Burn disease affects the functioning of almost all functional systems, having a negative effect on them due to the development of endotoxicosis and vascular disorders. At the same time, the greatest attention is paid to the shifts that form in the systemic hemodynamics, primarily in the coronary and pulmonary blood flow, which can cause the development and progression of life-threatening conditions. At the same time, the microcirculatory system is also undergoing changes, but these changes have been studied in much less detail. The aim of this research was to study the state of skin microcirculation in healthy and burned rats. The experiment was carried out on 20 adult male Wistar rats divided into two equal groups. The first group of animals (n=10) was a control group (intact), with its representatives there were not any manipulations, except for a single study of the state of microcirculation. Rats of the second (main) group (n=10) were simulated contact thermal burn using their own patented method. The assessment of microcirculation parameters in the main group of animals was carried out by laser Doppler flowmetry on the device "LAKK-M" on the 1st day (2-3 hours after the injury), on the third and tenth days after the burn period. It was found that thermal injury has a negative impact on the microcirculation system, which is seen in a decrease in the intensity of blood flow through small-diameter vessels. This is achieved by reflex vasospasm, reducing the release of nitrogen oxide by endothelial cells and activating precapillary sphincters, and simultaneously activating the shunting ways of cardiac shunt from the arterial directly to the venous channel, bypassing the capillaries. Thus, in the post-thermal period, a compensatory "robbing phenomenon" is realized in the border zone of the burn, which requires pathogenetic correction.

Relevance. Occlusal splints, made for the patients with temporomandibular disorders (TMD), influence proprioceptive sensitivity, sensory characteristics of pain, parameters of microcirculation of the pulp and periodontium of the healthy teeth. Condition of the pulp and periodontium of the healthy teeth should be assessed in patients with TMD associated with connective tissue dysplasia (CTD).Materials and methods. The study examined 36 TMD patients aged 26.3 ± 1.3 лет (М ± m) with CTD (n = 20, main group) and without CTD (n = 16, control group); with natural teeth, without complaints of TMD, masticatory muscles and periodontal condition. Laser doppler flowmetry (LDF) helped to analyze blood flow parameters in the periodontium and pulp of the healthy teeth. Received results were statistically analyzed.Results. Wavelet analysis revealed different microcirculatory flowmotion: high-amplitude irregular type was in patients of the main group, low-amplitude regular type was in controls. In the main group, perfusion is supported by high values of flux and vasomotion, ergotropic sympathetic effects (controlled by catecholamines) prevail, myogenic tone is reduced, elevated microvascular tone reflects possible changes in rheologic blood properties, microcirculatory signs of pain syndrome and relatively elevated ischemic index in pulp microcirculation of healthy teeth.Conclusion. Sensory and regulatory mechanisms, reflected by the condition of periodontium and pulp blood flow, can be regarded as early signs of inflammation and chronic pain in the diagnosis of TMD in patients without complaints.

The aim: Of work is to determine changes in blood flow in the vessels of the anterior abdominal wall that occur after plastic surgeries in order to improve the results of operations and to develop new methods for the prevention of complications. Materials and methods: The study was conducted in 132 patients. Patients were divided into 2 groups: main group 64 and control group 68 patients. Main group has patients who underwent abdominoplasty in combination with liposuction; control group has patients who underwent abdominoplasty without liposuction. In both groups we make different simultaneous operations. Laser Doppler Flowmetry and Ultrasonic Doppler Flowmetry were performed to determine the blood flow indices in the flaps. Results: Liposuction volumes averaged 3.57 ± 0.74 liters of lipoaspirate. In the main group there were totaly 4 complications, in the control group complications developed in 9 patients. Comparing daily indicators between the two groups, no statistically significant difference in the dynamics of MI changes was found during the entire study period (p = 0.767). Increase in caliber of vessels, on average, from 1.55 ± 0.8 mm in the preoperative period to 1.68 ± 0.75 mm on the 14th day of the postoperative period was statistically significant (p < 0.05). Conclusions: The combination of abdominoplasty with liposuction and simultaneous operations does not lead to greater development of complications and allows to achieve good aesthetic results.

