A Candida albicans é a espécie presente na cavidade oral responsável por cerca de 95% das candidíases, sendo considerada o principal fator etiológico da estomatite protética, condição inflamatória que acomete a mucosa oral coberta pela base da prótese. O uso de antifúngicos demonstra grande eficácia no tratamento da candidíase oral, apesar de alguns efeitos colaterais, como gosto desagradável, alergias e reações gastrointestinais. Com isso, a fitoterapia vem se destacando como terapia alternativa devido ao seu poder antimicrobiano e a redução de efeitos adversos. Desta forma, o objetivo desta revisão de escopo é explorar sistematicamente a literatura, mapeando e sumarizando as evidências disponíveis quanto aos produtos naturais utilizados no tratamento da candidíase oral. Esse protocolo foi desenvolvido de acordo com as recomendações do Manual do Instituto Joanna Briggs e registrado na Open Science Framework. A busca pelos artigos será realizada em três etapas, utilizando as bases de dados PubMed, Scopus, Web of Science, Embase, Cochrane Library e LILACS, bem como fontes adicionais da literatura cinzenta e lista de referências dos estudos incluídos. Os termos “phytotherapy”, “biological products”, “oral candidiasis", “denture stomatitis”, “candida albicans”, e palavras relacionadas serão usadas na estratégia de busca. Os dados serão extraídos através de um formulário e apresentados na forma de quadro e/ou imagens e de forma descritiva. Este protocolo auxiliará o desenvolvimento da revisão de escopo para a identificação de lacunas e síntese do conhecimento para a condução de futuros estudos primários.

Abstract Background: Despite evidence that loneliness increases during times of transition, and that the incidence of loneliness is highest in young adults, loneliness during pregnancy and new parenthood has not been developed as a program of research. Because loneliness research has primarily focused on older adults and other high-risk populations, the concept of loneliness and its effects on this population are not well understood, leaving a gap in our understanding of the psychosocial needs and health risks of loneliness on pregnant people and new parents. A scoping review has been completed in order to map and synthesize the literature to date on loneliness experienced during pregnancy and the first five years of parenthood.Methods: To address the aim of this review, a wide net was cast in order to detect experiences of perinatal or parental loneliness, and/or instances where loneliness was measured in this population. Among the inclusion criteria were loneliness in people who were pregnant, who were parents in the postpartum period, or who had children aged five years or younger. A search for literature was conducted in December 2020 using nine databases: MEDLINE (Ovid), EMBASE (embase.com), SCOPUS (scopus.com), Cochrane Library including CENTRAL (Wiley), CINAHL (Ebscohost), PsycINFO (Ebscohost), Dissertations & Theses Global (ProQuest) and Sociological Abstracts (ProQuest) and the Web of Science Core Collection (Clarivate).Discussion: Perinatal and parental loneliness studies are limited and have rarely been targeted and developed through a program of research. Loneliness inquiry in this population has frequently been studied in relation to other concepts of interest (e.g., postpartum depression). Alternatively, the importance of loneliness has emerged from study participants as relevant to the research topic during qualitative inquiry. Across studies, the prevalence of loneliness ranged from 32% to 100%. Loneliness was commonly experienced alongside parenting difficulties, with parents feeling as though they were alone in their struggles. As loneliness has been called a sensitive indicator of wellbeing, we believe screening will help healthcare professionals identify common difficulties and early signs of depression experienced during pregnancy and parenthood.The protocol is available on Open Science Framework at DOI 10.17605/OSF.IO/BFVPZ.

