Repairing bone defects in oral surgery often requires the use of bone regeneration techniques. Silicon is an element that has been employed as regeneration material in several studies. In our study, silicon was combined with autologous bone and platelet-rich fibrin (PRF) membranes to analyse the behaviour of this element in bone regeneration. Four circumferential defects were created in the cranial vault of five New Zealand rabbits. The following elements were applied to the regeneration of the defects: (P): PRF; (S): silicon and (B): autologous bone, with the following distribution of study groups: Group 1 (PSB); Group 2 (PS); Group 3 (SB) and Group 4 (CONTROL): unregenerate group. The animals were sacrificed after 3 weeks. Computed microtomography studies (μ-CT) were carried out, as well as histomorphometric ones. The ANOVA statistical test was used with a Bonferroni post-hoc test to compare the results (p ≤ 0.05). Radiologically, groups PSB and SB were better as far as quantity and percentage of healthy bone observed, but not significantly compared to the control group. The PS group was significantly worse. The histological test revealed that the PSB group was the one to present the largest area, percentage and perimeter of mineralised bone. On evaluating the forming bone (osteoid), no difference was observed across the groups with the exception of the bone perimeter, where the SB group was significantly better. The bone height variable showed no significant differences. In conclusion we can state that the combination of PRF, autologous bone and silicon provides good results at 3 weeks whilst the PS group shows the worst results. This highlights the importance of autologous bone forming part of the graft material in order for the bone to mineralise.

Our study was aimed to analyze the osteoinductive effect of powdered and block type autogenous bone graft along with bone morphogenetic protein (BMP-2) as compared to synthetic bone graft. Three circular bicortical defects were made in the calvaria of each rabbit and randomly divided into three groups as follows: powdered tooth biomaterial+BMP-2, block tooth biomaterial+BMP-2, and control group: synthetic bone+BMP-2. The samples taken from these defects after 4 and 8 weeks were analyzed histologically along with micro CT analysis. In our study, both powered and block type tooth autogenous bone graft successfully stimulated mesenchymal cells leading to endochondral ossification and bone regeneration. We observed that the powered bone graft material which is acid insoluble especially is preferable as a carrier for BMP-2.

Abstract Background Zinc transporter 8 autoantibodies (ZnT8Abs) together with glutamic acid decarboxylase autoantibodies (GADAbs), insulinoma antigen 2 autoantibodies (IA-2Abs) and insulin autoantibodies (IAbs) are markers of type 1 diabetes mellitus (T1DM). We studied the prevalence of ZnT8Ab in children with autoimmune thyroid diseases (AITDs) to assess the association of AITDs and T1DM at the serological level. Methods The study groups consisted of 44 children with Graves’ disease (GD), 65 children with Hashimoto’s thyroiditis (HT), 199 children with T1DM with or without AITDs and 58 control children. ZnT8Ab, GADAb, IA-2Ab, IAb, 21-hydroxylase autoantibodies (21-OHAbs) and acetylcholine receptor autoantibodies (AChRAbs) were measured. Results ZnT8Abs were found in 4/44 (9.1%) patients with GD, and 4/44 (9.1%) patients with GD were positive for GADAb. Of the 65 HT patients, six (9.2%) were positive for ZnT8Ab, while four (6.2%) were positive for GADAb. In the T1DM group, 128/199 (64%) of the patients were positive for ZnT8Ab, 133/199 (67%) for GADAb and 109/199 (55%) for IA-2Ab. One GD patient and one HT patient were positive for all the four diabetes-associated autoantibodies. Two HT patients were positive for three diabetes autoantibodies. Two GD (4.5%) and five HT (7.7%) patients were positive for 21-OHAb only. None of the patients had AChRAb. In the control group, 2/58 (3.4%) were positive for GADAb and 2/58 (3.4%) were positive for ZnT8Ab. Conclusions Diabetes-associated autoantibodies including ZnT8Ab were found in children and adolescents with GD and HT.

