Aim of study. Determination of the diagnostic significance of laboratory biomarkers of renal tissue damage in remote nephrolithotripsy in patients with urolithiasis.Material and methods. On the basis of the urology department of the Republican Clinical Hospital (Ufa), 35 patients with urolithiasis were examined, who underwent remote shock wave lithotripsy sessions. The laboratory parameters were determined in patients: the number of erythrocytes, leukocytes in the blood, the level of lipocalin and microalbumin in the urine, as well as alpha2-microglobulin and cystatin C in the blood serum. The control group included 14 healthy donors. To determine the diagnostic efficacy of biomarkers of renal injury, characteristic curves were plotted, and lipocalin level shifts were interpreted taking into account the data on the critical difference criterion value.Results. When studying the urinary level of lipocalin in patients with urolithiasis, it was found that the difference in the concentration of the biomarker in them and in healthy individuals is statistically insignificant (0.68 pg/ml versus 0.4 pg/ml). After the first session of extracorporeal lithotripsy, an increase in urinary excretion of lipocalin by 5 times is noted, after the second - by an additional 1.6 times, and after the third - by another 1.7 times (the differences are statistically significant). To analyze the prognostic efficiency of markers of renal injury, characteristic curves were plotted. The area under the ROC curve for lipocalin varied from 0.77 to 0.80 depending on the number of sessions, which indicates a high diagnostic efficiency of this biomarker. The determination of the criterion of critical difference (CCD) showed that an increase in the level of lipocalin in the urine after the first session of lithotripsy more than 2.1 times is statistically significant. The concentration of the specified biomarker in urine exceeding 4.5 pg/ml, 6 pg/ml and 10 pg/ml after the first, second and third sessions of lithotripsy, respectively, can be considered as a basis for changing treatment tactics (delaying the second procedure) or performing lithotripsy in another way.Conclusion. Urinary lipocalin, associated with neutrophil gelatinase, is an informative biomarker of renal injury in the assessment of complications associated with the lithotripsy procedure.

AbstractIn this study we examined the serum activity of lactate dehydrogenase (LDH) and its isoenzyme patterns in 28 calves of a lowland black spotted breed and its crossbreeds at the age of 2-6 months suffering from clinically noticeable manifested respiratory diseases - bronchopneumonia (BRD Group). As a control group we used 35 clinically healthy calves of the same age, breed and nutrition (Healthy Group). The sick calves did not show clinical signs or pathological lesions on other organ systems. The results found in sick calves showed a significantly higher total activity of LDH than in clinically healthy animals (P<0.01). The mean activity of LDH was 2012 U/l in healthy calves and in calves with respiratory diseases 2529 U/l. The differences in all LDH isoenzyme patterns between both groups of animals were significant (P<0.001) and in calves with respiratory diseases are characterized by a marked increase of the LDH 1 fraction and a decrease in the proportion of the other four LDH isoenzymes. Our results differ from those observed and presented in respiratory diseases in human medicine or in sheep. The explanation for the obtained results in calves and the determination of their diagnostic significance needs further studies and investigations using more animals with various severity of clinical signs and pathological changes, including analysis and determination of lactate dehydrogenase isoenzyme patterns in healthy and affected cattle lung tissue.

Summary. Introduction. The aim of the work is the determination of functioning of the single kidney (SK) by complex scintigraphic research with using nephrotrophic radiopharmaceuticals (NRP) of different mechanisms of elimination and the detection of its diagnostic significance. Materials and methods. 90 children (range 5-16 years) with SK were examined: dynamic (DRSG) and static (SRSG) scintigraphy with angiography was made them. 150 scintigraphic examinations with different NRP were made: 99mTc-DTPA (diethyleneaminopentacetate) – 51, 99mTc – PP (pyrophosphate) – 40,99mTc – DMSA (dymethylenesuccinateacetate) – 34, 99mTc MAG3 (mer- captoacetyle-3-glycine) – 15, 99mTc-EC (ethylenedicysteine) – 10. Time options of renal blood flow and transport of NRP, SGF, ERP were determined. Control group consists of15 children. Results.The main scintigraphic features ofSK are: presence of single kidney on scintigraphic image; slowdown of time options of renal haemodynamic, increasing of SGF and ERP of SK on 50-60 %; decreasing of standard parameters SGF and ERP on 40-50%. The slowdown of SK excretion ability is associated with retention NRP in parenchyma. Proportions of SK increase on 2-3 sm average, and size of scintigraphic picture – on 40%. The main type of renographic curve in children with SK is parenchymatose, and it is found in 50% ofpatients. The accumulation and distribution of NRP in SK in 73,3 % is quite uniform. Conclisions. Thus, SK in children functions in strain way of all compensation mechanisms that provide blood purification from products of vital functions, which shows the slowdown of main options of the SKfunctioning.

