Incompleteness in filling out the inpatient pain assessment sheet will have an impact on the discontinuity of information and will affect the quality of health service facilities. The purpose of this study was conducted to determine the completeness of the Inpatient Pain Assessment Sheet at Sumber Kasih Hospital, Cirebon City.This type of research is quantitative descriptive. The population is the entire medical record document from January to March in 2020 with a sample of 99 medical record documents. This research instrument uses observation sheets. The results of this study indicate that there are 1 (1%) medical record documents that are completely filled and 98 (99%) incomplete medical record documents on the pain assessment sheet form. As for the conclusion in this research the completeness of the study sheet of pain in Sumber Kasih Cirebon City Hospital 1 while the incomplete study sheet of 99. And also the advice in this research is advice for hospitals should be evaluated to maintain the quality of health services improve the performance of medical record officers in the Sumber Kasih Cirebon City Hospital.

The level of family knowledge is a very important factor in the actions of families of people with Diabetes Mellitus, behavior based on knowledge will be easier to carry out than those not based on knowledge. Diabetes Mellitus is a disease that requires proper and immediate treatment efforts because it can cause complications such as heart disease, kidney failure, and damage to the nervous system. One way to overcome the effects of Type 2 Diabetes Mellitus is by applying a diet. This study aims to determine the relationship between the level of family knowledge with efforts to prevent complications of type 2 diabetes mellitus in the work area of Babakan Puskesmas, Cirebon Regency. This research method uses descriptive correlational, this research uses cross section research. The population is all families with type 2 Diabetes Mellitus in the work area of Babakan Puskesmas in Cirebon Regency as many as 144 respondents and sampling using Purposive Sampling obtained 60 respondents. The instruments used were questionnaire sheets, and data analysis using chi-square test that is the significance value α = 0.1. The results of the above study indicate between the level of family knowledge and prevention of complications of type 2 diabetes mellitus in getting p-value = 0.04, because the p-value of 0.04 <0.1 then Ho is rejected and Ha is accepted which means there is a relationship between the level of family knowledge and prevention of complications of type 2 diabetes mellitus in the work area of Babakan Health Center, Cirebon Regency.

Antenatal care is examination pregnancy in do for pregnant women during the her pregnancy to prevent complication of pregnancy and to prepare for a healthy birth. The purpose of this research conducted to determine the implementation of the antenatal care center Kalijaga Cirebon City in 2020. The kind of research use is descriptive survey. The population in research it is a whole pregnant women who performs the first visit in the work area Kalijaga Cirebon as many as 195 pregnant women with a sampleof 66 respondents. The instrument used in this study used a checklist. The research results show that pregnant women get the service measurement of the height bodies and weight as many as 60 (91%), the measurement of blood pressure 60 (91%), measurements the upper arm circumference 60 (91%), examinatin fundus uteri high 54 (82%), screening for TT immunization 47 (71%), administration of Fe tablets 39 (59%), percent the fetus and fetal heartbeat 33 (50%), a laboratory test of 43 (65%), managementof cases kasus 33 (50%) and a dialogue 60 (91%). Conclusion obtained in this research that there are pregnant women who should be given at the time of the first. Suggested health workers to improve services to pregnant women in accordance with the care of service standards that has been determined by the government.

Diabetes mellitus is a disease that has clinical signs and symptoms due to chronic declining function. Type 2 diabetes mellitus always increases every year as much as 9.3% of people. Patients with type 2 diabetes mellitus are required to always be routine control, but patients with diabetes mellitus will experience stress caused by various stressors, to reduce these problems interventions need to provide self-efficacy to patients and coping strategies are also needed so that patients are able to adapt to their stress in order to create coping strategies the good one.The purpose of this study was to determine the relationship of self-efficacy with coping strategies in patients with type 2 diabetes mellitus who use insulin in the Polyclinic of Waled District Hospital Cirebon. This type of research uses descriptive correlational with cross sectional approach. Sampling using purposive sampling technique amounted to 34 respondents. The instrument used was a questionnaire. Analysis of data using rank test speraman.Place of research in Waled District Hospital Cirebon in July 2020. The results showed that most respondents' self-efficacy in the high category (58.8%) and coping strategies were mostly in the good category (61.8%). Spearman rank test results P value <a and r <1 then H0 = rejected means that there is a strong and positive relationship between self-efficacy and coping strategies in patients with type 2 diabetes mellitus in the Polyclinic of Waled District Hospital Cirebon (P value = 0,000: a = 0 .05 r = 0.719). It is expected that nurses should provide more motivation and support to patients with type 2 diabetes mellitus and educate about the importance of coping strategies

