Oxidized low density lipoprotein (ox-LDL) is a product of oxidative stress. In this cross-sectional study, we compared the ox-LDL concentrations in diabetic patients with normoalbuminuria (n=28), microalbuminuria (n=28), and macroalbuminuria (n=28) with their first degree relatives (n=28) and healthy control people (n=31). They were selected by consecutive patient selection method. The ox-LDL level was assayed using ELISA. We measured blood pressure, lipid profile, fasting plasma glucose (FPG), and HbA1c in all groups. There was no significant difference in ox-LDL concentrations among normoalbuminuric, microalbuminuric, and macroalbuminuric diabetic groups. In diabetic patients with micro- and macroalbuminuria, ox-LDL concentration was higher than their first degree relatives (P=0.04andP=0.03) and control group (P=0.001andP=0.03, resp.). In normoalbuminuric diabetic persons, ox-LDL concentration was just higher than that of healthy people (P=0.02). There was no statistically significant difference in ox-LDL levels between normoalbuminuric diabetic patients and their first degree relatives. In conclusion, the presence and progression of albuminuria in diabetic patients are not related to ox-LDL concentration and genetic predisposition influences the plasma OX-LDL level. Larger sample size is needed to confirm this conclusion in future studies.

Abstract Background: Human Fascioliasis is a zoonotic infection caused by the adult stage of Fasciola species. Fasciola hepatica is a parasite that, following living in its vertebrate host, reduces the host's inflammatory immune responses to infectious diseases, including COVID-19. Therefore, because of the completely unproven and limited data on this issue, we decided to evaluate the prevalence of antibodies against this parasite in patients admitted to the hospital due to relatively severe symptoms of COVID-19.Methods: This cross-sectional study was conducted on the serum samples of 182 patients with COVID-19 disease who were diagnosed by PCR test and 210 healthy subjects in Alborz Province, Iran between March and June 2020. Anti-Fasciola antibodies status was determined in all serum samples, using ELISA technique. A self-developed questionnaire was used to measure demographic information.Results: The frequency of Fasciola infection was found to be significantly higher in COVID-19 patients as compared to healthy control (n = 4, 2.2%, n = 9, 4.3%; respectively) (P = 0.041). There was no significant difference between COVID-19 patients and control group in age, education, and gender (P > 0.05).Conclusions: This study indicates that a significant association between Fasciola hepatica seropositivity and COVID-19 disease. Our finding suggests that Fasciola infection may be able to lead to decreased immune responses, thereby reducing the severity and symptoms of COVID-19 disease.

Lunula is the white, half-moon shaped area seen in proximal ends of some nails. Though a few studies have described the nail changes that can occur in association with HIV infection, none of these paid much attention to lunula. Aims and Objectives. To study the lunula in fingernails among HIV infected patients. Materials and Methods. An observational, cross-sectional study to record presence of lunula in 168 HIV-positive patients and compare it with age and sex matched 168 healthy HIV-negative control. Anolunula (absence of lunula) in HIV-positive patients was correlated with CD4 counts, stages of HIV infection, time since patient was diagnosed as HIV-positive, and status of antiretroviral therapy. Results. Anolunula was present in significantly more fingernails in HIV-positive patients compared to HIV-negative controls. There was a highly significant difference for total anolunula (anolunula in all fingernails) in study and control group. Incidence of total anolunula was directly proportional to the stage of HIV infection, increasing progressively as the HIV infection advances from stage 1 to stage 4. Conclusion. Absence of lunula is related to not only HIV infection per se but also the stages of HIV infection.

The metabolic fitness (MF) is a component of athletes’ physical conditioning. This study aims to investigate the effects of quercetin supplementation on Turkish Junior athletes’ lipid and protein metabolism relating to MF after one month classic boxing training. Totally 20 voluntary junior male athletes were separated into two equal groups as the experimental group (EG) and control group (CG). The participants were supplemented with 500 mg quercetin fifteen minutes before each workout in one month boxing training program. Blood samples during pre and post training were taken from athletes in order to determine metabolic fitness related parameters. Lipid profile contains low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), trigliserid (TG), total cholesterol (TC) variables while protein metabolism includes the albumin, total protein, direct bilirubin and total bilirubin parameters. The Mann Whitney U analyzes were used for comparison of the means between experimental and control groups during pre and posttest and between pre and post test results in experimental and control groups. This study showed that EG had a similar physical characteristic with CG. There were significant decrease in TC and LDL-C and an increase in HDL-C in EG while there was only significant increase in HDL-C of in controls. A significant difference of HDL-C was observed between EG and CG during pretest. In other side, TC and LDL-C and HDL-C were significantly differentiated between EG and CG during posttest. Conclusion: it can be concluded that quercetin plays an important role on lipid metabolism not protein.