Considering the particular danger of remote skin reactions to ultraviolet irradiation (UVI), it is advisable to use ointments with antioxidant activity to reduce its negative effect on the skin. The rationale for the choice of ointments with antioxidant activity was the fact that they reduce the damaging effect of ultraviolet radiation in the erythemal and early post-erythemal period. The presence of a regular connection between the development of the early and late periods has given reason to assume the protective effect of ointments on the remote skin reactions. Objective: to study the effect of thiotriazoline ointment and thiotriazoline ointment with silver nanoparticles on the state of the morphological structures of the skin of guinea pigs after local UVI. Material and methods of research. The study involved 132 albino guinea pigs weighing 400-500 g, divided into 4 groups: 1 - intact, 2 - control (guinea pigs subjected to local UVI), 3 and 4 main ones. The third main group included guinea pigs that after UVI were administered thiotriazoline ointment in the treatment and prophylactic regime, the fourth main group included guinea pigs that after UVI were administered thiotriazoline ointment with silver nanoparticles in the same mode as Group 3. Ointments were applied 1 hour before irradiation and daily until erythema disappeared. Ultraviolet erythema was caused by irradiation in 1 minimum erythemal dose. After 2, 4 hours, on the 3rd, 8th, 15th, 21st, 28th day, the fragments of irradiated skin were investigated using histochemical and morphometric methods (fibroblast density and epidermis thickness). Results. Morphological changes in the skin after applying ointments with antioxidant activity were unidirectional. It was revealed that in the early periods after irradiation, thiotrazoline ointment and thiotrazoline ointment with silver nanoparticles do not affect changes in the thickness of the epidermis, but statistically significantly reduce the density of fibroblasts in the dermis on the 3rd day of the experiment compared to the control group. In the later periods, under the influence of thiotriazoline ointment, a gradual decrease in the thickness of the epidermis, which reached the norm by the end of the experiment, was observed. On the 8th day, the maximum density of fibroblasts was recorded, in the subsequent periods of the experiment, the index gradually decreased, which was accompanied by collagenization of the papillary layer in the loci of damage to collagen and elastic fibers detected in 50% of cases. In later times, under the influence of thiotriazoline ointment with silver nanoparticles, the processes of restoring the morphological structures of the skin occurred faster. In parallel with the decrease in the density of fibroblasts in the loci of the previous damage to the collagen and elastic fibers of the papillary layer, thickening of collagen fibers was observed, replacing them with segments of destruction of elastic fibers. In this group, at the end of the experiment, the collagenization locus was small, single, occurring in 16.7% of cases. Conclusions Ointments with antioxidant activity exert a positive effect on the state of morphological structures of the skin, damaged as a result of local UVI, in erythemal and post-erythemic periods. In the early periods after the local UVI, there was a general tendency for the effect of both ointments, as they reduced the density of fibroblasts on the 3rd day, but did not result in complete normalization. In the late period after local UVI , under the influence of thiotriazoline ointment and thiotriazoline ointment with silver nanoparticles, thickness of the epidermis (by 21st and 15th day, respectively) and density of fibroblasts (by the 28th day) decreased to normal while without treatment both indicators exceeded the norm by several times for 28 days of the experiment.

