TMJ MORPHOLOGICAL FEATURES OF DIFFERENT TYPES OF MALOCCLUSION CAUSING PATHOLOGICAL LOWER JAW POSITION ASYMMETRY

According to Russian researchers, the prevalence of temporomandibular joint dysfunction ranks third in the structure of dental morbidity among adults. In recent decades, the attention of clinicians to the problem of TMD has been growing rapidly due to the difficulties in the diagnosis and treatment of patients with malocclusion, complicated by joint pathology. The purpose of this article is to search for new knowledge that will improve quality of diagnosis of dysfunctional changes in the TMJ of patients with various malocclusion pathologies by identifying correlations between anatomical and morphological features of the joint and dentoalveolar anomalies. Understanding the trends in the development of characteristic morphofunctional disorders will make it easier for doctors to plan treatment for orthodontic patients and increase its effectiveness. Such patients require special attention from orthodontists. When writing this article, we have studied a lot of modern literature, as well as studies of foreign and Russian scientists who is involved in the treatment of dysfunction. However, we have not found the information that would show a direct relationship between anatomical and morphological features and malocclusion pathology, which was the purpose of our work. The article describes in detail the morphological and functional changes that occur at different stages TMD, the relationship between the shape of the glenoid fossa morphology process, included classification of the forms condyloid process described features of the development of bone structures of the TMJ depending on the type of pathological occlusion. Revealed statistics demonstrate the dependence of the frequency of dysfunction on the shape of the articular head and fossa. The analysis of computer tomograms of patients of the Department of Pediatric Dentistry and Orthodontics of the First Sechenov Moscow State Medical University allows us to conclude that the oval shape of the TMJ head and the rounded shape of the articular surface of the temporal bone can become a prerequisite for the development of dysfunctional changes. The results of our study will help orthodontists improve the quality of diagnostic measures and the effectiveness of treatment of patients with suspected TMD.

SummaryArthrographic and intraoperative evaluations of stifles affected with cranial cruciate disease were compared. Arthrography did not appear to be helpful in predicting cranial cruciate ligament pathology. The caudal cruciate ligament was consistently not visualized in the arthrograms and was normal at surgery. The menisci were visualized consistently in the arthrograms, but conclusions could not be made as to the benefit of arthrography in predicting meniscal pathology. Arthrography was not helpful in predicting joint capsule and femoral articular surface pathology. Survey radiographic evaluation was better than arthrography in evaluating joint pathology. When cruciate injury is suspected, after history and physical examination, survey radiographs are better than positive contrast arthrograms at supporting the diagnosis.Positive contrast arthrography was evaluated as a diagnostic aid in canine cranial cruciate ligament disease. It did not appear to be useful in predicting joint pathology. With arthrography, both menisci could be visualized and evaluated for abnormalities. Joint effusion and presence of osteophytes evaluated on survey radiographs was better than arthrography in evaluating joint pathology.

The morphological and histological features of the skin and wool cover of sheep as the basis for the quality of fur sheep pelts have been studied. The most important properties of sheep pelts (uniformity, thinness and density of wool) are provide the possibility of producing high-quality fur semi-finished products from them. However, the features of the histostructure of fine-wool sheep determine the low mechanical strength of the “facial” layer of skin. As a result, the “front” layer during processing often cracks to the upper border of the reticular layer or even peels off from the latter, making the sheep pelt unsuitable for use on fur products. These defects in fur practice are called “cracking” and “peeling” of the facial layer. They are mainly peculiar to sheep pelts of fine-wooled sheep. In these animals due to the high density and tone of the coat, the roots and hair follicles, root vaginas, secretory departments, excretory ducts of the glands and other structures occupy a significant share of the volume in the thickness of the Pilar layer (up to 25–30 %). The share of fibrous structures remains less volume, and these structures themselves are relatively weakly developed, located loosely and loosely intertwined with each other. The accumulations of fat cells that occur here also cannot be attributed to skin-strengthening elements. In fine-fleece sheep the pilar layer is on average 60 % of the thickness of the dermis. Therefore, more than half of its thickness is a weakened zone. The strength of the “front” layer is not the same in different fine-wool breeds of sheep and in different animals within the breed. For example, the average breaking load for cod of the “front” layer in Soviet Merino pelts is 1,25 kg, and in Precoce is 2,49 kg.