O objetivo deste protocolo é garantir a transparência do processo da revisão de escopo, pré-definindo os objetivos, os métodos, os critérios de elegibilidade das fontes de evidências e os dados relevantes a respeito das reações adversas em mucosa oral e face provocadas pelo uso dos produtos naturais. Este protocolo foi registrado na Open Science Framework e seguiu as recomendações do Manual do Instituto Joanna Briggs. A estratégia de busca será estruturada para o PubMed e posteriormente adaptada para as outras bases de dados, como Scopus, Web of Science, Embase, LILACS e LIVIVO, além da literatura cinzenta (Google Scholar e OpenGrey), utilizando os seguintes termos e suas palavras relacionadas: “Biological Products”, “Natural Products”, “Phytotherapy”, “Chemical Burn”, “Adverse Effect”, “Oral mucosa” e “Face”. Adicionalmente, será feita a busca nas listas de referências dos artigos incluídos. Relatos de caso que apresentem reações adversas à mucosa oral e à face, sem distinção de sexo, idade ou etnia dos pacientes serão incluídos na revisão. Estudos clínicos, in vitro, em modelos animais, revisões de literatura e artigos de opinião serão excluídos. A seleção dos estudos será conduzida em duas fases por dois examinadores independentes e participação do terceiro revisor em caso de discordância. Os dados também serão extraídos de maneira independente, com o uso de formulário desenvolvido pelos próprios autores, e apresentados em tabela e/ou figuras e de forma narrativa. Este protocolo auxiliará no desenvolvimento da revisão de escopo, para mapeamento e sumarização das evidências.

O envelhecimento da população faz surgir a necessidade de cuidados especiais com a saúde. O edentulismo, por exemplo, é uma condição bucal que afeta, principalmente, os idosos, sendo comum o uso de próteses dentárias que muitas vezes geram lesões orais. Este protocolo propõe uma revisão de escopo sobre a prevalência de lesões na mucosa oral em decorrência do uso de prótese dentária na população idosa. Desse modo, o objetivo do referido protocolo é apresentar a metodologia que será adotada, de acordo com as recomendações de estruturação do Joanna Briggs Institute para revisões de escopo, sendo este documento previamente registrado no repositório Open Science Framework. A estratégia de busca será elaborada de modo a identificar as possíveis fontes de evidências publicadas nas seguintes bases de dados: Cochrane, PubMed via MEDLINE, LILACS, Web of Science e Scopus. No que se refere à literatura cinzenta, serão feitas buscas através do Google Scholar. Serão utilizados os seguintes descritores do vocabulário controlado Descritores em Ciências da Saúde/Medical Subject Headings: idoso/aged, prótese dentária/dental prosthesis e manifestações bucais/oral manifestations, seus sinônimos e variações ortográficas. A estratégia PCC (População, Conceito, Contexto) servirá de base para elaboração da pergunta de pesquisa e definição dos critérios de inclusão e exclusão. Os resultados da revisão de escopo proposta deverão mostrar os fatores que influenciam a prevalência de lesões em mucosa oral de pessoas idosas que fazem uso de prótese dentária.

Candida albicans is the most common Candida species isolated from the oral cavity both in healthy and diseased. Candida albicans is a dimorphic fungus existing both in blastopore phase (yeast phase) and the hyphal or mycelial phase. Although these organisms typically colonize mucocutaneous surfaces, the latter can be portals of entry into deeper tissues when host defences are compromised. Denture stomatitis is a common form of oral candidiasis that manifests as a diffuse inflammation of the maxillary denture bearing areas & is associated with angular cheilitis. At least 70% of individuals with clinical signs of denture stomatitis exhibit fungal growth & these conditions most likely result from yeast colonization of the oral mucosa combined with Bacterial colonization. Candida species act as an endogenous infecting agent on tissue predisposed by chronic trauma to microbial invasion. At one time, oral fungal infections were rare findings in general dentist's office. They were more commonly seen in hospitalized and severely debilitated patients. However with enhanced medical and pharmaceutical technology, increasing numbers of ambulatory immunosuppressed individuals with oral fungal infections are seeking out general dentists for diagnosis and treatment of these lesions.

