Aims and Objectives: The main aim of the study was to find out the association of serum homocysteine (HCY) in diabetic neuropathy patients. Methods: All the patients who were diagnosed with Type II diabetes mellitus will be included. Their serum levels of fasting blood sugar, postprandial blood sugar, glycated hemoglobin, and associated blood parameters will be assessed. Diabetic neuropathy will be confirmed using nerve conduction testing, electromyography, and quantitative sensory testing with clinically correlated. The serum HCY levels will be measured and correlated with other blood parameters. Results: Of 1000 patients, 46 were Type I diabetic and 954 were Type II. The prevalence of neuropathy in diabetic patients was 156. Mean serum HCY without diabetic neuropathy was 6.8+2.9 and serum HCY with diabetic neuropathy was 21.6+0.29 and p value was found to be 0.0017. The correlation between serum HCY and diabetic neuropathy was found to be 14.5 with p=0.001. Conclusion: There has been a significant increase of HCY in diabetic patients. It can be clearly seen that elevated serum HCY level has led to some of the complications of diabetic neuropathy.

Introduction: Diabetic neuropathy is one of the early complications of diabetes mellitus patients which is very difficult to face in the daily living activities. The purpose of the present study was to see the demographic characteristics of diabetic neuropathy patients. Metarials & Methods: This descriptive type of cross-sectional study was conducted in the Department of Neurology including Neuropathy Clinic and in collaboration with department of Endocrinology at Banghabandhu Sheikh Mujib Medical University, Dhaka from January 2012 to December 2013 for a period of two (2) years. Adult diabetic patients presented with neuropathic pain with symmetrical involvement of distal limbs from indoor and outpatient department of Neurology including Neuropathy clinic as well as indoor and outpatient department of Endocrinology, BSMMU were enrolled in the study population. Data was collected by face to face interview. Information was collected by taking medical history and clinical examinations and subsequent laboratory investigations. Results: A total number of 102 cases were recruited for this study who were clinically diagnosed as painful diabetic polyneuropathy. Female was predominant than male 55(53.9%) cases and 47(46.1%) cases respectively. The male and female ratio was 1:1.2. Majority were in the age group of more than 55 years which was 55(53.9%) cases. The mean age with SD of the study population was 52.79±9.42 years. Among 102 patients type II DM was predominate than type I patients which were 95(92.2%) cases and 8(7.8%) cases respectively. The mean duration of DM with SD was 6.51±3.6 years. However the mean duration of neuropathic pain was 1.68±1.155 years. Conclusion: In conclusion majority of the diabetic neuropathy patients are female suffering from type II DM in the middle age. Medicine Today 2019 Vol.31(1): 27-30

Objective: The evaluation of serum osteocalcin (OSN) for Iraqi infertile patients to see the effect of osteocalcin insufficiency, which may lead to a decreased level of testosterone production in males that may cause infertility. Methods: Forty two newly diagnosed infertile males age range (24–47) years and thirty two apparently healthy males as controls age range (25–58) years. Serum levels of testosterone (TEST), stimulating follicle hormone (FSH) and luteinizing hormone (LH), prolactin (PROL), osteocalcin OSN, and fasting blood sugar (FBS) were performed in both patients and controls. Estimation of serum OSN by Immulite1000 auto-analyzer, TEST, FSH, LH, PROL, and FBS by Immulite2000 auto-analyzer. Results: Infertile patients show significantly elevated serum levels for follicle-stimulating hormone (FSH), LH, and PROL as compared with controls according to p-value ( 0.000), (0.044), and (0.000), respectively. On the other hand, the infertile patients have lowered serum levels for OSN and TEST as compared with controls according to p-value (0.000) and (0.000), respectively. Fasting blood sugar (FBS) is evaluated within the normal levels for both patients and control groups. Conclusion: Osteocalcin, act as a regulator of male fertility via its role in the biosynthesis of testosterone, so it serves as a biomarker for evaluation of male infertility. Therefore, osteocalcin could be used as a therapeutic for the treatment of disorders related to male reproduction, including male infertility and low testosterone levels.

