Transcutaneous Carbon Dioxide Measurement in Adult Patients with Neuromuscular Disorders: A quality Level Assessment

Background: Carbon dioxide tension (PCO2) monitoring during sleep, is crucial to identify respiratory failure in patients with neuromuscular disorders (NMD). Transcutaneous PCO2 monitoring is an available technique to measure PCO2. Objectives: To assess the quality level of transcutaneous blood gas measurements via SenTec monitor. Methods: A 12-month analysis of SenTec measurements was conducted in a Belgian Centre for Home Mechanical Ventilation (HMV). Over two consecutive nights; SpO2 and PCO2 measurements, the presence of PCO2 drift and drift correction with SenTec, were reviewed and scores (0, 1, 2 for poor, medium and high level) were assigned to estimate the quality of measurements. Results: Sixty-nine NMD patients met the inclusion criteria, of which 48/69 used HMV. PCO2 drift and drift correction were present in 15% and 68% of the 138 recordings, respectively. The quality level of measurements throughout night 1, scored 1.55 (0–2). The relevance of our clinical findings from SenTec scoring 1.94 (1–2); was considered highly satisfactory. HMV was ineffective in 24/48 patients. Among 12 patients with hypercapnia, 8 patients improved PCO2 between night 1 and 2. Among 12 patients with hypocapnia, PCO2 improved in 4/12 patients, who reached the range of normal PCO2 (35–47 mmHg). Conclusions: The quality of SenTec measurements was acceptable in the majority of recordings and clinical findings were deemed satisfactory in all cases. A single SenTec measurement was sufficient to determine the need for NIV. However, two SenTec registrations were insufficient to both improve NIV effectiveness in 50% of cases, and, to ensure follow-up of our interventions.

Abstract Transcutaneous carbon dioxide tension (tc-pco2) at 37, 39, 41, 43, and 45 degrees C, and transcutaneous oxygen tension (tc-po2) at 41, 43, and 45 degrees C were measured simultaneously in 10 healthy adults during hyperventilation and inhalation of O2/CO2 gas. Nine electrodes were applied to each subject: Five CO2 electrodes, one O2 electrode, and three combined O2/CO2 electrodes. The CO2 electrodes had negligible temperature coefficients in the calibration gases, but the O2 electrodes showed an increase in po2 of 4.5% per degree C. With skin application, tc-pco2 increased approximately 4% per degrees C between 37 and 45 degrees C, which is close to the anaerobic temperature coefficient of pco2 in blood. The tc-po2 increases on the skin with increasing temperature appeared to be more dependent on changes in blood flow in skin, but in the temperature range 43 to 45 degrees C, tc-po2 showed the expected decrease in the temperature coefficient with increasing po2. The correlation between transcutaneous and capillary pco2 was close at all transcutaneous electrode temperatures, even 37 degrees C, provided the skin was preheated (via the electrode) to 45 degrees C. For tc-po2, an electrode temperature of at least 43 degrees C was necessary to produce a reasonable correlation between tc-po2 and capillary po2. The combined O2/CO2 electrodes measured slightly higher pco2 values than the single CO2 electrodes, but there were no differences in po2 readings, stabilization time, imprecision, or electrode drift between the two electrode types. The imprecision (CV, %) of tc-pco2 and tc-po2 measurements was approximately twice that of the corresponding capillary blood-gas measurements.

The quality of air in sacral buildings is an important factor that affects users. Church buildings are characterized by a specific microclimate, because they accommodate many people in short periods of time, which results in rapid changes in air quality. Sacral buildings usually only have natural ventilation through doors, return air inlets and window leaks, which can cause uneven distribution of fresh air. The aim of this article is to analyze the results of research on the basic parameters of air quality: carbon dioxide, relative humidity and air temperature in the church. The analysis carried out on research results allowed to state that the measured parameters of air quality - the concentration of carbon dioxide and the level of relative humidity significantly exceeded the standards recommendations. High level of air humidity was recorded at 75%. In the analyzed sacral building there is the need to improve air quality in terms of carbon dioxide and relative humidity. Ways of improving air quality, such as installation of mechanical ventilation, will help prevent damage to structures and equipment, and improve comfort of people in the church.

Abstract The long-term precision and stability of a new quality-control system for blood pH and gas measurements are compared to that of tonometered bicarbonate solutions and serum-based preparations. The new system, consisting of gas-equilibrated bicarbonate solutions in glass ampuls, is shown to be as stable as the serum-based preparation, and as reproducible as either of the other methods. The new system, offering three discrete sets of control values, has certain advantages in the simultaneous quality of pH, carbon dioxide tension, and oxygen tension measurements.


The article addresses the concept of transport service quality as an essential tool for improving customer focus and the efficiency of rail transport in the implementation of cargo traffic. Issues have been investigated on current reforms going on in the railway transport market with regard to transport service. Valid procedures (both foreign and Russian ones) have been studied on freight owners’ evaluating transport service quality including All-Russian Project “Quality Index”. By this method, dynamics of change has been assessed in the fulfillment of basic indices of transport service quality, and causes of their inadequacy to ideal fulfillment have been described. Presented is the transport service quality management system for freight owners. The system is based on combination of natural assessment methods, optimization of quality level and assessment of efficiency of measures to increase the quality level. Based on logics of assuring transport service quality, a sequence has been proposed to analyze the transport quality indices systems: “quality of technical means → quality of operational work → quality of transport service”. Moreover, the quality of a subsequent system is the most objective characteristics of the previous system (systems) quality. It has been concluded that one of the major priorities in development of railway transport is to assure high-level quality of transport service. In addition to the most complete satisfaction of clients with railway transport services, the service quality makes it possible to increase attractiveness of railway companies in the transportation field and is the most important factor of competitiveness and the most promising for development as compared with alternative price factors.