The first COVID-19 case in South Brebes was in the Paguyangan Community Health Center area, which originated from the Gowa cluster. One of the efforts that can be used to stop the transmission of COVID-19 is to use PPE (Personal Protective Equipment). The knowledge of medical personnel regarding the use of PPE when handling COVID-19 is important as a way to prevent transmission from COVID-19 patients to medical personnel. This study aims to determine the relationship between knowledge and attitudes of nurses and compliance with the use of personal protective equipment (PPE) during the coronavirus disease pandemic at Paguyangan Public Health Center in 2020. The design of this study is a correlation analysis with a cross sectional approach using total sampling method and 32 nurses were obtained as a sample. Data collection began in October-December 2020. The questionnaire used consisted of a questionnaire about Covid-19 knowledge and a questionnaire about nurses' attitudes in using PPE, each with 20 question items. To assess the adherence of nurses using the WHO adherence checklist for use of PPE adaptation. The results of the analysis showed that there was a significant relationship between knowledge of nurses and adherence to the use of PPE with a value of p = 0.021 (p <0.05) and a significant value of the relationship between nurses' attitudes and compliance with the use of PPE during the Covid-19 pandemic at the Paguyangan Health Center in 2020. p = 0.017 (p <0.05). So it can be concluded that there is a significant relationship between attitudes and compliance with nurses at Paguyangan Health Center. The results of this study can be used as a reference for nurse credentialing in the use of ADP and for further research, research can be carried out by adding variables such as length of service, availability of facilities, level of nurse education and supervision of the use of PPE.

Knowledge of nutritional intake of pregnant women is the result of knowing about the amount of nutrition that enters through daily food consumption by pregnant women. Chronic lack of energy is an occurrence where pregnant women experience a shortage of nutrients that lasts for a long time or is caused by an imbalance in nutritional intake, so that the nutrients needed by the body are not fulfilled. This study aims to determine the relationship of nutritional intake knowledge with chronic energy deficiency events in pregnant women at Kalijaga Health Center. This research method uses analytical survey, with a cross-sectional research design. The population is children of pregnant women the total sample is 24 pregnant women. The sampling technique used total sampling. The instruments used were questionnaire sheets, Upper arm circumference and observation sheets. Data analysis used Spearman rank with significance value α = 0.05. The results of univariate analysis, knowledge of good nutritional intake (70.8%), those who have sufficient knowledge (12.5%), and those who have less knowledge (16.7%), while pregnant women who experience chronic energy deficiency amounted to (16.7%) and pregnant women who do not experience chronic energy deficiency (83.3%). Bivariate analysis with the Spearman rank test shows the p-value <ᾳ (p-value = 0,000: ᾳ = 0.05) r0.808, so that H0 is rejected, meaning that there is a very strong and positive relationship between knowledge of nutrient intake with less energy events chronic in pregnant women at UPT. Kalijaga Health Center, Cirebon City.

