The osseointegration process of dental implants begins with osteoblast differentiation and proliferation, which is an important aspect of the bone regeneration process. Platelet-rich fibrin (PRF) is a platelet that contains a large amount of fibrin and growth factor, which is widely used to accelerate bone regeneration. This study aims to determine the effect of PRF on the acceleration of osteoblast proliferation after endosseous dental implant insertion. Twelve male Wistar rats weighing 250 to 300 gr of 8 to 15 weeks as an animal model were divided into two groups, the control group and treatment group, based on days and PRF treatment. Three ml of Wistar rat blood was obtained and centrifuged for 12 minutes at a speed of 2700 rpm to make PRF. Afterward, platelet-rich fibrin was applied to implant bed, and the dental implant was inserted at the lateral epicondyle of the right femur in 3 mm depth and 1.8 mm diameter dimension. Incontrol groups, dental implants were immediately inserted after implant bed preparation without PRF administration. Observation of the Wistar rats was carried out on days 14 and 28 for each group. The rats were terminated accordingto the timeline of group design. The epicondylus lateralis femoris dextra bone of the rats was taken and fixed with 10% buffered formalin solution. Then, histological samples were made with hematoxylin-eosin staining. Sample observation was done under a light microscope to calculate the number of osteoblasts. The data were analyzed using the two-way ANOVA test, followed by LSD post hoc test. Based on the two-way ANOVA test, there was a significant difference between control groups and treatment groups (p<0.05), and there were significant differences between the observation result of days 14 and 28 (p<0.05). The application of PRF increased osteoblast proliferation for the first 14 days, but the rate decreased after that. Based on the LSD post hoc test, there were differences in osteoblast proliferation between the treatment and control group (p<0.05). Thus, it is concluded that platelet-rich fibrin increased the acceleration ofosteoblast proliferation after endosseous dental implant insertion.

Repairing bone defects in oral surgery often requires the use of bone regeneration techniques. Silicon is an element that has been employed as regeneration material in several studies. In our study, silicon was combined with autologous bone and platelet-rich fibrin (PRF) membranes to analyse the behaviour of this element in bone regeneration. Four circumferential defects were created in the cranial vault of five New Zealand rabbits. The following elements were applied to the regeneration of the defects: (P): PRF; (S): silicon and (B): autologous bone, with the following distribution of study groups: Group 1 (PSB); Group 2 (PS); Group 3 (SB) and Group 4 (CONTROL): unregenerate group. The animals were sacrificed after 3 weeks. Computed microtomography studies (μ-CT) were carried out, as well as histomorphometric ones. The ANOVA statistical test was used with a Bonferroni post-hoc test to compare the results (p ≤ 0.05). Radiologically, groups PSB and SB were better as far as quantity and percentage of healthy bone observed, but not significantly compared to the control group. The PS group was significantly worse. The histological test revealed that the PSB group was the one to present the largest area, percentage and perimeter of mineralised bone. On evaluating the forming bone (osteoid), no difference was observed across the groups with the exception of the bone perimeter, where the SB group was significantly better. The bone height variable showed no significant differences. In conclusion we can state that the combination of PRF, autologous bone and silicon provides good results at 3 weeks whilst the PS group shows the worst results. This highlights the importance of autologous bone forming part of the graft material in order for the bone to mineralise.

Background: The extract of ramania Bouea macrophylla Griff) and binjai (Mangifera caesia) leaf have flavonoid compounds that function as antioxidants to balance the amount of Reactive Oxygen Species (ROS) in tissues and optimize wound healing by helping synthesis of hydroxyproline which is used as a collagen synthesis material. Objective: To compare the effect of 15% concentration of ramania leaf extract gel, 15% concentration of binjai leaf extract gel on collagen density in back incision wounds of male Wistar rats on day 7 and day 14. Methods: This study used a true experimental design with a posttest-only with control group design. The study sample used male wistar rats that were healthy and active, aged 2-3 months with a body weight of 250-300 grams. The total sample was 18 rats divided into 6 groups. The 15% concentration of ramania and binjai leaves was given topically, then the rats were euthanized on the 7th and 14th day. Collagen index measurement was using hydroxyproline concentration. Results: Two-way Anova data analysis showed a significant value of 0.00 (p<0.05), which means that there was a difference in effect between the treatment gel and wound day. Bonferroni Post Hoc test showed a significant value in all treatment gel groups. Conclusion: There is a difference in the effect of 15% concentration of ramania leaf extract gel and 15% concentration of binjai extract gel on collagen density. Binjai leaf extract gel at 15% concentration is more effective for collagen density than Ramania leaf extract at 15% concentration and placebo. Keywords : Binjai Leaf Extract Gel, Collagen, Hydroxyproline, Ramania Leaf Extract Gel.