Background: Cardiac dysfunctions have been recognized as a common complication of sickle cell anaemia (SCA), and together with pulmonary disorder accounts for many deaths in these patients. However, sickle cell traits appear clinically normal, although they have genetic abnormality. The aim of this study was to assess the effect of sickle cell trait on cardiac prognostic markers by measuring high density lipoprotein (HDL-C), low density lipoprotein (LDL-C), cardiac creatine kinase (CK-MB), ultra-sensitive C reactive protein (us-CRP), total homocysteine (Hyc), and N-terminal pro-brain natriuretic peptide (NT-pro BNP) tests in adult Sudanese patients with sickle cell trait.Methods: A cross-sectional study was performed in 200 healthy volunteers as a control group and 200 diagnosed patients with sickle cell trait. It was carried out in Khartoum Specialized Hospital, Al-Bayan Hospital, Obayed Clinical Center and Dr. Nadir Specialized Hospital, Sudan between January 2015 and January 2016. All participants were between 20-32 years old. LDL-C, HDL-C, CK-MB, NT-proBNP and hs-CRP concentrations were measured by Hitachi 912 full-automated Chemistry Analyzer (Roche Diagnostics, Germany) as manufacturer procedure, while homocysteine level was measured by ELISA technique using special kit.Results: When compared to control group, the levels of LDL-C, hs-CRP and NT-proBNP revealed significant increase in patients’ sera (p<0.001), while Hyc and CK-MB levels were increased insignificantly in patients with SCT (p=0.069, p=0.054 respectively). On the other hand, comparison to control group, HDL-C showed insignificant reduction in patients (p=0.099).Conclusion: The results suggest that sickle cell trait increased the risk of patient-related complication secondary to cardiac dysfunction.

ABSTRACT This cross-sectional study sought to evaluate motor development in infants exposed to ZIKV born with normal head circumference (HC). Thirty one children, distributed into two groups, participated in the study: 15 whose mothers were infected by ZIKV during pregnancy, born with HC from −1.9 to +2 Z-scores, adjusted for sex and gestational age (exposed group); and 16 randomly selected infants without known prenatal exposure to ZIKV, paired by sex and age (control group). Alberta Infant Motor Scale (AIMS) was used to evaluate gross motor development. We found no significant difference between the exposed and control groups. However, considering that AIMS is a screening test that assesses only the gross motor development and the small size of our sample, infants exposed to ZIKV during pregnancy should be continuously evaluated for different aspects of their development.

Background and Aim: Subclinical hypothyroidism (SCH) is frequently seen in diabetic patients. Elevated levels of uric acid (UA) were also reported in diabetic patients. No study assessed the relation between SCH and UA levels in diabetic patients. We aimed to evaluate this relation and the association of both conditions with other clinical and laboratory parameters in diabetic patients. Subjects and Methods: This cross-sectional study included 100 T2DM patients in addition to 50 age and sex matched healthy controls. Diabetic patients comprised 50 patients with SCH and 50 euthyroid patients. All participants were subjected to careful history taking, thorough clinical examination and standard laboratory work up. The performed investigations included fasting and postprandial blood sugar, fasting insulin levels, HbA1c levels, thyroid hormones (FT3, FT4 and TSH), renal profile and serum UA. Results: Comparison between the studied groups regarding serum UA levels revealed significantly higher levels in the diabetic group (5.4 ± 1.9 versus 4.2 ± 1.0 mg/dl, p<0.001). SCH + DM patients had significantly higher UA levels in comparison to DM group (6.1 ± 1.8 versus 4.8 ± 1.7 mg/dl, p<0.001) and control group (6.1 ± 1.8 versus 4.2 ± 1.0 mg/dl, p<0.001). SCH + DM patients had significantly higher HbA1c levels (8.9 ± 1.1 versus 7.6 ± 1.3 %, p<0.001), HOMA-IR (3.9 ± 0.8 versus 2.8 ± 1.0, p<0.001) and UA levels (6.1 ± 1.8 versus 4.8 ± 1.7, p<0.001). Correlation analysis identified a significant direct correlation between serum UA and HOMA-IR in DM + SCH patients (r=0.4,p=0.004). In univariate analysis, presence of SCH [OR (95% CI): 2.57 (1.07-6.15), p=0.034] and nephropathy [OR (95% CI): 4.57 (1.77-11.8), p=0.002] were significant predictors of higher (upper tertile) UA in the studied patients. However, in multivariate analysis, only nephropathy [OR (95% CI): 4.25 (1.62-11.17), p=0.003] continued to be significant while SCH showed a marginal trend [OR (95% CI): 0.43 (0.17-1.08), p=0.073]. Conclusions: The present study suggests an association between SCH and increased UA levels in diabetic patients.

