Wound healing is a highly coordinated process which leads to the repair and regeneration of damaged tissue. Still, numerous diseases such as diabetes, venous insufficiencies or autoimmune diseases could disturb proper wound healing and lead to chronic and non-healing wounds, which are still a great challenge for medicine. For many years, research has been carried out on finding new therapeutics which improve the healing of chronic wounds. One of the most extensively studied active substances that has been widely tested in the treatment of different types of wounds was Substance P (SP). SP is one of the main neuropeptides released by nervous fibers in responses to injury. This review provides a thorough overview of the application of SP in different types of wound models and assesses its efficacy in wound healing.

Bicellular tight junctions (TJs) are intercellular junctions comprised of a variety of transmembrane proteins including occludin, claudins, and junctional adhesion molecules (JAMs) as well as intracellular scaffold proteins such as zonula occludens (ZOs). TJs are functional, intercellular structures that form a barrier between adjacent cells, which constantly seals and unseals to control the paracellular passage of molecules. They are primarily present in the epithelial and endothelial cells of all tissues and organs. In addition to their well-recognized roles in maintaining cell polarity and barrier functions, TJs are important regulators of signal transduction, which modulates cell proliferation, migration, and differentiation, as well as some components of the immune response and homeostasis. A vast breadth of research data is available on TJs, but little has been done to decipher their specific roles in wound healing, despite their primary distribution in epithelial and endothelial cells, which are essential contributors to the wound healing process. Some data exists to indicate that a better understanding of the functions and significance of TJs in healing wounds may prove crucial for future improvements in wound healing research and therapy. Specifically, recent studies demonstrate that occludin and claudin-1, which are two TJ component proteins, are present in migrating epithelial cells at the wound edge but are absent in chronic wounds. This indicates that functional TJs may be critical for effective wound healing. A tremendous amount of work is needed to investigate their roles in barrier function, re-epithelialization, angiogenesis, scar formation, and in the interactions between epithelial cells, endothelial cells, and immune cells both in the acute wound healing process and in non-healing wounds. A more thorough understanding of TJs in wound healing may shed new light on potential research targets and reveal novel strategies to enhance tissue regeneration and improve wound repair.

Wound healing is one of the most complex processes in the human body. It involves the spatial and temporal synchronization of a variety of cell types with distinct roles in the phases of hemostasis, inflammation, growth, re-epithelialization, and remodeling. With the evolution of single cell technologies, it has been possible to uncover phenotypic and functional heterogeneity within several of these cell types. There have also been discoveries of rare, stem cell subsets within the skin, which are unipotent in the uninjured state, but become multipotent following skin injury. Unraveling the roles of each of these cell types and their interactions with each other is important in understanding the mechanisms of normal wound closure. Changes in the microenvironment including alterations in mechanical forces, oxygen levels, chemokines, extracellular matrix and growth factor synthesis directly impact cellular recruitment and activation, leading to impaired states of wound healing. Single cell technologies can be used to decipher these cellular alterations in diseased states such as in chronic wounds and hypertrophic scarring so that effective therapeutic solutions for healing wounds can be developed.

Chronic wounds seem to be a big problem for the medicinal, social and commercial area, especially for elder patients or people with cancer, metabolic or autoimmune diseases. In this respect, in the modern regenerative medicine there are intensive studies on methodologies that stimulate healing of chronic wounds (diabetic foots, ulcers, burns). In tissue engineering new solutions in wound healing are based on cellular therapies which consisting of growth factors and various types of scaffolds. In this way, there are created skin substitutes which are composed of cellular auto/allografts (stem cells and differentiated cells) and most commonly biodegradable scaffolds; they aim is not only to fill the tissue but also to stimulate wound healing. In this article we demonstrate the current knowledge about biological properties of Adipose- -derived Stem Cells (ASCs), methods of their isolation and potential for use in therapies for non-healing wounds. Adipose tissue seems to be an attractive and abundant stem cells source with therapeutic applicability in diverse phase of the repair and regeneration of the chronically damaged tissues. Additionally, it is believed that secreted by ASCs growth factors, cytokines and exosomes are decisive in the clinical effects. In this review, we also present the current clinical trials using stem cells derived from adipose tissue. Increasingly, the use of cell therapy in wound healing treatment draws attention to the safety, reproducibility and quality of stem cells. Researches go on and therapy approaches are possible but the detailed knowledge of the ASCs biology must be thoroughly investigated before these cells would be widely used in the clinical trials.