Abstract Background One reason that asthma remains poorly controlled in children is poor inhaler technique. Current guidelines recommend checking inhaler technique at each clinical visit. However, they do not specify how best to train children to mastery of correct inhaler technique. Currently many children are simply shown how to use inhalers (brief intervention) which results in less than 50% with correct inhaler technique. The aim of this scoping review is to explore published literature on teaching methods used to train children to master correct inhaler technique. Methods This scoping review will follow the Arksey and O'Malley framework and the Joanna Briggs Institute guidelines. We will search (from inception onwards): MEDLINE, Embase, Scopus, Web of Science, CINAHL and the Cochrane library. We will include quantitative studies, (e.g. randomised controlled trials, cohort studies and case-control studies), published from the year 1956 to present, on teaching the skill of inhaler technique to children with asthma. Two reviewers will complete all screening and data abstraction independently. Data will be extracted onto a data charting table to create a descriptive summary of the results. Data will then be synthesised with descriptive statistics and visual mapping.Discussion This scoping review will provide a broad overview of currently used educational methods to improve inhaler technique in children with asthma. The analysis will allow us to refine future research in this area by focusing on the most effective methods and optimising them. Objective: this study aimed to investigate the main therapeutic options for denture stomatitis in patients using complete removable prostheses, to determine the best treatment method, avoiding relapses and improving patients' quality of life. Material and Methods: a search was made in the databases and electronic search tools PubMed, LILACS, SciELO and Web of Science using keywords, in Portuguese and English, such as denture stomatitis, Candida albicans, antifungal, oral candidiasis, and treatment. Results: studies have shown that the main drugs used in the treatment of denture stomatitis are miconazole, fluconazole, itraconazole, nystatin, amphotericin B, ketoconazole, clotrimazole and chlorhexidine. Regardless of the treatment employed, there is an improvement in the disease’s clinical symptoms, but it does not prevent recolonization of the oral mucosa in patients with poor hygiene of the dental prosthesis. Conclusion: it is not possible to determine the best therapy for denture stomatitis, since the studies evaluated showed drug efficacy, but with relapses, especially in patients with poor hygiene of the dental prosthesis.

Denture stomatitis is a common manifestation of oral candidiasis affecting some 65% of denture wearers. This condition is initiated by the adherence of Candida albicans to denture base acrylic resin. The present study aimed to test the in vitro effect of traditional and novel fabrication methods on Candida albicans adhesion to denture base samples. Denture based acrylic discs were fabricated using: (i) computerized milling, (ii) 3D printing, (iii) heat curing, and (iv) cold curing. Discs were tested for surface roughness (Ra), hydrophobicity (contact angle), mucin adsorption (Bradford assay), and Candida albicans adhesion. 3D printing significantly increased microbial cell adhesion as compared with heat curing, and computerized milling significantly decreased it. These results were associated with mucin adsorption levels rather than surface roughness. Results suggest that 3D printing may increase the risk for developing denture stomatitis, whereas computerized milling may decrease it as compared with traditional heat curing denture base fabrication.

A polifarmárcia é o termo aplicado para referir-se ao uso de múltiplas combinações medicamentosas e, atualmente, tem recebido muita atenção devido aos inúmeros impactos à saúde e à frequência com a qual é utilizada. Dessa forma, a polifarmácia é um problema crítico, passível de atingir qualquer pessoa, sendo mais danosa para os idosos devido aos potenciais riscos de eventos adversos e colaterais, que podem influenciar na saúde sistêmica e bucal. O objetivo desse protocolo é apresentar etapas metodológicas para realização de uma revisão de escopo, a fim de explorar os impactos da polifarmácia na saúde bucal de idosos, mapeando e sumarizando as evidências quanto às manifestações e quadros patológicos mais recorrentes devido à condição estudada, além de verificar como os estudos vêm sendo conduzidos e identificar lacunas na literatura. Esse protocolo foi desenvolvido de acordo com as recomendações do Manual do Instituto Joanna Briggs e registrado na Open Science Framework. Serão utilizadas as bases de dados PubMed via MEDLINE, Science Direct (Elsevier), Web of Science, Scopus, LILACS e Google Scholar para busca dos estudos. Os dados serão extraídos e apresentados na forma de quadros e/ou imagens e de forma descritiva, conforme indicado no checklist proposto pelo PRISMA-ScR. Espera-se que esse protocolo possibilite a realização de uma revisão de escopo, com metodologia consistente e, a partir dela, a obtenção de um maior conhecimento acerca dos impactos da polifarmácia na saúde bucal de idosos, ampliando a visão que se tem sobre o tema.

