The awareness of care provided by stroke care unit (SCU) nurses in Japan to patients with an acute cerebrovascular accident (CVA) and the characteristic differences in their actual nursing practice were evaluated. A cross-sectional web-based questionnaire survey was administered to 1040 SCU nurses. Data collection and reporting procedures followed the STROBE Statement Checklist for cross-sectional studies. Exploratory factor analysis, using 52 observation items, identified eight factors with a factor loading > 0.4. For all factors, the actual practice was significantly lower than the awareness of the importance of nursing care for patients with acute CVA. Awareness and actual practice of recognition of patients’ physical changes (RPPCs) were high. The actual practice of RPPCs and preventing the worsening of acute stroke and related symptoms varied, depending on years of experience in acute phase stroke care. RPPCs in actual practice had a significantly higher score among certified nurses or certified nurse specialists. Their awareness of the importance of collaborating with therapists was low. On-the-job training can improve nurses’ competence and prevent worsening conditions in patients with CVA. An emphasis on enhancing practice experience toward patients with acute CVA and facilitating the deployment of certified nurses in SCUs can improve nursing care practice.

In recent years, additional expressions such as ‘sensation of breathing discomfort’ and ‘discomfort of dyspnea’ are being used in daily nursing care in Japan. To better understand the current status of the use of these terms by nurses, and to ascertain what the term ‘dyspnea’ may not express, we designed an original questionnaire and conducted a study with all nurses at our hospital. The questionnaire included questions to determine if nurses used these terms, and in what context. Of the 279 nurses in our hospital, 225 (80.6%) responded. Three-quarters of nurses indicated that they use these terms in clinical nursing practice. There was no difference in the usage of these terms between nurses who had or had not worked in a respiratory outpatients/ward. However, the percentage of nurses using these terms was higher amongst those with 10 years or less nursing experience compared with those with more than 10 years’ experience. Open-ended questions revealed that these terms were used to communicate information between nurses and between nurses and patients’ families. Our observations need to be verified in large-scale studies to determine if these terms are meaningful for nursing practice in that they describe something not expressed with ‘dyspnea’. There is the possibility of confusion due to the use of inappropriate terms and a lack of education on the subject. Many nurses used these terms, and there may be things that the term ‘dyspnea’ could not express. The results of this study can be used to identify something that is lacking in communication about dyspnea between nurses, nurses and patients, and nurses and patients’ families.

Abstract Background Following the publication of the National Stroke Audit in 2015 with below the national average rates of thrombolysis – 3.3% versus 11% - there has been a number of initiatives launched to improve stroke care in this hospital. In 2018 we enrolled in the National Quality Improvement Project for the care of Patients with Acute Ischaemic Stroke run by the RCSI. A stroke steering committee was established consisting of a multi-disciplinary group encompassing all areas of acute stroke care. Methods Acute stroke care practice and factors causing sub-optimal management were examined by the committee and compared with national standards. A ‘3 jobs’ proforma for management and communication of Fast positive cases was designed to address the difficulties identified in stroke care and tailored to the resources available in this hospital. These simplified and standardised roles for all staff members involved, many of whom were unfamiliar with the practical delivery of thrombolysis and thrombectomy. Educational sessions were initiated for all those involved in acute stroke management. The acute stroke program was implemented as a 6 month pilot before official launch in April 2019 with necessary changes assessed weekly by the stroke committee. Stroke data is continually audited with the National Stroke Register. Key Performance Indicators (KPI’s) in the pilot were thrombolysis/thrombectomy rates and door to needle time. Results There were 164 patients admitted to the stroke unit in 2018. In 2018 prior to this initiative, the hospital had a 3% thrombolysis rate in 2018 and a 1% thrombectomy rate. Amongst FAST positive patients during the pilot period, 10/68 patients were thombolysed (15%) and 6/68 thrombectomies (9%). Average door to needle time for patients was 96 minutes. Conclusion The redesigning of the acute stroke care program has led to significant improvements in the identified KPI’s although door to needle times remain below the national target of 30 minutes.

Background: Caring behavior is the main focus act in nursing practice which contains humanistic values, a responsive attitude, and responsible for fulfilling patients’ expectations. The results of the patient satisfaction survey and observation result showed the lack of caring behavior of the attending nurses in Inpatient Ward of Wisma Prashanti General Hospital. This study aimed at determining the relation of nurses’ working motivation towards caring behavior of attending nurses in Inpatient Ward of Wisma Prashanti General Hospital Tabanan. Methods: The design of this study was cross-sectional using a quantitative approach. Data was collected by filling out questionnaires by 37 of attending nurses from Juni-August 2018 in Inpatient Ward of Wisma Prashanti General Hospital Tabanan. Data analysis included univariate and bivariate analysis with Kendall's Tau_b test. Result: Univariate analysis showed that most nurses had the high working motivation (89.2%) and had good caring behavior (91.9%). Bivariate analysis showed a significant relationship between working motivation and caring behavior of attending nurses in Inpatient Ward of Wisma Prashanti General Hospital Tabanan (p = 0,00). Conclusion: Fulfillment of the supporting factors of nurses’ working motivation is needed to maintain or improve the working motivation of nurses, therefore, nurses have an inner impulse to provide caring in order to provide optimal and professional nursing care.

