Role conflicts and role overloads in the doctor's activity as a socio-psychological problem

Currently, the leading socio-psychological problem in modern health care is role overloads and role conflicts among medical professionals. Role-based problems can lead to neuropsychiatric stress and become a source of professional stress for doctors. Therefore, in order to provide social assistance and support to doctors, it is necessary to study the sociopsychological factors that determine role conflicts in the professional activity of a doctor and further implement a socially oriented approach in the conditions of modernizing the modern healthcare system.

Introduction. Development of methodical base of information hygiene, systematization of data on levels of information loadings and degree of their influence on working conditions of workers is actual.The purpose of the study was to measure and evaluate the levels of information mental load of office workers, to study the patterns of development of adverse functional States in this professional group.Materials and methods. The pilot study included 25 employees of scientific and educational institutions aged 25 to 65 years. Measurement of information mental load of workers was carried out on the basis of a point assessment. Psychodiagnostics of performance of participants of research was carried out by means of a package of standard techniques. Heart rate telemetry was carried out in employees in the conditions of professional activity.Results. The integral indicator of information mental load of the studied professional group was 37.3±7.2 points and was in direct connection with the coefficient of comfort of the workplace (r=0.5, p=0.01). A reliable dependence of the speed of information processing on the length of work experience (r=0.71, p=0.0001) was also established. In the low-stress group, high indices of stress, fatigue, monotony were recorded in the workers, in the senior group — signs of chronic fatigue. The employees during the shift registered an increase in sympathetic activation.Conclusions. The results of the psychophysiological survey of employees indicate the need to regulate the information load, the development and implementation of preventive measures to reduce professional stress.

A comprehensive examination of 213 interns of both sexes was carried out. A system of psychotherapeutic correction of maladaptive states and psycho-preventive support of interns during the period of professional training has been developed. Levels of adaptation of internship doctors for professional activity were emphasized: high (14,1 %), average (20,2 %) and low level (65,7 %). It was described the asthenical (25,8 % of men and 22,5 % of women), hypersthenic (20,6 % of men and 23,2 % of women), anxiodepressive (11,3 % and 20,6 % respectively), psychosomatic (14,5 % of men and 18,1 % of women), asthenic-apathetical (12,3 % and 11,2 % respectively) and addictive (15,5 % of men and 4,4 % of women) variants of maladaptive reactions among the internship doctors. The developed model of the formation of disorders of adaptation of internship doctors is presented by a complex of pathogenic factors. It was established that the basis for the formation of maladaptive reactions among the internship doctors is the presence of somatic pathology, craniocerebral injury and neuroinfection in the anamnesis and the tendency to addictive behavior. Prognostically important factors in the formation of adaptation disorders are conflicts of family and professional relations, disturbing suspiciousness, low communicativeness, difficult working conditions, lack of positive emotions, awareness of inadequate level of competence, low level of motivation, and imperfectiveness of mechanisms of psychological protection. Triggers for the development of maladaptive states are the high level of professional stress, depletion of adaptation, prolonged mental stress, frustration of significant basic needs, and high rates of clinical scale of anxiety and depression HDRS. An individualized three-stage system of medical and psychological support during the professional training period, which involves the application of complex psychodiagnostic, psychotherapeutic, psychoeducational and psychoprophylactic influences, has been developed and tested.

Abstract Introduction. Currently, the work of managers is characterized by a number of factors having an unfavorable impact on human health. Stress as the main risk factor related to the professional activity is considered to be the major cause of possible poor health among the managers. Depression may result from a stress overload of managers. Aim. The authors wanted to determine the vulnerability to depression related to professional stress among healthcare managers and to assess gender and managerial work experience-specific differences. Materials and methods. A total of 235 healthcare managers working in the Republic of Belarus with various length of managerial work experience (women n=142, men n=93; mean age 45.7±1.21 years) underwent a psychological testing aimed at determining their vulnerability to depression. The results of the study were analyzed using the STATISTICA 7.0 software. Results. The vulnerability to depression in men differed from that in women (p=0.002). The predisposition to depression had significant differences between the groups with various experience of managerial work (p=0.03). The vulnerability to depression among healthcare managers increased with the length of managerial work. The highest level of vulnerability to depression was in healthcare managers with >20 years of experience (p=0.02) both in men (p=0.003) and in women (p=0.04). Conclusion. Thus, acquiring professional competences as a factor contributing to stress resistance, skills of coping with stress and alleviating its impact on the health status is very important through the whole professional activity. Healthcare managers with the length of work experience of 1-5 years are especially in need of the appropriate knowledge and skills.


The purpose of this chapter is to explore why juggling all the different and demanding roles of a medical professional is by no means an easy task. Perhaps the biggest challenge for doctors is time management and multitasking. Much of this is part and parcel of an ordinary doctor's life, but due to the peculiar nature and complex paradigms of modern health care services, special emphasis must be put on empowering fledgling medical professionals with such managerial skills. Resident medical physicians and surgeons should at least be aware of the countless opportunities available as well as how to get the best out of them.

Paying attention to role cognition and the employment strategies of female rural laborers has great significance for the improvement of the status of women and the rational transfer of labor forces. However, there are few studies of role cognition amongst the rural female labor workforce. There is even less research on the relationship between role cognition and employment transfer strategies. In this study, a survey of 425 female rural laborers in the Sichuan Province, China, was conducted to construct a role-based cognitive scale in order to measure the cognitive levels of subjective ability, self-role expectations and role conflicts. This paper divides employment strategy into four aspects: Actual workplaces, workplace preferences, actual industries, and industry preferences. An unorganized multi-classification logistic regression model was constructed in order to explore the relationship between the role cognition and employment strategy selections. The results indicated that there was a significant difference between employment expectations and realities. Rural females with more equal modern role cognition had a higher expectation of employment. Meanwhile, those who had a strong feeling of role conflicts, tended to work in other places and choose secondary and tertiary industries. However, in reality, most were unemployed. The conflict of roles clearly restricts employment options of the rural female workforce. This study enhances our understanding of the relationship between the perceptions of the Chinese female labor workforce and the employment strategies used. The findings can help to provide a reference for guiding employment policies and the flow of the female labor workforce.

