Introduction. The development of telemedicine systems intended to remotely monitor the health status of patients constitutes one of the most promising areas of contemporary science. Particularly relevant is the use of such systems to track the health of surgically treated patients. Since such patients are mobile, sensors measuring vital signs should not cause any discomfort in regular use. Thus, wearable smart trackers can be used for these purposes.Aim. The paper aims to study the possibility of using smart trackers in a telemedicine system for monitoring patient health.Materials and methods. In the study, a series of measurements was performed using GSMIN WR 41 smart bracelets in order to estimate their accuracy when measuring pulse, blood pressure, and oxygen saturation for different research subjects at rest and during intense physical exercises. In addition, the average measurement interval of the smart bracelet was determined; the operation of the smart bracelet in the telemedicine system for monitoring vital signs was considered.Results. The studies show that an average error in the measurement of vital signs (except for systolic pressure) using the smart bracelet does not exceed 10 %. In order to avoid the high error in measuring systolic pressure, an algorithm for assessing the general health of patients was developed. In addition, it is shown that the smart bracelet can detect an abrupt change in the vital signs of patients, with the average time of their measurement and data transmission to the server of the telemedicine system coming to 45 seconds.Conclusion. The study results indicate that smart bracelets can be used to remotely monitor the health of patients in real-time. These findings are confirmed by the telemedicine system designed on the basis of the smart bracelets considered in the paper.

Telemedicine has become an increasingly important part of the modern healthcare infrastructure, especially in the present situation with the COVID-19 pandemics. Many cloud platforms have been used intensively for Telemedicine. The most popular ones include PubNub, Amazon Web Service, Google Cloud Platform and Microsoft Azure. One of the crucial challenges of telemedicine is the real-time application monitoring for the vital sign. The commercial platform is, by far, not suitable for real-time applications. The alternative is to design a web-based application exploiting Web Socket. This research paper concerns the real-time six-parameter vital-sign monitoring using a web-based application. The six vital-sign parameters are electrocardiogram, temperature, plethysmogram, percent saturation oxygen, blood pressure and heart rate. The six vital-sign parameters were encoded in a web server site and sent to a client site upon logging on. The encoded parameters were then decoded into six vital sign signals. Our proposed multi-parameter vital-sign telemedicine system using Web Socket has successfully remotely monitored the six-parameter vital signs on 4G mobile network with a latency of less than 5 milliseconds.

[Purpose] AHA/ACC guidelines recommend routine use of 12-lead ECG and advance notification for patients with acute coronary syndrome. However, transmission of out-of-hospital 12-lead ECG to emergency department is still not spread and ECG interpretation on the prehospital and emergency department is not established. Therefore, we have developed and tested the clinical usefulness of the mobile telemedicine system to transmit 12-lead ECG between moving ambulances and physicians in cardiovascular emergency. [Method] We set up the mobile telemedicine using the third-generation digital mobile phone to promote communications between an ambulance and diverse hospital. Compatibility issue among device vendors was solved by the implementation of open-standard medical waveform encoding rule with motion noise-reduction system. Real time 12-lead ECG was transferred together with vital signs and live video during transfer the patient by an ambulance. The performance of the mobile telemedicine system in the field-test was checked to transfer 12-lead ECG in different scenarios such as transferred ECG from a volunteer moving hand or leg, coughing or twisting body in an ambulance driving on common road or highway. In the next step, we set up the mobile telemedicine on an ambulance to promote communications between moving 5 ambulances in Suita-city and National Cardiovascular Center since 2008 June 2. To establish the efficacy of real-time transmission of out-of-hospital 12-lead ECG, the time-line from the onset of acute myocardial infarction to reperfusion is analyzed. [Results] Totally 36 patters of 12-lead ECG were checked in the field test and all of them were comparable than those original one in the ambulance. Time-delay for transmission of 12 lead ECG was within 10seconds and for one-lead ECG monitoring and vital signs including BP, HR and oxygen saturation was 1 second without the difficulties for the interpretation. [Conclusion] Those results indicate the usefulness and reliability of transmission of 12-lead ECG using the mobile telemedicine system from the ambulance. Further investigation is on-going to determine the efficacy in clinical conditions to reduce the treatment delay for acute myocardial infarction.