We compared outcomes in elderly patients with intra-articular distal radius fractures treated by closed reduction and plaster immobilization or open reduction and internal fixation with a volar plate. Ninety-seven patients older than 60 years were randomly allocated to conservative (47 patients) or surgical (50 patients) treatment. Over a 2-year period, we assessed patient-rated wrist evaluation score, DASH (disability arm, shoulder and hand) questionnaire, pain, wrist range of motion, grip strength, and radiological parameters. The functional outcomes and quality of life were significantly better after volar plating fixation compared with conservative treatment. We found that restoration of the articular surface, radial inclination, and ulnar variance affected the outcomes, but the articular step-off did not. Twenty-five per cent of the patients with conservative treatment had secondary loss of reduction. We conclude that surgical plating leads to better outcomes than conservative treatment for elderly patients with intra-articular distal radius fractures. Level of evidence: I

The review analyzes the capabilities of modern technologies of structural neuroimaging of the retina, standard perimetry, and studies of ocular blood flow in the early diagnosis and management of glaucoma. The relevance of the search for those structural and functional changes that are primary in the development of glaucomatous optical neuropathy (GON) and the diagnostic method that has the greatest clinical significance is discussed. Progress in understanding the pathogenesis of glaucoma and the expansion of scientific understanding of key risk factors for the development and progression of the disease, including genetic factors, can be crucially important to substantiate new strategies for preclinical diagnosis and the development of radically new approaches to personalized and preventive glaucoma therapy. However, the search for what arises most early with in primary open-angle glaucoma — changes in structure or function — will not have clinical relevance unless you take into account the capabilities of specific methods of structural and functional neuroimaging that represent information at various levels of organization of the visual system. The search for a single primary factor in the pathogenesis of GON can lead to an erroneous exaggeration of the close relationship between the variables being studied, which in reality either does not exist as a causal relationship or is significantly less than what is supposed — the phenomenon called “illusory correlation”. The reliable diagnosis of early changes that occur before the clinical manifestation of glaucoma is most likely to be based on a combination of structural, functional, and hemodynamic indicators, aimed not only to increase the sensitivity of diagnosis in detecting the earliest events in the development of GON, but rather to dramatically improve the understanding and quality of interpretation of those markers that we own.

Aim. To estimate dynamics of secretory and motor-evacuational functions of the stomach in patients with type 1 diabetes mellitus and gastrointestinalform of diabetic neuropathy. Materials and methods. 32 patients with DM1 without gastrointestinal pathology allocated to different groups depending on DM duration (gr. 1 lessthan 10 yr, gr. 2 over 10 yr). Vegetative equilibrium was estimated from the Kerdo index, rehabilitative potential from its basic constituent (morphophysiologicalindex). The motor-evacuational function of the stomach was studied with the use of a scintillation gamma-chamber, the gastric secretoryfunction by pH measurements. Results. Half of the patients in gr 2 presented with hypersympathicotony. The frequency of hypertonic form of gastric tone increased with durationof DM while the acid-producing and evacuational functions of the stomach decreased (as estimated by pH-measurement and gastroscintiographyrespectively). The propulsive function most significantly decreased in the pyloric part. The efficacy of rehabilitation of diabetic patients with gastrointestinalform of diabetic neuropathy was much lower than in those with preserved vegetative function of the stomach. Conclusion. Impairment of evacuational function of the stomach and duodenum with DM1 duration may be a cause of unstable blood glucose level.Hypomotor dyskinesia of the upper gastrointestinal tract due to DM1 and deficit of parasympathetic innervation occurs more frequently in patientswith low rehabilitative potential. Functional changes in the gastrointestinal tract of DM1 patients do not depend on the quality of compensation ofmetabolic disorders but correlate (r=-0.39) with DM duration. It is concluded that the gastrointestinal form of diabetic neuropathy impairs rehabilitativepotential of fhe patients.

Postmenopausal women and events like childbirth, and aging may cause structural and functional changes in women genitalia. The arising indications do not only cause psychological distress to women but negatively affect the sexual well-being and deteriorate the quality of their lives. Regenerative/ cosmetic gynecology procedures enable women to treat the functionality issues and modify the physical structure of vagina. This review discusses the latest developments in this field with regards to various kinds of procedures that are available, particularly the use of energy-based devices, and adipose tissue derived stem cells therapy for fat grafting which have revolutionized the regenerative gynecology procedures. These offer non-invasive modalities to treat the conditions like urinary incontinence among others which occur in high prevalence among women. Despite the advancements made in this field, it lacks regulatory guidelines and standardized procedures which imposes one of the biggest challenges of the field. Alongside, we have documented a procedure called Intimacell® which has been standardized for fat grafting procedures in vulvovaginal region.

