Objectives: The aim of this study is to compare patients with fibromyalgia, their first degree relatives; osteoarthritis patients, their first degree relatives; patients with somatization disorder and healthy controls in terms of psychological symptoms, somatic complaints and ways of coping with stress.Materials and Methods: The study is planned as crosssectional. Patients who presented to the Düzce University Faculty of Medicine, Physical Medicine and Rehabilitation Outpatient Clinic between June 2016 and January 2017 and who diagnosed with fibromyalgia and osteoarthritis, and their first-degree relatives, patients with somatization disorders and a healthy control group who filled out the informed consent form was included in this study. The socio-demographic information query form, Coping Inventory for Stressful Situations (COPE) Form, Psychological Symptom Checklist (SCL- 90) and the Visual Analog Scale (VAS) were applied to all volunteers. Covariance analysis was used to compare groups in terms of scale scores.Results: It was determined that the groups were not homogeneous in terms of age, VAS, education level, sex, marital status and occupation. The groups were compared taking into account the impacts on the COPE and SCL-90 scores of these characteristics which are thought to be confounders. As a result of the evaluations, it was determined that the patients with fibromyalgia and somatization preferred the problem-focused coping and emotional-focused coping attitudes significantly less and the non-functional coping attitude significantly more. Osteoarthritis patients were found to be in the middle of both sides on many occasions. However, there was no significant difference among the groups in terms of psychological symptoms when the effect of the confounding factors were eliminated.Conclusion: Since the findings achieved suggest that fibromyalgia and somatization disorder are the same diseases, it has been concluded that more research should be conducted on the subject.International Journal of Human and Health Sciences Vol. 01 No. 01 January’17. We aimed to reveal psychological symptoms of young university students (18-25 y/o) with no secondary disease with complaints of migraine headaches. We compared the psychological symptoms with a young control group that did not present headaches and other studies examining migraines in different age groups to determine any similarities. The study consisted of 75 migraine cases; 13 with aura and 62 without aura) and 42 non-headache control subjects. The International Classification of Headache Disorders 3 (ICHD-3) with criteria questions and the Psychological Symptom Research Scale were applied to all participants and SPSS Statistics Program evaluated the data. Findings: 1) In the test of Descriptive Statistics for the Sample of Students, average values of both sexes were taken and found the average values of the migraine groups were higher than control group. 2) The Anova Test found differences in the diagnostic group besides depression (Somatisation p<., others p<.01) with no significant difference in depression (p=0.315). 3)The Pearson Correlation Analysis, found no association between age and psychological symptoms. There’s a significant relationship between psychological symptoms with somatisation (besides paranoid) (p<.05). There’s a significant relationship between depression and other psychological symptoms (p<.05). There’s a significant relationship between anxiety, obsession, interpersonal relationships, psychoticism, paranoia, anger, phobia, additional scale and other psychological symptoms (p<.01). Result: The study demonstrated psychological symptoms in young migraineurs. The average psychological symptoms of both migraine groups were higher than the control group, this showed psychological symptom similarities between young migraineurs and migraine patients of different age groups.

This study examined the relationship between psychiatric conditions and narcissism in 78 patients with physical rehabilitation for illnesses such as cerebrovascular disease. The frequency of psychiatric disorders (Major Depression and Adjustment Disorder) was significantly higher for the patient group than for the healthy control group. The patients had significantly higher scores on measures of psychiatric symptoms including depression and lower scores on a narcissistic factor (Self-sufficiency) than the controls. In the patient group, the Self-sufficiency scores were significantly and positively correlated with Vigor scores In the patient and control groups, although the correlation coefficient was not high and no significant difference was noted, the Self-sufficiency scores were negatively correlated with the Depression scores. These findings suggest that, in patients undergoing physical rehabilitation, poor Self-sufficiency is related to a decrease in vigor as a depressive symptom, perhaps due to the loss of physical functioning.

