Antimicrobial resistance (AMR) in Neisseria gonorrhoeae is a major problem worldwide. In the former Soviet countries including Russia, the knowledge regarding AMR has been highly limited. However, in 2004 the Russian gonococcal antimicrobial susceptibility programme (RU-GASP) was initiated. The aims of this study were to examine and describe the prevalence of N. gonorrhoeae AMR in 2007 and 2008 in Russia, and reveal trends in the period from 2005 to 2008. Gonococcal isolates (660 in 2007 and 900 in 2008) from 36 surveillance sites were examined using agar dilution method. From 2005 to 2008, the proportion of isolates resistant to spectinomycin increased from 0% to 7.2%, and remained high for those resistant to ciprofloxacin (approximately 49%). The resistance to azithromycin was 2.3% and 0.4% in 2007 and 2008, respectively. All isolates between 2005 and 2008 were susceptible to ceftriaxone. In conclusion, the AMR of N. gonorrhoeae in Russia is high, as in most countries in the European Union, and ceftriaxone should be the first line for treatment. If there is no access to ceftriaxone or in the presence of severe beta-lactam antimicrobial allergy, spectinomycin should be used; however, the resistance to spectinomycin has increased. Regular, quality-assured national and international surveillance of AMR in N. gonorrhoeae is crucial globally for public health.

Background: Mycoplasma bovis (Gram-positive bacteria) belongs the class Mollicutes and to the family Mycoplasmataceae (Maunsell and Donovan, 2009). It is a cell wall-less bacterium and are instead enveloped by a complex plasma membrane. In cattle, M. bovis is widely known causes various diseases, such respiratory disease, mastitis, arthritis and otitis.Methods: The present study was aimed to determine the antimicrobial susceptibility and identify the genes for antimicrobial resistance of Mycoplasma bovis PG45, Staphylococcus aureus and Escherichia coli. M. bovis PG45, S. aureus and E.coli were subjected to test for their sensitivity to various clinically important antibiotics (Cefotaxime, Cefuroxime, Cefaclor Cefalexin, Ofloxacin, Norfloxacin, Nalidixic acid, Amikacin, Ampicillin, Oxacilin, Amoxyclav, Rifampicin, Penicillin G and Tylosin). The minimal inhibitory concentration (MIC) of each antimicrobial agent was determined by applying an agar dilution method. Polymerase Chain reaction (PCR) was used to amplify specific DNA fragments and thus to determine the presence or absence of a target gene (VspA, tet k and tetA). Result: Showed the MIC values and the presence of VspA, tetK and tetA in M. bovis PG45, S. aureus and E. coli respectively.

Background. Enterococci, once considered as a harmless commensal of intestine, have now emerged as medically important pathogens and are associated with both community-acquired and nosocomial infections. They bear the potential to exhibit resistance against all commonly used antibiotics either by inherent or acquired mechanism, posing a therapeutic challenge.Objectives. This study aimed to characterize enterococci up to the species level and study their antibiogram with special regard to vancomycin.Methods. A descriptive cross-sectional study was conducted in the Department of Microbiology, B.P. Koirala Institute of Health Sciences, Dharan, Nepal, from February to May 2017. A total of 91 enterococcal isolates recovered from clinical specimens were investigated in this study. Their identification and speciation were done according to standard microbiological guidelines. Kirby–Bauer disc diffusion technique was used to study antimicrobial susceptibility pattern, whereas minimum inhibitory concentration of vancomycin was determined by the agar dilution method, with reference to Clinical and Laboratory Standards Institute guidelines.Results. Seven different species of enterococci were isolated,E. faecalisandE. faeciumaccounting about 45% each. The other species encountered wereE.avium,E.cecorum,E.dispar,E.durans, andE.raffinosus. Highest proportion of antimicrobial susceptibility was recorded for linezolid (97.8%), followed by teicoplanin (95.6%) and high-level gentamicin (81.3%). Sensitivity to vancomycin was seen in 79.1% isolates. Likewise, 82.1% of urinary strains were susceptible to nitrofurantoin. A total of 4 disparities were observed between the disc diffusion technique and agar dilution method in determining vancomycin resistance. Multidrug resistance was observed in 31.9% isolates. The overall prevalence of vancomycin-resistant enterococci based on the standard minimum inhibitory concentration method was 25.3%.Conclusions.Enterococcus faecalisandE. faeciumwere the predominant species in causing enterococcal infections. The alarming rise in prevalence of vancomycin and multidrug resistance strains warrants immediate, adequate, and efficient surveillance program to prevent and control its spread.

