Abstrak.Penyakit tuberkulosis masih tetap merupakan masalah kesehatan yang utama di dunia. Penyakit ini menyebabkan angka kesakitan yang tinggi pada jutaan penduduk dunia setiap tahunnya. Mycobacterium tuberculosis (M.Tb) memiliki kemampuan aktifitas replikasi dan metabolisme yang tinggi dan memiliki kemampuan untuk bertahan hidup di dalam fagosit profesional. Respon imun seluler sangat berperan penting untuk proses eliminasi M. Tb. Sel TH1 yang teraktivasi akan mengeluarkan interferon gamma yang akan mengaktivasi makrofag. Interferon gamma memiliki peran yang sangat penting dalam imunitas protektif terhadap infeksi M.Tb.(JKS 2017; 2: 131-137)Kata kunci: Tuberkulosis, Mycobacterium tuberculosis, respon T­H1,Interferon gammaAbstract. Cellular immune responses play an important role in the elimination process of M. Tb. The activated TH1 cell will secrete interferon gamma that leads to activation of macrophages. Abstract Romania is recognized as the European country with the highest tuberculosis rate in Europe. The association of tuberculosis with silicosis determines synergistic immunosuppression of the alveolar macrophages resulting in a higher grade of pulmonary parenchymal destruction and consequently respiratory failure. The case report approaches a patient with third stage of silicosis and associated active secondary pulmonary tuberculosis disease with positive smears. The impact of the Mycobacterium tuberculosis (MTB) infection’s activation is known to be severe, worsening the prognosis of silicosis and reducing the patient’s quality of life. Regarding the high morbidity rate of tuberculosis, an early diagnosis of tuberculosis in patients with silicosis is paramount, and sometimes cannot be achieved by usual bacteriological tests. Therefore, a better strategy is to be considered in silicotuberculosis, namely to prevent the progression of the latent tuberculosis foci by testing the positive predictive value of up-to-date tests such as IFN-γ inducible protein 10 biomarker, which may allow early detection and treatment.

Pulmonary tuberculosis is a major health problem in Bangladesh that is responsible for about 7% of total death in a year. This study was conducted to isolate and identify Mycobacterium tuberculosis from sputum and to evaluate the efficacy of PCR as a modern diagnostic tool, for diagnosis of pulmonary tuberculosis, especially in the smear negative cases. One hundred and fifty suspected pulmonary TB patients (male/ female: 97/53) were included in this study. Single morning sputum was collected from each patient and diagnostic potential of PCR was compared with staining and culture. Twenty five (16.7%) sputum were positive by ZN stained smear. Among 125 smear negative samples, 13 (10.4%) yielded growth in culture in LJ media and 21 (16.8%) samples were positive by PCR. The sensitivity and specificity of PCR in smear negative cases was 100% and 92.9% respectively. Mean detection time in PCR was 24 hours. PCR detected M. tuberculosis in 21 smear negative and 9 culture negative samples. For diagnosis of tuberculosis in smear negative cases, PCR directly from sputum was a very sensitive and accurate method. INTRODUCTION: Influenza A H1N1 2009 is associated with a high morbidity rate among children around the world, including Brazil. This survey was conducted on samples of symptomatic children (< 12 years) to investigate the influenza virus as the etiological agent of respiratory infections in a day care school in a health facility during the first and second pandemic wave of H1N1 (2009-2010) in São Paulo, Brazil. METHODS: Influenza infections were determined by real-time PCR in 34% (47/137) of children with a median age of 5 years (8 months - 12 years), from June to October 2009 and in 16% (14/85) of those with median age of 6 years (1-12 years), from March to November 2010. RESULTS: In general, most positive cases (64%) occurred in children aged 5-12 years, this age group was significantly the most affected (39.8%, p = 0.001, OR = 8.3, CI 95% 1.9-36.9). Wheezing was reported by 31% (19/61) and dyspnea by 23% (14/61) of the studied patients. An outbreak of influenza H1N1 with an attack rate of 35.7% among children (median age 6 years) was documented in April 2010, before the vaccination campaign against the pandemic virus was extended for children up to 5 years in Brazil. CONCLUSIONS: Therefore, the study reinforces the recommendation to immunize school children to reduce the incidence of the disease.

