EVALUATION OF THE QUALITY OF THE RESULTS OF ORTHODONTIC TREATMENT IN THE CLINICS OF ST. PETERSBURG

To date, various methods of qualitative and quantitative assessment of dental-facial anomalies have been described in the medical literature. They describe the causes of dental anomalies, determine the localization of disorders, consider the functional state of the dental system, as well as quantify the degree of violations of facial aesthetics, occlusion, morphology and function in various dental-facial anomalies. To assess the effectiveness of orthodontic treatment, it is necessary to conduct a quality assessment in patients with dentofacial anomalies who have completed orthodontic treatment. The essence of the proposed method is that on the basis of tables developed and structured according to several blocks, an assessment of the quality of orthodontic treatment is proposed, including an assessment of the aesthetics of the face and smile, articulation and occlusal ratios, malocclusion anomalies in the sagittal, vertical and transversal directions, the state of periodontal tissues and hard tissues of the teeth, as well as the state of the temporomandibular joint in patients who have completed orthodontic treatment. The assessment of the quality of orthodontic treatment consists of three blocks: assessment of the quality of the results of orthodontic treatment, assessment of the process of orthodontic treatment, assessment of the maintenance of medical records. According to the results of filling in which, a conclusion is made about the quality of the orthodontic treatment. The overall assessment of the quality of orthodontic treatment is obtained by summing up the results of the three assessments received, and the assessment of the quality of clinical work is crucial. The method developed by us allows us to make an expert assessment of the process of orthodontic treatment, the quality of its results and the maintenance of medical records in patients who have completed orthodontic treatment. The article presents the results of the assessment of the quality of orthodontic treatment in the clinics of St. Petersburg.

They are reduced to determining the localization of disorders, describing the causes of their occurrence, considering the functional state of the dentofacial system, as well as to quantifying the degree of violations of facial aesthetics, occlusion, morphology and function in various dentofacial anomalies. The method developed by us for assessing the quality of the results of orthodontic treatment allows us to conduct an expert assessment of the quality of the orthodontic treatment performed on the basis of tables developed and structured according to several blocks. Assessment of the quality of orthodontic treatment consists of three blocks: assessment of the quality of the results of orthodontic treatment, assessment of the process of orthodontic treatment, assessment of the maintenance of medical records. The task of the proposed method for assessing the quality of orthodontic treatment is to improve the quality of diagnosis of dental-facial anomalies and the quality of the results of orthodontic treatment. This paper presents the results of assessing the quality of the clinical component of orthodontic treatment in clinics of various forms of ownership in St. Petersburg. The clinical evaluation of the results of orthodontic treatment consists of an assessment of: the aesthetics of the face, the aesthetics of the smile, the occlusal ratios of the dentition, the articulation ratios of the dentition, the temporomandibular joint, the position of individual teeth and dentition, the integrity of the dentition, the integrity of the teeth, the condition of the periodontium and the timing of orthodontic treatment. The study was conducted in state-funded health care institutions and in private, including network, clinics. According to the results of the study, it can be concluded that the data on assessing the quality of clinical results of orthodontic treatment in clinics of different ownership forms do not have significant differences.

The article presents the results of clinical and microbiological studies and immunological features of the state of periodontal tissues in patients with moderate periodontitis, which developed as a complication of orthodontic treatment. It was found that at the later stages of orthodontic treatment with the development of periodontitis, a complex of persistent changes in the periodontal tissues at different levels of pathogen-esis was revealed in patients. In this connection, in order to plan the complex treatment of patients with periodontitis, which has developed as a complication of orthodontic treatment, and to predict the course of the disease, it is necessary to influence the established and mutually supportive links and indicators of pathogenesis.

In the 21st century, due to the improvement of modern fixed orthodontic structures, the need for orthodontic treatment, both among the child population and among the adult population, has greatly increased. Recently, in orthodontics, there is a tendency to evaluate the work of a doctor on the final result, which consists of subjective and objective criteria. To assess the effectiveness of orthodontic treatment, it is necessary to conduct an examination of the quality of medical care in general and to assess the quality of orthodontic treatment of patients with dentofacial anomalies in particular. Subjective evaluation criteria are satisfaction with the results of treatment, the absence of complaints from the patient and the data of a sociological survey. Objective criteria – timely access to dental care, objective assessment of the results of treatment (expert assessment), based on cephalometric measurements of the skull in lateral or direct projections, the study of the face or its image in the photo-photogrammetry, the qualifications of the specialist, the state of health of the patient. This study is devoted to the subjective assessment of the results of orthodontic treatment in children and adults in St. Petersburg. To study the opinion of patients about the quality of orthodontic care, a questionnaire consisting of 21 questions was developed, which allowed to conduct a sociological survey of patients who have completed orthodontic treatment, in relation to their various aspects that make up the concept of quality of dental care. According to the results of the survey, it was revealed that patients who have completed orthodontic treatment are more satisfied with the results of treatment. It should be noted that in addition to the subjective analysis of the results of treatment, an expert assessment of the clinical results of treatment was carried out and there is a discrepancy between the subjective and objective assessment of the results obtained.

Purpose Craniofacial anomalies (CFA) often result in growth abnormalities of the facial skeleton adversely affecting function and appearance. The functional problems caused by the structural anomalies include upper airway obstruction, speech abnormalities, feeding difficulty, hearing deficits, dental/occlusal defects, and cognitive and psychosocial impairment. Managing disorders of the craniofacial skeleton has been improved by the technique known as distraction osteogenesis (DO). In DO, new bone growth is stimulated allowing bones to be lengthened without need for bone graft. The purpose of this clinical focus article is to describe the technique and clinical applications and outcomes of DO in CFA. Conclusion Distraction can be applied to various regions of the craniofacial skeleton to correct structure and function. The benefits of this procedure include improved airway, feeding, occlusion, speech, and appearance, resulting in a better quality of life for patients with CFA.

Lung cancer is one of the most commonly diagnosed cancers across Northern America and Europe. Treatment options offered are dependent on the type of cancer, the location of the tumor, the staging, and the overall health of the person. When surgery for lung cancer is offered, difficulty swallowing is a potential complication that can have several influencing factors. Surgical interaction with the recurrent laryngeal nerve (RLN) can lead to unilateral vocal cord palsy, altering swallow function and safety. Understanding whether the RLN has been preserved, damaged, or sacrificed is integral to understanding the effect on the swallow and the subsequent treatment options available. There is also the risk of post-surgical reduction of physiological reserve, which can reduce the strength and function of the swallow in addition to any surgery specific complications. As lung cancer has a limited prognosis, the clinician must also factor in the palliative phase, as this can further increase the burden of an already compromised swallow. By understanding the surgery and the implications this may have for the swallow, there is the potential to reduce the impact of post-surgical complications and so improve quality of life (QOL) for people with lung cancer.

Political democracy should be equivalent to the economic development of the quality of democracy, economic democracy if not upright, even the owner of the ruling power and money, which is parallel to force global corporatocracy. Consequently, the economic oligarchy preservation reinforces control of production and distribution from upstream to downstream and power monopoly of the market. The implication, increasingly sharp economic disparities, exclusive owner of the money and power become fertile, and the end could jeopardize the harmony of the national economy. The loss of national economic identity that makes people feel lost the “pilot of the state”. What happens then is the autopilot state. Viewing unclear direction of the economy, the national economy should clarify the true figure.
