AbstractOver the past 50 years, ketamine has solidified its position in both human and veterinary medicine as an important anesthetic with many uses. More recently, ketamine has been studied and used for several new indications, ranging from chronic pain to drug addiction and post-traumatic stress disorder. The discovery of the rapid-acting antidepressant effects of ketamine has resulted in a surge of interest towards understanding the precise mechanisms driving its effects. Indeed, ketamine may have had the largest impact for advancements in the research and treatment of psychiatric disorders in the past few decades. While intense research efforts have been aimed towards uncovering the molecular targets underlying ketamine’s effects in treating depression, the underlying neurobiological mechanisms remain elusive. These efforts are made more difficult by ketamine’s complex dose-dependent effects on molecular mechanisms, multiple pharmacologically active metabolites, and a mechanism of action associated with the facilitation of synaptic plasticity. This review aims to provide a brief overview of the different uses of ketamine, with an emphasis on examining ketamine’s rapid antidepressant effects spanning molecular, cellular, and network levels. Another focus of the review is to offer a perspective on studies related to the different doses of ketamine used in antidepressant research. Finally, the review discusses some of the latest hypotheses concerning ketamine’s action.

The review conducted research of the Russian and foreign literature of recent years on the methods of using photo and video recording in dentistry in the databases of the Russian Science Citation Index, SCOPUS, Web of Science, Pubmed and others. The data of the possibilities of using the method of registration of the dental status and the state of the tissues of the oral cavity using photo and video fixation in the diagnosis, treatment and observation of patients are presented.

Abstract Background and Aims Clinical studies in recent years have revealed a close relationship between hormonal disorders in women with CKD and the duration and quality of life, bone mineral and related disorders of the cardiovascular system. In individual studies, there is a tendency to improve the indicators of mineral and bone metabolism and the state of the cardiovascular system in hormonal or other drug-induced correction of hormonal dysfunctions in women. - Aims to study the effect of estrogen deficiency on bone and mineral metabolism in a population of women suffering from CKD stages III-V Method The study included 52 women who met the clinical criteria for the possible appointment of hormone therapy (both replacement and combined oral contraceptives) for the purpose of a detailed examination of the state of their cardiovascular system and bone-mineral metabolism in dynamics (with an interval of 10-12 months) in order to assess the degree of influence of estrogen-deficient conditions on the course of such common complications of CKD as cardiovascular diseases and pathology of the bone system. The age of the patients ranged from 26 to 61 years (mean age-50.65±9.17 years). The duration of CPN averaged 77.02 months.. The stages of CKD were determined according to the K/DOQI (2012) criteria, and the glomerular filtration rate was calculated using the CKD-EPI formula. The following parameters were evaluated: the concentration of sclerostin, osteoprotegerin, fibroblast growth factor 23 (FGF-23), parathyroid hormone, total calcium, phosphorus, alkaline phosphatase, creatinine, and urea. Follicle-stimulating hormone( FSH), luteinizing hormone(LH) and estradiol were determined by solid-phase chemiluminescent enzyme immunoassay (commercial sets of Alkor-Bio, St. Petersburg). Serum concentrations of sclerostin, sRANKL (soluble RANKL), and osteoprotegerin were determined by the enzyme-linked immunoassay using Biomedica gruppe test systems. Results The examined patients showed hormonal dysfunctions (82%), accompanied by changes in the content of sex hormones: the concentration of estradiol was below normal: 123.4±72.5 pmol/l and 150.0-450.0 pmol/l, respectively, in patients and in normal (p<0.01), which confirms the presence of estrogen deficiency in the examined patients . Concentrations of FSH and LH exceeded the norm in the group of patients as a whole: 91.6±46.1 IU/l and 3.0-8.0 IU/l FSH content in patients and normal; 51.8±32.1 IU/l and 3.0-10.0 IU/l LH in patients and normal. In the group of patients as a whole, an increase in the level of sclerostin to 28.5 ± 9.2 pmol/l was detected ( norm 12±33.45 pmol/l), an increase in the level of osteoprotegerin to 6.9±0.4 pmol/l (norm 2.7 pmol/l). Positive and negative correlations were found between the levels of morphogenetic proteins, sex hormones, and characteristic parameters of hormonal dysfunctions Conclusion Pre-and postmenopausal women with CKD have hormonal dysfunctions, including disorders of sexual and reproductive function, menstrual cycle, decreased fertility, increased risks of miscarriage at its onset, the basis of hormonal dysfunctions is the absence of LH peaks and changes in the concentration of estradiol depending on the phase of the cycle, hypoestrogenemia. It is assumed that there is a pathogenetic link between hormonal dysfunctions and disorders in the system of bone metabolism regulatory proteins in patients with CKD.

