Usage of the Medical Product Ketanov in the Maxillofacial Surgery

Pain is the psychophysiological condition of a person arising in as a result of the action of ultra-strong or destructive irritants causing organic or functional disturbances in the organism. The purpose of our research was to evaluate analgesic efficiency of the medicinal product ketanov (ketorolac tromethamine), produced by the firm “Ranbaxy” in patients with diseases of the maxillofacial region. We monitored 127 cases, which we divided into the following groups: Group I of 44 patients after performing dental surgery (extraction of tumors and tumor-like formations of jaws and soft tissues, plastic and reconstructive operations); Group II – 23 patients with mandibular fractures; Group III – 27 patients with inflammatory diseases of the soft tissues; Group IV – 19 patients with rhythmic diseases of the maxillofacial region (odontogenic neuralgia, post-traumatic and post-operative neuralgo-neuritis); Group V – 14 patients who have undergone the surgical phase of dental implantation. Ketanov (ketorolac tromethamine) is a highly effective analgesic and is recommended for use in the post-operative period after removal of tumors and tumor-like formations of the jaws and soft tissues of the face and neck, after plastic and reconstructive operations, in case of jaw fractures, purulent inflammatory processes, odontogenic neuralgia, postoperative and post-traumatic neuralgo-neuritis of peripheral branches of the trigeminal nerve, as well as after the surgical stage of dental implantation.

Introduction: Mandible is one of the most commonly fractured bones inmaxillofacial trauma. Mandibular symphysis is a common site of mandibular fractures. Compareto conventional miniplates, three dimentional plates were found more effective in terms of postoperative occlusion, post operative mobility of reduced fractured segments and the degree ofanatomic reduction. Objectives: Compare the conventional 2.0mm mini plates versus 3D miniplates in the fixation of isolated mandibular symphysis fractures. Study Design: Interventionalstudy. Setting: Oral & Maxillofacial Surgery Department, Abbasi Shaheed Hospital Karachi.Period: Two years from 1st January 2013 to 2nd January 2015. Material & Methods: Onehundred and fifty four patients were randomly selected and allocated into two groups. Group‘I’in which patients were treated with conventional mini plates and group‘II’ where patients weretreated with 3D mini plates. Results: Mean (+SD) age of patients was 29.53 (±7.88) years.Among 154 patients in this study, majority (75.3%) of cases had age 20 - 35 years. Majority,124 (81.2)were males and 29 (18.8%) were females. In Group I, anatomic reduction at 3 monthpost-operative was observed in 30 (39%). In Group II (3D Plates) at 3 months post-operatively,ideal molar relationship was higher (89.6% Vs. 98.7%; p-value = 0.016), fracture mobility waslower (14.3% Vs. 2.6%; p-value = 0.009) and higher anatomic reduction. Conclusion: Threedimensional miniplates are more effective compared to conventional miniplates.

Medical rehabilitation of patients with defects, deformities of the jaws is an urgent problem in reconstructive maxillofacial surgery. Such defects cause significant dysfunctions of the organs of the maxillofacial region, disfigurement of the soft tissues of the lower face zone. A complex problem of maxillofacial surgery is the development of methods for surgical restoration of the integrity of organs and their function. Currently, various surgical methods and materials are used to restore the defect in the jaw bone tissue [12].