Bone defects lead to aesthetic and functional losses, causing dental rehabilitation to be more difficult. The objective of this work is to histologically assess the hard tissue healing of bone defects filled with platelet-rich fibrin (PRF) alone or as an adjuvant for mixing with and covering anorganic bovine bone (ABB), compared to ABB covered with a resorbable collagen membrane (CM). This study was designed as a crossover animal study. Four 5-mm tibia defects, 5 mm apart from each other, were surgically created on the tibias of 6 sheep. The defects were randomly filled with ABB + CM; PRF alone; ABB+PRF; or were left empty. The animals were euthanized on days 10, 20, and 40 post-operatively. No group showed any signs of bone necrosis. Inflammation was observed in 2 control and 3 test defects with no statistically significant difference between groups at each time point. The ABB + CM and ABB + PRF groups experienced the highest bone regeneration ratios. No differences between the empty-defect and PRF groups were observed in regard to bone regeneration. No statistical difference was observed between the ABB+PRF and ABB + CM groups in regard to bone regeneration and the amount of residual graft material at each time point. The use of PRF should be preferred due to its autogenous origin, low cost, and ease of use.

Object.To determine the incidence of postoperative bleeding for oral osteotomy carried out under continued monoantiplatelet therapy with clopidogrel and dual therapy with clopidogrel/aspirin.Design.Retrospective single center observatory study of two study groups and a control group.Methods.A total of 64 and 60 oral osteotomy procedures carried out under continued monoclopidogrel therapy and dual clopidogrel/aspirin therapy, respectively, were followed for two weeks for postoperative bleeding. Another 281 similar procedures were also followed as a control group. All oral osteotomy procedures were carried out on an outpatient basis.Results.We observed postoperative bleeding in 2/281 (0.7%) cases in the control group, in 1/64 (1.6%) cases in the clopidogrel group, and in 2/60 (3.3%) cases in the dual clopidogrel/aspirin group. The corresponding 95% confidence intervals are 0–1.7%, 0–4.7%, and 0–7.8%, respectively, and the incidences did not differ significantly among the three groups (P>0.09). Postoperative hemorrhage was treated successfully in all cases with local measures. No changes of antiplatelet medication, transfusion, nor hospitalisation were necessary. No major cardiovascular events were recorded.Conclusions.Our results indicate that minor oral surgery can be performed safely under continued monoantiplatelet medication with clopidogrel or dual antiplatelet medication with clopidogrel/aspirin.

The objective of the work: to study the changes of the functional activity of the endothelium and the plasma-coagulation link of hemostasis in women with chronic inflammatory processes of the internal genital organs (CIPIGO) with and without varicose veins of the small pelvis (VVSP).Materials and methods: 68 women with chronic inflammatory processes of the internal genital organs against the background of varicose veins of the small pelvis (group 1); 52 women with chronic inflammatory processes of the pelvic organs without varicose veins of the small pelvis (group 2); 30 practically healthy women (control group) were examined before and after the treatment. The age of women was between 18 and 45 years old. All the patients of the study groups received the treatment offered by us, which included a diosmine-containing phlebotropic preparation normoven, 500 mg twice a day in continuous mode and a donor of nitric oxide tivortin aspartatum, 5 ml 3 times per os for 14 days each month during six months.Determination of concentration of endothelin-1, prostacyclin was performed by immunoassay using a set of reagents from the firm “Biomediсa” (Austria); the determination of D-dimers in blood plasma was performed by latex-test of the firm “Organon-Tekhnika”.Results of workIn women with CIPIGO with and without VVSP, the increased production of endothelin-1, which has a pro-aggregate and pro-inflammatory effect, was determined. At the same time, the decrease of the synthesis of prostacyclin, which has anti-aggregant properties, was noted. In evaluating the indicators of coagulograms in patients of the studied groups, the propensity for hypercoagulation was revealed, as evidenced by an increase in the level of fibrinogen and a rise of index of the phospholipid-dependent coagulation tests. The decrease of the percentage of fibrinolytic blood activity and the increase in plasma concentration of the molecular marker of thrombophilia D-dimer can be considered as activation of intravascular coagulopathy and the presence of endothelial dysfunction.Conclusions: 1. Our studies found endothelial dysfunction and violations of the plasma-coagulation linkage of hemostasis in both studied groups. 2. After the performed treatment, the improvement of the functional activity of the endothelium and normalization of the main parameters of the plasma-coagulation linkage of hemostasis were observed.