Oxidized low density lipoprotein (ox-LDL) is a product of oxidative stress. In this cross-sectional study, we compared the ox-LDL concentrations in diabetic patients with normoalbuminuria (n=28), microalbuminuria (n=28), and macroalbuminuria (n=28) with their first degree relatives (n=28) and healthy control people (n=31). They were selected by consecutive patient selection method. The ox-LDL level was assayed using ELISA. We measured blood pressure, lipid profile, fasting plasma glucose (FPG), and HbA1c in all groups. There was no significant difference in ox-LDL concentrations among normoalbuminuric, microalbuminuric, and macroalbuminuric diabetic groups. In diabetic patients with micro- and macroalbuminuria, ox-LDL concentration was higher than their first degree relatives (P=0.04andP=0.03) and control group (P=0.001andP=0.03, resp.). In normoalbuminuric diabetic persons, ox-LDL concentration was just higher than that of healthy people (P=0.02). There was no statistically significant difference in ox-LDL levels between normoalbuminuric diabetic patients and their first degree relatives. In conclusion, the presence and progression of albuminuria in diabetic patients are not related to ox-LDL concentration and genetic predisposition influences the plasma OX-LDL level. Larger sample size is needed to confirm this conclusion in future studies.

Addiction is the most critical form of Addiction. It is a chronic disease with a potential for fatality if not treated. In this work, 180 samples of male individuals were collected in this study. They classified into three groups, groups:  G1 who were healthy control; G2 who was addicted to methamphetamine (meth); G3 who was addicted to tramadol (Tra). Each group consists of 60 heavy smokers Iraqi male individuals in the age range of 18-43 years. The results showed a highly significant increase (p<0.0001) in the level of Glucose of the two addicted groups in comparison with the healthy group. A highly significant decrease (p<0.0001) could be seen in the level of Zn of the two addicted groups G2, G3 compared to the control group, while the level of Cu of the two addicted groups were highly significant increased (p<0.0001). Also, the results showed a highly significant difference (p<0.0001) in BMI for the studied groups, G2, G3 in comparison with the control group. All addictive individuals under this study were at normal weight depending on their BMI.

Purpose: This study aimed to compare the neutrophil-to-lymphocyte (NLR) and plateletto- lymphocyte (PLR) ratios in patients with retinal artery occlusion (RAO) with those from a healthy control population and to identify the relationship between them. Methods: Forty-six patients with RAO and fifty-one healthy control subjects were included in this retrospective case-control study. RAO was diagnosed following an ophthalmic examination and fluorescein angiography (FA). Blood neutrophil, lymphocyte, and platelet counts were recorded for each of the 97 subjects, from which NLR and PLR values were calculated. Results: There were 46 patients (28 male [M], 18 female [F]) in the RAO group and 51 patients (27 M, 24 F) in the control group. No significant differences were found between patients with RAO and the control subjects in terms of gender and age (P > 0.05). Patients with RAO had significantly increased NLR values (2.85 ± 1.70) than the control subjects (1.63 ± 0.59, P < 0.001). The mean PLR in patients with RAO was 123.69 ± 64.98, while that in control subjects was 103.08 ± 36.95; there was no significant difference between the two groups (P = 0.055). A logistic regression analysis revealed that NLRs were 3.8 times higher in patients with RAO than in control subjects (odds ratio = 3.880; 95% confidence interval = 1.94 to 7.74; P < 0.001). Conclusion: NLRs were significantly increased in patients with RAO compared to the control subjects.

Background: Oxidative stress induced cancer cell formation. Gene polymorphism plays roles in carcinogen metabolism, antioxidant and DNA repairing pathway was susceptibility to oxidative stress. This study aim to determine the association between CAT-21 A/T polymorphism with breast cancer susceptibility. Methods: Case control study was conducted on 65 breast cancer patient and 65 healthy control group. The whole blood samples were isolated from 65 breast cancer patients in Haji Adam Malik General Hospital Medan and 65 healthy control group. The CAT-21A/T polymorphism was analyzed by PCR-RFLP procedure. PCR-RFLP product was electrophoresed and visualized in agarose 4%. Results:The AA CAT-21 genotype were lower in breast cancer (BC) than healthy control (HC) group (31/47.7% vs 40/61.5%), in the contrary AT+TT genotype was greater in BC than HC group (34/52.3% vs 25/38.5%) with (p=0.159, OR=1.755, CI=0.874–3.525). A allele CAT-21 were found lower in BC than HC group (89/68.5% vs 105/80.8%) then T allele were greater in BC than HC group (41/31.5% vs 25/19.2%) with (p=0.033, OR=1.935;CI=1.022-3.428). Conclusions: There was significant difference in allele distribution of CAT-21 A/T between case and control group but no in genotype distribution. In this population study showed that allele of CAT -21 A/T polymorphism could represent as a risk factor to breast cancer. Bangladesh J Medicine July 2021; 32(2) : 79-84

