The state of microcirculation and identification of microcirculatory predictors of chronic pain in younger patients with functional disorders of the masticatory system affected by connective tissue dysplasia

Relevance. Occlusal splints, made for the patients with temporomandibular disorders (TMD), influence proprioceptive sensitivity, sensory characteristics of pain, parameters of microcirculation of the pulp and periodontium of the healthy teeth. Condition of the pulp and periodontium of the healthy teeth should be assessed in patients with TMD associated with connective tissue dysplasia (CTD).Materials and methods. The study examined 36 TMD patients aged 26.3 ± 1.3 лет (М ± m) with CTD (n = 20, main group) and without CTD (n = 16, control group); with natural teeth, without complaints of TMD, masticatory muscles and periodontal condition. Laser doppler flowmetry (LDF) helped to analyze blood flow parameters in the periodontium and pulp of the healthy teeth. Received results were statistically analyzed.Results. Wavelet analysis revealed different microcirculatory flowmotion: high-amplitude irregular type was in patients of the main group, low-amplitude regular type was in controls. In the main group, perfusion is supported by high values of flux and vasomotion, ergotropic sympathetic effects (controlled by catecholamines) prevail, myogenic tone is reduced, elevated microvascular tone reflects possible changes in rheologic blood properties, microcirculatory signs of pain syndrome and relatively elevated ischemic index in pulp microcirculation of healthy teeth.Conclusion. Sensory and regulatory mechanisms, reflected by the condition of periodontium and pulp blood flow, can be regarded as early signs of inflammation and chronic pain in the diagnosis of TMD in patients without complaints.

Burn disease affects the functioning of almost all functional systems, having a negative effect on them due to the development of endotoxicosis and vascular disorders. At the same time, the greatest attention is paid to the shifts that form in the systemic hemodynamics, primarily in the coronary and pulmonary blood flow, which can cause the development and progression of life-threatening conditions. At the same time, the microcirculatory system is also undergoing changes, but these changes have been studied in much less detail. The aim of this research was to study the state of skin microcirculation in healthy and burned rats. The experiment was carried out on 20 adult male Wistar rats divided into two equal groups. The first group of animals (n=10) was a control group (intact), with its representatives there were not any manipulations, except for a single study of the state of microcirculation. Rats of the second (main) group (n=10) were simulated contact thermal burn using their own patented method. The assessment of microcirculation parameters in the main group of animals was carried out by laser Doppler flowmetry on the device "LAKK-M" on the 1st day (2-3 hours after the injury), on the third and tenth days after the burn period. It was found that thermal injury has a negative impact on the microcirculation system, which is seen in a decrease in the intensity of blood flow through small-diameter vessels. This is achieved by reflex vasospasm, reducing the release of nitrogen oxide by endothelial cells and activating precapillary sphincters, and simultaneously activating the shunting ways of cardiac shunt from the arterial directly to the venous channel, bypassing the capillaries. Thus, in the post-thermal period, a compensatory "robbing phenomenon" is realized in the border zone of the burn, which requires pathogenetic correction.

The aim: Of work is to determine changes in blood flow in the vessels of the anterior abdominal wall that occur after plastic surgeries in order to improve the results of operations and to develop new methods for the prevention of complications. Materials and methods: The study was conducted in 132 patients. Patients were divided into 2 groups: main group 64 and control group 68 patients. Main group has patients who underwent abdominoplasty in combination with liposuction; control group has patients who underwent abdominoplasty without liposuction. In both groups we make different simultaneous operations. Laser Doppler Flowmetry and Ultrasonic Doppler Flowmetry were performed to determine the blood flow indices in the flaps. Results: Liposuction volumes averaged 3.57 ± 0.74 liters of lipoaspirate. In the main group there were totaly 4 complications, in the control group complications developed in 9 patients. Comparing daily indicators between the two groups, no statistically significant difference in the dynamics of MI changes was found during the entire study period (p = 0.767). Increase in caliber of vessels, on average, from 1.55 ± 0.8 mm in the preoperative period to 1.68 ± 0.75 mm on the 14th day of the postoperative period was statistically significant (p < 0.05). Conclusions: The combination of abdominoplasty with liposuction and simultaneous operations does not lead to greater development of complications and allows to achieve good aesthetic results.