Objectives: Diabetes Mellitus being a major issue faced by developingcountries like Pakistan. Non-adherence is a major issue in achieving a desired therapeuticgoal. This study aims at determining relation with the drug non-adherence associated with thetreatment regime, treatment satisfaction and educational status of the person. Study Design:This was a cross sectional study. Place and Duration: This study was done at the Diabetesclinic, Nishtar hospital, Multan. Total 242 diabetic patients were studied over a period of 10months. Methodology: The inclusion criteria was set as patients who have been diagnosedwith Diabetes Type II and had been on oral hypoglycemic medications for about last 5 monthsor more. Those patients who were diagnosed with Type I diabetes and Type II diabetesless than 5 months were not included in the study. Also those patients who were on Insulintherapy were also excluded. Socio-demographic data was obtained along with therapy typeand educational status. Drug adherence level was studied with the 8-item Morisky (MMAS-8). Treatment satisfaction was studied with the help of treatment satisfaction questionnaire formedication. The data was analyzed on the SPSS version 23 with significant p-value <0.05.Results: Among 242 patients 152(62.81%) were females and 90(37.19%) were males. Meanage of the patients were 47.68±15.13 years. Patients on combination therapy were (n=134,55.3%) and on monotherapy were (n=108, 44.6%). Mean adherence score was 5.4±1.1.Themean for four domains of Treatment Satisfaction Questionnaire for Medication(TSQM) were:effectiveness= 66.93±15.27, side effects=32.09±26.91, convenience =62.59±14.49 andsatisfaction= 65.49±16.13.Non-adherence was found to be associated with age, illiteracy,side-effects, satisfaction and convenience (p<0.05). Conclusion: Non-adherence must beovercome in patients to achieve appropriate glucose levels. Significant factors affecting theadherence levels were Illiteracy, Side effects, convenience and satisfaction of the patient. Publicawareness campaigns and regular clinic visits should be encouraged. Drugs having lower sideeffects,higher satisfaction levels and combined generics for once daily dosing having greaterconvenience should be used.

SummaryWe have demonstrated previously that fibrin enhanced plasmin formation by the vascular plasminogen activator was significantly impaired, when components isolated from the plasma of three uncontrolled diabetic patients (type I) were used to study plasminogen activation in vitro. In the present study it can be demonstrated that functional properties of the vascular plasminogen activators as well as of the plasminogens from the same three diabetic patients are significantly improved after normalization of blood sugar levels and improvement of HbAlc values. Most pronounced the Km of diabetic vascular plasminogen activator in the presence of fibrin returned to normal values, and for diabetic plasminogen the prolonged lag period until maximal plasmin formation occurred was shortened to almost control values. From these data we conclude that the observed abnormalities of in vitro fibrinolysis are not primarily associated with the diabetic disease, but might be secondary to metabolic disorders caused by diabetes.

• Introduction - Recently, the prevalence of acute myocardial infarction (AMI) has been increasing in the younger generation. With the advances in the field of medicine, elevated serum levels of various biomarkers like c-reactive protein (CRP), various lipoproteins and homocysteine have been identified to be related to AMI. • Objective - To clinically diagnose acute myocardial infarction in young patients and correlate the levels of serum lipoprotein, serum homocysteine and serum hs-crp and to calculate the level of risk depending on their levels. • Method - A hospital based observational study in which 50 subjects were enrolled in the study as per the inclusion and exclusion criteria. • Results - The results support the current global findings of the younger generation, especially the males, being affected by AMI (Acute Myocardial Infarction). Sedentary lifestyle and BMI >25 can be considered as major risk factors leading to AMI. A considerable proportion of our AMI patients had significantly elevated levels of homocysteine, hs-CRP and lipoprotein A and low levels of HDL-C indicating their correlation with AMI.