Background and Objectives. Tissue plasminogen activator (t-PA) within 4.5 hours from onset improves outcome in patients with ischemic stroke and has been recommended by several international guidelines. Since its approval in 1996, the debate among emergency physicians continues particularly around the result interpretation of the first positive randomized controlled trial, the National Institute of Neurological Disorders and Stroke (NINDS) clinical trial. This lack of consensus might negatively affect the delivery of effective stroke care. Here we aimed to assess the knowledge and attitude of Saudi emergency physicians toward t-PA use within 4.5 hours of onset in acute ischemic stroke. Methods. A web-based, self-administered, locally designed questionnaire was sent to all emergency physicians practicing in the city of Riyadh from January to September 2017. Results. Out of 450 emergency physicians, 122 from ten hospitals in Riyadh participated in the survey, with a 27% response rate. The majority of participants were men (78%), and their mean age was 40 ± 8 years. Half of the participants were board certified, and 36% were consultants. Half of the participants consider the evidence for t-PA use in stroke within 4.5 hours of stroke onset to be controversial, and 41% recommend against its use due to lack of proven efficacy (37%), the risk of hemorrhagic complications (35%), lack of stroke expertise (21%), and medicolegal liability (9%). Nearly half were willing to administer IV t-PA for ischemic stroke in collaboration with remote stroke neurology consultation if telestroke is implemented. Conclusion. Our study detected inadequate knowledge and a negative attitude among Saudi emergency physicians toward t-PA use in acute stroke. This might negatively impact patient outcome. Therefore, we recommend developing urgent strategies to improve emergency physicians’ knowledge, attitudes, and beliefs in the management of acute stroke.

Abstract Background Scientific knowledge and theory constitute part of the nurse’s competence and evidence-based nursing practice. To obtain and maintain these skills, nurses require access to research utilization. The aim of the present study was therefore to describe and compare nurses in nursing homes and home-based nursing care and their use of research knowledge in their practice in elderly care in Norwegian rural districts. Methods The Research Utilization Questionnaire (RUQ) was employed in cross-sectional quantitative design. One hundred nurses were recruited from ten rural municipalities that participated in the study. Inclusion criteria for participating were registered nurses and employees working in the municipal elderly care service for 6 months or more. Results Most participants were younger than 55 years old, worked in permanent jobs, and were educated more than 5 years ago. The result showed that nurses in nursing homes were significantly more positive compared to nurses in home-based nursing care when analyzing all three domains in the RUQ together, as well as for attitudes towards research when testing each domain separated. Overall, each item in the domains revealed opinions that were more positive for nurses in nursing homes. The regression analysis showed that attitudes towards research, as well as availability and support of research utilization predicted the use of research in daily practice. Conclusions Positive attitudes, availability, and support for research utilization can contribute to greater use of research in nursing practice and improve the quality of service. Younger nurses’ knowledge about using research should be shared with senior colleagues, who possess much experience in practice. In collaboration, they can develop evidence-based practice by the implementation of research seen in the context of nurses’ experiences, user involvement, and person-centred practice. The i-PARIHS (Promoting Action on Research Implementation in Health Services) framework can be a useful tool in this implementation process.