AbstractGene–diet interactions may play an important role in the inter individual diversity observed in on cardiovascular disease (CVD) risk factors. Therefore, in the current study, we examined the interaction of CETP TaqB1 polymorphism with dietary insulin index and load (DII and DIL), in altering on CVD risk factors among type 2 diabetes mellitus (T2DM). In this cross-sectional study, blood samples were collected from 220 type 2 diabetic patients (134 females and 86 male) with a mean age of 52.24 years in Tehran, Iran. DIL and DII were obtained via validated food-frequency questionnaire (FFQ). Taq1B polymorphism was genotyped by the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method. Biochemical markers including total cholesterol (TC), low-density lipoprotein (LDL), high-density lipoprotein (HDL), triglyceride (TG), superoxide dismutase (SOD), C-reactive protein (CRP), total antioxidant capacity (TAC), pentraxin-3 (PTX3), isoprostaneF2α (PGF2α). interleukin 18 (IL18), leptin and ghrelin were measured by standard protocol. Patients with B1B1 genotype had lower lipid profiles include LDL/HDL (P < 0.001) and TG (P = 0.04) when they consumed diets higher on the DIL and DII index. Moreover, carriers of B2B2 genotype who were in the last tertile of DIL had higher antioxidant and inflammatory markers include SOD (P = 0.01), PGF2α (P = 0.04) and CRP (P = 0.02). Further, a significant interaction between CETP TaqB1 and DII was shown in terms of WC (P = 0.01), where the highest WC were observed in B2B2 genotype carriers following a DII score. However, the highest inflammatory and antioxidant markers include CRP (P = 0.04), TAC (P = 0.01), SOD (P = 0.02), and PGF2α (P = 0.02) were observed in B2B2 genotype carriers when they consumed diets higher on the DII index. Based on the current study, it could be proposed that CETP polymorphism may be associated with CVD risk factors in T2DM patients with high following insulin indices, including DII and DIL. It seems that CETP Taq1B polymorphism can invert the result produced by insulin. This conclusion illustrates that the CETP Taq1B B1 allele could counteract the CVD risk induced by high DII and DIL.

Objective: Preeclampsia characterized systematically by extensive vascular endothelial dysfunction and microangiopathy on mother, dNK is very important for the success of placentation. They are the key mediator of maternal immune system interactions with fetal cells. dNK cells are also involved in the modulation of EVT and the remodeling of spiral arteries. Methods: Analytic research with cross-sectional study, with samples of pregnant women who suffer from severe PE and aterm pregnancy which came to H. Adam Malik Hospital and Networking Hospital, November 2015-April 2016. The samples are 46 women, who met the inclusion criteria. Results: Immunohistochemistry examination dNK cell in the severe PE case group and control group, statistically found p<0,05. dNK placenta expression in the severe preeclampsia case group gives an overview of expression with a mean of 2.55±2.31, while the control group of normal pregnancy had higher mean is 8.66±3.16. Conclusion: The examination of immunohistochemistry of dNK cells showed there is a significant difference in the expression of Immuno-histochemistry dNK cells between severe PE case group and non severe PE.

Abstract Objective. To investigate the relationship between sarcopenia and diabetic nephropathy. Methods. 56 diabetic patients without complications, 50 diabetic patients with nephropathy, 53 healthy controls included in this present study. Demographic characteristics such as sex, age, anthropometric measurements such as weight, body mass index [BMI], hip circumference, waist circumference and upper arm circumference were measured. Sarcopenia diagnosis was based on European Working Group on Sarcopenia in Older People [EWGSOP] criteria which consist of hand grip strength, 6-meter walking test and muscle mass. Results. The frequency of sarcopenia increased gradually from 15.1% in healthy control group to 21.4% in the diabetes group, and 34% in diabetic nephropathy group (X2 for trend, p = 0.029). The frequency of sarcopenia was similar in diabetes and diabetic nephropathy group. However, the frequency of sarcopenia was higher in diabetic nephropathy than healthy controls (OR = 2.89, CI [1.11-7.51] in logistic regression). Conclusion: In the present study, the prevalence of sarcopenia was higher in patients with diabetic nephropathy compared to healthy controls.