The study of Doppler parameters of hemodynamics of internal genital organs was carried out in 102 patients with chronic pelvic pain syndrome, 54 men and 48 women. It was found that against the background of extracorporeal magnetic stimulation of the neuromuscular apparatus of the pelvic floor and peloid therapy in patients of the main group, there is an improvement in the Doppler parameters of the hemodynamics of the internal genital organs in the form of a decrease in angle-independent indices: systolic-diastolic ratio, resistance and pulsation index, an increase in blood flow velocity on average by 36,2% in men and 42,1% in women from the initial values with the results being preserved for 6 months. In patients of the control group, Doppler measurements of the arteries of the prostate and ovaries showed no significant dynamics of vascular indices and blood flow velocities in the presence of a tendency to unidirectional shifts in parameters with the main group.

Purpose. To study the effect of undifferentiated connective tissue dysplasia (UCTD) on the clinical features of the course of school myopia. Material and methods. The object of study – 120 children (240 eyes) aged 12–15 years with moderate axial myopia. In addition to standard examination methods, computer accommodography and measure of convergence were carried out. Two observation groups were formed: the main – 80 people (160 eyes) with clinical manifestations of UCTD, the control – 40 people (80 eyes) without clinical manifestations of UCTD. According to the severity of clinical manifestations of UCTD in the main group, 3 subgroups were distinguished: 1–A – 25 children (50 eyes) with a weak degree of UCTD, 1–B – 38 children (76 eyes) with a moderate degree of UCTD, 1–C – 17 children (34 eyes) with a pronounced degree of UCTD. Results. Locomotor syndrome prevails in the structure of UCTD in children of the main group. A comparative analysis of the frequency of peripheral retinal degeneration revealed that most often they are detected in children with myopia associated with UCTD. In children of the main observation group, weakness of accommodation prevails in the structure of accommodation disorders (p<0,05). In the general population of examined in 39 children (32,5%), the presence of esophoria of varying degrees of compensation was diagnosed. A statistically significant difference was revealed between the control group and the subgroup of children with myopia associated with a pronounced degree of UCTD. Conclusion. Phenotypic signs of locomotor syndrome prevails in the structure of UCTD. School myopia in children with UCTD is characterized by: an increase in the frequency of peripheral retinal degeneration in 2,9 times; an increase in the proportion of children with a weakness of accommodation – in 1,7 times and violations between accommodation and convergence – in 2,2 times, compared with children with myopia without UCTD.

Aim. To study the functional state of peripheral and microcirculatory blood flow in patients with pulmonary hypertension associated with systemic connective tissue diseases (PH-SCTD).Material and methods. The study included 22 patients (50,1±14,7) with PH-SCTD I–III functional class. 18 patients had systemic scleroderma, 3 – systemic lupus erythematosus, and 1 patient suffered from rheumatoid arthritis. The control group (CG) comprised 25 healthy volunteers (39,3±10). The studies were performed using finger photoplethysmography (FPG), laser Doppler flowmetry (LDF), and capillaroscopy (CS).Results. According to the FPG data, patients with PH-SCTD relative to CG have lower values of occlusion index in the amplitude of 1,51±0,37 and 2,28±0,46 respectively (<0.05). According to LDF data, signs of venular plethora are observed in patients with PH-SCTD, which is manifested by an increase in the amplitude of respiratory-induced fluctuations in blood flow to 0,17±0,11 (PU) versus 0,13±0,07 (PU) in CG (<0.05), an increase in the constrictor reaction in the cold test was 56,4±16,8 % and 42,1±17,9 % (<0.05) and a venous occlusion test was 56,8±18,7 % and 45,2±17,9 % (<0.05), respectively. According to CS data, patients with PH-SCTD have a noticeable increase in the degree of hydration of the interstitial space, which manifests itself in an increase in the size of the pericapillary space to 160,8±37,3 μm against 114,1±17,4 μm in the CG (<0.00001).Conclusions. In patients with PH-SCTD, all regulatory mechanisms and structural components of microvessels, including endothelial and smooth muscle cells, are involved in the pathological process, which is manifested by an increase in the constrictor activity for all types of stimuli and a violation of the filtration-reabsorption mechanism of metabolism.