Over the past years, numerous Internet of Things (IoT)-based healthcare systems have been developed to monitor patient health conditions, but these traditional systems do not adapt to constraints imposed by revolutionized IoT technology. IoT-based healthcare systems are considered mission-critical applications whose missing deadlines cause critical situations. For example, in patients with chronic diseases or other fatal diseases, a missed task could lead to fatalities. This study presents a smart patient health monitoring system (PHMS) based on an optimized scheduling mechanism using IoT-tasks orchestration architecture to monitor vital signs data of remote patients. The proposed smart PHMS consists of two core modules: a healthcare task scheduling based on optimization and optimization of healthcare services using a real-time IoT-based task orchestration architecture. First, an optimized time-constraint-aware scheduling mechanism using a real-time IoT-based task orchestration architecture is developed to generate autonomous healthcare tasks and effectively handle the deployment of emergent healthcare tasks. Second, an optimization module is developed to optimize the services of the e-Health industry based on objective functions. Furthermore, our study uses Libelium e-Health toolkit to monitors the physiological data of remote patients continuously. The experimental results reveal that an optimized scheduling mechanism reduces the tasks starvation by 14% and tasks failure by 17% compared to a conventional fair emergency first (FEF) scheduling mechanism. The performance analysis results demonstrate the effectiveness of the proposed system, and it suggests that the proposed solution can be an effective and sustainable solution towards monitoring patient’s vital signs data in the IoT-based e-Health domain.

IntroductionEmergency department (ED) overcrowding is a major healthcare problem associated with worse patient outcomes and increased costs. Attempts to reduce ED overcrowding of patients with cardiac complaints have so far focused on in-hospital triage and rapid risk stratification of patients with chest pain at the ED. The Hollands-Midden Acute Regional Triage—Cardiology (HART-c) study aimed to assess the amount of patients left at home in usual ambulance care as compared with the new prehospital triage method. This method combines paramedic assessment and expert cardiologist consultation using live monitoring, hospital data and real-time admission capacity.Methods and analysisPatients visited by the emergency medical services (EMS) for cardiac complaints are included. EMS consultation consists of medical history, physical examination and vital signs, and ECG measurements. All data are transferred to a newly developed platform for the triage cardiologist. Prehospital data, in-hospital medical records and real-time admission capacity are evaluated. Then a shared decision is made whether admission is necessary and, if so, which hospital is most appropriate. To evaluate safety, all patients left at home and their general practitioners (GPs) are contacted for 30-day adverse events.Ethics and disseminationThe study is approved by the LUMC’s Medical Ethics Committee. Patients are asked for consent for contacting their GPs. The main results of this trial will be disseminated in one paper.DiscussionThe HART-c study evaluates the efficacy and feasibility of a prehospital triage method that combines prehospital patient assessment and direct consultation of a cardiologist who has access to live-monitored data, hospital data and real-time hospital admission capacity. We expect this triage method to substantially reduce unnecessary ED visits.