Subject. At the present stage of society development cancer incidence remains among the most pressing medical and social problems. In the structure of oncopathology a special place is occupied by malignant neoplasms of lymphoid and hematopoietic tissues which include chronic myeloid leukemia (CML). In some patients, the disease can appear with dental symptoms associated with a decrease in local immunity of the oral cavity — the appearance of aphthae, plaque on the oral mucosa, angular cheilosis, bleeding gums, dry mouth. It is diagnostically significant that such oral symptoms may precede general clinical symptoms, that is, they can be considered as early symptoms of CML. In addition, their severity often prompts a patient with CML to see a dentist. Thus, the importance of the professional knowledge of the dentist in general clinical and dental symptomatology of CML, the correct routing of the patient for further clinical and laboratory examination, treatment and observation by a hematologist is undeniably high. The goal is to study and identify the features of the dental status of patients with chronic myeloid leukemia. Methodology. The work carried out a comprehensive assessment of the state of the oral cavity and the need for dental treatment of patients with chronic myeloid leukemia in which patients with preserved systemic status and patients with chronic myeloid leukemia took part. Results. According to the results of a complex dental examination and the calculation of the main dental indices, patients with CML were found to have serious disorders in the state of their oral health. Conclusions. Patients with CML showed a high need for various types of dental care, primarily in conservative oral cavity sanitation, periodontal treatment and rationalization of oral hygiene protocols (professional and individual).

Abstract: Removable partial dentures can be an alternative treatment for tooth loss in the elderly. Users of removable partial dentures without good treatment can cause a variety of changes to the conditions of hard tissue and soft tissues of the oral cavity which is the first driveway for human digestive system. In addition, the use of removable partial dentures in the elderly are particularly susceptible to the occurrence of disorders of the oral cavity. The purpose of this study is to provide a picture of the oral cavity in elderly users of removable partial dentures in elderly nursing home in Minahasa. This is a descriptive cross sectional study. This study used a total sampling method, with respondents is a 30 elderly users of removable partial dentures in elderly nursing home in Minahasa. The results showed that the presence of ulcers as a result of artificial teeth in the upper jaw 66.67%, ulcers due to artificial teeth on the lower jaw 50%, 30% gingival redness, swelling of the gingiva 30%, 36.67% gingival bleeding, gingival recession at 46, 67%, 50% Denture stomatitis, angular celulitis 46.67%, Plaque 43.33%, 56.66% calculus, caries 93.33%. The research shows elderly users of removable partial dentures in elderly nursing home in Minahasa have a conditions change for hard and soft tissues of the oral cavity. Suggested the elderly to pay more attention to health and hygiene denture for more vigorous control the state of the denture and the state of the oral cavity in dentistry.Keywords: oral cavity condition, elderly, dental removable partial denturesAbstrak: Gigi tiruan sebagian lepasan dapat menjadi salah satu alternatif perawatan terhadap kehilangan gigi pada lansia. Pengguna gigi tiruan sebagian lepasan tanpa perawatan yang baik dapat menyebabkan berbagai perubahan terhadap kondisi jaringan keras dan jaringan lunak pada rongga mulut yang merupakan jalan masuk pertama sistem pencernaan manusia. Disamping itu, pemakaian gigi tiruan sebagian lepasan pada lansia sangat rentan terhadap kejadian kelainan – kelainan pada rongga mulut. Tujuan dari penelitian ini yaitu untuk memberikan gambaran keadaan rongga mulut pada lansia pemakai gigi tiruan sebagian lepasan di panti werda Kabupaten Minahasa. Penelitian ini merupakan penelitian deskriptif cross sectional. Penelitian ini menggunakan metode total sampling, dengan responden sebanyak 30 orang lansia pemakai gigi tiruan sebagian lepasan yang ada di panti werdha Kabupaten Minahasa. hasil penelitian menunjukan bahwa adanya Ulser akibat gigi tiruan pada rahang atas 66,67%, Ulser akibat gigi tiruan pada rahang bawah 50%, Kemerahan gingiva 30%, pembengkakan gingiva 30%, pendarahan pada gingiva 36,67%, Resesi pada gingiva 46,67%, Denture stomatitis 50%, angular celulitis 46,67%, Plak 43,33%, Kalkulus 56,66%, karies 93.33%, Kegoyangan 23,33%. Hasil penelitian menunjukan lansia pengguna gigi tiruan sebagian lepasan di panti werda Kabupaten Minahasa mengalami perubahan kondisi jaringan keras dan jaringan lunak pada rongga mulut. Disarankan para lansia untuk lebih memperhatikan kesehatan dan kebersihan gigi tiruan untuk lebih giat mengontrol keadaan gigi tiruan dan keadaan rongga mulut di dokter gigi.Kata Kunci: kondisi rongga mulut, lansia, gigi tiruan sebagian lepasan

