Choledochal Cyst is a relatively rare condition. Even rarer is a choledochal cyst in association with a gallbladder carcinoma. This study reports a rare case of choledochal cyst coexisting with gallbladder carcinoma in a Bangladeshi patient. A 35 year old lady presented at IBN Sina Medical College Hospital, Kallyanpur, Dhaka with the history of recurrent right upper quadrant abdominal pain from childhood, which became severe for last 4 days before admission. The pain was colicky in nature and radiated to the back. Episodes were associated with low-grade fever, anorexia as well as vomiting. The preoperative diagnosis was made by abdominal ultrasound and MRCP. Exploratory laparotomy, enbloc cholecystectomy with excision of the choledochal cyst and roux-en-Y hepaticojejunostomy was also done. Post operative recovery was uneventful. Patient was followed up for six months and no obvious complication was noticed. Early suspicion of this rare pancreato-billiary disease is important because surgical treatment is the only way to avoid the complications of the disease.Bangladesh Journal of Medical Science Vol.16(4) 2017 p.602-605

Splenic cysts are rarely detected, and clinically diagnosed commonly as spleenomegaly . Thenature of the cyst could not be diagnosed pre-operatively because of its’ anatomical location andonly be confirmed by the histopathological assessments. These cysts are found as a sequel ofsplenic infarct followed by the abscess formation. This is a very rare condition of a minor boy ofonly 10 years’ old who presented with a hugely enlarged spleen. The common investigation(s)did not prove that it was in the haematological conditions. Ultrasonogram (USG) investigationshows spleenomegaly due to the cystic lesion. CT scan of the abdomen showed large andwell defined cystic mass which was 13.4cmX7.9cmX15.3cm in size, multi-loculated, septated,sharply demarcated as such well delineated cyst. After necessary preparation like vaccination,blood grouping, cross matching and other fitness, laparotomy and total spleenectomy was doneas the cyst occupied most of the Splenic tissue. The post operative recovery was uneventful andthe final diagnosis was confirmed by histopatholgical examinations as epidermoid cyst of thespleen. A girl named Tania, aged 5yrs was brought to Shaheed Ziaur Rahman Medical College Hospital, Bogra with the complains of swelling of both legs for 5days & low grade intermittent fever for 1month & she also had severe mental retardation, facial dysmorphism, hepatosplenomegaly, umbilical hernia, corneal clouding, large calvaria & features of dysostosis multiplex. Background: Tendo Achilles injuries are usually related to poor ankle flexibility and strength and to overuse. Morphology of tendo Achilles is an important tool for its assessment which can be done by measuring tendon thickness, cross-sectional area and length of tendon. Objective: The aim of the study was to correlate length of tendo Achilles with stature in several age groups of sedentary people as measured by ultrasonography. Materials and method: This cross sectional analytical type of study was conducted in the department of Anatomy, Dhaka Medical College Hospital, Dhaka, Bangladesh from July 2013 to June 2014. The present study was performed on 200 tendo Achilles of 100 people (50 male and 50 female) randomly selected from the patients who came to the Radiology department of Dhaka Medical College Hospital for ultrasonography of any region of their body other than leg. Results: In this study length of tendo Achilles was measured in both legs of adult people and was correlated with stature. Highly significant correlation was found between length of tendo Achilles with stature (p<0.001). Conclusion: The data of length of tendo Achilles obtained from the present study may provide valuable information in different aspects of medical science as a guide line for physiotherapists, radiologists, sports professionals and ortho-surgeons in early detection and monitoring of rehabilitation especially of professional athletes. Choledochal cyst is an aneurysmal dilatation of the bile duct. It is a rare condition. Here we report a case of choledochal cyst in a 15 year-old female patient. She presented to the out patient clinic with the complaints of recurrent pain and lump in the upper abdomen along with fever for 1 year. Introduction: Visual inspection of cervix after application of 3-5% acetic acid (VIA) is a potential alternative to Pap smear cytology for screening of cervical cancer in resource poor settings. VIA has gained popularity and proven itself in many clinical trials as an adequate screening test. VIA is an attractive alternative to Pap smears for its ease of use, low-cost and fewer physician visits. Currently VIA is done in tertiary level hospitals by trained health care providers to generalize its efficacy.Objective: The main objective of this study was to