Specialty-Training Program During COVID-19 Pandemic: A Single Center Survey on over 300 Trainees and Trainers (Preprint)

BACKGROUND COVID-19 pandemic caused significant modifications such as limiting the number of residents in the clinics, cancelling elective surgical procedures, stopping face to face practical education, and transforming theoretical education into distance learning platforms resulted in alterations in the curriculum. OBJECTIVE We addressed to assess the situation of trainees’ education using an online questionnaire from the trainees’ and directors’ perspective during the pandemic. METHODS The survey platform SurveyMonkey® was used to distribute the survey and to collect responses. We generated a list of multiple-choice questions about how social distancing affected the delivery of medical education, potential compromise in core training and difficulties in conducting clinical research for the thesis. RESULTS A total of 364 trainees among 552 (65.9%) under training at our university hospital and 90% of the directors (37 of 41) responded the survey. Almost 78 percent of the trainees claimed that they have been negatively affected during the pandemic. Although majority of the trainees (60,3%) reported that extension of their education program is not necessary, most of the program directors were in tendency of extending the duration of the speciality training period. The participants predominantly considered that online training would keep on being a part of the training program after the pandemic. CONCLUSIONS Education programs are negatively affected during pandemics. However, authorities should manage this deficiency by a new perspective since present trainees are familiar to use technology-driven virtual sources for their education. After the pandemic, computer-assisted online learning and web-based programs should be integrated into educational curriculum. Introduction: Clostridioides (Clostridium) difficile can be isolated from stool in 3% of healthy adults and in at least 10% of asymptomatic hospitalized patients. C. difficile, the most common cause of hospital-acquired infectious diarrhea in the developed world, has re-emerged in recent years with increasing incidence and severity. In an effort to reduce the spread of the pathogen, published recommendations suggest isolation and contact precautions for patients suffering from C. difficile infection (CDI). However, asymptomatic colonized patients are not targeted by infection control policies, and active surveillance for colonization is not routinely performed. Moreover, given the current changes in the epidemiology of CDI, particularly the emergence of new virulent strains either in the hospital or community settings, there is a need for identification of factors associated with colonization by C. difficile and CDI. Methods and analysis: We are carrying out a prospective, observational, cohort study in Edouard Herriot Hospital, Hospices Civils de Lyon, a 900-bed university hospital in Lyon, France. All consecutive adult patients admitted on selected units are eligible to participate in the study. Stool samples or rectal swabs for C. difficile testing are obtained on admission, every 3–5 days during hospitalization, at the onset of diarrhea (if applicable), and at discharge. Descriptive and logistic regression analyses will be completed to mainly estimate the proportion of asymptomatic colonization at admission, and to evaluate differences between factors associated with colonization and those related to CDI. Ethics: The study is conducted in accordance with the ethical principles of the Declaration of Helsinki, French law, and the Good Clinical Practice guidelines. The study protocol design was approved by the participating units, the ethics committee and the hospital institutional review board (Comité de protection des personnes et Comission Nationale de l’Informatique et des Libertés; N°: 00009118). Dissemination: The results of this study will be disseminated by presenting the findings locally at each participating ward, as well as national and international scientific meetings. Findings will be shared with interested national societies crafting guidelines in CDI.

AbstractThe sequential mixed-mode strategy has become standard practice in the survey management of longitudinal studies, in order to achieve consistently high response rates. To realise this aim in a cost-efficient way, a first mode is often an online questionnaire, towards which the target persons are pushed, and a second mode is then a telephone interview, offered to those who do not respond to the initial mode. However, the rationale for using the sequential mixed-mode plus “push-to-web” strategy to reduce the burden of choosing between survey modes, in order to maximise survey participation, could be undermined if there is an overlapping field period during which the target persons could choose between two or more offered modes. The use of reminders might be useful in solving this problem of competing risks. In the context of a multiple-panel study, this question was investigated by utilising longitudinal paradata from the fieldwork, as well as procedures of event history analysis that are adequate for the analysis of processes with competing risks. First, for a web survey as the initial mode and computer-assisted telephone interview (CATI) as the subsequent mode, it was found that the idea of a sequential mixed-mode plus “push-to-web” strategy does work even in the case of competing risks in the choice of a survey mode. Second, it was confirmed that reminders are a useful instrument for stimulating panellists to choose the running survey mode. Third, and finally, it was found that this feature of survey management is effective in countering panellists’ procrastination in regard to responding to a survey.

