Iatrogenic or drug induced disease is of great clinical significance in therapeutics. Treatment with antidepressants like selective serotonin reuptake inhibitors (SSRIs) and antipsychotics reveal Bruxism (excessive grinding and clenching of the teeth) as such type of iatrogenic disease, however it is rarely found with atypical antipsychotic drugs like Olanzapine. A 50-year male patient, suffering from schizophrenia was under regular antipsychotic medication receiving Tab. Olanzapine 5 mg O.D., Tab. Clonazepam 0.5 mg O.D. regularly for past 25 years. Recently he gradually developed excessive grinding and clenching of teeth despite of any dental and ENT clinical complications. Biochemical and hematological tests also showed normal result. On admission with the complication of bruxism, tab. Olanzapine dose was reduced to 2.5 mg OD, however even after dose reduction there was not much improvement in bruxism. On the 4th day of the admission tab. Olanzapine has been substituted with tab. Clozapine 50 mg TDS. After 1 week of withdrawal of Olanzapine there was no complain of bruxism and he was discharged. On follow up after 1month the complication was occasional. Causality assessment revealed a probable relation between Olanzapine and Bruxism. Though among the atypical neuroleptics, extrapyramidal side effects are thought to be less common with olanzapine, but it may cause fatal consequences. Therefore, this case study and proper investigation may be helpful in designing the treatment strategy in more effective way and also aware all the healthcare professionals to monitor the patients on neuroleptics.

Resumen: La llegada masiva de extranjeros en las últimas décadas ha dado lugar a un nuevo colectivo que acude a los centros sanitarios. El objetivo del presente trabajo es confirmar la hipótesis de que una fase de contacto previa o encuentro extra rutinario entre el intérprete y el profesional sanitario contribuye a una mejora de los resultados en la interpretación en consulta y un aumento en la calidad. Para ello, se estudian factores clave: la comunicación, los roles, el código deontológico, la legislación, la entrevista clínica y la visibilidad. Se ha realizado un estudio observacional, descriptivo con seguimiento y un estudio de caso para conocer las opiniones de los intérpretes profesionales y los profesionales sanitarios. Los resultados confirman la necesidad de un encuentro previo. En este sentido, afirmamos que la interpretación médica es un sector en crecimiento y dada la escasez de estudios sobre estos factores, es aquí donde se engloba el presente estudio. Abstract: The massive arrival of foreign people in the last decades has originated a new group of people with new healthcare needs. The aim of this paper is to ensure the hypothesis that a previous interpreter-doctor contact or an out of the routine encounter contributes to improve the interpreting services during the consultation and it enhances better quality. To that end, some key factors such as communication, roles, code of ethics, law, clinical interview stages and visibility are studied in this research work. We have carried out an observational, descriptive and follow-up study and a case study to discover the opinions of professional interpreters and healthcare professionals. Finally, the previous encounter assumption is confirmed based on the findings of the study. In this sense, medical interpreting constitutes an interesting growing field. This research focuses on the mentioned field due to the lack of studies.

Drug-induced parkinsonism (DIP) is one of the most frequent extrapyramidal disorders that develops against the background of prescribing a large number of medications. Initially, DIP was described as an adverse drug reactions (ADRs) against the background of the use of antipsychotic drugs, but later recognized as ADRs of a number of other drugs, including prokinetics, antidepressants, calcium channel blockers and antiepileptic drugs. The relative risk of developing LIP on the background of taking typical antipsychotics increased by 2.92 times compared to patients who do not take these drugs. The risk of developing DIP in patients receiving flunarizine is increased by 2.75-4.07 times. The risk of DIP with the use of antidepressants is increased by 2.14 times, among the drugs of this group with an increased risk of DIP, the use of selective serotonin reuptake inhibitors is most often associated with DIP (relative risk 1.24). Among other antidepressants, there is evidence of the development of DIP against the background of the use of duloxetine, mirtazapine, amitriptyll clomipramine, venlafaxine, trazodone. Among anticonvulsants, DIP can rarely develop against the background of the appointment of valproic acid, gabapentin, pregabalin, carbamazepine, oxcarbazepine. The risk of DIP in patients receiving metoclopramide is extremely low (0.06%), but it is 2.16 times higher compared to people who do not take this drug. Among drugs from other groups, DIP can occur against the background of the use of lithium carbonate, tacrolimus, cyclosporine, amiodarone, captopril, amphotericin B. If DIP develops, it is necessary, if possible, to reduce the dose or cancel the inducer drug, or replace it with another drug with minimal risk of DIP. Symptoms of DIP most often regress within a few weeks or months after dose reduction or withdrawal of the drug inducer. If the symptoms persist longer, it is necessary to exclude the presence of Parkinson’s disease or dementia with with Lewy bodies.

