Diabetes is a chronic disease that requires good therapeutic management and self care to achieve optimal glycemic target. The important barrier to achieve good glycemic control is a lack of knowledge about optimal glycemic targets. The purpose of this study was to determine the relationship of the level of knowledge to clinical outcome of type 2 diabetes mellitus in Yogyakarta and Bantul primary health care. This was an observational study with a cross sectional design. The sampling technique was done by accidental sampling for patients visiting the primary health care from October to December 2018 that involved 200 type 2 diabetes mellitus patients who were included in the inclusion criteria. The level of knowledge was measured by the Diabetes Knowledge Questionnaire (DKQ)-24 which had 24 questions and the clinical outcome was obtained from laboratory measurement data as well as from medical record data when the patient was in routine control. Sociodemographic data were analyzed descriptively. The relationship between the level of knowledge and clinical outcomes was analyzed using chi square. The results showed that the knowledge level score had an average value of 12. The most incorrect answer was in etiology and diabetes self care, which is eating too much sugar and sweet foods is the cause of diabetes (97%), exercise regularly increasing the need of hormone insulin or diabetes medication (82%), the insulin reaction is caused by too much food (84%) and diabetics should clean the wound with betadine and alcohol (88%). There were 106 (53%) patients who achieved good glycemic control. Chi square test showed no relationship between the level of knowledge of clinical outcomes (p = 0.328). The result of this study may become a material and education content evaluation in diabetic patients.

ABSTRACT Periodontal disease and type 2 diabetes mellitus are considered chronic diseases that at their core have a deep relationship with inflammation. It is assumed that there is a bidirectional relationship between periodontal disease and type 2 diabetes mellitus. It is estimated that approximately 10% of the world’s population is affected by periodontal disease, in its most severe form, almost the same percentage estimated for people with diabetes, which is considered a 21st century emergency. The World Workshop for the Classification of Periodontal and Peri-implant Diseases and Conditions took place from September 9-11, 2017. The aim of this study is to analyze the results of this workshop with regard to the relationship between periodontal diseases/conditions and diabetes mellitus, in addition to conducting an integrative review on the topic. A literature review was conducted, using the Medline electronic databases via Pubmed, Scientific Electronic Library Online, Scientific and Technical Literature of Latin America and the Caribbean and Virtual Health Library. A new classification of periodontal disease included tools for individual assessment of the patient and recognizing risk factors that might negatively interfere in response to treatment. The occurrence of metabolic lack of control in periodontal patients with type 2 diabetes mellitus is now considered a factor of great importance for the assessment of individual susceptibility to the progression of periodontitis. Diabetes is believed to promote a hyper inflammatory response to bacterial challenge by modifying the tissue response of periodontal tissues.

Pendahuluan. Kata Kunci: albuminuria, defisiensi vitamin D 25(OH)D, DM tipe 2  The Association between Vitamin D 25(OH)D Level and Albuminuria in Type 2 Diabetes MellitusIntroduction. Vitamin D 25(OH)D deficiency was reported as a possible risk factor for the development of diabetic nephropathy in several epidemiologic studies. Whether vitamin D 25(OH)D deficiency plays a role in the development of diabetic nephropathy in Indonesia is unknown. This study aims to determinate the association between vitamin D 25(OH)D level with albuminuria in patients with type 2 diabetes mellitus in Indonesia. Methods. A cross-sectional study was conducted in 96 patients with type 2 diabetes mellitus at outpatient clinic of MetabolicEndocrine Cipto Mangunkusumo Hospital. Serum vitamin D level was assessed using Diasorin kit with CLIA method, while albuminuria was assessed using random urine sample. Statistical analysis was conducted using chi square for bivariate analysis and regression logistic method for multivariate analysis. Results. The prevalence of vitamin D 25(OH)D deficiency in patients with type 2 diabetes mellitus was 49% with a median value 16,35 (4,2-41,4) ng/mL. There was no significant correlation between vitamin D deficiency with the severity of albuminuria (OR 0,887; 95% CI 0,335 to 2,296). Confounding factors such as poor blood glucose control and overweight strongly influenced the association between vitamin D deficiency with the incidence of albuminuria in patients with type 2 diabetes mellitus. Conclusion. The results of this study showed that there was no association between vitamin D deficiency with the severity of albuminuria in patients with type 2 diabetes mellitus in Indonesia.

In 2010, the Global Burden of Disease research revealed that cases of chronic kidney disease had increased and became a serious health problem. Indonesia Basic Health Research data in 2013 revealed that the proportion of patients with chronic kidney failure aged ≥ 15 years old based on doctor's diagnosis was 0,2% and 0,6% kidney stones. Therefore the aim of the study is to analyze the relationship between type 2 diabetes mellitus and chronic kidney disease in the population of Indonesia in 2014-2015. This type of research is observational with a cross-sectional design. The study used secondary data from the Indonesia Family Life Survey (IFLS) 5 in 2014-2015. The sampling technique used total sampling, which found 34,012 people who met the inclusion and exclusion criteria. The independent variables of this study are type 2 diabetes mellitus and chronic kidney disease as the dependent variable. Covariate variables are age, sex, smoking status, history of hypertension, and history of high cholesterol. Analysis bivariate of the relationship between type 2 diabetes mellitus and chronic kidney disease in this study using chi square and multivariate analysis using logistic regression test. The magnitude of the effect expressed in the prevalence odds ratio (POR) with confident interval (CI: 95%.) This study shows the prevalence of chronic kidney disease in Indonesia population in 2014 – 2015 is 1%. The result of chi-square test of the relationship between type 2 diabetes mellitus and chronic kidney disease in the Indonesian population showed POR=2.48 (p-value =0.0002; 95%CI 1.422-4.071). Multivariate analysis with logistic regression tests shows that smoking status, history of high cholesterol, and history of hypertension influence the relationship of type 2 diabetes mellitus and chronic kidney disease.