Introduction: Surgical removal of 3rd molars causes significant swelling and trismus which are unpleasant and uncomfortable for the patient even when teeth are removed by gentle surgical technique. Our surgical procedures can lead to serious inflammatory reactions in the facial soft tissues and should be minimized as much as possible. The aim of this study was to compare the effect of a single intramuscular (I/M) dose of two different corticosteroids on postoperative facial soft tissue swelling and trismus.Methodology: This interventional study was conducted in oral and maxillofacial surgery department of BSMMU. 60 patients (33 male and 27 female)were included in this study and were randomly divided into two groups. In Group-I, 30 patients were included and 4mg of Dexamethasone was injected into the Deltoid region 30 minutes prior to extraction of mandibular 3rd molars. In Group-II,another 30 patients were included and 40mg of Methylprednisolone was injected into theDeltoid region 30 minutes prior to extraction of mandibular 3rd molars.Facial swelling was evaluated preoperatively, on 2nd POD and 7th POD by using measuring tape. Trismus was determined by measuring the maximum inter-incisal distance pre-operatively, on 2nd POD and 7th POD by vernier caliper.Result:Result showed that swelling and trismus was significantly reduced in both groups, p>0.05(not significant).Conclusion:The result concluded that the preoperative single dose of I/M administration of Dexamethasone or Methylprednisolone significantly reduces postoperative swelling and trismus after surgical removal of mandibular 3rd molars.J Shaheed Suhrawardy Med Coll, December 2017, Vol.9(2); 60-64

The purpose of this article is to present the case of maxillofacial trauma patient with maxillary sinus fracture diagnosed with cone-beam computed tomography (CBCT) and to explore the applications of this technique in evaluating the maxillofacial region. A 23-year-old male patient attempted to our clinic who had an injury at midface with complaints of swelling, numbness. The patient was examined before in emergency center but any diagnosis was made about the maxillofacial trauma. The patient re-examined clinically and radiographically. A fracture on the frontal wall of maxillary sinus is determined with the aid of CBCT. The patient consulted with the department of maxillofacial surgery and it is decided that any surgical treatment was not necessary. The emerging technique CBCT would not be the primary choice of imaging maxillofacial trauma. Nevertheless, when advantages considered this imaging procedure could be the modality of choice according to the case.

Introduction: In recent years, procedures with the use of dental implants have increased worldwide, reaching approximately one million dental implants per year. In recent years, a platelet concentrate called FRP (fibrin-rich plasma) has been the subject of clinical studies. Associated with this, the biomaterial Bio-Oss® (Geistlich), as it is biodegradable, biocompatible, non-toxic, and has low immunogenicity, and bio stimulators can act in the regeneration of bone tissue, as it establishes with the cells the appropriate biological niche (favorable microenvironment) for bone growth. Objective: Therefore, the present study aimed to evaluate, through a brief systematic review, the results that involve bone formation for dental implantation, with the use of biomaterials such as fibrin-rich plasma and Bio-Oss®. Methods: The model used for the review was PRISMA. Was used databases such as Scopus, Scielo, Lilacs, Google Scholar, PubMed. Results: Fibrin-rich plasma (FRP) as an autologous biomaterial for use in oral and maxillofacial surgery presents most leukocytes, platelets, and growth factors, forming a fibrin matrix, with three-dimensional architecture. The Bio-Oss® biomaterial (Geistlich), as it is biodegradable, biocompatible, non-toxic, and has low immunogenicity and bio stimulators can act in the regeneration of bone tissue, since it establishes with the adenomatous mesenchymal stem cells the appropriate biological niche for bone growth and, thus, allowing the dental implant to be as effective as possible. Conclusion: The use of FRP associated with Bio-Oss® seems to illustrate high success rates with minimal costs, which may reduce the amount of bone graft needed to fill the sinus cavity, reducing the costs of the procedure.