Introduction. Despite a long history of the disease, genital prolapse still have not been definitively solved. A relevance of pelvic organ prolapse in women has been increasing in recent years largely due to a change in the quality of life and desire to sustain their youth. The number of studies pinpointing the cause of the disease in connective tissue dysplasia (CTD) has been increasing annually.Aim: to conduct a systematic analysis and determine most common signs of CTD in women with genital prolapse.Materials and methods. CTD criteria and the relationship with genital prolapse were analyzed. The study was conducted using questionnaires and including clinical studies examining residents of the Belgorod region (135 women). The main group included 91 patients who had signs of pelvic organ prolapse, and control group consisted of 44 healthy women. CTD intensity score proposed by T.Yu. Smolnova et al. (2003) was used to assess signs of dysplasia in women examined allowing to build up three groups in which each symptom was evaluated as severity score.Results. Asthenic constitution was noted in high percentage of women suffering from genital prolapse (16.70 ± 0.38 %) compared with healthy women (2.30 ± 0.15 %; p = 0.016). Mild bruising, increased tissue bleeding tended to rise in main vs. control group (16.50 ± 0.37 % in main group and 0.0 in control group; p = 0.016). Signs of varicose veins and hemorrhoids requiring no surgical treatment were found in 33.00 ± 0.47 % and 4.50 ± 0.21 % in main vs. control group (p = 0.0002), respectively. Genital prolapse and hernia in first-line relatives were recorded in 9.90 ± 0.30 % in main group (p = 0.031), women without signs of prolapse did not indicate prolapse in close relatives.Conclusion. Asthenic constitution, hernias, varicose veins and hemorrhoids, female pelvic and hernial prolapse in close female relatives, a tendency to mild bruising, skeletal anomalies, elastosis skin were among common CTD signs in women with genital prolapse.

Introduction. Back pain is not only a high prevalence, but also a costly health problem. That is why the search for new and the optimization of existing methods of dorsopathies treatment acquire an important clinical and medico-social significance. In recent years, the medical community has increased the interest in non-drug methods of treatment, including osteopathy. Previous studies have substantiated the possibility of effective application of osteopathic correction methods in the treatment of patients with dorsopathies. A specific object of the osteopath′s work is somatic dysfunction (SD). And if earlier SD was perceived from the standpoint of purely biomechanical disorders, then in recent years there have been serious changes in understanding the heterogeneity of this state nature. The biomechanical, rhythmogenic and neurodynamic components are conventionally distinguished in the structure of SD. One of the neurodynamic disorders types is the violation of motor stereotypes, the so-called static-dynamic disorders, revealed through special dynamic tests and manifested in the functional impossibility of building of active movement at various levels. At the same time, the generally accepted biomechanical approaches do not allow to completely eliminate violations of the statodynamic stereotype.The goal of research — the study was to research the effectiveness of the osteopathic approach using in combination with kinesitherapy for correction of the statodynamic stereotype violations in patients with dorsopathy at the cervicothoracic level.Materials and methods. A controlled randomized prospective study was conducted on the basis of a private rehabilitation center «Ezramed-Clinic» in Omsk in the period from February 2019 to December 2019. 52 patients with a diagnosis of dorsopathy at the cervicothoracic level were observed. 12 people were knocked out during the study in accordance with the exclusion criteria. As a result, 40 patients participated in the study. Depending on the applied treatment method, the patients were divided by simple randomization into two groups (main and control), each of which consisted of 20 people. Both groups of patients received osteopathic correction three times with a frequency of receptions 1 time in 7–10 days. The main group of patients additionally independently performed daily for 10 minutes a set of exercises aimed to restore the disturbed motor stereotypes (normal synkinesis). The control group of patients additionally independently performed a set of exercise therapy for the cervical spine every day for 10 minutes. All patients, regardless of the group, underwent an osteopathic examination before and after treatment with the formation of an osteopathic conclusion; the severity of pain syndrome and the volume of active movements in the cervical spine were assessed. The severity of the pain syndrome and the range of active movements were assessed before and immediately after treatment, as well as 3 months after the first session.Results. The use of osteopathic correction in conjunction with kinesitherapy (both special exercises and a complex of exercise therapy) in patients with a diagnosis of dorsopathy at the cervicothoracic level leads to a statistically significant increase in the range of motion in the cervical spine in the sagittal and frontal planes (p<0,05). The combination of osteopathic correction together with exercise therapy in patients of the control group led to a statistically more significant increase (p<0,05) in the range of motion in the cervical spine in the frontal plane after treatment compared with the results of the main group. However, 3 months after treatment, the indicators in patients of both groups did not have statistically significant differences.The combination of osteopathic correction in conjunction with kinesiotherapy in the form of special exercises for the motor stereotype correction in the main group of patients with diagnosed dorsopathy at the cervicothoracic level led to a statistically significant (p<0,05) decrease in the intensity of the pain syndrome at the 2nd session.Conclusion. In order to increase the effectiveness of treatment, osteopathic correction of somatic dysfunctions in patients with dorsopathy at the cervicothoracic level can be supplemented with complex kinesitherapy, both in the form of traditional exercise therapy, and in the form of special exercises for correction of altered motor stereotype.