The article analyzes the key approaches to the phenomenon of stress in the teacher's professional activity. The views of scientists on its nature, the factors that give rise to its appearance and deployment are presented. The concepts of "work stress", "organizational stress", "professional stress" are distinguished. Professional stress among teachers is viewed through the prism of an individual's individual response to the impact of a difficult situation (psychological approach). The teacher's reaction to the increased demands of the social environment for his activities is investigated (social approach). The psychological state of the teacher, determined by the powerful action of factors associated with the implementation of professional tasks (professionally oriented approach) is described. The importance of an integrated approach to the study of psychological characteristics of professional stress of teachers is emphasized. Currently, there is an acute lack of personal resources for the effective solution of current pedagogical problems, which leads to stress. The author has characterized the types of professional stress by the nature of the impact on the personality of the teacher and his/her activities. The factors determining the transformation of situational stress into chronic occupational stress have been determined. The features of informational, emotional and communicative stress in the teacher's professional activity are described. The types of stressors in pedagogical activity are highlighted. Particular attention is paid to objective and subjective stressors, in particular to interpersonal, mid-personal and motivational. General and special occupational stressors are described in detail. The differences from stressors that are controlled and not amenable to control in the conditions of pedagogical activity are determined. Attention is focused on the fact that professional stress among teachers can become a serious threat to the successful deployment of professional activity and lead to professional burnout. Cases in which professional stress becomes a stimulus prompting a teacher to develop and improve are identified.

The specific features and content of boxing coach-to-be professional training at Ukrainian higher educational institutions are substantiated in the article. It is established that boxing coach-to-be professional training is an integrated multicomponent system that is aimed at harmoniously developed specialist formation. The content of boxing coach-to-be professional training determines the list and volume of normative and elective disciplines, the sequence of their learning, specific forms of study, etc. It is established that the practice-oriented approach is aimed at getting not only knowledge but skills and practical experience. The analysis of the complex of literary sources made it possible to establish the fact that practice-oriented approach is fairly often considered by the researchers to be the method of teaching and learning in combination with practical activity, the orientation of educational process to the final education product that is the formation and development of professional competences. In this research, a practice-oriented approach is considered to be one of the most effective means of professionalization of boxing coach-to-be training since it brings the students as much as possible to future professional activity. It is aimed at the organization of the educational process in the context of the future profession by reflection in forms and methods of students’ educational activity of real psychological and social cases which accompanies the professional activity of a coach. It focuses our attention on the composite design principle as one of the factors of increasing specialists-to-be training quality. It is proved that a complex of practical works is to be the heart of professional training of a coach. The classification of educational practices is carried out; their volume and content are investigated. We see prospects for further research in the direction of a set of educational practices for the sports specialization of the boxing coach-to-be and in the development of an integrated network of disciplines.

The aim: Determining the need to differentiate the criminal liability of medical workers for damage caused in the course of professional activity. Materials and methods: This following research is based on an analysis of laws (21 laws), court judgments and case files (108 judgments and 8 case files), judgments of the European Court of Human Rights (4) and the legal doctrine of criminal law. Comparative, systematic, analytic, and empiric methods have been used in this research. During the preparation of this article the results of personal experience of the scientific work (17 years), the experience of advocacy (11 years), and the experience of the Supreme Court's judge have been applied. Results: The analysis of case files, thoughts of scientists and lawyers-practitioners allowed to propose criteria and indicators influencing increasing or decreasing of social dangerousness of actions committed by medical professionals. It is noted that the necessity of the legislator to consider the close interrelation of professional medical services and influence on the health of persons who demand medical services or need health care during differentiation of criminal liability. Conclusions: The necessity of differentiating approach to the criminal liability of medical professionals who inflict health damages or death is stated in comparison with the liability of general subjects of a crime.

Тенденции развития современного российского общества, как глобализация и цифровизация, повышают требования не только к интенсивности работы государственных служащих, но и развитию их как субъектов профессиональной деятельности. Ведущая идея статьи заключается в том, педагогическими основами профессионального развития молодых госслужащих является не только система основного и дополнительного образования, но и институт наставничества, что актуализирует вопросы субъектно-ориентированного подхода и развитие госслужащих как субъектов профессиональной деятельности через различные системы их подготовки и переподготовки. В этой системе подготовки и переподготовки госслужащих особое внимание уделяется институту наставничества на основе теоретического анализа изучаемой проблемы и результатов пилотного исследования. The article examines issues related to the pedagogical foundations of training personnel for the civil service on the example of the Republic of Tatarstan (RT) in the context of their professional formation and development. The development trends of modern Russian society, such as globalization and digitalization, increase the requirements for not only the intensity of the work of civil servants, but also their development as subjects of professional activity. The leading idea of the article is that the pedagogical foundations of the professional development of young civil servants are not only the system of basic and additional education, but also the institute of mentoring, which actualizes the issues of the subject-oriented approach and the development of civil servants as subjects of professional activity through various systems of their training and retraining. In this system of training and retraining of civil servants, special attention is paid to the institution of mentoring on the basis of a theoretical analysis of the problem under study and the results of a pilot study.
