Corona virus Disease 2019 (COVID-19) pandemic is causing a major health crisis across the globe. With the increasing number of fungal infections associated with COVID-19 being reported, it is imperative to understand the spectrum of such infections. Most documented cases have been reported in patients with diabetes mellitus or treatment with immunomodulators. The most common causative agents are Aspergillus, Candida or Mucorales. This series aims to portray the spectrum of fungal infections associated with COVID-19.

The treatment of corona virus disease 2019 (COVID-19)remains in debate, and the use of chloroquine has not been validated by accurate clinical trials.The aim of this study was to provide the possible cardiotoxicity effect of chloroquine in patients with COVID-19. This study was a case-series of prolonged QT interval of COVID-19 patients treated with chloroquine in a hospital in Bali, Indonesia. There were two cases of COVID-19 with exhibited a prolonged QT interval after being administrated of chloroquine. The prolonged QT interval returned to normal after chloroquine was stopped.These cases alert us the cardiotoxicity effect of chloroquine and the need for serial electro-cardiography monitoring before and during therapy. In conclusion, although antiviral and anti-inflammation properties of chloroquine on COVID-19 are promising, its cardiotoxicity effects should be monitored closely for less harm to the patients.

Introduction: Advanced cardiac imaging (ACI), including transesophageal echocardiogram (TEE), cardiac CT and cardiac MRI, is widely considered a crucial element in the work-up of cryptogenic and ESUS stroke. Current data on diagnostic yields are variable, though, and selection of patients to undergo such imaging is largely anecdotal. Methodology: We conducted a retrospective case series review of all cryptogenic ischemic strokes who received ACI, admitted to a large urban academic single-center over a 2.5-year period. Patient characteristics and imaging findings were analyzed to determine diagnostic yields and to identify positive and negative predictors that ACI would uncover etiology. Results: Out of 277 total cryptogenic ischemic strokes reviewed, ACI determined etiology in 20 cases (7.2%). Median age was 61 (IQR = 52-73). Patient sex was 49.5% female. Race demographic was 1.8% Asian, 50.9% black, 14.1% latino(a), and 33.2% white. Etiology was found by TEE in 16 of 20 cases, and the most common finding was left atrial thrombus/smoke. Latino(a) race was significantly more likely (15.4%) for ACI to find etiology and white race was significantly less likely (1.1%), p = .03 and .005, respectively. The table included shows other notable findings. Analogous trends were observed in a subgroup analysis of patients greater or equal to 60 years old. Conclusion: Our data indicate that in cryptogenic stroke premorbid non-use of antithrombotic, elevated troponin and left atrial dilatation are each individually associated with increased likelihood that ACI will find an etiology. Meanwhile, patients with diabetes mellitus are significantly less likely to have etiology found by ACI; and, smokers with combined hypertension, hyperlipidemia, and diabetes mellitus trend towards ACI less frequently uncovering etiology. Finally, the data suggest potential racial disparities of finding etiology with ACI, the determinants of which remain to be studied.

Background: It is generally acknowledged that patients with diabetes mellitus are more susceptible to fungal infections, particularly with Candida albicans. Oral infection by Candida can result in a number of clinical lesions, including median rhomboid glossitis (central papillary atrophy), denture stomatitis, squamous cell carcinoma, Radiation therapy, immunocompromised status, etc. Different studies have shown that patients with diabetes mellitus have increased frequency of oral candidal carriage and increased risk of candidiasis, which is related to poor metabolic control, neutrophil dysfunction, reduced salivary flow, high glucose concentration in blood and saliva and in medications.Materials and Methods: Subjects of both the groups were given 10 ml of sterile normal saline and asked to rinse the mouth for one minute. The subjects were then asked to return the oral rinse in a sterile clean, broad-mouthed container which was capped, labelled and taken to the laboratory. The samples were then inoculated onto the culture medium (Sabouraud’s dextrose agar with Chloramphenicol) with minimal delay (within 6-8 hours of collection of oral rinse). Candidal colonies were counted and compared with non-diabetics.Results: Statistically significant increase in colony forming units (p=0.0324) were obtainedin patients with diabetes mellitus.Conclusion: The results indicate significant increase in colonization and carriage of candida in the oral cavity among diabetics when compared with non-diabetics. However, further research using larger samples is required which may lend credibility to the suggestion of increased candidal CFUs in diabetics serving as a surrogate marker of serum glucose levels.Journal of Pathology of Nepal (2015) Vol. 5, 733-738