Statistical modeling of historical crash data can provide essential insights to safety managers for proactive highway safety management. While numerous studies have contributed to the advancement from the statistical methodological front, minimal research efforts have been dedicated to real-time monitoring of highway safety situations. This study advocates the use of statistical monitoring methods for real-time highway safety surveillance using three years of crash data for rural highways in Saudi Arabia. First, three well-known count data models (Poisson, negative binomial, and Conway–Maxwell–Poisson) are applied to identify the best fit model for the number of crashes. Conway–Maxwell–Poisson was identified as the best fit model, which was used to find the significant explanatory variables for the number of crashes. The results revealed that the road type and road surface conditions significantly contribute to the number of crashes. From the perspective of real-time highway safety monitoring, generalized linear model (GLM)-based exponentially weighted moving average (EWMA) and cumulative sum (CUSUM) control charts are proposed using the randomized quantile residuals and deviance residuals of Conway–Maxwell (COM)–Poisson regression. A detailed simulation-based study is designed for predictive performance evaluation of the proposed control charts with existing counterparts (i.e., Shewhart charts) in terms of the run-length properties. The study results showed that the EWMA type control charts have better detection ability compared with the CUSUM type and Shewhart control charts under small and/or moderate shift sizes. Finally, the proposed monitoring methods are successfully implemented on actual traffic crash data to highlight the efficacy of the proposed methods. The outcome of this study could provide the analysts with insights to plan sound policy recommendations for achieving desired safety goals.

The present study investigated what complaints are prominent in psychologically distressed Vietnamese in Vietnam beyond standard symptoms assessed by Western diagnostic instruments for anxiety and depression. To form the initial Vietnamese Symptom and Cultural Syndrome Addendum (VN SSA), we reviewed the literature, consulted experts, and conducted focus groups. The preliminary VN SSA was then used in a general survey (N = 1004) of five provinces in Vietnam. We found that the VN SSA items were highly and significantly correlated with a measure of anxious-depressive psychopathology (a composite measure of the General Anxiety Disorder-7; Posttraumatic Diagnostic Scale; and Patient Health Questionnaire-9). The VN SSA item most highly correlated to anxious-depressive psychopathology was “thinking a lot” ( r = .54), reported by 15.8% of the sample. Many other symptoms in the addendum also were prominent, such as orthostatic dizziness (i.e., dizziness upon standing up; r = .41), reported by 22.9% of the sample. By way of comparison, somatic complaints more typically assessed to profile Western anxious-depressive distress, such as palpitations, were less prominent, as evidenced by being less strongly correlated to Western psychiatric symptoms and being less frequent (e.g., palpitations: r = .31, 7.1% of the sample). Study results suggest that to avoid category truncation when profiling anxious-depressive distress among Vietnamese that items other than those in standard psychopathology measures should also be assessed.

Methodology/Technique - The research subjects were all first national ranking tournament badminton players in 2012. The study uses convenience sampling to conduct a questionnaire survey. 350 questionnaires were distrubuted, with 288 valid questionnaires being returned (male 195, female 93). The rate of effectiveness was 82%. All of the data collected was analyzed using descriptive statistic and hierarchical regression analysis. Findings – The research subjects were all first national ranking tournament badminton players in 2012. The study uses convenience sampling to conduct a questionnaire survey. 350 questionnaires were distrubuted, with 288 valid questionnaires being returned (male 195, female 93). The rate of effectiveness was 82%. All of the data collected was analyzed using descriptive statistic and hierarchical regression analysis. Novelty – The study highlights that badminton players who participate in leisure-based activities are able to effectively adjust their perception of pressure to a "reduced sense of accomplishment" which has an effect on athletic burnout. Additionally, the study results give rise to variety of proposed solutions or suggestions for dealing with burnout or stress. High-quality sleep is essential to our daily lives, and real-time monitoring of vital signs during sleep is beneficial. Current sleep monitoring solutions are mostly based on wearable sensors or cameras, the former is worse for sleep quality, the latter is worse for privacy, dissimilar to such methods, we implement our sleep monitoring system based on COTS WiFi devices. There are two challenges need to be overcome in the system implementation process: First, the torso deformation caused by breathing/heartbeat is weak, how to effectively capture this deformation? Second, movements such as turning over will affect the accuracy of vital signs monitoring, how to quickly distinguish such movements? For the former, we propose a motion detection capability enhancement method based on Rice-K theory and Fresnel theory. For the latter, we propose a sleep motion positioning algorithm based on regularity detection. The experimental results indicated the performance of our method.