The structure, classification, function, and regulation of matrix metalloproteinases in normal and abnormal wound healing is discussed. Results from key studies suggest that neutrophil-derived matrix metalloproteinase 8 (MMP-8) is the predominant collagenase present in normal healing wounds, and that overexpression and activation of this collagenase may be involved in the pathogenesis of nonhealing chronic leg ulcers. Excessive collagenolytic activity in these chronic wounds is possible because of the reduced levels of tissue inhibitor metalloproteinase 1 (TIMP-1). However, until recently, there have been no studies evaluating levels of matrix metalloproteinase or tissue inhibitors of metalloproteinase activity in chronic diabetic foot wounds. Improving basic knowledge and pharmaceutical intervention in this area ultimately may help clinicians identify and proactively intervene in an effort to prevent normal wounds from becoming chronic. The review of the world literature, is devoted to the role of collagen in the process of wound healing. The problems of epidemiology of chronic wounds and ulcers of various genesis, physiology and pathophysiology of wound healing phases are considered. The pathogeneticrole of different types of collagen, as well as the mechanisms of collagen, macrophage, fibroblasts, matrix metalloproteinases and other cytokines in healing ulcers are discussed. The prospects of development and use of medical products and preparations based on collagen in the treatment of patients with chronic wounds and ulcers are shown. The results of clinical studies on the effectiveness of collagen with preserved (native) and unsaved (fractionated) structure are presented. The advantages of using native collagen in the treatment of diabetic foot syndrome are demonstrated.

It is undeniable that many patients worldwide suffer from various types of wounds, especially from chronic wounds. The complex and intricate process of wound healing has a severe impact on the patient's quality of life as well as causing an economic burden on healthcare institutions. Although various new therapies have become available for treating patients with acute and chronic wounds for the past decade, the available therapies are often expensive or accompanied by undesirable side effects. Hence, the discovery of a new arsenal for wound healing remains a hot topic of research. Recently, plants or herbs and their derivatives have garnered significant attention as a source of therapeutic agents to treat wounds. This is because plants provide a rich reservoir of phytochemicals that could potentially become effective and affordable therapeutic agents. Thus, the present review attempted to outline wound healing mechanisms and analysed some renowned medicinal plants with potential wound healing properties from the existing literature from various electronic databases. This review also sheds light on the plant's underlying molecular mechanisms and, wherever available, acknowledges the biologically active substances found in these plants.

Introduction Chronic, non-healing wounds are often characterised by an excessive, and detrimental, inflammatory response. We review our experience of using a combined topical steroid, antibiotic and antifungal preparation in the treatment of chronic wounds displaying abnormal and excessive inflammation. Methods A retrospective review was undertaken of all patients being treated with a topical preparation containing a steroid (clobetasone butyrate 0.05%), antibiotic and antifungal at a tertiary wound healing centre over a ten-year period. Patients were selected as the primary treating physician felt the wounds were displaying excessive inflammation. Healing rates were calculated for before and during this treatment period for each patient. Changes in symptom burden (pain, odour and exudate levels) following topical application were also calculated. Results Overall, 34 ulcers were identified from 25 individual patients (mean age: 65 years, range: 37–97 years) and 331 clinic visits were analysed, spanning a total time of 14,670 days (7,721 days ‘before treatment’ time, 6,949 days ‘during treatment’ time). Following treatment, 24 ulcers demonstrated faster rates of healing, 3 ulcers showed no significant change in healing rates and 7 were healing more slowly (p=0.0006). Treatment generally reduced the burden of pain and exudate, without affecting odour. Conclusions In normal wound healing, inflammation represents a transient but essential phase of tissue repair. In selected cases, direct application of a steroid containing agent has been shown to improve healing rates, presumably by curtailing this phase. Further evaluation is required to establish the role of preparations containing topical steroids without antimicrobials in the management of chronic wounds.