The emergence of antimicrobial-resistant Salmonella organisms, especially Salmonella Typhimurium DT104, has been reported in many countries, including the United States and Canada. The purposes of this study were to determine the antimicrobial resistance patterns of Salmonella isolated from hog, beef, and chicken carcasses from provincially inspected abattoirs in Ontario and to determine the agreement between the agar dilution method and the microbroth dilution method for measurement of antimicrobial resistance of the isolates. Antimicrobial resistance of Salmonella isolates from hogs (n = 71), beef (n = 24), and chicken (n = 295) to amikacin, ampicillin, cephalothin, chloramphenicol, ciprofloxacin, gentamicin, streptomycin, sulfamethoxazole,and tetracycline was determined using the two methods. None of the 390 isolates were resistant to ciprofloxacin at levels of 0.125 μg/ml. All chicken and hog isolates were sensitive to amikacin, whereas all beef isolates were sensitive to both amikacin and gentamicin. Multiple antimicrobial resistance (resistance to more than one antimicrobial) was found in 29% of bovine isolates and 42% of porcine isolates using both methods for testing and in 42% by the agar dilution and 33% by the microbroth dilution methods in the chicken isolates. Overall, there was good agreement between the two test methods for resistance to most of the antimicrobials, with disagreement found in the results in 1.3% of the isolates for ampicillin and sulfamethoxazole, 8.2% for streptomycin, 5.6% for cephalothin, and 1.0% of the isolates for tetracycline. The lack of agreement between the two test methods was found mostly among the chicken isolates.

Antimicrobial susceptibility testing of anaerobes is challenging. Because MIC determination is recommended by both CLSI and EUCAST, commercial broth microdilution and diffusion strip tests have been developed. The reliability of broth microdilution methods has not been assessed yet using the agar dilution reference method. In this work, we evaluated two broth microdilution kits (MICRONAUT-S Anaerobes® MIC and Sensititre Anaerobe MIC®) and one gradient diffusion strip method (Liofilchem®) for antimicrobial susceptibility testing of 47 Clostridiales isolates (Clostridium, Clostridioides and Hungatella species) using the agar dilution method as a reference. The evaluation focused on comparing six antimicrobial molecules available in both microdilution kits. Analytical performances were evaluated according to the Food and Drug Administration (FDA) recommendations. Essential agreements (EA) and categorical agreements (CA) varied greatly according to the molecule and the evaluated method. Vancomycin had values of essential and categorical agreements above 90% for the three methods. The CA fulfilled the FDA criteria for three major molecules in the treatment of Gram-positive anaerobic infections (metronidazole, piperacillin/tazobactam and vancomycin). The highest rate of error was observed for clindamycin. Multicenter studies are needed to further validate these results.

Abstract:Antimicrobial susceptibility test (AST) is widely used to provide the minimum inhibitory concentration of bacteria, and crucial to provide appropriate uses of antibiotics and to address the issue of drug-resistance bacteria. However, ASTs require the time-consuming incubation about 16-20 h for the visual determination of the growth of bacterial colonies, which has been a major obstacle to on-site applications of ASTs. In this study, we propose a rapid and non-invasive method based on laser speckles to evaluate the bacterial growth movements in real time, thus reducing the time for the agar dilution method. With a simple configuration compatible with conventional agar plates, the analysis of laser speckle from samples enables the early detection of the presence of growth as well as its detailed history of the colony-forming movement on agar plates. Using the samples prepared through the same procedure as the agar dilution method, we obtained the AST results at least 4-8 hours earlier than the conventional method without compromising the accuracy. This technique does not require for the use of exogenous agents, but works for most bacteria regardless of their species. Furthermore, the distinctive responses of several species to microbial agents were revealed through the present technique supporting a comprehensive analysis of the effect of the antibiotics. The findings suggest that this new method could be a useful tool for rapid, simple, and low-cost ASTs in addition to providing the historical information of the bacterial growth on agar plates.