In the experiment on the white Wistar rats, the authors evaluated kidneys changes in urolithiasis modelling (Kidney stone disease, KSD) and the possibility of correcting these changes by internal use of the mineral water (MW) with high organic matter content. The results of the studies revealed structural pathological changes in the kidneys in the form of changes of the part of glomeruli capillaries and gross changes in the kidneys tubules and interstitial layers. At the same time, the functional kidneys activity is manifested in the form of increased urination and a significant increase in urination. Urine metabolic characteristics are also change. The use of MW leads to an improvement in the capillaries of the renal corpuscles structure and almost complete restoration of the tubules structure. The functional activity of the kidneys and the quality of urine have significantly improving. The authors believe that the organic compounds of MW, which mainly determine its biological activity, contribute to the improvement of the regulation of metabolic processes and, accordingly, have a positive effect on the structural and functional parameters of the kidneys.. ABSTRACT Objectives To investigate adolescent orthodontic patient experiences and quality of life with fixed appliances compared to Invisalign. Materials and Methods Adolescent patients in active treatment with Invisalign or fixed appliances for a minimum of 6 months were provided with the Child Oral Health Impact Profile-Short Form 19 questionnaire, along with additional items of interest that were assessed separately. Pearson's χ2 test was used to compare responses (P < .05), and unpaired t-tests (P < .05) were used to test for differences in mean satisfaction, quality of life, and domain scores. Results In total, 74 patients (37 in each treatment group) participated. Overall, no significant differences were noted in the mean quality of life, satisfaction, or domain scores between the two groups. A significant difference was noted in the time taken to adjust to appliances, with the Invisalign group demonstrating faster adaptation. Additionally, the fixed appliance group was 3.8 times more likely to report missing school because of their appliance (95% confidence interval [CI]: 1.2, 12.5) and 2.7 times more likely to report having difficulty eating certain foods (95% CI: 1.1, 7.1). When the sample of females between the ages of 14 and 18 was analyzed, the Invisalign group reported feeling attractive more often than the fixed appliance group. Conclusions Both treatment groups were generally very satisfied with their treatment modality. The overall quality of life of adolescent orthodontic patients undergoing treatment with fixed appliances and Invisalign for a minimum of 6 months was similar.

<b><i>Introduction:</i></b> While eccentric (ECC) training appears to be more efficient than concentric (CON) training at improving body composition in adolescent with obesity, its impact on health-related quality of life (HRQOL) has never been studied. <b><i>Objective:</i></b> The aim of this study is to compare the effects of 2 cycling training modalities, i.e., ECC vs. CON, in adolescents with obesity on HRQOL and health perception (HP). <b><i>Methods:</i></b> A total of 24 adolescents with obesity, aged 12–16 years, were randomized to either a 12-week ECC or a CON cycling training program performed at the same oxygen consumption (VO<sub>2</sub>). Anthropometric measurements, body composition, maximal incremental tests, HRQOL (Vécu et Santé Percue de l’Adolescent [VSP-A], Medical Outcome Study Short Form [SF-36]), and HP were assessed at before and after training. <b><i>Results and Conclusion:</i></b> Both CON and ECC cycling trainings promoted significant improvements in BMI, VO<sub>2peak</sub>, total fat mass, and fat-free mass, with better improvements in body composition parameters in the ECC group (<i>p</i> &#x3c; 0.05). The VSP-A total score increased after CON (<i>p</i> &#x3c; 0.01) and ECC (<i>p</i> &#x3c; 0.001) training, with better enhancement for the ECC group (<i>p</i> &#x3c; 0.05). The SF-36 physical score increased after both CON (<i>p</i> &#x3c; 0.01) and ECC (<i>p</i> &#x3c; 0.001) trainings. The global HP score increased only after ECC training (<i>p</i> &#x3c; 0.001). Except for the energy-vitality item, no significant correlation was found between changes in HRQOL and its subdomains and anthropometric, body composition, and functional parameters. Both ECC and CON cycling trainings are associated with positive changes in HRQOL and HP. However, ECC seems to induce greater improvements in HRQL and HP than CON cycling training, which is probably not due to the anthropometric, body composition, and functional changes.