<p><strong>OBJECTIVE:</strong> The objective of the study was to investigate the effect of immersion in water strategy during labor on postpartum bleeding by calculating the postpartum reduction rates of the hematocrit values of the patients.</p><p><strong>STUDY DESIGN:</strong> The study groups consisted of 84 women undergoing vaginal delivery with immersion in water during labor (group 1) and the control group (group 2) of 84 women undergoing normal vaginal delivery. Patients who have received additional medical and surgical interventions for alleviation of postpartum hemorrhage, patients who have undergone an episiotomy and/or perineal trauma were not included in the study. All data were taken from patients who have delivered with spontaneous vaginal delivery. Postpartum hematocrit fall rates of the groups have been compared and the effect of immersion in water on postpartum hemorrhage has been evaluated.</p><p><strong>RESULTS:</strong> The study groups consisted of 84 women undergoing vaginal delivery with immersion in water during labor (Group 1) and the control group (Group 2) of 84 women undergoing vaginal delivery at the hospital. The women in the two groups were matched with respect to age, parity, birth weight and gestational age. The mean age of the women was 29.8±4.8 and 30.5±4.9 respectively. The mean hematocrit difference in the first group was 2.08 ± 1.88 and in the second group was 3.81 ± 1.55. The mean percentage of hematocrit reduction in the first group was 5.71% and in the second group 10.23%.</p><p><strong>CONCLUSION:</strong> Our data showed that mean hematocrit level decreases among women following vaginal delivery more than women who give birth vaginally within immersion in water during labor. The percentage of hematocrit reduction in the water birth group was lower than in the control group. Water birth seems to facilitate uterine contractions more efficiently following vaginal delivery.</p>

The osseointegration process of dental implants begins with osteoblast differentiation and proliferation, which is an important aspect of the bone regeneration process. Platelet-rich fibrin (PRF) is a platelet that contains a large amount of fibrin and growth factor, which is widely used to accelerate bone regeneration. This study aims to determine the effect of PRF on the acceleration of osteoblast proliferation after endosseous dental implant insertion. Twelve male Wistar rats weighing 250 to 300 gr of 8 to 15 weeks as an animal model were divided into two groups, the control group and treatment group, based on days and PRF treatment. Three ml of Wistar rat blood was obtained and centrifuged for 12 minutes at a speed of 2700 rpm to make PRF. Afterward, platelet-rich fibrin was applied to implant bed, and the dental implant was inserted at the lateral epicondyle of the right femur in 3 mm depth and 1.8 mm diameter dimension. Incontrol groups, dental implants were immediately inserted after implant bed preparation without PRF administration. Observation of the Wistar rats was carried out on days 14 and 28 for each group. The rats were terminated accordingto the timeline of group design. The epicondylus lateralis femoris dextra bone of the rats was taken and fixed with 10% buffered formalin solution. Then, histological samples were made with hematoxylin-eosin staining. Sample observation was done under a light microscope to calculate the number of osteoblasts. The data were analyzed using the two-way ANOVA test, followed by LSD post hoc test. Based on the two-way ANOVA test, there was a significant difference between control groups and treatment groups (p<0.05), and there were significant differences between the observation result of days 14 and 28 (p<0.05). The application of PRF increased osteoblast proliferation for the first 14 days, but the rate decreased after that. Based on the LSD post hoc test, there were differences in osteoblast proliferation between the treatment and control group (p<0.05). Thus, it is concluded that platelet-rich fibrin increased the acceleration ofosteoblast proliferation after endosseous dental implant insertion.