evaluate the efficacy of visual inspection based screening approach in the detection of precancerous & early cancerous lesions of the cervix.Materials and Methods: This study was done as a part of an ongoing screening program in Rajshahi Medical College Hospital from July 2008 to December 2009. VIA was carried out in 540 eligible women attending Gynae OPD for gynecological problems. The women underwent a complete clinical evaluation. Detection of well-defined, opaque, acetowhite lesion close to squamocolumnar junction (SCJ) or in transitional zone (TZ), well-defined circumferential, acetowhite lesions or dense acetowhitening of ulceroproliferative growth on the cervix constituted a positive VIA. All screened women evaluated by colposcopy and biopsy were taken from colposcopically suspected areas or in cases of VIA negative from different quadrants of the cervix. The final diagnosis was based on histology, which allowed direct estimation of sensitivity, specificity, and predictive values of VIA. Those with CIN I or cancerous lesions diagnosed by histology were considered as true positive.Results: Out of 540 patients screened, 328(61%) were VIA negative and 212(39%) were VIA positive. Out of positive cases 87 (41%)%) seemed to have pathology. Colposcopy yielded normal results in 340 (63%) cases, low grade CIN in 138 (26%) cases, high grade CIN in 44 (8%) cases and cancer in 18 (3%) cases making total 200 cases. Of the 200 (37%) patients with positive colposcopy, 98 (49%) turned out to be negative on histology. There were biopsy proven chronic cervicitis and metaplastic changes in 423 (78%) cases, CIN I in 66 (12%) cases, CIN II in 25 (5%) cases, CIN III/ carcinoma-in-situ in 5 (1%) cases. Eighteen (3%) cases of cervical carcinoma were diagnosed on colposcopy but ultimately 21(21%) cases of invasive cancer were detected on histology. The sensitivity of VIA for biopsy proven CIN I was 74.36%, specificity 75.8%, positive predictive value 41.04%, & negative predictive value 90.85%.Conclusion: VIA can differentiate a normal cervix from a precancerous cervix with reasonable accuracy. Till now a good number of studies had been carried out in different countries of the world and now it is well established that the sensitivity of VIA equaled or exceeded the reported rates for conventional cervical cytology. A 10 year old boy suffering from prolonged low grade fever, progressive pallor, one episode of haematemesis and melaena, was found to have hepatospenomegaly, features of portal hypertension on abdominal ultrasound, and grade II varices in upper gastrointestinal endoscopy. During hospital stay for diagnostic workup, he developed features of hepatic failure and pancytopenia. Bone marrow aspirate revealed hemophagocytosis and plenty of Leishman-Donovan bodies. The child received Injection Sodium Stibogluconate to treat leishmaniasis and received supportive therapy for hepatic failure and pancytopenia. The incidence of a normal live foetus along with a partial molar placenta is extremely rare. Although triploidy is the most frequent association, a foetus with normal karyotype can survive in cases of partial molar pregnancy. A case is reported in the Department of Obstetrics & Gynaecology, Dhaka Medical College Hospital, Dhaka that of partial molar placenta in which a live male baby was delivered at 34 weeks gestation by a 25 years old woman. At the 23rd week, ultrasonographic examination revealed a normal foetus with a huge, multicystic placenta. There were per vaginal bleeding up to the 34th gestational weeks. The patient underwent an emergency caesarean section and delivered a 1.6 kg male baby. The baby was admitted in SCABU. The foetus was normal and no complication was there. Background : Poor genital hygiene has been responsible for high prevalence of excessive vaginal discharge. Leucorrhoea is the clinical evidence of infection and can be treated satisfactorily whenever diagnosed. Occurrence of various cervical epithelial lesion associated with discharge can be easily prevented if detected early. Majority of the patients with various cervical epithelial lesion attending in the hospital present with varying degree of vaginal discharge and cytological cellular aberrations in the cervical epithelium.Objective : The aim of the present study is to observe the various patterns of cervical epithelial lesions in cytopathology and associated infections in patients with Leucorrhoea.Methodology : This observational study was done over a period of ten months in Popular Medical college hospitals of Bangladesh. A total of 230 cases were included in the study with women of age range 15-45 years, complaining of leucorrhoea. After a thorough vaginal examination Pap smears were taken and immediately fixed in absolute alcohol and stained according to the papanicolaous technique. The cytopathological changes observed in the cervical smears were graded according