The implementation of coding done in the medical record should be done very thoroughly, precisely and accurately according to the diagnostic code that exists in ICD-10. If there is an error in coding, it will have a bad impact on the patient, puskesmas or hospital. But in the fact found in the field there are still problems in the implementation of the accuracy of the encoding of disease diagnosis based on ICD-10. The purpose of this research is to describe the accuracy of disease code in the medical record in Puskesmas Plumbon. The population in this study was the entire quarterly medical record document of year 2020 with a total of 1,098 medical record documents with a total sample of 92 documents using simple random sampling technique. This study was conducted in Plumbon Puskesmas Medical record unit on 20 June 2020. The method used is descriptive with a quantitative approach. The data collection procedures used are by observation and checklist sheet. The population in this study was the entire quarterly medical record document of year 2020 with a total of 1,098 medical record documents with a total sample of 92 documents using simple random sampling technique. This study was conducted in Plumbon Puskesmas Medical record unit on 20 June 2020. The results of the study obtained from 92 samples of medical record documents, the appropriate disease code in accordance with the ICD-10 as much as 39 (42.39%) and improper code of 53 (57.61%). There is still an imprecision of unsuitable disease code due to coding officers that do not include the 4th character. It is best to need a fixed procedure in accordance with WHO provisions for coding the disease to make the koder more thorough in determining the disease code.

Physical Processes of processing raw materials to purify furniture can making the hazardous materials such as dust of wood. Workers who are in the area of higher dust levels can overcome complaints of respiratory caused by one of them from the use of masks. Based on a preliminary study 12 out of 16 furniture workers deal with the subjective complaints of respiratory system including coughing, phlegm, and shortness of breath.This purpose study to determine the correlation of the use of masks with a subjective complaints of respiratory system on furniture home industry workers in the Cikeduk Village, Cirebon District. The type of research used is descriptive correlation with retrospectively study design. The population in this study were all workers in 24 furniture home industries in Cikeduk Village totaling 232 workers with sample of 70 people. The sampling technique uses quota sampling. The statistical test used is Chi Square. The results obtained p value of 0,032 and smaller than the value of α = 0,05, this indicates a relationship use of the masks with subjective complaints of respiratory system on furniture home industry workers in the Cikeduk Village, Cirebon District. The need for workers to improve discipline in using masks so that it can help improve the subjective complaints of respiratory system and check their health with routine on health services.

Minimum service standards are a technical spesification regarding service benchmarks provided by public service bodies to the public. Minimum service standards have several fields especially in the field of medical records. One the indicators in the field of medical record is the time of provision of inpatient medical record documents. Minimum service standards based on Kepmenkes 129 of 2008 ≤ 15 minutes. The purpose of this study was to determine the time provision of inpatient medical record documents in Hospital Sumber Waras Cirebon Regency.  The type of research used is descriptive research with a quantitative approach. The population in this study was 1.242 documents with a total sample of 92 documents and sampling in this study using accidental sampling. The research instrument used wa an observasion sheet in the form of a checklist sheet. Dat collection procedure is done by determining the inclusion and exclusion.  Based on the results of research when providing inpatient medical record documents as much as 53% of 49 documents and 47% of 43 documents that are not appropriate. The average time for providing inpatient medical record documents is 23 minute 13 seconds.  The conclusions obtained in this study are time for providing inpatient medical record documents in Hospital Sumber Waras Cirebon Regency source not in accordance with Kepmenkes standard number 129 of 2008, that for the time of providing inpatient medical record documents is ≤ 15 minutes.

 Family support is a process of relationship between the family and its social environment that is accessible to families that can be supportive and provide help to family members Anxiety (Ansiety) is a feeling of fear that is not clear and is not supported by the situation. Pregnancy is an event that occurs in a woman from the process of fertilization (conception) to the birth of a baby. The purpose of this study was to determine the relationship of family support with anxiety in third trimester pregnant women in the Kalijaga Health Center, Cirebon City. This research method uses quantitative descriptive correlational research, with cross sectional method carried out on 30 respondents at Kalijaga Health Center in Cirebon City with independent variables of family support and anxiety dependent variables in trimester III pregnant women by using questionnaire sheet measurement tools. Data analysis used the Spearman rank test. The research place was at the Kalijaga Public Health Center, Cirebon City. The results showed that the P value is smaller than alpha, which means that Ho is rejected and Ha is accepted, so it can be concluded that there is a significant relationship between family support and anxiety in third trimester pregnant women at the Kalijaga Health Center in Cirebon City, P value 0.017 is smaller than alpha 0, 05. Nurses are expected to be able to perform their roles optimally as providers of nursing care, advocates, educators, coordinators, and collaborators in providing family support to pregnant women who experience anxiety.