Drugs induced nephrotoxicity now considered as an essential reason for kidney failure. Aminoglycoside anti-infection agents, for example, gentamicin, which causes ototoxicity and nephrtotoxicity as a side effect , this research is centered around the utilization of natural materials as an antioxidants against the lethal oxidative activity that applied on the kidney cells ,the most imperative one of these materials is the honey. This work aimed to assess the defensive impact of citrus honey against the histopathological and biochemical harms of gentamicin induced nephrotoxicity .24 locally breed rabbits (Oryctolagus cuniculus) were used and divided into 4 study groups (6 rabbits per each group), group 1 received I.P daily dose of normal saline (control), group 2 received (80 mg/kg/day) I.P dose of gentamicin, group 3 received (80mg/kg/day) of gentamicin I.P dose in combination with oral dose of Citrus Honey625 mg/kg/day for 14 days and group 4 received (80mg/kg/day) of gentamicin I.P dose in combination with extra dose of Citrus Honey orally for 14 days. All animals (at 15th day) were anesthetized by ether and sacrificed; blood samples were gathered for the subsequent measurement of the serum creatinine, urea and albumin  while an isolated kidney was kept in 10 % of formaldehyde for the histopathological examination. The results demonstrated that gentamicin causes nephrotoxicity showen by elevation of serum level of creatinine, urea and  a decrease in the serum albumin. While the administration of honey in combination with gentamicin reduced the nephro-toxic effect of gentamicin that represented by a reduction of the serum creatinine and urea with improvement of the kidney histological findings.This study concluded that, honey decreased nephrotoxic effect of  gentamicin   http://dx.doi.org/10.25130/tjps.25.2020.001

AbstractBackground:Different barrier membranes and augmentation techniques are used in oral surgery to recover lost bone structures with varied success. Recently, a combination between bone graft materials and Platelet-Rich-Fibrin (PRF) is implemented in the periodontology and implantology.Aim: The aim of this case report was to assess the possibility for augmentation of the alveolar ridge in the frontal region of the upper jaw, utilizing a combination of bone graft material, injectable platelet-rich-fibrin (i-PRF) and advanced platelet-rich fibrin (A-PRF).Materials and methods:An 18 year-old male with expulsion of tooth 11 and partial fracture of the alveolar ridge was treated with augmentation of the alveolar ridge using bone graft material, injectable platelet–rich-fibrin(i-PRF) and advanced platelet-rich-fibrin (A-PRF). Clinical results were reviewed 4 months after the augmentation and a dental implant was placed.Results:The postoperative period was uneventful. The control CBCT scan showed good organization of new bone allowing placement of a dental implant.Conclusion:The successful clinical and radiographic results of the case suggest that using A-PRF and i-PRF can be beneficial for bone augmentation of the alveolar ridge before implant placement.

The regeneration of bone defects caused by periodontal disease or trauma is an important goal. Porous hydroxyapatite (HA) is an osteoconductive graft material. However, the hydrophobic properties of HA can be a disadvantage in the initial healing process. HA can be coated with TiO2 to improve its hydrophilicity, and ultraviolet irradiation (UV) can further increase the hydrophilicity by photofunctionalization. This study was designed to evaluate the effect of 5% TiO2-coated HA on rabbit calvarial defects and compare it with that of photofunctionalization on new bone in the early stage. The following four study groups were established, negative control, HA, TiO2-coated HA, and TiO2-coated HA with UV. The animals were sacrificed and the defects were assessed by radiography as well as histologic and histomorphometric analyses. At 2 and 8 weeks postoperatively, the TiO2-coated HA with UV group and TiO2-coated HA group showed significantly higher percentages of new bone than the control group (p < 0.05). UV irradiation increased the extent of new bone formation, and there was a significant difference between the TiO2-coated HA group and TiO2-coated HA with UV group. The combination of TiO2/HA and UV irradiation in bone regeneration appears to induce a favorable response.
