Background: The increasing rates of Caesarean section (CS) beyond the WHO standards (10–15%) pose a significant global health concern. Objective: Systematic review and meta-analysis to identify an association between CS history and maternal adverse outcomes for the subsequent pregnancy and delivery among women classified in Robson classification (RC).Search Strategy: PubMed/Medline, EbscoHost, ProQuest, Embase, Web of Science, BIOSIS, MEDLINE, and Russian Science Citation Index databases were searched from 2008 to 2018.Selection Criteria: Based on Robson classification, studies reporting one or more of the 14 adverse maternal outcomes were considered eligible for this review.Data Collection: Study design data, interventions used, CS history, and adverse maternal outcomes were extracted.Main Results: From 4,084 studies, 28 (n = 1,524,695 women) met the inclusion criteria. RC group 5 showed the highest proportion among deliveries followed by RC10, RC7, and RC8 (67.71, 32.27, 0.02, and 0.001%). Among adverse maternal outcomes, hysterectomy had the highest association after preterm delivery OR = 3.39 (95% CI 1.56–7.36), followed by Severe Maternal Outcomes OR = 2.95 (95% CI 1.00–8.67). We identified over one and a half million pregnant women, of whom the majority were found to belong to RC group 5.Conclusions: Previous CS was observed to be associated with adverse maternal outcomes for the subsequent pregnancies. CS rates need to be monitored given the prospective risks which may occur for maternal and child health in subsequent births.

<p>Aorto-digestive fistulas are a rare but extremely dangerous cause of massive gastrointestinal bleeding with a high risk of death. The aim of the review was to examine the modern principles of aorto-digestive fistula diagnosis and optimal treatment modalities.</p><p>Scientific publications and their reference lists were searched on PubMed database, Google Scholar and Russian Science Citation Index. Articles relevant to the topic, published over the past 25 years (1996-2021), were included and they were searched and categorised using the following key words: ‘gastrointestinal bleeding’, ‘aorto-digestive fistulas’, ‘diagnosis’ and ‘treatment’. Inclusion criteria were limited to gastrointestinal bleeding associated with aorto-digestive fistulas.</p><p>Patients with aortic aneurysms or those who have undergone prosthetics should have increased alertness regarding the formation of aorto-digestive fistulas. With a presence of gastrointestinal bleeding and exclusion of other sources based on multispiral computed tomography data with intravenous bolus contrast enhancement, this will allow for quick verification of the diagnosis and also enable timely medical measures to be taken. In an emergency situation, to achieve rapid hemostasis in unstable patients, endovascular aortic replacement is most justified. Open reconstruction of the aorta in situ with simultaneous elimination of the hollow organ defect and sanitation of fistula-associated foci of infection should be considered as a radical intervention for aorto-digestive fistulas.</p><p>Received 19 May 2021. This study extended the author's previous research in the field of scientometrics of media researchers on the topic “Mass information. Journalism. Mass media” in Russian electronic library and Russian Science Citation Index. The methodology was a census of the personalities of the first hundred authors ranked by the level of the h-index in 2020 compared to 2017, and in some aspects — from 2016. The study analyzed the changes in the h-index over the years under study, changes in the authors' geography by federal districts and cities of the Russian Federation, their academic position, the distribution of doctors and candidates of sciences in scientific majors in accordance with the awarded degree, as well as the distribution of rating personalities by actual scientific interests and taught courses in their affiliated scientific and educational organizations. The study revealed the importance of scientometric indicators for authors and scientific and educational organizations to determine their place in the academic community, the relevance of topics and authors’ research in the general academic landscape, and material incentives. At the same time, it was noted that the system of scientometric indicators is changing: perhaps the h-index the number of published works and the number of citations in the RSCI will remain as an object of research by historians of science to determine the common place of Russian (and Soviet) scientists in the science development, and in addition or to replace them there will be new indicators. One of them is the "percentile" recently introduced into the list of scientometric indicators. Studies of the values of scientometric indicators will remain relevant in the future.

The review presents up-to-date information on the health effects of ambient fine particulate matter, obtained in large cohort epidemiological studies, as well as in meta-analysis of pooled data. In addition, it summarizes the current data on the potential pathological mechanisms and existing monitoring systems. The literature search used the Scopus, PubMed, Russian Science Citation Index databases for 19902020. The results of epidemiological studies carried out in different countries indicate that fine particles in ambient air pose a serious threat to health. Scientific publications assessing the health impact of particulate matter show a wide range of adverse effects from the increasing incidence of upper and lower respiratory tract diseases, including exacerbations of bronchial asthma, pneumonia, chronic obstructive pulmonary disease, to a high incidence of myocardial infarction, strokes, diabetes mellitus type 2, as well as an increase in overall mortality from natural causes, mainly mortality from respiratory diseases, cardiovascular and cerebrovascular diseases, lung cancer. The effects of short-term exposures are described in more detail, while the effects of long-term exposure to fine particles are not well understood. Potential mechanisms of the harmful effects of fine particulate matter include oxidative stress, inflammatory reactions, disorders of autonomic regulation and heart rhythm, fine particles translocation through the alveolar barrier into the vascular bed with endothelial damage and thrombus formation, and genotoxicity. Ambient fine particulate matter is a manageable risk factor, and reductions in air pollution will have a significant impact on public health outcomes.