The therapeutic alternatives available for use against ciprofloxacin-resistant enteric fever isolates in an endemic area are limited. The antibiotics currently available are the quinolones, third-generation cephalosporins and conventional first-line drugs. In this study, the MICs of various newer drugs were determined for 31 ciprofloxacin-resistant enteric fever isolates (26 Salmonella enterica serovar Typhi and 5 S. enterica serovar Paratyphi A). MICs for ciprofloxacin, ofloxacin, gatifloxacin, levofloxacin, cefotaxime, cefixime, cefepime and azithromycin were determined using Etest strips and the agar dilution method. By Etest, all of the ciprofloxacin-resistant isolates had ciprofloxacin MICs ≥32 μg ml−1. S. Typhi showed MIC90 values of 0.50, 0.25 and 0.38 μg ml−1 for cefixime, cefotaxime and cefepime, respectively. For the cephalosporins, a negligible difference in MIC90 and MIC50 values for S. Typhi and S. Paratyphi A was observed. A single isolate of S. Typhi showed a high azithromycin MIC of 64 μg ml−1. The MIC90 value for azithromycin in S. Typhi and S. Paratyphi was 24 μg ml−1. Gatifloxacin demonstrated lower resistance (80.8 %) compared with the other quinolones (92–100 %) in S. Typhi. The rise in MIC levels of these antimicrobials is a matter for serious concern.

Introduction: Antimicrobial resistance of Salmonella poses a key threat to public health worldwide. Salmonella Enteritidis (S. Enteritidis) (119 isolates) from children under 10 years old with diarrhea in Shanghai from 2010-2012 were characterized for antimicrobial susceptibility, virulence gene profiles and pulsed field gel electrophoresis (PFGE) patterns. Methodology: The minimum inhibitory concentration for the 119 S. Enteritidis isolates was determined using an agar dilution method. The presence of virulence genes was detected by polymerase chain reaction (PCR). All the isolates with antimicrobial resistance were subjected to PFGE analysis. Results: Among these isolates, 71.4% (85) were resistant to sulfafurazole, 59.7% (71) were resistant to ampicillin, 47.1% (56) were resistant to streptomycin, 7.6% (9) were resistant to ceftiofur and 3.4% (4) were resistant to ceftriaxone. Multidrug resistance (MDR) was found in 63.9% (76) of the isolates; 23 resistance profiles were identified. All isolates harbored the ssaQ and sopE virulence genes in the 16 virulence profiles (VPs); VP1 accounted for 70.59% of the 119 isolates. There were 57 PFGE patterns among the 92 isolates tested, mainly grouped into five clusters (A to E). All of the 76 MDR isolates carried multiple virulence genes. Conclusions: Our study provides useful microbiological data for the successful treatment of S. Enteritidis infections in Shanghai. Although broad spectrum antimicrobials may be useful in the treatment of invasive S. Enteritidis infections, clinicians need to be aware of common microbiological traits, because of the high prevalence of MDR.

Antimicrobial resistance is recognised as a growing problem that seriously threatens public health and requires prompt action. Concerns have therefore been raised about the potential harmful effects of making antibiotics available without prescription. Because of the very serious concerns regarding further spread of resistance, the over-the-counter (OTC) availability of antibiotics was analysed here. Topical and systemic OTC antibiotics and their indications were determined across 26 European Union (EU) countries and Norway by means of a European survey. We identified a total of 48 OTC products containing 20 different single antibiotics and three antibiotic combinations as active substances, used mainly as topical preparations in short treatment courses. Given the relevance of these medicines and the increasing risk of antimicrobial resistance, it is important to limit the availability of OTC antibiotics and to monitor their use.

Introduction: Tigecycline is a novel glycylcycline  derivative of the tetracycline with activity against a wide range of  organisms including Methicillin resistant Staphylococcus aureus, Vancomycin  resistant  Enterococcus , Extended spectrum beta lactamase   producing  (Escherichia coli , Klebsiella  pneumonia)  and Acinetobacter species.  The aim of the study was to assess effectiveness of the drug against methicillin resistant Staphylococcus aureus (MRSA), vancomycin resistant enterococci (VRE), ESBL producers and carbapenem resistant Acinetobacter baumannii and to compare the efficacy of different methods of antimicrobial susceptibility testing for Tigecycline.Methods: A total of 250 clinical isolates were processed and identified by conventional methods. In all the 250 isolates, antimicrobial susceptibility was carried out by disc diffusion method , Minimum inhibitory test by agar dilution method (MIC) and in 30 isolates of A baumannii  MIC was also done by E test.Results: Out of 250 isolates, 236 isolates were sensitive to tigecycline by agar dilution method while only 159 were sensitive by disk diffusion method.Conclusion: Marked discordance was observed between the results of two different methods (DDT & Agar dilution method) for E coli, Klebsiella spp and A baumannii, where significant number of isolates were resistant to tigecycline by DDT as compared to AD method. But results of MIC by agar dilution method & E test were in concordance for A. baumannii.
