Possibilities and problems of mycotic keratites therapy in the hospital

Fungi are most common cause of severe inflammation of the cornea, resulting in blindness and loss of vision [6]. According to various researchers, fungi are the cause of keratitis in 40-50% of all microbial keratitis cases [1, 3]. Treatment of keratomycosis in the Russian Federation is carried out by methods other than intended. Lack of skills and availability of diagnostics, official etiotropic therapy for use in ophthalmology for ophthalmologists, unequal conditions in the fight against ophthalmomycosis. This is partly due to the lowered level of «myco-alertness», which leads to long-term ineffective «healing» of antibacterial drops, the duration of the patient’s suffering and results in the loss of the eye functionally and — in some cases — anatomically [7]. Global eye removal counts revealed that this method was used in 8-11%, which represents an annual loss of 84,143-115,697 eyes [6]. Key words: keratomycosis, mycotic keratitis, treatment, therapy.

The presented study develops theoretical and methodological foundations for assessing the fiscal sustainability of the constituent entities of the Russian Federation in perspective.Aim. The study aims to develop a systems approach to assessing the fiscal sustainability of the constituent entities of the Russian Federation in the medium and long term.Tasks. The authors analyze the major approaches to assessing the fiscal sustainability of federal subjects and determine the significance of quantitative and qualitative assessment methods in the development of a methodology for assessing the fiscal sustainability of federal subjects in the medium and long term.Methods. This study uses scientific methods of cognition, analysis and synthesis, comparison and analogy, systems and institutional approaches to assess the fiscal sustainability of federal subjects.Results. The authors examine the major approaches to assessing the fiscal sustainability of federal subjects developed by Russian scientific schools and disciplines; approaches used by state and local authorities; approaches to assessing the fiscal sustainability of federal subjects used by international and national rating agencies; foreign experience. In general, this implies the development of a universal system of indicators for assessing the fiscal sustainability of federal subjects.Conclusions. It is substantiated that under the current conditions of new challenges, particularly in the context of the coronavirus pandemic, it is necessary to assess the long-term balance and sustainability of the budgets of federal subjects using a systems approach based on quantitative and qualitative methods, making allowance for the medium- and long-term prospects to make efficient management decisions at different levels of the economic system.

Purpose. To evaluate the clinical and functional results of the technique of transcleral fixation of the artificial iridochrustalic diaphragm during its decentration in a patient with aniridia and aphakia. Material and methods. Under observation was a 32-year-old patient K. with a diagnosis on the right eye: iridochrustalic diaphragm decentralization, posttraumatic aniridia, posttraumatic aphakia, who underwent a method of transcleral fixation of a displaced artificial iridochrustalic diaphragm developed at the Academician S. N. Fedorov Eye Microsurgery of the Moscow Ministry of Health of the Russian Federation. Results. Patient K. is subjectively satisfied with the visual functions obtained in the right eye. Conclusion. Thus, this clinical case demonstrates a very successful implementation of the fixation of the iridochrustalic diaphragm with its displacement to obtain satisfactory visual functions in the patient. "The transition of Russia to sustainable economic growth is impossible without stimulating the use of the achievements of science and education, high technologies, promoting innovative activity as an essential premise for the development of social production and subjects of economic relations. For the state, the promotion of innovation is a priority in the economic policy sector, since it is the level of scientific and technological development that largely determines the long-term strategic advantages of the country, and the prospects for the development of the state largely dependent on the level of innovation development. For a long time, the Ministry of Economic Development of the Russian Federation has been implementing the Strategy for Innovative Development of the Russian Federation. It includes 20 key steps and is developed based on the provisions of the concept of long-term development of the Russian Federation. The main results should have been, firstly, an increase in spending on science and innovation, and secondly, an increase in the share of industrial enterprises that introduce innovations in production. It was assumed that their share will increase by 4-5 times by 2020, compared with 2010 year. Not everything has been achieved, and there is a lot to strive for. This study is devoted to the driver of scientific and technological progress - the chemical complex of Russia. The chemical complex plays a significant role in the implementation of the most important socio-economic programs of the country. The study is devoted to the consideration of the most important prospects and key alternatives for the development of the chemical industry related to different areas of innovation. Conclusions will be made on key technologies and products that can change the position of the chemical industry in the global market."