Gastrointestinal parasites of wild birds in a tropical riverine urban ecosystem in Heredia, Costa Rica. In urban ecosystems, rivers provide various ecosystem services, among them, serving as interurban biological corridor, allowing birds to move and shelter in the middle of the city. However, high levels of pollution expose them, and even humans, to several health problems. Hence the importance of performing environmental diagnoses that allows to identify alterations and mitigate them in a timely manner. This study provides a first diagnosis of the parasitic situation of wild birds along the Pirro river which is located in an urban environment, fragmented and with high levels of surface water pollution. The sampling was carried out at the upper and middle part of the Pirro river in Heredia, Costa Rica. Mist-nets were used to capture the birds, and a general objective exam (GOE) was conducted in order to evaluate their nutritional and hydration state, as well as their physical state. In addition, stool samples were collected in order to characterize the gastrointestinal parasites (GPI). This procedure was carried out on 158 birds, belonging to 30 species, three PGI groups were identified: protozoa, nematodes and cestodes. The highest percentages of infection were represented by coccidia, with 97.6 %, followed by nematodes and cestodes with 2.4 %. Within the group of coccidios, it was possible to identify Eimeria spp., Isospora spp., in the nematodes: at the Strongylida group level and at the Capillaria spp., and for the cestodes: Choanotaenia spp. GOE resulted regular-to-good in all individuals captured, however, a general linear model was used to identify that the oral cavity and the state of the plumage could be the more linked variables with the percentage of infection by gastrointestinal parasites in birds, due to the fact that the oral cavity is the bird’s main route of ingestion, as well as the state of the plumage shows its vigor. In recent decades, both in the world and in our country, thyroid gland diseases are not only widespread, but also have a tendency to constant growth. Currently, more attention is paid to diseases of the thyroid gland that are associated with endemic foci of iodine defi ciency in soil and water.The aim of the study – to investigate the state of periodontal tissues in individuals with primary hypothyroidism.Materials and Methods. We conducted a survey of 86 people with primary hypothyroidism, 16 of them with manifestation and 70 with subclinical forms. A comparison group was 45 people without thyroid gland diseases. Index assessment of periodontal tissues included the defi nition of a simplifi ed oral hygiene index of the oral cavity of the Green-Vermillion (Oral Hygiene Index-Simlifi ed, Green-Vermillion, 1964), the degree of gingival infl ammation was determined by Parma (C. Parma, 1960, Papillary-Marginal Alveolar Index)), the state of periodontal tissues in the complex periodontal index (KPI) (1987), destructive changes in the bone tissue of the alveolar appendix were determined by the Fux index.Results and Discussion. The article presents the results of the index estimation of periodontal tissues in individuals with primary hypothyroidism. An analysis of the conducted index evaluation found that they had a poor level of hygienic state of the oral cavity. This was manifested by the development of infl ammatory and, to a lesser extent, infl ammatory and dystrophic lesions of periodontal tissues, as evidenced by papillarymarginalalveolar (PMA) and complex periodontal indices (KPI).Conclusions. Even more signifi cant changes in these indicators were found in individuals with a manifestation of hypothyroidism. So, we can conclude that in persons with primary hypothyroidism metabolic processes ofthe organism are violated, which leads through a number of pathophysiological changes to the activity of parodontopathogenic microfl ora and the development of infl ammatory and infl ammatory and dystrophic periodontal diseases.

This article describes changes in the state of the oral cavity in the pathology of the digestive system, shows the main clinical manifestations of these processes and their relationship. The information presented in the work emphasizes the advisability of including the results of a dental examination in a medical record, which will optimize the work of both pediatric dentists and pediatricians.