Abstract Background and Aims The definition of subclinical renal damage markers in patients with metabolic syndrome (MS) and evaluation the relations between the indicators of subclinical renal injury and endothelial dysfunction. Method The study included 62 patients with MS, 32 (51.6%) men and 30 (48.4%) women aged 51.2±5.4 years. The control group included 25 practically healthy individuals, 13 (52.0%) men and 12 (48.0%) women, aged 47.3±6.1 years. All patients were underwent clinical and laboratory examination, calculation of glomerular filtration rate (GFR) using the formula CKD-EPI, ultrasound of the carotid arteries, determination of the insulin resistance index (IR) (HOMA-IR), as well as determination of the content of endothelin-1 (ET-1) in serum and albumin excretion in a single portion of urine by IFA. Results When calculating GFR, hyperfiltration was detected in 10 (16.1%), optimal filtration - in 23 (37.1%), slightly reduced - in 29 (46.8%). At the same time, in patients with MS, a statistically significant excess of urinary albumin excretion (UAE) in a single dose was revealed compared with the control group (26.6±5.8 mcg/ml and 9.2±3.4 mcg/ml, respectively; p &lt;0.001). Based on the increase in level (ET-1) in patients of the main group, compared with the control (6.3±3.2 fmol/ml and 3.4±1.0 fmol/ml, respectively; p=0.03) may indicate the development in patients of the main group of endothelial dysfunction. A correlation analysis revealed a direct correlation between the level of UAE and ET-1 (R = 0.45; p = 0.005). A direct correlation was found between UAE and IR-HOMA (R = 0.68; p &lt;0.007), interventricular septum thickness (R = 0.71; p &lt;0.001), and intima–media thickness (IMT) (R = 0.49; p &lt;0.008) and systolic blood pressure (R = 0.47; p = 0.026). Similar associations were noted also for the ET-1 level: IR-HOMA2 (R = 0.78; p &lt;0.001), with the thickness of the interventricular septum (R = 0.76; p = 0.001), IMT (R = 0.46; p &lt;0.033) and systolic blood pressure (R = 0.45; p = 0.035). Conclusion In patients with metabolic syndrome, in addition to the well-known manifestations, in the early stages, may be a violation of renal function and endothelium, manifested by the change of glomerular filtration rate, increased levels of UAE and increased concentration of ET-1 in the blood. Direct correlation of the level of UAE and ET-1 level, and the coincidence of the correlation relationships of these indicators with a number of clinical and laboratory indicators of the metabolic syndrome will make it possible to consider an increasing of the level of UAE as one of the markers of endothelial dysfunction.

In this brief report, the authors demonstrated the importance of harmonious relations between the defined indicators in the human body. The purpose of this work was to define the significance of Wurfs in clinical practice in patients with different pathologies of the internal organs. The ratio of minoproteinic and cholesterol complexes, cholesterol, pigment and mixed stones with a special formula has been studied. The authors carried out a comparison of the results with the &#34;gold&#34; Wurf. The control group included 103 people, the patients with chronic active hepatitis – 43, with chronic persistent hepatitis – 51, with cirrhosis of the liver – 7, with micro spherocytes hemolytic anemia – 48, with alcoholic liver disease – 25. The authors found that there is no diagnostic significance of determination of Wurf for stones.

ABSTRACT The fractional determination of 17-ketosteroids in the daily urine was performed in nine cases of hyperemesis gravidarum and in four control cases, in the first trimester of pregnancy both before and after corticotrophin administration. The excretion of total 17-KS is similar in the two groups. Only in the hyperemesis group does the excretion of total 17-KS increase significantly after corticotrophin administration. The fractional determination reveals no difference between the two groups of patients with regard to the values of the fractions U (unidentified 17-KS), A (androsterone) and Rest (11-oxygenated 17-KS). The excretion of dehydroepiandrosterone is significantly higher in the hyperemesis group than in the control group. The excretion of androstanolone seems to be lower in the hyperemesis group than in the control group, but the difference is not statistically significant. The differences in the correlation between dehydroepiandrosterone and androstanolone in the two groups is significant. The high excretion of dehydroepiandrosterone and low excretion of androstanolone in cases of hyperemesis gravidarum is a sign of adrenal dysfunction.