Background Familial Mediterranean Fever and celiac disease are both related to auto-inflammation and/or auto-immunity and they share some common clinical features such as abdominal pain, diarrhea, bloating and flatulence. Objectives We aimed to determine the association of these two diseases, if present. Methods Totally 112 patients diagnosed with Familial Mediterranean Fever and 32 cases as healthy control were included in the study. All participants were examined for the evidence of celiac disease, with serum tissue transglutaminase IgA levels (tTG IgA). Results Totally 144 cases, 112 with Familial Mediterranean Fever and 32 healthy control cases were included in the study. tTG IgA positivity was determined in three cases with Familial Mediterranean Fever and in one case in control group. In that aspect there was no significant difference regarding the tTG IgA positivity between groups (P=0.81). Duodenum biopsy was performed to the tTG IgA positive cases and revealed Marsh Type 3b in two Familial Mediterranean Fever cases and Marsh Type 3c in the other one while the biopsy results were of the only tTG IgA positive case in control group was Marsh Type 3b. In HLA evaluation of the celiac cases; HLA DQ2 was present in two celiac cases of the Familial Mediterranean Fever group and in the only celiac case of the control group while HLA DQ8 was present in one celiac case of the Familial Mediterranean Fever group. Conclusions We did not determine an association of Familial Mediterranean Fever with celiac disease. Larger studies with subgroup analysis are warranted to determine the relationship of these two diseases.

Abstract Objectives Vitamin K is generally regarded as a procoagulant drug with physicians, concerns have been raised about its effects on hemostasis in the healthy population. We aimed to investigate whether vitamin K2 affects activities of individual vitamin K dependent coagulation factors in healthy individuals without anticoagulation treatment. Methods Forty healthy volunteers between 25 and 40 years old were recruited. They received 90 μg of vitamin K2 every day for 30 days. Prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), international normalized ratio (INR), fibrinogen (FIB) levels and blood coagulation factors II, VII, IX, and X activity levels (F II : C, FⅦ : C, FⅨ : C,FⅩ : C), protein induced by vitamin K absence or antagonist-II (PIVKA-II), which is uncarboxylated prothrombin were measured at day 0, and day 30 after vitamin K2 administration. Plasma diluted 1:10 from vitamin K2 group and healthy control group were assayed for the activity of factors II, VII, IX, and X. Results PT, APTT, TT, and FIB did not show significant difference at day 30 when compared with baseline. The activities of coagulation factors II, VII, IX, and X was not significantly different with baseline (97.28 ± 12.42% vs. 99.96 ± 10.24%, P = 0.24 for F II: C; 76.12 ± 15.82% vs. 76.40 ± 12.33%, P = 0.92 for FⅦ: C; 97.65 ± 13.98% vs. 99.65 ± 13.30%, P = 0.47 for FⅨ: C; 89.18 ± 10.76% vs. 92.01 ± 10.46%, P = 0.1 for FⅩ: C) . PIVKA-II levels were not changed with 30 days vitamin K2 supplementation (21.62 ± 3.21 vs. 23.87 ± 2.65 mAU/ml, P = 0.16). After 30 days vitamin K2 administration, factor II, Ⅶ, Ⅸ, and Ⅹ activity of plasma diluted up to 10 times were proportionally decreased, and did not show significant difference with the healthy control without vitamin K2 exposure (10.32 ± 1.24% vs. 10.97 ± 1.55%, P = 0.38 for F II: C; 9.52 ± 2.94% vs. 9.14 ± 1.79%, P = 0.68 for FⅦ: C; 11.78 ± 2.12% vs.11.65 ± 1.54%, P = 0.87 for FⅨ: C; 8.22 ± 1.28% vs. 8.92 ± 1.13%, P = 0.21 for FⅩ: C). Conclusions Vitamin K2 supplementation at recommended dosage does not affect vitamin K-dependent coagulation factors activity in healthy subjects. Uncarboxylated prothrombin (PIVKA-II) in healthy individuals is not decreased with vitamin K supplementation. Funding Sources None.