Latar Belakang : Bertambahnya masalah gangguan jiwa diperlukan peningkatan pelayanan kesehatan terutama keperawatan. Penanganan gangguan jiwa diperlukan sikap caring yaitu memberikan perhatian terhadap seseorang. Salah satu cerminan sikap caring adalah berkomunikasi dengan baik. Tujuan Penelitian : Penelitian ini bertujuan untuk mengetahui hubungan Persepsi, Motivasi, dan Gaya kepemimpinan Dengan Sikap Caring Perawat di Rumah Sakit Jiwa Daerah Dr. RM. Soedjarwadi Jawa Tengah 2018. Metode Penelitian : Desain penelitian ini adalah correlation dengan pendekatan cross sectional dilakukan di Rumah Sakit Jiwa Daerah Dr. RM. Soedjarwadi Jawa Tengah tanggal 11 Mei 2018 – 4 Juni 2018. Jumlah sampel menggunakan Tehnik Purposive sampling yaitu 70 responden. Alat ukur kuesioner. Analisis bivariate menggunakan uji Somers’d. Hasil Penelitian : Persepsi p-value<α (0,035<0,05), Motivasi p-value>α (0,080>0,05), gaya Kepemimpinan p-value<α (0,043<0,05) dan analisa multivariat menggunakan analisis Regresi Berganda 4 Variabel, tidak terdapat hubungan secara partial maupun simultan antara persepsi, motivasi, gaya kepemimpinan dengan sikap caring. Kesimpulan :Terdapat hubungan antara persepsi dengan sikap caring perawat, tidak ada hubungan antara motivasi dengan sikap caring perawat, ada hubungan antara gaya kepemimpinan dengan sikap caring perawat di Rumah Sakit Jiwa Daerah Dr. RM. Soedjarwadi Jawa Tengah. Saran : Hasil penelitian bagi Stikes Bethesda, dapat dijadikan refrensi tambahan tentang faktor yang mempengaruhi sikap caring perawat.   Kata Kunci : persepsi, motivasi, gaya kepemimpinan, caring     ABSTRACT    Background : Caring is giving attention to others. One reflection of caring is to maintain good communication. In mental nursing, maintaining a good communication is a key of successful mental nursing care practice. Caring can be influenced by nurses, in terms of perception and motivation and it can also be influenced by unit manager’s leadership style. Objective : To know the correlation between perception, motivation and leadership style with caring attitude of nurses in RSJD Dr. RM. Soedjarwadi Central Java in 2018. Method : It was a correlation research with cross sectional approach done in RSJD Dr. RM. Soedjarwadi Central Java in 11th May – 4th June 2018. The sample was 70 respondents taken with total sampling technique. Data was collected with questionnaire. Analysis was done with Kendall’s Tau test. Result : The result shows df 3, p-value > α (0,388>0,05), (0,170>0,05), (0,725>0,05) and multivariate analysis with 4 variable multiple regression shows there is no correlation either partially or simultaneously between perception, motivation, leadership style to caring attitude. Conslusion: There is no correlation between perception, motivation and leadership style with caring attitude of nurses in RSJD Dr. RM. Soedjarwadi Central Java in 2018. Suggestion : This research can be an additional reference about factors that influence caring attitude of nurses.   Keyword : perception, motivation, leadership style, caring

Background: Stroke is a major cause of morbidity and disability worldwide. However, its outcomes have improved in the last few years with advancement in acute stroke treatment, including the use of tissue plasminogen activator (t-PA) within 4.5 hours of onset, which led several international guidelines to adopt it as the standard of care. In this study, authors sought to assess the knowledge, practices, and attitudes of emergency and medicine staff in Qassim, Saudi Arabia toward acute ischemic stroke care.Methods: A quantitative observational cross-sectional study involving 148 physicians from emergency and medicine departments (only three neurologists) was conducted in three main hospitals of the Qassim region, Saudi Arabia. Information was obtained from a self-administered questionnaire. A logistic regression model was used to control for potential confounding factors.Results: Ninety-two percent of participants were aware of t-PA. Eighty-seven percent of participants thought that t-PA was an effective treatment for acute ischemic stroke. Only 20% of participants had given t-PA or participated in the use of t-PA in acute ischemic stroke. Moreover, 64% of participants believed that allowing blood pressure to remain high was the most appropriate action in the first 24 hours in acute ischemic stroke patients who presented outside the t-PA window.Conclusion: Most of the emergency and medicine staff are well informed about t-PA, but the majority of these physicians have never given t-PA or participated in the administration of t-PA to a stroke patient. The main finding here is the positive outlook among emergency and medicine physicians in Qassim toward training in acute stroke care and administering t-PA for stroke, which will positively impact patient outcomes.

Nursing Care Systematization (NCS) is an organized and systematized process that concerns nursing professionals who provide quality assistance, being a mandatory tool in all health centers. Despite this factor, there is a visible resistance from nursing to the operationalization of a systematized care. Thus, the present study aims to analyze the applicability of nursing care systematization and the knowledge that primary care nurses have of this process. This is a descriptive, cross-sectional study with quanti-qualitative approach, conducted with nurses working in primary care teams from a regional health zone in the state of Paraná, Brazil. Out of the 44 participant nurses, only 20.5% claimed to take all steps in the nursing process during their care practice, and 43.2% do not know about the Resolution of the Federal Nursing Council that addresses NCS application. This low NCS applicability is worrisome, since it is related to the quality, resolution and security of care provision. The applicability of the systematization within the assessed teams is quite fragmented and small; besides, their NCS knowledge is insufficient considering that this is such a relevant tool to a nurse’s professional practice. It is possible to notice numerous fragilities and difficulties in the application of NCS to primary care, with highlight to time availability, overworked nurses, and need for training.