Burnout syndrome is common among healthcare professions, including resident physicians. We aimed to assess the prevalence of burnout among resident physicians in Jordan, and a secondary aim was to evaluate the risk factors associated with the development of burnout syndrome in those residents, including gender, working hours, psychological distress, training sector, and specialty. In this cross-sectional study, 481 residents were recruited utilizing multistage stratified sampling to represent the four major health sectors in Jordan. Data were collected using an online questionnaire, where the Copenhagen Burnout Inventory (CBI) was used to assess the prevalence of burnout. The prevalence, group differences, and predictors of burnout were statistically analyzed using STATA 15. Overall, 373 (77.5%) residents were found to have burnout. Factors associated with higher levels of burnout were psychological stress (β = 2.34, CI = [1.88–2.81]), longer working hours (β = 4.07, CI = [0.52–7.62], for 51–75 h a week, β = 7.27, CI = [2.86–11.69], for 76–100 h a week and β = 7.27, CI = [0.06–14.49], for >100 h a week), and obstetrics/gynecology residents (β = 9.66, CI = [3.59–15.73]). Conversely, medical sub-specialty residents, as well as private and university hospital residents, had lower burnout levels. We concluded that decreasing the workload on residents, offering psychological counseling, and promoting a safety culture for residents might help in mitigating burnout consequences.

The aim was to examine the impact on interpreters’ health knowledge, attitudes and self-evaluated skills after they participated in a pilot health introduction course at a university hospital in Denmark. The study was conducted as an intervention study using a questionnaire with both closed and open-ended questions. The questionnaire was distributed to interpreters one week before the six-week course started, and again at one week and at three months after course completion. Level of knowledge was calculated based on the number of answers to 18 multiple-choice questions on common health issues, diagnoses and treatments. Of the 100 interpreters who registered for the course, 86 completed the course, and 61 of these participants (70%) completed both the baseline and the one-week questionnaire. The mean knowledge score increased from 48 ( SD 6.9) at baseline to 52 ( SD 3.4; p < 0.001) one week after the course and was 51 ( SD 7.3; p < 0.001) three months after the course ( n = 55). Participants who increased their knowledge score the most were those with the least interpreter experience ( p = 0.001). One week after the course, most participants (83–95%) agreed that the individual lessons had been useful in their subsequent interpreting activities and that they had gained useful information. The health introduction course appeared to be beneficial for interpreters. This study highlights the need for greater focus on education for interpreters working in the healthcare sector.

The fear of vaccines has led to population rejection due to various reasons. Students have had their own inquiries towards the effectiveness of the vaccination, which leads to vaccination hesitancy. Vaccination hesitancy can affect students’ perception, hence, acceptance of e-learning platforms. Therefore, this research attempts to explore the post-acceptance of e-learning platforms based on a conceptual model that has various variables. Each variable contributes differently to the post-acceptance of the e-learning platform. The research investigates the moderating role of vaccination fear on the post-acceptance of e-learning platforms among students. Thus, the study aims at exploring students’ perceptions about their post-acceptance of e-learning platforms where vaccination fear functions as a moderator. The current study depends on an online questionnaire that is composed of 29 items. The total number of respondents is 630. The collected data was implemented to test the study model and the proposed constructs and hypotheses depending on the Smart PLS Software. Fear of vaccination has a significant impact on the acceptance of e-learning platforms, and it is a strong mediator in the conceptual model. The findings indicate a positive effect of the fear of vaccination as a mediator in the variables: perceived ease of use and usefulness, perceived daily routine, perceived critical mass and perceived self-efficiency. The implication gives a deep insight to take effective steps in reducing the level of fear of vaccination, supporting the vaccination confidence among educators, teachers and students who will, in turn, affect the society as a whole.

In recent years, there has been widespread interest around the potential for technology to transform learning. As investment in education technology continues to grow, students, parents, and teachers face a seemingly endless array of education technologies from which to choose—from digital personalized learning platforms to online courses to text message reminders to submit financial aid forms. Amid the excitement, it is important to step back and understand how technology can help—or in some cases hinder—learning. This review article synthesizes and discusses rigorous evidence on the effectiveness of technology-based approaches to education in developed countries and outlines areas for future inquiry. In particular, we examine randomized controlled trials and regression discontinuity studies across the following categories of education technology: (i) access to technology, (ii) computer-assisted learning, (iii) technology-enabled behavioral interventions in education, and (iv) online learning. We hope this synthesis will advance academic understanding of how technology can improve education, outline key areas for new experimental research, and help drive improvements to the policies, programs, and structures that contribute to successful teaching and learning. Background: The (HELLP) syndrome is a severe health hazard in pregnancy described by elevated liver enzymes, low platelet count and hemolysis. It happens in 0.4 to 0.7% of all gestations and in 10-12% of cases with severe preeclampsia. Patients and methods: The present study was a prospective observational study that was made at Department of Obstetrics and Gynecology, Kasr Al-Ainy University Hospital, Cairo University, from September 2018 – March 2019. It included One Hundred and Thirty pregnant women diagnosed with hypertension in the current pregnancy complicated with severe preeclampsia, or HELLP Syndrome associated with abdominal ascites, recruited from the attendees of the Obstetric Emergency Department. The study was approved by the local institutional review board of the Faculty of Medicine, Cairo University. All ladies signed an informed consent. Results: There were statistically significant differences between the HELLP group and the severe preeclamptic group regarding maternal ICU admission and the need of multiple drugs to control the blood pressure being less in severe preeclampsia group. The need for ICU admission was much higher in the HELLP group (43.1%) compared to the severe preeclampsia group (9.2%) showing statistical significance (P<0.001). Conclusion: Maternal and neonatal sicknesses are elevated between cases with HELLP syndrome. So, early diagnosis and definitive treatment can be made to enhance maternal and neonatal results.