to the Bethesda system for reporting cervical cytology.Results : Out of 230 cases the cytological patterns were found Normal in 4 (1.7%), Inflammatory 204 (88.3%), Low grade squamous intraepithelial (LSIL)10 (4.8%), High grade squamous intraepithelial (HSIL) 6(2.6%), Atypical squamous cells of undetermined significance (ASCUS) 6 (2.6%) and the distribution of different types of pathogens detected were Trichomonas 33 (70.62%), Candida albicans 19 (40.66%).Conclusion : In this study significant numbers of Leucorrhoea patients were detected with cervical squamous intraepithelial lesions (SIL) in which LSIL (Low grade squamous intraepithelial) was higher. Associated infections were Trichomonas and Candida albicans. Therefore cytological screening for early detection of cervical squamous intraepithelial lesion is essential routine medical examination for Leucorrhoea patients in gynaecological practice.Northern International Medical College Journal Vol.9(1) July 2017: 252-254

Objectives: To investigate the clinical characteristics, treatment and prognosis of leiomyomatosis peritonealis disseminata (LPD) following laparoscopic surgery with uncontained morcellation and to summarize clinical features of iatrogenic LPD based on published literature together with our own experience.Methods: A cohort of 13 cases with iatrogenic LPD diagnosed and treated in Peking Union Medical College Hospital from 2011 to 2020 was reported focusing on clinical characteristics, treatment and prognosis.Results: All the patients had a history of laparoscopic myomectomy with uncontained morcellation. The average age was 35.6 (range 25–47) years. The interval between initial laparoscopic surgery and first diagnosis of LPD was 6.08 years on average (range 1–12). Most of the patients had no obvious symptoms. The accuracy of pre-operative diagnosis was low. Two patients had been treated with gonadotropin-releasing hormone agonist (GnRH-a) before surgery without obvious effect. The nodules of LPD are usually located in the lower half of the peritoneal cavity. The most commonly involved site was the pouch of Douglas. The number of nodules ranged from 3 to over 10, and they ranged in size ranged from 0.3 to 22 cm. All patients underwent surgical treatment: six patients underwent laparoscopy and seven underwent laparotomy. Pathology results confirmed LPD. The immunohistochemical profile indicated LPD tends to be positive strongly for desmin, caldesmon, ER, PR and SMA. Only one patient underwent post-operative treatment with GnRH-a. All patients were followed for an average period of 49 months without recurrence.Conclusion: Iatrogenic LPD is a relatively rare condition. Patients usually exhibit no hormonal stimulation factors. Surgery is the main method of treatment, and hormone suppressive therapy is only rarely used. The nodules are usually large and less numerous, and most involve the pelvis. The prognosis of iatrogenic LPD seems good.

Background: Polycystic ovary syndrome (PCOS) is a complex medical condition characterized by elevated androgen levels, menstrual irregularities, and small cysts on one or both ovaries. The prevalence of PCOS is 6 to 10% in women. C-reactive protein (CRP) is an acute-phase protein produced by the hepatic cells and it’s levels increased in inflammation that increases interleukin-6 by macrophages and T cells. Aims and Objective: A correlative study of role and usefulness of CRP in women with PCOS and its correlation with different biochemical parameters. Materials and Methods: This was hospital based case-control study carried out among PCOS was conducted in the Department of Obstetrics & Gynaecology, Index Medical College Hospital, Indore. This study was conducted from 1st January 2018 to 31st December 2019. A total of 260 subjects with age group between 15 to 45 years were divided into two group; cases (130) and controls (130). Results: Among a total of 260 cases, based on clinical and different biochemical parameters, 130 were diagnosis with PCOS and 130 were apparently healthy women. The mean ±SD of various parameters among PCOS cases were; body mass index (BMI) 32.97±8.466, P (<0.001) total cholesterol (TC) 188.42±31.126, P (<0.001), triglyceride (TG) 134.43±50.01, P (<0.001), high density lipoprotein (HDL) 36.29±9.55 TC/HDL ratio, 5.54±1.865 serum CRP, 3.41±0.94 versus BMI 22.87±2.470, P (<0.001), TC155.42±26.333, TG 110.00±42.19, HDL 41.22±10.912, TC/HDL ratio 4.08±1.39, serum CRP 2.25±0.83 P (<0.001) in healthy control. Conclusion: In this study, the role of inflammation and different biochemical markers were studied among PCOS. It was found that a majority of PCOS patients were obese, having insulin resistance. The levels of CRP as a marker of chronic low grade inflammation were higher in newly diagnosed PCOS as compared to the controls. The CRP values correlated well (statistically significant) with increased in BMI and age.