Placement of dental implants in the aesthetic zone needs to be planned to consider both atraumatic tooth extraction and the preservation of the socket. The authors presented two clinical cases of patients with indication of dental extraction of the upper central incisor which was planned to use a two-armed lever as atraumatic alveolar preservation technique and managed in two different ways. Case one referred to a traditional approach that included dental extraction, guided bone regeneration, and provisionalization, with a period of healing of 6 months to enable the bone regeneration. Subsequently, it was placed a dental implant TSV 3.7*13 mm (Zimmer Dental). Five months after of clinical and radiographic controls, the second phase was started with the rehabilitation. The second case was about a patient who required dental extractions of maxillary left central incisor due to a fracture of the crown with a poor prognosis, in which the dental extraction was planned to use a two-armed lever. After reclassifying the fresh socket, a dental implant TSV of 3.7 x 13 mm (Zimmer Dental) was placed with modified drilling technique, implant placement resulted in primary fixation with insertion torque level lower than 30 Ncm so the immediate load was not performed. Both patients received a ceramic crown with functional and esthetic results after 16 months of continuous checks and controls. It was concluded that the alveolar preservation in the aesthetic zone is a strategy that starts from the extraction technique. The authors recommend the two-armed lever as atraumatic alveolar preservation technique and different ways to place dental implants.

Introduction: Green tea and tocopherol are potent antioxidants used to treat melasma. However, whether green tea or tocopherol is superior remains unclear. Objective: To compare the effectiveness between green tea and tocopherol in decreasing the amount of melanin and tyrosinase level in wistar rats exposed to UVB rays. Methods: The experimental study took 30 male Wistar rats randomly and divided them into 3 groups. The control group (C-G) was given basic topical cream, GT-G was given green tea topical cream and TC-G was given tocopherol topical cream. All rats were exposed to UVB every Monday, Wednesday and Friday for 4 weeks, whereas topical creams were smeared every day. Topical creams smearing on the same day with UVB exposure was performed 20 minutes before exposure and 4 hours after UVB exposure. The doses of UVB were 50 mJ/cm2 in first week, 70mJ/cm2 in the second week and 80mJ/cm2 in the third and fourth weeks. The amount of melanin was measured using pixel method and the tyrosinase level was measured using ELISA.Results: Anova analysis indicates that the amount of melanin and tyrosinase level are significantly different between groups, p<0.05. Post Hoc LSD analysis indicates that the amount of melanin in GT-G and TC-G are significantly lower than that of C-G, p<0.05. The amount of melanin in GT-G is lower than that of TC-G, p<0.05. The tyrosinase level in GT-G is significantly lower than that of C-G and TC-G, p<0.05. Meanwhile, the tyrosinase level in TC-G is lower than that of C-G but insignificantly, p>0.05. Conclusion: Green tea topical treatment is significantly capable of decreasing the amount of melanin and tyrosinase level better than tocopherol.