Relevance. It is currently relevant to study and compare the effectiveness of the autologous connective tissue grafts and the combination of collagen-based and autologous platelet-rich plasma in the surgical treatment of Miller Class I gingival recessions.Materials and methods. We examined and treated 48 (20 male (41.67%) and 28 female (58.33%)) patients aged from 25 to 40 years with Miller Class I gingival recessions. All gingival recessions were treated surgically using a modified twolayer tunnel technique. The patients were divided into two groups according to the graft type. Group I (24 patients (50%) had a connective tissue graft from the hard palate. Group II (24 patients (50%) used the combination of the autologous platelet-rich plasma and 3D collagen matrix Fibromatrix for the regeneration of oral soft tissues. We removed the sutures on the 14th day. The patients were followed up on the 7th and 14th days and in 1.3 months.Results. 48 Miller Class I gingival recessions were treated between 2018 and 2020. The depth of gingival recessions averaged 3.5 ± 1.13 mm before treatment. The level of the attached keratinized gingiva regarding the cementoenamel junction significantly (p < 0.001) improved in both groups after the surgery. The width and thickness of the keratinized gingiva best increased in group II. The mean effectiveness of gingival recession treatment was 84% in study group I and 96% – in study group II. Pain syndrome, fibrinous plaque and soft tissue edema were insignificant in group II.Conclusion. The combination of the autologous platelet-rich plasma and Fibromatrix, collagen 3D matrix, for the regeneration of the oral soft tissues is a more effective technique for the treatment of Miller Class I gingival recessions. This technique has several advantages. It is minimally invasive, less painful, soft tissue postoperative swelling is less and the received volume of the attached keratinized gums is larger than with a connective tissue graft.

Aim: to explore the mandibular bone fractures in elderly patients with reference to etiology of trauma. Methodology: This was a descriptive study conducted in the department of Oral and Maxillofacial Surgery KEMU/ Mayo Hospital Lahore. Elderly patients (age 60-100years) with mandibular bone fractures. Results: Aetiology of trauma leading to mandibular fractures was as follows; there were 79(65.83%) cases of RTA, 32(26.66%) falls, 4(3.33%) assaults and there were only 3(2.5%) cases of industrial injury. Conclusion: To conclude, this study depicts that road traffic accidents were the predominant cause of injury in the studied age group. The issues of financing in a medical organization are one of the most relevant areas of healthcare in the Russian Federation. Providing departments with the necessary financial resources to provide effective and accessible medical care to the population is important for both the administration and doctors and nurses. In some regions of the Volga Federal District, the departments are combined (Penza, Saransk), that is, all types of medical care are provided to both adults and children in one place, which leads to a number of problems associated with the irrational use of medicines. One of these is the department of Maxillofacial Surgery of the N.N. Burdenko National Clinical Hospital, on the basis of which the disadvantages of using drugs for parenteral use are considered. The purpose of this work is an economic analysis of the effectiveness of the use of solutions for parenteral use in children on the basis of an adult medical organization. The materials of the study were data from patients ' medical histories, prescribing lists, invoices from the pharmacy organization, and data on the cost of medicines. The results of the work are an analysis of the existing problems of combined type departments related to drug provision, a number of medicines for parenteral use that are not available to order, but are included in the standards of treatment of children with injuries of the maxillofacial region. The ABC - analysis of expenses under the item "medicines" is calculated, the data of the VEN – analysis is presented, which includes vital, necessary and secondary medicines for the treatment of children with injuries of the maxillofacial region. An algorithm for the selection of effective and rational therapy has been developed, using the example of antibacterial agents for parenteral administration.

Due to the high biotolerance, favourable mechanical properties, and osseointegration ability, titanium is the basic biomaterial used in maxillofacial surgery. The passive layer of titanium dioxide on the surface of the implant effectively provides anticorrosive properties, but it can be damaged, resulting in the release of titanium ions to the surrounding tissues. The aim of our work was to evaluate the influence of Ti6Al4V titanium alloy on redox balance and oxidative damage in the periosteum surrounding the titanium miniplates and screws as well as in plasma and erythrocytes of patients with mandibular fractures. The study included 31 previously implanted patients (aged 21–29) treated for mandibular fractures and 31 healthy controls. We have demonstrated increased activity/concentration of antioxidants both in the mandibular periosteum and plasma/erythrocytes of patients with titanium mandibular fixations. However, increased concentrations of the products of oxidative protein and lipid modifications were only observed in the periosteum of the study group patients. The correlation between the products of oxidative modification of the mandible and antioxidants in plasma/erythrocytes suggests a relationship between the increase of oxidative damage at the implantation site and central redox disorders in patients with titanium miniplates and screws.