 Objectives: The objective of this case study is to evaluate the importance of detection, assessment, and reporting of an adverse drug reaction and to improve the medication perseverance. Methods: This is an observational type of case report which was observed and analyzed in Silchar Medical College and Hospital on regular ward rounds. Results: A case report of toxic epidermal necrolysis due to carbamazepine (CBZ) administration by a 47-year-old male patient who presented as a follow-up case of ischemic stroke. Causality assessment of the event done with Naranjo’ causality assessment scale suggests to be “Probable.” Conclusion: Daily, CBZ is being increasingly prescribed for control of pain in neuralgias and diabetic neuropathy, apart from control of seizures. Awareness about the drugs implicated in life-threatening drug reactions will help physicians in preventing them by cautious use of the drugs. Moreover, proper counseling to the patient regarding the use of medications is of utmost importance, in such life-threatening conditions where treatment guidelines remain indistinct.

This case report describes the positive outcome of a therapeutic intervention that integrated an intensive, residential component with follow-up telepractice for a 21 year old male who stutters. This therapy utilized an eclectic approach to intensive therapy in conjunction with a 12-month follow-up via video telepractice. The results indicated that the client benefited from the program as demonstrated by a reduction in percent stuttered syllables, a reduction in stuttering severity, and a change in attitudes and feelings related to stuttering and speaking.

This research was conducted to determine the Effectiveness of Jakarta Siaga 112 Emergency Services in Fire Management by UPT. Disaster Data & Information Center of BPBD DKI Jakarta Province by paying attention to aspects contained in the Effectiveness of the Jakarta Siaga Emergency Service Program 112. The research method was carried out with a case study method with data collection techniques using interview methods and document review. Interviews were conducted on 10 (ten) key informants, document review focused on documents related to the Jakarta Emergency Alert Service 112 Effectiveness research in Fire Management. The results showed that the Effectiveness of Jakarta Siaga 112 Emergency Services in Fire Management by UPT. The Center for Disaster Data & Information BPBD DKI Jakarta Province Its effectiveness is still low, due to the Implementation of Emergency Services Jakarta Standby 112 in Fire Management implemented by UPT. Disaster Data & Information Center of BPBD DKI Jakarta Province in terms of the Target Group Understanding of the Program, the Achievement of the Program Objectives aspects, and the Program Follow-up aspects. It is recommended to continue to disseminate this Emergency Service to the public, it is necessary to increase the firm commitment of the Head of 8 SKPD related to fire management so that all units play a role in accordance with the Standard Operating Procedures (SOPs) for Fire Management and the evaluation and follow-up of program services that are held periodically 3 once a month.Keywords: Effectiveness, Emergency Services, Fire Handling

Paranasal mucoceles are a type of cysts that evolve slowly and are asymptomatic; this poses a difficulty in diagnosing the patient because the symptoms can go unnoticed. The mucocele evolves unpredictably. On the one hand, it can become infected turning into pyoceles and on the other hand, it can invade important regions such as the orbital, cranial or genian regions, creating facial asymmetry. This is a retrospective case study of 37 patients diagnosed with sinus mucoceles, followed up by clinical examination and paraclinical tests such as CT and MRI scans. The biochemical components of the liquid from within the mucocele were analyzed and the following criteria were recorded: NaCl-, Cl-, Na+ and cholesterine as well as cellular components such as mastocytes, macrophages, hematocytes and leucocytes. In all cases, the treatment option was surgery with favorable post-operative and follow-up evaluation. The mucoceles that appeared post-operatively (maxillary and ethmoid sinuses) evolved more rapidly than the mucoceles that were induced byan external injury. Longer follow-up of operated patients permitted a more timely diagnosis of recurrences.