Abstract Background and Aims Balloon angioplasty (BA) without the use of stents has unsatisfactory results, which may cast doubt on its expediency. At the same time, BA is a very expensive treatment method. We conducted a comprehensive comparative analysis of the native arteriovenous fistula (AVF) patency rates in hemodialysis patients with central venous stenosis (CVS) after endovascular BA and «open» palliative surgery. Method A retrospective study included 80 patients with confirmed central vein stenosis: subclavian, brachiocephalic veins, vena cava inferior, or multiple lesions. The main group included 39 patients who underwent percutaneous balloon angioplasty. The control group included 41 patients who did not have balloon angioplasty for various reasons. In this patients we performed only «open» palliative interventions: thrombectomy, proximalization of arteriovenous anastomosis, AVF blood flow reduction. Results Functional primary patency (the time interval between the start of AVF using and the first intervention) did not differ: groups were comparable in time of stenosis manifestation (fig. 1A). Primary patency (the time interval between the first and second interventions) after BA was statistically significantly better than in the main group (fig. 1B), but difference was minimal: median survival in the study group of 8 months [95% CI 6; 10] vs. - 6 months [95% CI 4.9; 7.1]. There was the strong negative correlation between the primary patency and functional primary patency in the main group (r = –0.627 [95%CI –0.787; –0.388], p &lt;0.0001) but not in the control group (r = 0.049 [95%CI –0.262; –0.351], p = 0.7599). Thus, the later manifestation of CVS related with lower effectiveness of BA. The functional secondary patency (total duration of AVF use) in the main group was significantly better: median survival was 47 months [95% CI 40.9; 53.1] vs. 34 months [95% CI 29.8; 38.2] as well as secondary patency (the time interval between the first intervention and the complete cessation of AVF use): median survival was 16 months [95% CI 12.5; 19.5] vs. 7 months [95% CI 4.9; 9.1] (fig. 1 C and D). The occlusion-free period from the moment of starting the AVF use (functional primary assisted patency – fig. 1E) was higher in the main group, but difference was minimal: median survival was 39 months [95% CI 36.5; 41.5] vs. 32 months [95% CI 27.5; 36.5], as well as occlusion-free period from the moment of the first surgical intervention (primary assisted patency – fig. 1F) median survival was 9 months [95% CI 7; 11], in the control group - 7 months [95% CI 5.6; 8.4]. The need for open interventions was lower in the main group: 0.374 [95% CI 0.24; 0.556] and 2.451 [95% CI 1.1963; 3.023] per 10 patient-months, incidence rate ratio (IRR)= 0.153 [95% CI 0.095; 0.237], р&lt;0.0001; as well as overall need for interventions: 1.511 [95% CI 1.225; 1.843] and 2.451 [95% CI 1.963; 3.023] per 10 patient-months, IRR 0.617 [95% CI 0.461; 0.825] p=0.0011. The value of AVF volume blood flow had a strong negative correlation with the primary patency in both groups (r = –0.529, p =0.0027; r = –0.419, p =0.0101). Conclusion 1. Central vein stenosis is inevitably leads to loss of vascular access on the ipsilateral side. 2. Balloon angioplasty allows to extend the period of AVF use but it is not a radical treatment method of CVS. 3. The results of balloon angioplasty are significantly affected by the length of the period from the start of AVF use to the CVS manifestation. 4. Multiple repeated BA are apparently justified in patients for whom the possibility of creating a new vascular access is doubtful. 4. The AVF volume blood flow is an important factor determining the severity of CVS clinical manifestations and the need for repeated surgical interventions.

Introduction. The COVID-19 pandemic multiplies the incidence of long-term adverse events. Family history of heart disease predicates the necessity of study hemostasis in hypertensive patients. The purpose was to study the microcirculation in hypertensive COVID-19 survivors. Materials and methods. We selected patients treated at the hospitals of Samara from January to March 2021. COVID-19 survivors (diagnosis was confirmed by polymerase chain reaction analysis) were divided into 2 groups and analysed: the group with family history of hypertension (167 patients) and control group with no relevant past medical history (68 patients). The main microcirculation characteristics were evaluated by laser Doppler flowmetry of skin blood flow. Results. The average age of COVID-19 survivors was 52.3±14.2 years, n=86 (51.4 %) – male. The majority of patients (n=61/36.5 %) were 45 to 54 years. A fifth of the patients (n=35/20.9 %) showed no complaints about the underlying disease, the clinical manifestations of the rest were general weakness, fever, shortness of breath, cough, headaches, unstable blood pressure, inflammation of the ENT organs. By comparison the microcirculation parameters of the case patients and control group, a decreased perfusion was detected. It requires a preventive medicine to prevent from early and distant micro and macrothrombosis. Conclusion. The study of microcirculation essential features by the LDF of skin blood flow revealed a pathological type with a predominance of vasoconstrictions in patients with a history of hypertension. Taking into account the proven increase in the incidence of thrombotic events in COVID-19 survivors, the noticed changes in microcirculation require a new approach to prevent from adverse events concerning the hypercoagulation process, especially in patients with famil y history of heart disease.