Wound healing is a complex biological process that is impaired under diabetes conditions. Chronic non-healing wounds in diabetes are some of the most expensive healthcare expenditures worldwide. Early diagnosis and efficacious treatment strategies are needed. microRNAs (miRNAs), a class of 18–25 nucleotide long RNAs, are important regulatory molecules involved in gene expression regulation and in the repression of translation, controlling protein expression in health and disease. Recently, miRNAs have emerged as critical players in impaired wound healing and could be targets for potential therapies for non-healing wounds. Here, we review and discuss the mechanistic background of miRNA actions in chronic wounds that can shed the light on their utilization as specific wound healing biomarkers.

Abstract: Indonesia has a lot of herbs. One of the well-known and frequently used by people is miana. Empirical experience showed that miana is used to healing wounds. The chemical substances of miana leaves such as essential oil, flavonoid, tanin, and other active substances are expected to involved in the process of wound healing. The purpose of this reseach is to find out the healing effect of miana leaves to the insicion wound on rabbits’ skin. This research used an experimental method by using 5 rabbits as the experimental animals. Skin insicions were generated over  rabbits’  left and right back by 5 cm each. The wound on the left back is given miana leaves, and the wound on the right back without miana leaves. The wound healing was observed on third, seventh and fourteenth day, with the length, the superficial and the edges of the wounds as the evaluation criteria. The incision wounds on the rabbits’ back that given miana leaves are dry and heal faster than the rabbits without miana leaves. Using miana leaves for the rabbits’ insicion wounds can speed up the wound healing process. Keywords: miana leaves, wound healing, insicion wound.     Abstrak: Indonesia memiliki banyak tanaman yang berkhasiat obat. Salah satu tanaman yang berkhasiat, dikenal, dan digunakan masyarakat yaitu tumbuhan iler. Pengalaman empiris menunjukkan tumbuhan iler digunakan sebagai obat luka. Kandungan kimia daun iler seperti minyak atsiri, flavonoid, tanin, dan zat-zat aktif lainnya diduga terlibat dalam penyembuhan luka. penelitian ini bertujuan untuk mengetahui efek penyembuhan daun iler terhadap penyembuhan luka insisi pada kulit kelinci. Penelitian ini menggunakan metode eksperimental, dengan menggunakan lima ekor kelinci sebagai hewan uji. Kelinci diinsisi pada punggung kiri dan kanan sepanjang 5 cm. Luka pada punggung kiri diberi daun iler, sedangkan luka pada punggung kanan tidak diberi daun iler. Penyembuhan luka diamati pada hari ke-3, 7, dan 14, dengan kriteria penilaian panjang, permukaan dan tepi luka. Luka insisi pada kulit kelinci yang diberi daun iler terlihat lebih cepat kering dan menutup dibandingkan dengan luka yang tidak diberi daun iler. Pemberian daun iler pada luka insisi kulit kelinci dapat mempercepat proses penyembuhan luka. Kata Kunci: daun iler, penyembuhan luka, luka insisi.

Chronic wounds, including diabetic ulcers, pressure ulcers, venous ulcers, and arterial insufficiency ulcers, are both difficult and expensive to treat. Conventional wound care may sometimes lead to suboptimal wound healing and significant morbidity and mortality for patients. The use of skin substitutes provides an alternative therapy showing superior efficacy and, in some cases, similar cost-effectiveness compared to traditional treatments. This review discusses the different types of currently available commercial skin substitutes for use in chronic wounds as well as the paucity of strong evidence supporting their use. It then delves into the limitations of these skin substitutes and examines the most recent research targeting these limitations.