Background.Atrial fibrillation (AF) is the most common form of arrhythmia. Several trials have suggested that acupuncture may prevent AF. However, the efficacy of acupuncture for AF prevention has not been well investigated. Therefore, we designed a prospective, two-parallel-armed, participant and assessor blinded, randomized, sham-controlled clinical trial to investigate acupuncture in persistent AF (ACU-AF).Methods.A total of 80 participants will be randomly assigned to active acupuncture or sham acupuncture groups in a 1 : 1 ratio. Both groups will take the same antiarrhythmic medication during the study period. Patients will receive 10 sessions of acupuncture treatment once a week for 10 weeks. The primary endpoint is AF recurrence rate. Secondary endpoints are left atrium (LA) and left atrial appendage (LAA) changes in function and volume, and inflammatory biomarker changes.Ethics.This study protocol was approved by the institutional review boards (IRBs) of Kyung Hee University Hospital (number 1335-04). This trial is registered with clinicaltrials.govNCT02110537.

E-book perceptions and use in STEM and non-STEM disciplines: A comparative follow-up study. portal: Libraries and the Academy, 16(1), 131-162. https://doi.org/10.1353/pla.2016.0002 Abstract Objective – To compile a set of usability and collection development suggestions and to examine a possible statistical correlation between visiting the physical library, online resource use, and e-book use. Design – Online questionnaire survey. Setting – Major public research university in Maryland, United States of America. Subjects – 47,209 faculty, students, and staff. Methods – This survey is a follow-up to a similar 2012 study at the same institution. Survey respondents completed 14 multiple-choice and up to 8 open-ended questions about academic e-book discovery, perception, and usage patterns for both STEM and non-STEM respondents using the Qualtrics online research platform. Seven of eight open-ended questions were conditional (i.e., dependent on answers to multiple-choice questions), thus the number of questions answered by respondents could vary. The survey was available from October 1 to November 22, 2014, and promoted across a variety of communication channels (email, library website, social media, print flyers and handouts). Incentives for completing the survey included one iPad Mini and eight U.S. $25 Amazon gift cards. Main Results – 1,911 (820 STEM and 1,091 non-STEM) self-selected students, faculty, and staff from a total campus population of 47,209 faculty, students, and staff (4.2% response rate) participated in the survey, excluding 277 additional responses representing library personnel (70) and individuals not affiliated with the institution (207). 64% of respondents indicated more e-book use than three years before, with only 21.9% of respondents noting they never use e-books for academic purposes compared to 31% in 2012. 32.5% of respondents noted daily or weekly use of e-books for scholarly pursuits, with undergraduates reporting the most frequent use: 38.6% daily/weekly use versus 37.2% for graduate students, 16.2% for faculty, and 14.2% for staff. 38% of respondents reporting daily/weekly use were from STEM disciplines; 31.3% were from non-STEM fields. Computers, not e-readers, were the primary devices used for accessing e-books: 72.5% of respondents reported using laptops or desktops to this end versus tablets, 37.9%; mobile phones, 36.7%; Kindles, 25.6%; Nooks, 5.9%; and other e-readers, 3.3%. Top “mixed device access” responses were tablet/mobile phone/computer (98 responses); mobile phone/computer (93 responses); and tablet/computer (81 responses). The top three discovery tools respondents reported using for finding e-books were commercial sites (35.9%), free websites (26.8%), and the library website (26.2%). A weak-positive Spearman’s rho rank correlation of 0.25 provides some evidence that respondents who visit the library often are likely to use online resources and e-books. 35% of respondents reported they use e-books online “most of the time,” and 67% of respondents indicated they print out e-book content for use. Responses to the question “What, if anything, would make you more likely to use e-books for academic purposes?” included easier access via the library website (48% of respondents), better functionality for highlighting/annotating (44%), reduced cost (43.2%), easier downloading (38.5%), more e-books in area of research interest (37.3%), more textbooks (37.2%), and ownership of a dedicated e-reader (35.6%). In 2012, 52% of respondents reported never having downloaded an e-book for offline use. This percentage dropped notably in this study, with only 11.5% of respondents indicating they had never downloaded for later use. Conclusion – While this study indicates both STEM and non-STEM respondents at this institution are increasingly using e-books, preferences for electronic versus print format varied according to content type and type of user (e.g., STEM or non-STEM, undergraduate or graduate, student/faculty/staff). Key recommendations for usability and collection development include: improving discovery and awareness mechanisms, purchasing some content (e.g., references works, style guides) in e-format while ensuring multiple simultaneous use, taking advantage of print plus electronic options to serve users with different format preferences, and encouraging vendors to allow digital rights management free downloading and printing.
