The pandemic of coronavirus disease 2019 (COVID-19) struck the globe in December 2019, killing lakhs of people and it is continuing in many countries to create havoc. There are lakhs of publications creating evidence about the management of the disease. After seeing thousands of cases, we formed opinions in each field of management and these findings may look more logical. We accept that our opinions differ subtly from the evidence. Regarding the transmission of the disease, it is spread from person to person through inhaled aerosols. If this is going to be 100% true, then the need for personal protective equipment covering the legs seems illogical. Povidone-iodine is antiviral and spreading the same as an ointment inside the nose must be effective to decrease viral load. The other antivirals with questionable efficiency like remdesivir are being used frequently, especially in the pulmonary phase. There are no clear-cut guidelines for primary contacts. Any protective drug intake could have decreased the transmission. It’s a simple logic that the act of sneezing will decrease the viral load. The practice of inhaling turmeric smoke to induce sneezing in the outdoor could have decreased the viral load. We opine that a lymphopenia of <15% can predict a worse outcome in the next 2 days. A computerized computed tomography scan of the chest is to be taken 7 days after the onset of symptoms as the disease usually enters the pulmonary phase only then. The date of onset of symptoms rather than the positive testing date should be considered for timeline management of the case. The undue tachycardia in the 1st week and desaturation below 92% in the 2nd week are the warning signs. The steroids are to be usually prescribed after the 5–6 days. The roles of psychiatric counseling and nutrition were largely underplayed.

In this work, we discovered a new phenomenon—asymptomatic COVID-19 infection, or covert case, during the pandemic. All the 3 patients had a history of exposure, with no symptoms, and no abnormalities were found in computed tomography scan or lab tests. Except for case 2, the other patients’ severe acute respiratory syndrome coronavirus 2 (SARS-COV-2) nucleic acid tests were negative. But their anti-SARS-COV-2 nucleocapsid antibody showed a dynamic trend, consistent with the process of virus infection and clearance. A growing number of asymptomatic or covert cases need more attention. Lack of surveillance may lead to another outbreak. We hope to demonstrate our cases to attract the attention of governments or health authorities that covert cases should be the focus as well.

Background: Orbital meningocele is a rare congenital malformation characterized by herniation of the meninges into the orbit through a congenital defect in the orbital bones. Much less commonly, it occurs at the site of natural openings (e.g., optic foramen and sphenoidal fissure) or can be attributed to trauma. Cases Description: We report two patients with progressive proptosis found to have orbital meningoceles, respectively, attributed to congenital and traumatic lesions. The computed tomography scan in one case documented a traumatic orbital bony defect, but in the other case, led the mistaken diagnosis of an arachnoid cyst. Conclusion: Both patients underwent two operations each to ultimately achieve successful surgical correction of their respective traumatic and congenital orbital meningoceles.

A significant proportion of patients with intestinal obstruction will be evaluated with a CT scan of the abdomen. This study presents a group of 97 patients diagnosed with mechanical obstruction or ileus on CT scan over a 16-month period at a community based teaching hospital and follows the further management of these patients. Our study shows that 43.3 per cent of patients with mechanical obstruction, diagnosed by CT scan, eventually needed surgical treatment. On the other hand, even when CT indicates ileus, 20 per cent of these patients may still require surgical intervention.

hetan Bhagat is one of the most influential fiction writers of contemporary Indian English literature. Postmodern subjects like youth aspirations, love, sex, marriage, urban middle class sensibilities, and issues related to corruption, politics, education and their impact on the contemporary Indian society are recurrently reflected thematic concerns in his fictions. In all his fictions, he has mostly depicted the contemporary urban social milieu of Indian society. Though the fictions of Chetan Bhagat are romantic in nature, contemporary Indian society and its major issues are the chief of the concerns of all his fictions. He has focused on the contemporary issues of middle class family in his fictional works. All of the chief protagonists of his works are sensitive youth and they do not compromise with the prevalent situations of society. Most of the characters are like caricatures that represent one or the other vice or virtue of the contemporary Indian society. The author has a mastery to convince the reader about the prevalent condition of society so that one can easily reproduce in mind, a clear cut image of contemporary Indian society. The present article is a sincere endeavor to present the detailed literary analysis of the select fictions of Chetan Bhagat keeping in mind how the contemporary Indian society has been replicated in the fictions.

A tracheal diverticulum is a type of paratracheal air cyst and is usually an incidental finding after a computed tomography scan of the neck and thorax. With an incidence between 1% and 4% in adults, tracheal diverticula are rare entities that can be symptomatic in certain cases. We present a case of a COVID-19 positive patient who presented to our hospital and was diagnosed with multiple tracheal diverticula during his hospitalization.

Abstract Background We diagnosed a clinical case of pulmonary infection involving Mycobacterium tuberculosis and Tropheryma whipplei in a patient with acute respiratory distress syndrome. The diagnosis was assisted by metagenomic next-generation sequencing of bronchoalveolar lavage fluid. Case presentation A 44-year-old Han Chinese inmate was transferred to the emergency department because of dry cough, chest tightness, and shortness of breath. The patient’s body temperature rose to 39.3 °C following empirical cephalosporin treatment for 1 week. The blood CD4+/CD8+ ratio was 0.7, suggesting immunodeficiency. Routine microbiological tests were performed, and tuberculosis interferon gamma release assays were positive. Mycobacterium tuberculosis polymerase chain reaction was also positive. Chest computed tomography scan revealed miliary nodules and ground-glass opacifications, which were in accordance with tuberculosis. To fully examine the etiology, we performed routine laboratory tests and metagenomic sequencing, the results of which indicated the presence of Mycobacterium tuberculosis and Tropheryma whipplei. We administered anti-tuberculosis regimen in combination with trimethoprim/sulfamethoxazole. The patient recovered, with chest computed tomography scan showing absorption of lesions. Conclusions Compared with traditional diagnostic methods such as culture and serology, metagenomic next-generation sequencing has the advantage of detecting a wide array of microorganisms in a single test and therefore can be used for clinical diagnosis of rare pathogens and microbial coinfections. It is particularly useful for immunocompromised patients as they are more prone to infection by opportunistic microorganisms.

The human respiratory tract has a complex airflow pattern. If any obstruction is present in the airways, it will change the airflow pattern and deposit particles inside the airways. This is the concern of breath quality (inspired air), and it is decreasing due to the unplanned production of material goods. This is a primary cause of respiratory illness (asthma, cancer, etc.). Therefore, it is important to identify the flow characteristics in the human airways and airways with a glomus tumour with particle deposition. A numerical diagnosis is presented with an asymmetric unsteady-state light breathing condition (10 l/min). An in vitro human respiratory tract model has been reconstructed using computed tomography scan techniques and an artificial glomus tumour developed 2 cm above a carina on the posterior wall of the trachea. The transient flow characteristics are numerically simulated with a realizable (low Reynolds number) k–ɛ turbulence model. The flow disturbance is captured around the tumour, which influenced the upstream and downstream of the flow. The flow velocity pattern, wall shear stress and probable area of inflammation (hotspot) due to suspended particle deposition are determined, which may assist doctors more effectively in aerosol therapy and prosthetics of human airways illness.
