Features of performing anterior capsulorexis for the prevention of capsule block after cataract phacoemulsification with implantation of posterior chamber IOL

Purpose. Тo develop a method for the formation of anterior capsulorexis for the prevention of capsule block in cataract phacoemulsification with the implantation of a posterior chamber IOL. Material and methods. 135 people were studied, who were divided into two groups depending on the diameter of the performed anterior capsulorexis (CR). Group I (main) included 67 patients (67 eyes), who underwent CR according to the method proposed by the authors-in the form of an ellipse with a large axis of 7,0-7,5 mm along the tunnel incision and a small axis of 4.0-5.0 mm. Group 2 -68 patients (68 eyes), capsulorexis was produced in a round shape- 5-5.5 mm. Results. Complications during surgery – in patients of group 2, a capsule block and a tear in the anterior capsulorhexis were noted in 2.9% of cases, in 3% of cases posterior capsular fibrosis was diagnosed in each group, which required the performance of posterior capsulorhexis. In the late period in one patient of group 2 endothelial-epithelial dystrophy (EED) of the cornea developed, which required ultraviolet corneal crosslinking followed by keratoplasty. Conclusion. The proposed method for the creation of anterior capsulorexis in cataract phacoemulsification with the implantation of a posterior chamber IOL reliably eliminates the possibility of a capsule block due to the formation an oval shaped capsule hole. In addition, it reduces the risk of damage to the lens capsules, ensures reliable fixation of IOLs in the capsule bag and increases access for posterior capsulorhexis, if necessary. Purpose. To optimize the technique for the femtosecond laser-assisted anterior capsulorhexis creation in anterior capsular fibrosis. Material and methods. All patients were divided into two groups: group I included 12 patients with central and peripheral anterior capsule fibrosis who underwent anterior circular capsulorhexis creation using femtosecond laser LensX (Alcon, USA) with laser energy 10 and 15 μJ. Group II included 11 patients with central and peripheral anterior capsule fibrosis who underwent manual capsulorhexis. Results. In group II the operative complications occurred in 3 cases (27.3%), group I was uneventful. Conclusion. Femtosecond laser-assisted anterior capsulorhexis creation in patients with anterior capsule fibrosis allowed reducing the number of operative complications by 27% in comparison with manual capsulorhexis creation. It is advisable to use 10 μJ power to create the anterior capsulorhexis in the area of unaffected and slightly fibrotic anterior capsule cutting the fibrotic part of the anterior capsule by collet scissors. <strong>Aim</strong>: To compare different technique of nucleus delivery in small manual incision cataract surgery,regarding their safety and intra operative complication. <strong>Material &amp; Methods</strong>: A total of 285 patients with cataract undergoing SICS with posterior chamber IOL implantation were selected. Patients were randomly divided into 7 groups of nucleus delivery. All of the patients were followed up on first postoperative day and discharge. Patients were advised regular follow up at 1<sup>st</sup> week, 4<sup>th</sup> week and 6<sup>th</sup> week. <strong>Results</strong>: Out of total 285 patients 68 (23.86%) patients had intraoprative complications for all the techniques. There was no intraoperative difficulty to the surgeon for delivery of nucleus by various techniques in 210(73.68%) of cases. Group 1 (wire vectis) had minimum intraoperative complications whereas group 2 had maximum.31 (10.88%)had grade 1 (minimal) difficulty intraoperetively. 25 (8.77%)cases had grade 2 (moderate) difficulty. 19 (6.67) cases had grade 3(abandoned) difficulty. <strong>Conclusion</strong>: By adjusting parameters like size of cornean incision, hydrodissection or debulking of nucleus before delivery of nucleus in different grades of hardness of cataract, a surgeon may choose any one technique in which he or she is comfortable. Manual small incision Cataract surgery with its low complication rate has now come to be established surgical procedures for cataract surgery.

Background: Low back pain by osteoarthristis is one of the most common diseases in the world as well as in Vietnam, estimated 70-85% people in the world have low back pain sometime in their lives. Obiectives: To evaluate the effects of embedding therapy and electronic acupuncture combined with “Doc hoat tang ky sinh” remedy in the treatment of low back pain by spondylosis. Materials and methods: 72 patients diagnosed of low back pain by spondylosis, were examined and treated at Phu Yen Traditional Medicine Hospital, divided equally into 2 groups (group 1 and group 2). Results: In group 1: Effective treatment at good and fair good level accounted for 41.7% and 41.7%. In group 2: Good level occupied 33.3% and fair good level occupied 55.6%. Conclusion: The ratios of good and fair good in 2 groups were equal (p >0.05) Key words: Low back pain, spondylosis, embedding therapy, electronic acupuncture