Aim. To study the possibility of using and estimate the efficiency of a new tablet form of biologically active supplement in patients with vegetovascular dystonia syndrome. Materials and methods. One hundred patients with vegetovascular dystonia syndrome, including 50 patients of the main group and 50 patients of the control group were examined. There is presented the composition of prescription formula of biologically active supplement, based on participation of its components in the correction of disturbed metabolic processes of the nervous system. Results. Additional application of biologically active supplement in combination with drug significantly improves cerebral blood flow, decreases blood viscosity and risk of cellular membrane impairment, normalizes arterial pressure, elevates initiative and attention, reduces headache in patients of the main group with vegetovascular dystonia syndrome. Conclusions. Use of biologically active supplement has a positive effect in complex therapy of patients with vegetovascular dystonia syndrome.

The article contains the results of a study of two different methods for calculating the spectral parameters of laser Doppler flowmetry in patients with diabetic microangiopathy caused by type 2 diabetes mellitus (main group) and those with excluded diabetes mellitus (control group). Spectral indices were calculated using either average or maximum amplitudes of the frequency ranges. When comparing the contribution of respiratory and pulse fluxmotions using average amplitudes, there were significant (p < 0.05) differences between the main and control groups. On the contrary, when using the maximum amplitudes, no significant differences were noted (p > 0.05). Also, significant correlations were found between the contributions of respiratory and pulse fluxmotions and the estimated glomerular filtration rate in the main group, using both calculation methods. These studies indicate the feasibility of using a technique based on the analysis of average amplitudes to increase the specificity of laser Doppler flowmetry as a method for diagnosing diabetic microangiopathy.

The aim: was to improve the course of purulent wounds in diabetes by using physical therapies. Materials and methods: we investigated 122 patients with diabetes mellitus and wound processes. We divided all patients on the two groups. Our study groups were 50 patients who had therapy by ozone together with other general treatment and surgery treatment (daily dressings, ointments and solutions). Our control groups were 72 patients, who had treated only by general methods and surgery treatment (daily dressings, ointments and solutions).The patients of our explore group had received intravenous injection of saline ozone solution together with other general treatment and surgery treatment (daily dressings, ointments and solutions). Results: it had studied of indicators of oxidative modification of proteins, lipid peroxidation in blood plasma, antioxidant protection during this period, there were no significant changes in these parameters between the control and experimental groups. As the result of the study was found that the leukocyte index of intoxication in patients in the main and control groups was almost indistinguishable. Regarding the indexes of hematological index of intoxication, which decreased during the treatment of patients of the main group, and different from the patients of the control group, in which this indicator increased sharply. The result of the study of changes in the indicators of sorption capacity of erythrocytes showed that the level of sorption capacity of erythrocytes in patients in the main group during treatment, also decreased, compared with patients in the control group. Conclusions: Therefore, the use of ozone in the treatment of patients with complicated forms of diabetes does not cause a negative impact on the dynamics of homeostasis and indicators of the level of intoxication of the body. Ozone therapy has a beneficial effect on the course and wound process in patients with diabetes with purulent processes.

Relevance: the purpose of this work was to study the effectiveness of proposed treatment and rehabilitation complex for patients with generalized chronic periodontitis and bruxism.Materials and methods: we examined 73 patients aged 30 to 45 years diagnosed with generalized chronic periodontitis of moderate severity and bruxism. Control group included 15 healthy individuals of the same age for clarify normal functional parameters. Clinical, Doppler flowmetry and electromyographic methods were used for evaluation of the effectiveness of proposed treatment and rehabilitation complex.Results: the course of treatment resulted in the increase in blood flow perfusion volume by 37.7% (p < 0.05), of blood flow perfusion rate by 58.2% (p<0.05) from baseline values. At the same time, the decrease in bioelectric activity of temporal muscle at rest by 51.82% (p >< 0.05), at voluntary contraction – by 40.4% (p<0.05), the decrease in bioelectric activity of masseter muscle at rest – by 47.4% (p><0.05), at voluntary contraction - by 35.12% (p >< 0.05) from values before treatment, were observed.Conclusion: data obtained on the basis of electromyography and laser Doppler flowmetry showed that developed treatment and rehabilitation complex is an effective method for correcting the microcirculation in periodontal vessels and the functional activity of the muscles of maxillofacial area.