The paper is devoted to the examination of issues related to the increasing importance of budgetary regulation for the proper functioning of a modern innovative society. The key role of the budgetary regulation in the financial process of the State is particularly acute in the context of systemic crises that include socio-economic consequences caused by the spread of a new coronavirus infection (COVID-19) in Russia. In the course of the study, the features of changes in the state financial policy caused by the above-mentioned crisis phenomena are highlighted. The paper describes various approaches to the interpretation of the budgetary regulation as a category of financial law, explores various types and legal forms of methods of the budgetary regulation, analyses mechanisms and the impact of the State on the budget system through the existing legal structure of the budgetary regulation. It is determined that the rules of financial law governing the whole complex of public relations concerning the distribution and redistribution of the national product between the levels of the budget system of the Russian Federation constitute the institution of financial law, namely: the budgetary regulation. The author concludes that the approach based on the concentration of basic powers in the financial field at the federal level significantly slows down the dynamics of development of economic activity in the majority of regions of Russia, while the need for breakthrough innovative development of Russian society determinates the expansion of long-term tax sources of income for regional budgets. In this regard, it is proposed to consolidate additional regulation for revenues gained by regional and local budgets in the form of targeted deductions from federal taxes on a long-term basis.

Long-term socio-economic development forecast of the Russian Federation for the period till 2030 specifies that the main barriers in the implementation of innovative and accelerated scenarios are caused by the shortage of world-competitive professional personnel both at the level of corporations and public administration, the inefficiency of coordination mechanisms. This study is aimed at studying the readiness and possibilities of the system of vocational education and training of the Russian Federation to respond to external challenges, taking into account scenarios of the long-term forecast of the social and economic development of the Russian Federation for the period up to 2035. The study is also aimed at creating scientific, organizational and methodological conditions for the positive modernization of the professional education and training system of the Russian Federation on the basis of regional development strategies, which will allow implementing measures to develop infrastructure, personnel potential, create modern conditions for the implementation of professional educational programs. The study presents predictive scenarios of the development of secondary vocational education, gives their characteristics, and identifies risks and advantages. The presented scenarios allow us to determine the socio-economic potential of vocational education, to determine the goals and objectives in its development.

Knowledge and skills concentrated in human capital are increasingly important factors of economic development. However, there is a lack of a methodology for determining, which skills are necessary for the efficient industrial development. To this end, we examine skill requirements of regional employers potentially leading to an increase in economic indicators. Skills in demand were compared with predicted indicators based on a se mantic content analysis of vacancy databases in various regions of the Russian Federation. It was revealed that the list of demanded competencies depends not on a geographical aspect but on a specific profession. An analysis of the obtained data demonstrated that the growth in demand for highly qualified employees in the Russian Federation is correlated with an increase in gross value added of relevant industries. A linear correlation between gross value added per employee and the need for skilled specialists was demonstrated on the example of the transport sector. The proposed methodology can be used by educational organisations for targeted training of specialists, as well as by employers and experts for forecasting medium- and long-term socio-economic development of Russian regions.

Staphylococcal meningitis (SM) ranks fourth in the etiological structure of purulent bacterial meningitis in the Russian Federation. The most common cause of staphylococcal meningitis is Staphylococcus aureus, which accounts for 68 % of all cases. This meningitis is characterized by a higher mortality rate (30 %) than staphylococcal meningitis of other types of Staphylococcus (13 %) and mainly affects people over 25 years of age. With increasing age, the probability of death increases, and among people over 65 years of age, the mortality rate reaches 44 %.

This article presents the aspects of fetoscopic laser coagulation of anastomoses (FLCA) for treatment of twin-to-twin transfusion syndrome in the Russian Federation and worldwide. Own experience is correlated with the world one. There is a significant need for domestic medicine to increase the number of clinics that provide this type of intervention. Nevertheless, there is no obvious lag in comparing the results of FLCA in the Russian Federation. The article is devoted to the search for a criterion for grouping the regions of the Russian Federation, which allows to obtain adequate estimates of the correlation relationship between labor productivity and production, investment, information and innovation factors of economic development. Estimates based on the aggregate of data for all regions do not reflect a reliable relationship between the main and factor indicators. Conducting analytical procedures separately for regions with a share of the extractive industry in GRP of less than and more than 10% improved the correlation indicators. The conclusions are related to the development of programs for the long-term development of regions.