Aim. To study the influence of deproteinized dialysate from blood of milk-fed calves on the type of exudative discharge and dynamics of inflammatory reaction after hernia repair with plastics of the anterior abdominal wall with hernioendoprosthesis of polypropylene. Materials and Methods. The study involved 59 patients being on stationary treatment in the surgical department of Kursk Regional Clinical Hospital. Patients were hospitalized for the herniation of small or medium dimension. The patients were divided to two groups: the main (n=30) and control (n=29) groups. After endoprosthetics, the patients of the control group were given complex conservative treatment. The patients of the main group, besides standard treatment, were administered deproteinized dialysate from blood of milk-fed calves intravenously in drips 10 ml + 200 ml of 0.9% sodium chloride solution within 7 days. For cytological examination and determination of the type of cytograms, the traumatic discharge was collected and analyzed using the method of M.F. Kamaev and M.A. Palthsev. Results. Cytomorphometric examination was conducted on the third, fifth and seventh day after endoprosthetics with the aim of studying dynamic changes. Determination of cell composition and also of its changes characteristic of each studied period, was necessary for obtaining further information characterizing inflammatory process in the region of placement of the endoprosthesis. After endoprosthetics in patients who were administered hemodialysate, the inflammatory reaction was less pronounced than in patients who did not receive the preparation. This was associated with a more dynamic change of stages of the inflammatory process. In patients of the main group who received deproteinized dialysate, regenerative type of inflammation first appeared on the fifth day and made 6.9%; by the seventh day the share of patients with the regenerative type rose to 17.5%, while in the patients of the control group no regeneration stage was observed in both periods. Conclusion. Analysis of the efficiency of influence of hemodialysate on the inflammatory reaction in plastics of the anterior abdominal wall with polypropylene endoprosthesis evidences faster course of all stages of inflammation and reduction of its intensity by 10%.

Background: This study aimed to investigate the expression levels of microRNA (miRNA)-125b in serum exosomes and its diagnostic efficacy for asthma severity. Methods: The study included 80 patients with untreated asthma and 80 healthy volunteers. The patients were divided into 4 groups according to disease severity: 20 with the intermittent state, 20 with the mildly persistent state, 20 with the moderately persistent state, and 20 with the severely persistent state. The expression levels of miRNA-125b in serum exosomes of each group were detected using a quantitative polymerase chain reaction and compared. The Spearman correlation analysis was used to study the correlation between the expression levels of miRNA-125b in serum exosomes and asthma severity. The diagnostic efficacy of the expression levels of miRNA-125b in exosomes for asthma severity was evaluated using the Receiver Operating Characteristic (ROC) curve. Results: The expression levels of miRNA-125b in serum exosomes of patients with intermittent, mildly persistent, moderately persistent, and severely persistent asthma were all higher than those in the healthy control group, with statistically significant differences. The expression levels of miRNA-125b were also statistically significantly different among patients in each group. The Spearman correlation analysis showed a positive correlation of the relative expression of miRNA-125b in serum exosomes with asthma severity. The area under the ROC curve of the diagnostic efficacy of miRNA-125b in serum exosomes for patients with intermittent, mildly, moderately, and severely persistent asthma was 0.7770, 0.8573, 0.9111, and 0.9995, respectively. Conclusion: The expression levels of miRNA-125b in serum exosomes had a high diagnostic efficacy and might serve as a noninvasive diagnostic marker for asthma severity.

Abstract This study describes changes in haematological parameters, cytokine profile, histopathology and cortisol levels in Swiss mice experimentally infected with Angiostrongylus costaricensis. Twenty-eight Swiss mice were divided into two groups (G1 and G2) of 14 animals each. In each group, eight animals were infected orally with ten third-stage larvae of A. costaricensis and six were used as a control group. The mice of groups G1 and G2 were sacrificed 14 and 24 days after infection, respectively. Samples were collected for histopathological and haematological analyses and determination of the cytokine profile and cortisol levels. Granulomatous reaction, eosinophilic infiltrate and vasculitis in the intestinal tract, pancreas, liver and spleen were observed with varying intensity in infected animals. Our results showed that the mice developed normocytic and hypochromic anaemia, and that the histopathological lesions caused by the experimental infection influenced increases in cortisol, neutrophil and monocyte levels. In addition to this, we detected increased interleukin-6 and tumour necrosis factor alpha levels in the infected animals.