Background and Aims: Recipient demographics affect outcomes after kidney transplantation. The aim of this study was to assess, for kidneys retrieved from living donors, the effect of recipient sex, ethnicity, and body mass index (BMI) on delayed graft function (DGF) and one-year graft function, incidence of acute rejection (AR), and recipient and graft survivals. Methods: A systematic review and meta-analysis was performed. EMBASE and MEDLINE databases were searched using algorithms through Ovid. Web of Science collection, BIOSIS, CABI, Korean Journal database, Russian Science Citation Index, and SciELO were searched through Web of Science. Cochrane database was also searched. Risk of bias was assessed using the NHBLI tools. Data analysis was performed using Revman 5.4. Mean difference (MD) and risk ratio (RR) were used in analysis. Results: A total of 5129 studies were identified; 24 studies met the inclusion criteria and were analysed. Female recipients were found to have a significantly lower serum creatinine 1-year-post renal transplantation (MD: −0.24 mg/dL 95%CI: −0.18 to −0.29 p < 0.01) compared to male recipients. No significant difference in survival between male and female recipients nor between Caucasians and Africans was observed (p = 0.08). However, Caucasian recipients had a higher 1-year graft survival compared to African recipients (95% CI 0.52−0.98) with also a lower incidence of DGF (RR = 0.63 p < 0.01) and AR (RR = 0.55 p < 0.01). Recipient obesity (BMI > 30) was found to have no effect on 1-year recipient (p = 0.28) and graft survival (p = 0.93) compared to non-obese recipients although non-obese recipients had a lower rate of DGF (RR = 0.65 p < 0.01) and AR (RR = 0.81 p < 0.01) compared to obese recipients. Conclusions: Gender mismatch between male recipients and female donors has negative impact on graft survival. African ethnicity and obesity do not to influence recipient and graft survival but negatively affect DGF and AR rates.

Over the past 30 years, there has been a revolution in psychotherapy under the influence of evidence-based methodology. The methods of evidence-based psychotherapy have gained preference all over the world. The rapidity of implementation of research results into practice is predetermined by the phenomenon of scientific influence. To determine this phenomenon, the author proposes to use the RSCI (Russian Science Citation Index) core percentile. The rating of the scientific influence of St. Petersburg psychotherapists was compiled for the first time ever. In total, the rating includes 31 specialists (with the 133 RSCI core percentile from 4 to 51) representing 12 organizations, ranked in accordance with the average RSCI core percentile as follows: 1) Scientific Medical Research Center for Psychiatry and Neurology named after V. M. Bekhterev — 16.1; 2) St. Petersburg School of Psychotherapy and Psychology of attitudes — 17.7; 3) St. Petersburg State Pediatric Medical University — 31.0; 4) North-Western State Medical University named after I. I. Mechnikov — 39.5. It is important to note that the rating includes non-governmental organizations, which, unlike state-run ones, do not have access to direct state funding for scientific research. Methods of psychotherapy are presented in the rating as follows: psychoanalysis — 8.7; positive dynamic psychotherapy — 17.7; cognitive-behavioral psychotherapy — 31.3, and personality-oriented (person-centered) psychotherapy — 31.6. The results obtained can be taken into account by the heads of commercial structures, health care and educational institutions when drawing up long-term plans, as well as by doctors and psychologists when choosing educational programs for professional retraining in the field of psychotherapy.

An relevance of the topic was defined by the high occurrence, unfavorable prognosis, lack of diagnostic techniques for early stages of acute kidney injury (AKI) disclosed in patients with COVID-19 (Coronavirus Disease 2019). Screening of medical literature for selection of AKI preclinical biomarkers was considered as main aim of this review. More than 200 publications from Russian Science Citation Index (RSCI), Scopus, The Cochrane Library, and MEDLINE were reviewed. Such risk factors as hypoxemia, increased intrathoracic pressure associated with Acute Respiratory Distress Syndrome (ARDS), hypertension (HT) involving endothelial dysfunction, and Diabetes mellitus were considered to be associated with AKI. There were explicated cytopathic and immune-mediated (cytokine-induced) mechanisms of COVID-19 associated AKI pathogenesis. Multiple methodological approaches were defined for detection and identification of the biomarkers based on urine proteome and metabolome screening. Perspective ways in the preclinical diagnostics of AKI such as detection of the markers of injury of the hypoxia-sensitive proximal canaliculi and the ATP metabolites that reflect first stages of the energy metabolism disorder in the epithelium lining canaliculi were identified in this study. The instantaneous and non-invasive investigation of different markers was regarded as possible method of the prognostication. The accuracy of the diagnosis on the initial stages of AKI, substantiate for preventive start of therapy, and make projections on the disease`s outcome will be improved due to the identification of high-sensitive specific biomarkers.