Abstract Reliable data on the prognosis of functional motor disorder are scarce, as existing studies of the prognosis of functional motor disorder are nearly all retrospective, small and uncontrolled. In this study we used a prospectively recruited, controlled cohort design to assess misdiagnosis, mortality and symptomatic and health outcome in patients with functional limb weakness compared to neurological disease and healthy control subjects. We also carried out an exploratory analysis for baseline factors predicting outcome. One hundred and seven patients with functional limb weakness, 46 neurological and 38 healthy control subjects from our previously studied prospective cohort were traced for follow-up after an average of 14 years. Misdiagnosis was determined in a consensus meeting using information from records, patients and their GPs. Numbers and causes of death were collected via death certificates. Outcome of limb weakness, physical and psychiatric symptoms, disability/quality of life and illness perception were recorded with self-rated questionnaires. Outcome measures were compared within and between groups. Seventy-six patients (71%) with functional limb weakness, 31 (67%) neurological and 23 (61%) healthy controls were included in follow-up. Misdiagnosis was found in one patient in the functional limb weakness group (1%) and in one neurological control (2%). Eleven patients with functional limb weakness, eight neurological control subjects and one healthy control subject had died. Weakness had completely remitted in 20% of patients in the functional limb weakness group and in 18% of the neurological controls (P = 0.785) and improved in a larger proportion of functional limb weakness patients (P = 0.011). Outcomes were comparable between patient groups, and worse than the healthy control group. No baseline factors were independent predictors of outcome, although somatization disorder, general health, pain and total symptoms at baseline were univariably correlated to outcome. This study is the largest and longest follow-up study of functional limb weakness. Misdiagnosis in functional limb weakness is rare after long-term follow-up. The disorder is associated with a higher mortality rate than expected, and symptoms are persistent and disabling. It appears difficult to predict outcome based on common baseline variables. These data should help inform clinicians to provide a more realistic outlook of the outcome and emphasize the importance of active and targeted therapy.

Abstract Objectives This study was designed to evaluate the role of anti-CD74 antibodies in diagnosis of axial spondyloarthritis (axSpA) and their relationship to disease duration and disease activity. Methods Fifty patients with axSpA, 15 patients with RA and 15 healthy subjects were included in the study. Clinical examination and laboratory tests were done. The ESR, CRP level and ASDAS were measured as markers of the disease activity. Quantitative determination of human CD74 IgG antibodies was done. Results The mean age of the patients was 38.22 (S.D.12.20) years. The level of CD74 autoantibodies was significantly higher in axSpA in comparison to control groups. Most patients with positive articular and extra-articular manifestations were positive for CD74 autoantibodies. In patients with inactive disease, 33.3% were positive for CD74 autoantibodies, as were 83% with active disease. High percentages of patients with early and late axSPA were CD74 autoantibody positive. The majority of patients with positive disease activity in early and late axSpA were CD74 autoantibody positive. CD74 autoantibodies had 80% sensitivity vs both control groups with 87% specificity vs the healthy control group and 80% vs the RA control group in the diagnosis of axSpA. Conclusions The frequency of positive anti-CD74 IgG antibodies was as high in patients with early axSpA as in those with late axSpA, with no significant differences. There was a significant difference in the frequency of positive anti-CD74 IgG antibodies between patients with positive and negative disease activity. Based on the sensitivity and specificity of anti-CD74 IgG, this is a promising diagnostic tool to support the clinical diagnosis of axSpA.