Аннотация. В статье анализируется качество переводов рассказов В.Шукшина, осуществленных в середине 70-х годов С. Наматбаевым. Переводоведческий анализ вывил многочисленные стилевые расхождения между оригиналом и переводом. При переводе одного из лучших рассказов Шукшина «Чудик» искажения стиля не передали психологические тонкости рассказа. При переводе рассказа «Дядя Ермолай» философский аспект рассказа выпал, так как переводчик исказил жанровую специфику. Перевод рассказа «Два письма» оказался более качественным по сравнению с предыдущими. Переводчик рассказа «Два письма» сумел приблизиться к оригиналу, расшифровать его основную тональность, воспроизвести атмосферу напряженных поисков истины героем рассказа Николаем Иванычем. Этот перевод оказался более качественно выполненным по сравнению с предыдущим рассказом «Дядя Ермолай». Причину переводческого успеха можно объяснить, прежде всего, доступностью содержания рассказа, да и сам герой не столь сложная и глубокая личность, как повествователь в рассказе о дяде Ермолае. В целом, переводы С.Наматбаева являются своеобраз- ным этапом в переводческом деле. В настоящее время необходимо более полное и глубокое восприятие творчества В.Шукшина и создания новых адекватных переводов. Ключевые слова: качество переводов, стилевые расхождения, психологические тонкости, философский аспект, этапом. Аннотация. The quality of translation of the Shukshins stories is analyzed in the Middle 70- years. By S.Namatbaev. The translation analysis had idenh hed numerous style discrepancies between the original and the translation. The psychological feature of the compasihion were not be hansfered by the translation one the bestand famous story by. V.Shukshin due to distorhion of the style. By the translation of the story “The uncle Ermolai” had full a phisichal aspect of the story, because the translator had distorted the genre specifics. The translation of the stories “Two letters” was wade more qualitative comparated with another stories. Generally, the Namatbaevs translation are a sui generis stage in the translatiobs. We need currently more total and profound perception of the creation by V.Shukshin. The create of the new adeavate translation is the main task. The head of the theory and history of Russian literature department. V. Shukshin’s stories and problems of translation them into Kyrgyz language. This article is analysed the quality of translation of V. Shukshin’s stories, which were written in 70 th with S. Namatbaev. Translation analyse deduced many stylistic variations between translated version and original. There is a misrepresentation, was not transmitted psychological subtleties of the story in translation of one of the best V. Shuk- shin’s story “Wierdo”. Metaphysical dimension is disappeared in translation of the story “Uncle Ermolai”, since translator destort the genre specificity. The translation of the story “Two letters” has been more qualitative in comparison with previous translations. In common, S. Namatbaev’s translation is a genius stage in translation. It needs more full and deep comprehension of S. Shukshin’s work and to establish new suitable translation. The decellularized bovine pericardium and its potential use as a natural scaffold is a promising approach in the field of tissue engineering and regenerative medicine. The reaction of the host toward decellularized scaffolds depends on their biocompatibility, which should be satisfied being before applied in clinical use. Purpose: to evaluate the biocompatibility of the extracellular matrices, which were decellularized by trypsin enzyme and anionic sodium dodecyl sulfate (SDS) detergent. Material and methods. Pericardial sacs were acquired from 12-18 months’ age bulls. Tissue decellularization was performed by using 0.25 % Trypsin solution and 1 % ionic SDS for group I and 0.1 % SDS for group II samples. The implantation was performed on Wistar rats. The tissue samples were stained with hematoxylin & eosin, Congo red and Masson's Trichrome for histological analysis. Results. In group 1 in 3 months after subcutaneous implantation in rats we noticed the inflammation in surrounding tissue and degradation of the implant. Under the same conditions in animals of group 2 implant replacement with growing immature connective tissue was noted. Bio-implant of this group did not degrade, moreover it's integrated to the tissues of experimental rats. Conclusion. Our results showed that decellularized bovine pericardium by 0.1 % SDS can become an alternative material for tissue engineering and has the potential for further use in human surgery.

Objective: To investigate the clinical outcomes of three surgical approaches for ectopia lentis in Marfan syndrome (MS) patients who had undergone crystalline lens removal with posterior chamber intraocular lens (IOL) implantation techniques comprising the intrascleral fixation of IOL, sutured scleral fixation of IOL, and IOL implantation with the use of a Cionni capsular tension ring (CTR). Methods: This is a retrospective comparative study, including 35 eyes of 21 patients who underwent the intrascleral fixation of IOL (group 1), scleral IOL fixation with the Z-suture (group 2), and IOL implantation with the use of a Cionni CTR (group 3) following crystalline lens removal. The surgical indications were as follows: no improvement in visual function after eyeglasses or contact lens application due to excessive irregular astigmatism and advanced crystalline lens decentration in which the edge of the crystalline lens came up to the optical axis, or dislocation of the crystalline lens resulting in aphakia and secondary glaucoma due to lens dislocation. The surgical outcomes and complications due to surgery were compared between the groups. Results: The mean age of the patients in the study was 12.3 ± 8.7 years (5–32 years). There were 10 eyes in group 1, 13 eyes in group 2, and 12 eyes in group 3. Visual acuity improved significantly in each group after surgery. Ocular residual astigmatism did not differ significantly between the groups ( p = 0.51). Conclusion: There were no significant differences between the three surgical approaches in the current study in terms of the postoperative results and complications.

Implantable collamer lenses (ICL) have gained popularity for correction of myopia where kerato-refractive procedures are not indicated as in cases of high myopic refractive errors. Toxic anterior segment syndrome (TASS) is a very uncommonly reported postoperative complication following ICL implantation. A young patient developed severe corneal oedema and anterior segment inflammation on the first day after ICL implantation. Analysing retrospectively, possible idiosyncratic response to intracameral pilocarpine was considered as a cause for TASS. Prompt and intensive therapy with oral and topical potent steroids was visually rewarding. TASS, though a sterile inflammation can have catastrophic sequelae such as corneal decompensation and secondary glaucoma. Hence, timely identification and management is important.
