The Relationship Between Activity Energy Expenditure, Cardiorespiratory Fitness, Body Composition and Risk Factors in Young, Lean Men and Women

Abstract Background Physical inactivity due to cachexia and muscle wasting is well recognized as a sign of poor prognosis in chronic obstructive pulmonary disease (COPD). However, there have been no reports on the relationship between trunk muscle measurements and energy expenditure parameters, such as the total energy expenditure (TEE) and physical activity level (PAL), in COPD. In this study, we investigated the associations of computed tomography (CT)-derived muscle area and density measurements with clinical parameters, including TEE and PAL, in patients with or at risk for COPD, and examined whether these muscle measurements serve as an indicator of TEE and PAL. Methods The study population consisted of 36 male patients with (n = 28, stage 1–4) and at risk for (n = 8) COPD aged over 50 years. TEE was measured by the doubly labeled water method, and PAL was calculated as the TEE/basal metabolic rate estimated by the indirect method. The cross-sectional areas and densities of the pectoralis muscles, rectus abdominis muscles, and erector spinae muscles were measured. We evaluated the relationship between these muscle measurements and clinical outcomes, including body composition, lung function, muscle strength, TEE, and PAL. Results All the muscle areas were significantly associated with TEE, severity of emphysema, and body composition indices such as body mass index, fat-free mass, and trunk muscle mass. All trunk muscle densities were correlated with PAL. The product of the rectus abdominis muscle area and density showed the highest association with TEE (r = 0.732) and PAL (r = 0.578). Several trunk muscle measurements showed significant correlations with maximal inspiratory and expiratory pressures, indicating their roles in respiration. Conclusions CT-derived measurements for trunk muscles are helpful in evaluating physical status and function in patients with or at risk for COPD. Particularly, trunk muscle evaluation may be a useful marker reflecting TEE and PAL.

CrossFit (CF; CrossFit Inc., Washington, DC, USA) is a form of high-intensity functional training that focuses on training across the entire spectrum of physical fitness. CF has been shown to improve a number of indicators of health but little information assessing energy balance exists. The purpose of the present study was to investigate energy balance during 1 week of CF training. Men and women (n = 21; mean ± SD; age, 43.5 ± 8.4 years; body mass index, 27.8 ± 4.9 kg·m−2), with ≥3 months CF experience, had body composition assessed via air displacement plethysmography before and after 1 week of CF training. Participants wore ActiHeart monitors to assess total energy expenditure (TEE), activity energy expenditure, and CF energy expenditure (CF EE). Energy intake was assessed from TEE and Δ body composition. CF EE averaged 605 ± 219 kcal per 72 ± 10 min session. Weekly CF EE was 2723 ± 986 kcal. Participants were in an energy deficit (TEE: 3674 ± 855 kcal·day−1; energy intake: 3167 ± 1401 kcal·day−1). Results of the present study indicate that CF training can account for a significant portion of daily activity energy expenditure. The weekly expenditure is within levels shown to induce clinically meaningful weight loss in overweight/obese populations.

Abstract Background: Studies indicate that cardiorespiratory fitness, muscular strength and body composition are associated with mental health. However, research is scarce concerning this relationship in young adolescents in general and non-existent in Norwegian populations specifically. The aim of the study was to examine the relationship between different health-related aspects of physical fitness and self-reported mental health in Norwegian adolescents. Methods: Participants from four regions of Norway (n = 1486; mean age = 13.9; girls = 50.6%) completed the Strengths and Difficulties Questionnaire, and were measured for cardiorespiratory fitness, muscular strength and body composition. Linear mixed effects models were conducted to assess the associations between the health-related aspects of physical fitness and the total difficulties score of the questionnaire. Results: Body composition was not associated with total difficulties score. Muscular strength independently was associated with total difficulties score, but when all independent variables were entered in the fully adjusted model, only cardiorespiratory fitness was associated with total difficulties score. School clusters were included as random effects and all models were controlled for sex, socioeconomic status and birthplace (domestic or foreign). Conclusions: A small but significant negative association between cardiorespiratory fitness and total difficulties score indicated that higher cardiorespiratory fitness predicted better mental health in Norwegian adolescents. The results suggest that muscular strength is not associated with mental health in adolescents, when controlling for cardiorespiratory fitness. Future research on how exercise affects mental health should investigate whether there is a greater effect for participants who improve their cardiorespiratory fitness.