ABSTRACT  Diarrhea is a major health problem in Indonesia with high morbidity and mortality rates. Unhealthy environment and unhygienic behavior are closely related to diarrhea disease. This study aims to identify the association between environmental conditions and behavior with the occurrence of diarrhea in Jawa and Bali. Data source used was the integration data of March’s Susenas and Riskesdas 2018. A logistic regression analysis was chosen to elicit the relationship between sanitation and drinking water facilities, open defecation practice, and other factors with the prevalence of diarrhea. The results showed that open defecation (OR = 1.2; 95% CI: 1.12 to 1.29; P<0.001, and improper of household sanitation and drinking water (OR = 1.11; 95% CI: 1.04 to 1.2; P = 0.003) had significant association with diarrhea. Other factors are age 0-5 years (OR = 1.56; 95% CI: 1.46 to 1.66; P <0,001) and low education (OR = 1.33; 95% CI: 1.21 to 1.47; P <0,001). This study shows the hygienic behavior is the essential factors to reduce the incidence of diarrhea. Therefore, it is recommended to to prioritize aspects of behavior and environmental health, namely changing people’s behavior to defecate in a latrine, and provide proper sanitation and sufficient drinking water. Lingkungan yang tidak sehat dan perilaku tidak higienis sangat erat kaitannya dengan penyakit diare. Penelitian ini bertujuan untuk menganalisis hubungan antara kondisi lingkungan dan perilaku dengan penyakit diare di pulau Jawa dan Bali. Sumber data yang digunakan adalah data integrasi Susenas Maret dan Riskesdas 2018. Analisis dilakukan dengan menggunakan regresi logistik untuk melihat hubungan antara ketersediaan sanitasi dan air minum, perilaku buang air besar serta faktor lainnya terhadap kejadian diare. Hasil menunjukkan bahwa faktor perilaku buang air besar sembarangan (OR = 1,2; 95% CI: 1,12 - 1,29; P <0,001) serta akses sanitasi dan air minum rumah tangga tidak layak/tidak tersedia (OR = 1,11; 95% CI: 1,04 - 1,2; P = 0,003) memiliki hubungan dengan kejadian diare. Faktor lainnya yaitu: usia 0-5 tahun (OR = 1,56; 95% CI: 1,46 - 1,66; P <0,001) dan pendidikan rendah (OR = 1,33; 95% CI: 1,21 - 1,47; P <0,001).  Penelitian ini menunjukkan bahwa perilaku higienis sangat penting untuk menurunkan kejadian diare. Oleh karena itu direkomendasikan untuk mengutamakan aspek perilaku dan kesehatan lingkungan, yaitu merubah perilaku masyarakat untuk BAB di jamban, serta menyediakan sanitasi dan air minum yang layak. Background: Voluntary abortion for social reasons is illegal in Nigeria; however, the practice remains mostly clandestine and unsafe with varying consequences and determinants yet to be studied in all settings.Methods: A descriptive cross-sectional study design was used to assess the prevalence, practice and determinants of termination of pregnancy amongst 119 female Secondary School students in South-South Nigeria.Results: The prevalence of abortion was 57.1%. Most of the students were above 18years (58.8%), Christian (95.8%) and of rural residence (66.4%). While 58.8% had experienced an unwanted pregnancy, 61.4% had used some form of contraceptive with condom being the commonest (39.5%). Most (89.1%) have heard of abortions while 67.6% and 16.2% have had abortions once and twice respectively with the top reasons for abortion being that they were still in school (33.8%), too young (25.9%) and to avoid shame or stigma associated with the pregnancy (11.7%). Dilation and curettage was the predominant method employed (40.2%) mainly by medical doctors (34.1% and pharmacists (35.6%) while 51 (75%) had post-abortal complaints such as pain (41.2%) and bleeding (21.6%). There was a significant association between having an abortion and place of residence (rural more than urban), (p=0.04), being pregnant more than once (p<0.001), mothers` level of education (p=0.03), fathers` level of education (p=0.02) and mothers occupation (p=0.04).Conclusions: The prevalence of abortion is high and complicated by high morbidity rate despite a higher contraceptive prevalence rate whose major determinants were the socio-demographic characteristics of the parents. There is a need for early sex education from parents as this can influence abortion perception and practice in later years.

The global incidence of multiple sclerosis (MS) appears to be increasing. Although it may not be associated with a high mortality rate, this disease has a high morbidity rate which affects the quality of life of patients and reduces their ability to do their activities of daily living. Thankfully, the development of novel disease modifying therapies continues to increase. Monoclonal antibodies (MABs) have become a mainstay of MS treatment and they are likely to continue to be developed for the treatment of this disease. Specifically, MABs have proven to be some of the most efficacious treatments at reducing relapses and the inflammation in MS patients, including the first treatment for primary progressive MS and are being explored as reparative/remyelinating agents as well. These relatively new treatments will be reviewed here to help evaluate their efficacy, adverse events, immunogenicity, and benefit-risk ratios in the treatment of the diverse spectrum of MS. The focus will be on MABs that are currently approved or may be approved in the near future.

AbstractIn Japan, tuberculosis has been recognized as one of the major infections requiring urgent measures because of its high morbidity rate even now especially in elderly people suffering from tuberculosis during the past epidemic and its reactivation. Hence, many Japanese clinicians have made efforts to suppress the onset of tuberculosis and treat it effectively. The objectives of this study are to (1) identify covariate(s) that may explain the variation of rifampicin, which is the key antitubercular agent, under the steady-state by evaluating its population pharmacokinetics and (2) to propose an appropriate dosing method of rifampicin to Japanese patients. For this purpose, serum concentration–time data were obtained from 138 patients receiving rifampicin (300–450 mg) and isoniazid (300–400 mg) every day over 14 days, and analyzed using nonlinear mixed effects model. Thereby, population pharmacokinetic parameters were estimated followed by elucidating relations between the parameters and statistical factors. The analysis adopted one-compartment model including Lag-time by assuming that the absorption process is 0+1st order. The analyses demonstrate that meal affected the bioavailability, primary absorption rate constant, and zero order absorption time in the constructed model. A body weight calculated from the power model was selected as the covariate by the Stepwise Covariate Model method and found to highly affect the clearance in the range from −31.6% to 47.4%. We conclude that the dose in Japanese tuberculous patients can be well estimated by the power model formula and should be taken into consideration when rifampicin is administered.

Tuberculosis, a major health problem in developing countries, has reemerged in recent years in many industrialized countries. The increased susceptibility of immunocompromised individuals to tuberculosis, and many experimental studies indicate that T cell-mediated immunity plays an important role in resistance. The lymphokine interferon gamma (IFN-gamma) is thought to be a principal mediator of macrophage activation and resistance to intracellular pathogens. Mice have been developed which fail to produce IFN-gamma (gko), because of a targeted disruption of the gene for IFN-gamma. Upon infection with Mycobacterium tuberculosis, although they develop granulomas, gko mice fail to produce reactive nitrogen intermediates and are unable to restrict the growth of the bacilli. In contrast to control mice, gko mice exhibit heightened tissue necrosis and succumb to a rapid and fatal course of tuberculosis that could be delayed, but not prevented, by treatment with exogenous recombinant IFN-gamma.