Abstract Background: The stepped wedge cluster randomised trial is an increasingly common trial design. The design can be useful for informing real-world clinical decision-making, including decisions about the effectiveness of interventions in particular subgroups. However, there is little existing guidance about how to perform subgroup analyses in the stepped wedge design. We aim to determine the prevalence of subgroup analyses and describe statistical methods used to perform them in stepped wedge cluster randomised trials.Methods: We will conduct a systematic review following the methodology recommended in the Cochrane Handbook for Systematic Reviews of Interventions. We report this protocol according to the PRISMA-P checklist. The protocol has been registered in the Open Science Framework. We will search for terms related to ‘stepped wedge’. Sources will be PubMed, Embase, PsycINFO, Web of Science, CINAHL, Cochrane Library, and Current Controlled Trials Register up to 16 October 2020. Studies will be eligible if they are written in English, involve human participants and are primary or secondary reports of planned or completed stepped wedge cluster randomised trials. Two reviewers will first screen the titles and abstracts, then full texts, to select studies that should be included in the review. Disagreements will be solved by consensus through discussion with a third reviewer. We will extract data related to study characteristics including presence or absence of subgroup analyses, characteristics of subgroup variables examined, statistical methods used to perform subgroup analyses, and adherence to the most consistently recommendations suggested for subgroup analyses in general including in clinical trials. We will perform a qualitative synthesis of the extracted data.Discussion: This protocol offers a reproducible and transparent procedure for a systematic review of the literature. It will provide a portrait of the frequency and types of subgroup analyses performed in stepped wedge cluster randomised trials. These results will inform the development of recommendations for subgroup analyses in such trials.Systematic review registration: This protocol has been registered on Open Science Framework, Registration ID: https://osf.io/2kwrz.

The scarcity of parenthood-loneliness inquiries leaves a gap in our understanding of new parenthood and its effects on the health and well-being of parents and their children. Here, a scoping review protocol will be presented to address this gap. The objective of this study will be to summarize the current knowledge on loneliness experienced during pregnancy and by parents during the postpartum period through the first five years of the child’s life.MethodsA scoping review protocol was designed following Arksey and O’Malley’s framework. We will include all types of literature in English, including all study designs, reviews, opinion articles, dissertations, reports, books, and grey literature. To be considered for inclusion, sources should focus on loneliness in pregnant persons, postpartum people, and parents of children five years or younger. We will search the following electronic databases (from inception onwards): MEDLINE, EMBASE, CINAHL Complete, Cochrane Library, PsycINFO, Dissertations & Theses Global, Sociological Abstracts, Scopus, and Web of Science. Grey literature will be identified searching the British governmental website gov.uk, the Jo Cox Commission on Loneliness, the Campaign to End Loneliness, and the British Red Cross’s Action on Loneliness websites. Two reviewers, working independently of each other, will screen the titles and abstracts of the articles returned by the searches, then screen the selected full-text articles, and extract data. A third reviewer will cast the deciding vote in case no consensus is reached. Results will be given in narrative form, mapped, and illustrated.DiscussionThis scoping review will capture the state of the current literature on loneliness in pregnancy and new parenthood. Results will be published in a peer-reviewed journal. We anticipate that the study will identify gaps and make recommendations for future areas of study and related interventions.
