were significant differences in the lower glycemic control (p = 0.014), capillary glycemia (p = 0.018) and alcohol consumption (p = 0.015) for men as well as higher central obesity indexes for women (p = 0.000). It was observed high frequency of overweight, abdominal obesity, high blood pressure, elevated blood glucose levels and insufficient levels of physical activity. Conclusion: there is the need for nursing actions aimed at improving self-care and control of the clinical parameters in these patients.

ABSTRACT Objective: to investigate the association between resilience and self-care in people with diabetes mellitus treated in Primary Health Care. Method: Cross-sectional study, sample consisting of 362 people, aged 18 years or older, diagnosed with diabetes for at least one year. Descriptive analyzes and comparison of means were performed, assuming statistical significance with a value of p <0.05. The Resilience Scale and Questionnaire on Diabetes Self-Care Activities were applied, containing six dimensions: general food, specific food, physical activity, blood glucose monitoring, foot care, medication use, plus three items on smoking. Data collection took place between December and May 2016, in ten Health Centers in a city in the south of the country. Results: among the 15 self-care activities, four showed a statistically significant association when compared to the average resilience, highlighting: healthy eating and professional guidance, desirable sweet consumption, blood sugar assessment as recommended. Conclusion: the results obtained highlight the relationship between high averages of resilience and adequate performance in the care of diabetes mellitus.

ABSTRACT Objective: To evaluate the effect of educational intervention in the adherence to self-care activities and functional health literacy and numeracy in people with type 2 diabetes mellitus. Methods: This was a quasi-experimental study conducted in two units of the Brazilian Family Health Strategy, involving people with diabetes. Educational interventions occurred in three meetings, weekly, lasting 60 minutes on average. Data were collected using the Questionário de Autocuidado com o Diabetes, before and after the interventions. Results: 55 people participated in the study. After the interventions, the greatest difference for a better adherence to self-care was the item “inspecting the inside of the shoes before putting them on”, with 3.29 days in the week delta at analytical level. The worst was “taking insulin shots as recommended”, with 0.00 days a week delta at basic level. Conclusion: Educational interventions had a positive effect on adherence to self-care and functional literacy in health.

Abstrak Diabetes melitus merupakan sekelompok gangguan metabolisme tubuh, ditandai dengan hiperglikemik kronis yang dapat mengakibatkan komplikasi akut dan kronis. Salah satu peran perawat sebagai pemberi asuhan adalah memberikan asuhan keperawatan yang berkualitas melalui pendekatan Teori Self Care Orem. Penggunaan pendekatan teori ini diharapkan dapat membantu perawat dalam menanggulangi keterbatasan yang dimiliki pasien dan melibatkan pasien secara aktif dalam proses perawatannya melalui peilaku perawatan mandiri. Tujuan penulisan karya ilmiah akhir ini adalah menganalisis hasil proses belajar praktik klinik keperawatan medikal bedah melalui penerapan asuhan keperawatan, penerapan evidence based nursing practice, dan inovasi keperawatan. Data yang diperoleh selama praktik klinik menunjukkan sebagian besar pasien diabetes melitus mengalami komplikasi ulkus kaki diabetik yang disebabkan oleh perilaku perawatan kaki mandiri belum optimal. Hasil penerapan evidence based nursing practice berupa kegiatan edukasi perawatan kaki mandiri dapat meningkatkan pengetahuan dan membentuk perilaku perawatan kaki sebagai tindakan pencegahan dini. Hasil kegiatan inovasi keperawatan melalui pengkajian kesehatan mandiri adalah pasien dapat melakukan pengkajian mandiri terhadap kebutuhan edukasi dan mendapatkan edukasi sesuai kebutuhan mereka. Kesimpulan dari karya ilmiah akhir ini adalah perlunya peningkatan kualitas asuhan keperawatan pada pasien diabetes melitus melalui pendekatan teori keperawatan untuk meningkatkan kemandirian pasien dalam pengelolaan penyakitnya. Kata kunci: Diabetes Melitus, Teori Self Care Orem, Edukasi Perawatan Kaki Mandiri, Pengkajian Kesehatan Mandiri  Abstract Diabetes mellitus is a metabolic disorder that characterized by chronic hyperglycemia which could lead to acute and chronic complications. One of nurse roles in managing diabetes mellitus is deliver nursing care using Orem’s Theory of Self Care as guide of nursing practice. With this theoretical approach, nursing is expected to be able to overcome the limitations and inability of the patients and actively involve patients in the treatment process through self-care behaviors. The aims of this paper is to analyze the implementation of nursing care, application of evidence based nursing practice, and innovation program which integrated to clinical practice. Based on patient collective data, it was found that the majority of diabetes mellitus patients had been hospitalized with diabetic foot ulcers complications that caused by non-optimal foot self-care behavior. The results of evidence based nursing practice showed that the diabetic foot care education program can improve the knowledge and build the foot self-care behavior as early prevention action. The results of innovation program through self-health assessment program showed that patients could assess their needs and get health education according to the assessment results. In conclusion, it’s needed to improve the quality in caring for diabetes mellitus patients through application of nursing theory to increase the self-care behaviors in management of disease.   Keywords: Diabetes Mellitus, Orem’s Theory of Self Care, Diabetic Foot Care Education, Self-Health Assessment.

Management of Diabetes Mellitus (DM) aims to improve the quality of life for DM sufferers and reduce the risk of acute complications. Diabetes Self-Management Education (DSME) is a process that facilitates knowledge, skills, and abilities in self-care for DM patients based on evidence-based research. This activity aims to increase DM sufferers' knowledge and health status at Parangloe Health Center, Gowa Regency, South Sulawesi. The methods used are lectures, discussions, and demonstrations. The DSME administration was carried out in three sessions with 45 minutes and carried out diabetic foot gym training. Besides, the blood sugar level is checked at any time. The results achieved were an increase in knowledge of DM sufferers by 63.4% regarding definitions, classifications, signs and symptoms, risk factors, DM complications, diet, physical exercise, types and benefits of drugs given, foot care, and 96% being able to do foot exercises for diabetic. Also, there was an increase in the understanding and skills of cadres about DM self-care management. DM patients are also expected to check their blood glucose regularly and use the DM Self-Care Manual that has been provided.