Diabetes mellitus is known to compromise the various aspects of homeostasis, including the immune response and the composition of oral microflora. One of the oral manifestations of diabetes mellitus is tooth loss and the survival rate of dental implants chosen as a treatment modality for its rehabilitation is controversial. The current study aims to evaluate and compare the failure rate of dental implants between well-controlled diabetic and healthy patients. A retrospective study of case-control design was conceptualized with 121 well-controlled diabetic and 136 healthy individuals. Records of subjects who had undergone oral rehabilitation with dental implants between the periods of January 2013 to January 2016 were retrieved. Post-operative evaluation was carried out for all patients for about three years to assess the immediate and long-term success of the procedure. From a total of 742 dental implants, 377 were placed in well-controlled diabetic patients (case group) and 365 in healthy subjects (control group). A comparable (9.81%), but non-significant (p = 0.422) failure rate was found in the case group in comparison to the control group (9.04%). A non-significant (p = 0.392) raised number (4.98%) of failure cases were reported among females in comparison to males (4.44%). In respect to arch, the mandibular posterior region was reported as the highest failure cases (3.09%; p = 0.411), with 2.29% of cases reported in the mandibular anterior (p = 0.430) and maxillary posterior (p = 0.983) each. The maxillary anterior region was found to have the least number (1.75%; p = 0.999) of failure cases. More (4.98%; p = 0.361) cases were reported to fail during the functional loading stage in contrast to osseointegration (4.44%; p = 0.365). A well-controlled diabetic status does not impose any additional risk for individuals undergoing dental implant therapy.

The objective of this research was to investigate dental bridges with and without implants. Threedimensional (3D) mandible models were reconstructed by computer tomography (CT) to simulate biting behaviors. The dental implant is an important factor in dental bridge applications. Several studies have investigated finite element models for dental implants; however, few have examined a model for dental bridge with implant. The results revealed that stress was significantly increased when dental bridge was used with implant. Moreover, the dental bridge with implant group demonstrated a relatively big stress in mandible, which was 4.01% lower compared with that of the control group. Dental bridge would be an effective means of recovering dental performance. However, the present research stated that the implant of dental bridge has a potential to increase abnormal stress, and uniformly distributing stress in the dental bridges.

BACKGROUND: Restoring masticatory function and replacing missing teeth with minimal pain and discomfort are the most important issues for the patient and clinician. Nowadays dental implants became the most popular line of treatment to replace missing teeth; offering a comfortable long lasting prosthesis. Osseo-integration reflects the long term success of a dental implant. Many bio-modulators are used aiming to improve the osseointegration and healing around dental implants such as Low-Level Laser treatment (LLLT) and Platelet Rich Fibrin (PRF). PRF has been proven to improve bone repair process around the dental implant. LLLT is considered a noninvasive, safe technique that stimulates osteogenesis and alleviates post-operative pain. AIM: Evaluation of the bio-stimulatory effect of LLLT on a dental implant with PRF compared to PRF alone clinically and radiographically. METHODS: A randomised clinical trial with the split-mouth design was conducted on nine patients with bilaterally missing lower posterior tooth. All patients received one dental implant on each side with PRF. LASER application was performed to one side twice weekly for one month starting on the day of insertion. Post-operative pain was assessed daily through the first week using numerical rating pain scale (NRS) as the primary outcome. Relative peri-implant bone density was measured using direct digital intraoral radiography immediately after insertion, one, four and nine months postoperatively. Implants stability were measured using radio frequency assessment immediately after insertion, four and nine months post-operative as secondary outcomes. RESULTS: The NRS for pain was significantly decreased by the end of the first-week postoperatively in the intervention and control group with a mean of (2.22 ± 1.56) (2.11 ± 1.83) respectively. However, there was no statistically significant difference between the test groups at P-Value (0.892). The relative bone density values were decreased by the end of the ninth month of follow-up in the intervention and control group with a mean of (134.42 ± 16.13) (128.77 ± 33.54) respectively. No statistically significant difference was observed between the two test groups at P-value (0.863). The radiofrequency values for implant stability showed no statically significant difference after nine months of follow up when compared to the initial stability values at the day of insertion in the intervention and control group. The mean radiofrequency values were (67.24 ± 1.79) and (66.9 ± 2.57) respectively, and no statistically significant difference was observed between the two test groups at P-value (0.793) CONCLUSION: There are no statistically significant differences in post-operative pain values, implant stability and bone density between the implant sites treated with PRF augmented by Diode laser compared to implant sites treated by PRF alone.