The objective of this paper is an attempt to explain the determinants of the lowest governance quality level in one of the communes of the Opolskie Province, Poland. The first stage of the research consisted in developing a commune-level governance quality index in order to measure the quality of governance in the 60 communes of the Opolskie Province. Subsequently, the commune with the lowest score in the index was qualified for the second stage of the research which was based on the extreme case method. The major conclusion from the research is that the commune leader's governance style which allowed him to hold on to power for many terms of office was responsible for generating low governance quality. Furthermore, the low quality of governance was not only the effect of the governance style but also the strategy aimed at remaining in the commune leader office for many terms.

Research background and hypothesis. Replete schedule of competitions and intense training are features of contemporary team sports. Athletes, especially the most involved ones, may not have enough time to recover. As a consequence, aggregated fatigue can manifest in some undesirable form and affect athlete’s performance and health.Research aim. The aim of this study was to evaluate the changes in heart rate recovery (HRR) and investigate possible relations with sport-specifi c measures of effi cacy in professional basketball players during competition season.Research methods. Eight male high-level basketball players (mean ± SD, body mass, 97.3 ± 11.33 kg; height 2.02 ± 0.067 m, and age 23 ± 3.12 years) were investigated. The same basketball specifi c exercise was replicated several times from September till April during the practice sessions in order to assess the personal trends of HRR. Heart rate monitoring was performed using POLAR TEAM SYSTEM. Investigated athletes were ranked retrospectively according to the total amount of minutes played and the coeffi cients of effi cacy. Research results. There were signifi cant differences in the trends of HRR between the investigated players. The most effective players showed decreasing trends of HRR in all cases of ranking.Discussion and conclusions. Research fi ndings have shown that the quality of heart rate recovery differs between basketball players of the same team and could be associated with sport-specifi c effi cacy and competition playing time.Keywords: adaptation, autonomic control, monitoring training.

Investigation of failures is one of the critical activities of mining and haulage equipment operability assurance in mining. Maintaining failure investigation at the required quality level, it is possible to identify provisions, rules and procedures that should be revised or changed, operation conditions that should be improved, additional personnel training, if required, etc. Investigation of failures in mines is under responsibility of machine men and electricians of maintenance and operation services. In reality, factory management and setup for production condition weak concernment of these workers in quality investigation aimed at finding of sources of equipment failures. This article describes real-life results achieved in development and use of maintenance service operation, technology and management monitoring. The requirements are substantiated for quality improvement in failure cause finding and removal in mining and haulage equipment at Chernogorsky open pit mine, SUEK-Khakassia. Causes of the present quality of failure investigation by machine men of Chernogorsky Repair and Engineering Works and Chernogorsky open pit mine are revealed. The proposed recommended practices will improve quality of mining and haulage equipment failure investigation.

The continuous modernization of the education system makes the problems of the quality of teacher training increasingly relevant. Moreover, the measures taken to improve the system of teacher education are largely confined to the introduction of new organizational and managerial mechanisms and practically do not affect the internal content and technological structure of the teacher training process.Modern pedagogical universities are constantly looking for innovative models of training teachers that will be able to solve non-standard social and professional tasks. However, recent studies in this area do not fully take into account the nature of pedagogical activity and conditions of its formation. Thus, the need arises for a special study of the processes and means of updating the content and technologies of teacher training in order to control the level of students’ professional competencies development, as required by educational and professional standards. This means the creation of a special educational system in a pedagogical university, which can provide a harmonious and synchronous mastering by future specialists of both subject knowledge and methods of pedagogical activity.The article provides a theoretical study aimed at identifying key patterns of designing a new content for teacher education, the basis of which is the formation of a future teacher as a subject of his own professional activity. The author describes the experience of using a subject-oriented model of education, implemented at Bashkir State Pedagogical University n.a. M. Akmulla. The effectiveness of this model is confirmed by the high level of students’ mastery of designing methods and constructing the educational process, as well as their positive experience in the implementation of educational activities.

This study aims to describe the quality of the 2018 Mathematics National Examination (UN) in the HOTS types at the junior high level in terms of the level of validity, reliability, problem differentiation power, level of difficulty and distractor. This type of research is a descriptive study. The research was conducted at SMP Muhammadiyah 4 Surabaya and SMP Negeri 13 Surabaya for students in class VIII. The data collection technique used is a test. The test was taken from the 2018 math UN questions in odd semester VIII grade material including HOTS type. The number of UN mathematics questions in 2018 in the odd semester VIII class material consisted of 12 questions with 25% including LOTS types and 75% including HOTS types. The results showed: (1) 100% valid test questions, (2) high question reliability, (3) good problem differentiation power, (4) the difficuly level of the question 77,77% categorized as moderate and 2 question 22,23% are categorized as difficult, (5) there are 2 questions with one of the answer options do not work.