The aim: To evaluate the peculiarity of clinical manifestations of neonatal respiratory distress syndrome (NRDS) in deeply premature infants from mothers with phenotypic markers of undifferentiated connective tissue dysplasia (UCTD). Materials and methods: The study represent the results of a retrospective clinical and statistical analysis of 268 premature birth report card and newborn report sheet. .The main (1 group) included 50 pregnants with obvious phenotypic markers of UCTD, the comparison group (group 2) consisted of 50 pregnant women without phenotypic markers of UDCTD. Results: According to the study, in 12 (24%) pregnant women of the main group at the time of admission to the clinic had contractions,which required specific therapy. Cervical cerclage was performed in 38 (76%) patients of the main group due to the presence of cervical insufficiency (CI). In these cases, the severity of the CI on the Steinber scale was 7.2 &#177; 0.4 points in the main group against 4.4 &#177; 0.2 points in the comparison group (p &#60;0.05). Group I patients were more likely to have complications of labor such as:premature rupture of membranes, uterine contraction abnormalities and fetal distress, which required in most cases cesarean delivery (7% and 2%), respectively (p &#60;0.05). The incidence of neonatal complications requiring respiratory support was 67% in group I and 48% in group II. According to our observations, the clinical manifestations of bronchopulmonary dysplasia were twice as high in infants of the main group (66%) against (44%) of the comparison group (p &#60;0.05). Conclusions:1.Neonatal respiratory distress syndrome in premature infants is more often associated from mothers with UDCTD. 2. The high importance of steroid prophylaxis of NRDS and antioxidant therapy in reducing the frequency of mechanical ventilation and the development of bronchopulmonary pathology, especially in infants from mothers with UDCTD syndrome, has been proven. 3. The possibility of diagnosing disorders of functional maturation of the lungs in the fetal period using a non-invasive method of ultrasonography has been confirmed.

Results of randomized prospective study with participation of 94 patients aged from 20 to 70 years with monosegmental lumbar spine lesions are presented. Minimum invasive surgical interventions were performed in 55 patients from the main group. Control group consisted of 39 patients in whom decompressive-stabilizing operations via conventional posteromedian approach with skeletization of posterior segments of vertebral column were performed. Average size of operative wound in open interventions more than 10 times exceeded that size in minimum invasive interventions and made up 484 ± 56 and 36 ± 12 sq.cm, respectively. Mean blood loss was 326.6 ± 278.0 ml in the main group and 855.1 ± 512.0 ml in the comparative one. In the main group no one patient required substitution hemotransfusion, while in 13 patients from the comparative group donor erythrocytic mass and/or fresh-frozen plasma were used to eliminate the deficit of blood components. Intensity of pain syndrome in the zone of surgical intervention by visual analog scale in the main group was lower than in comparative group. In the main and comparative groups the duration of hospitalization made up 6.1 ± 2.7 and 9.7 ± 3.7 bed days, respectively. In no one patient from the main group complications in the zone of operative wound were noted. Three patients from the comparative group required secondary debridement and in 1 patient early deep operative wound suppuration was observed. Application of low invasive surgical techniques for the treatment of patients with degenerative lumbar spine lesions enabled to perform radical surgical treatment with minimal iatrogenic injury. The method possessed indubitable advantages over the conventional open operations especially intraoperatively and in early postoperative period.

The aim: Is to evaluate the effectiveness of the proposed method of constructing occlusal surfaces of orthopedic structures by monitoring the dynamics of functional indicators of occlusion and the functional condition of the masticatory muscles. Materials and methods: The object of the study was made up by 64 patients with included defects of the dentition of the 3rd class according to Kennedy. Patients were divided into two equal groups – the main group (patients were treated by using the suggested method of construction of occlusal surfaces of orthopedic structures) and control (dentition defects were replaced according to the traditional protocol). Results: The results of evaluating the effectiveness of the proposed method showed that the index of symmetry of the anterior bundles of the temporal muscles dropped by 18.5%, the index of symmetry of the masticatory muscles reduced by 22%, the torque of the mandible reduced by 32%, the index of stabilizing occlusal – by 27%, relative efforts of balance disturbance for more than 10 % regarding the left and right reduced by a factor of 4.4, the rate of opening time was 1.5 times higher than before, the rate of time of opening of the jaws – 1.8 times. Conclusions: The use of the suggested method of orthopedic treatment with temporary structures leads to probable changes in the studied functional parameters in comparison with the control group (p <0,05).