COVID-19 (Corona Virus Disease 2019) merupakan penyakit akibat infeksi SARS-CoV-2. Tenaga kesehatan merupakan kelompok yang paling rentan terpapar virus ini dikarenakan adanya kontak langsung dengan orang yang terinfeksi. Penelitian ini bertujuan untuk mengevaluasi kesehatan kerja petugas kesehatan di IGD RSUD Palembang BARI terhadap wabah COVID-19. Metode penelitian ini adalah penelitian kuantitatif dengan desain deskriptif case series. Hasil penelitian ini menunjukkan bahwa yang paling banyak ditemukan adalah karakteristik petugas kesehatan dengan usia antara 26–35 tahun, masa kerja >3 tahun dan lama kerja >8 jam, patuh menggunakan APD yang sesuai dengan rekomendasi WHO atau Kemenkes RI, jam kerja tetap, pendapatan yang tidak bertambah atau tetap, RT-PCR negatif, adanya rasa takut dan melakukan tindakan pencegahan terhadap COVID-19, serta motivasi kerja yang tidak rendah. Sehingga dapat disimpulkan bahwa petugas kesehatan di IGD RSUD Palembang BARI patuh menggunakan APD yang sesuai dengan rekomendasi WHO atau Kemenkes RI serta motivasi kerja yang tidak rendah. Saran bagi tenaga kesehatan diharapkan dapat terus mempertahankan kepatuhan dalam menggunakan APD, motivasi kerja maupun tindakan pencegahan.

The Corona virus disease 2019 (COVID-19) health crisis has not spared any country in the world, from the most advanced to the poorest. COVID-19 pandemic has exposed much vulnerability in systems and societies. In all resource-constrained countries, the response has been very complicated. The pandemic had to be dealt with very quickly with limited health and economic resources to avoid the hecatomb already predicted for these countries. In Côte d'Ivoire in particular, health and government authorities worked with state or private structures with infrastructure and human resource capacities to develop a rapid and effective response plan. Several measures have been adopted to reduce the spread of COVID-19. As a result of all these strategies, the number of cases of infected persons has been gradually decreasing. This new situation has favored the revision of certain restrictive measures, in this case the closure of air borders, which have of course been eased under certain conditions. Travelers are required to have a paper attesting to their negativity in COVID-19 test. Thus, Pasteur Institute of Côte d'Ivoire (IPCI), the reference laboratory for carrying out COVID-19 tests, was responsible for supplying a certificate of analysis to travellers. The purpose of this article is to show the internal process developed and different strategies implemented by IPCI to establish and deliver this document in order to meet the demand in the context of a pandemic.

The novel corona virus disease (COVID-19) is a pandemic due to the global health crisis it has created in the world and greatest challenges that it has introduced in this 21ST century. COVID-19 is the the 5th pandemic which was first reported in Wuhan, China and gradually spread into the other parts of the world. This virus is a spillover of an animal virus and then also adapted the ability of transferring human to human. The virus is highly contagious due to rapid spread and constantly evolves in the human population.  COVID-19 pandemic is a lot more than just a health emergency, but it is also socio-economic because people are losing jobs and income. The COVID-19 disease affects physical and mental health due to isolation. Moreover, more and more countries are affecting the cases of COVID-19 which are rising very rapidly day by day. Therefore, every country needs to several actions which accelerates their safety health, balance between the protection of health, prevent economic and social disruption due to this pandemic. In the spirit of the solidarity, we all need to be contributing our self to protect this situation. However, every country is equally responsible for determining the measures to prevent or decrease the viral transmission. Government authorizes also trying to act for reduce disease’s trajectory and they focus on develop the vaccination and therapeutic drugs.  Aim: To find causes, symptoms, awareness, Prevention, and treatment of COVID-19. Conclusion: The world is going by the critical situation and that is COVID-19 pandemic which is originated due to corona virus. Shortness of breath, fever and dry cough are most common symptoms of the novel corona virus. COVID-19 is highly contagious, so we must keep safe distance from other people and wear a mast hand wash our hands frequently. As it were together can we overcome the interwoven wellbeing and social and financial impacts of the widespread and anticipate its acceleration into an extended compassionate and nourishment security catastrophe, with the potential misfortune of as of now accomplished advancement picks up.

Background: A 2016 meta-analysis of pharmaceutical care for patients with diabetes mellitus showed that the following four components were most effective: (a) individual goal setting, (b) sending feedback to the physician, (c) reviewing the medication, and (d) reviewing blood glucose measurements. Methods: To formulate a hypothesis regarding the effect of these four pharmaceutical care components on glycemic control in patients with diabetes mellitus and the feasibility of these components in practice. Ten patients with type 2 diabetes were included in the case series and received medication therapy management over four months. Results: The four care components were feasible in everyday practice and could be implemented within one patient visit. The average visits were 49 and 28 min at the beginning and end of the study, respectively. The glycated hemoglobin values did not change over the study period, though the fasting blood glucose decreased from 142 to 120 mg/dl, and the number of unsolved drug-related problems decreased from 6.9 to 1.9 per patient by the study end. Conclusions: This case series supports the hypothesis that community pharmacists can implement structured pharmaceutical care in everyday pharmacy practice for patients with type 2 diabetes mellitus.

Mucormycosis is an angio-invasive fungal infection, associated with high morbidity and mortality. A change in the epidemiology of mucormycosis has been observed in recent years with the rise in incidence, new causative agents and susceptible population. The rise has been perceived globally, but it is very high in the Asian continent. Though diabetes mellitus overshadow all other risk factors in Asia, post-tuberculosis and chronic renal failure have emerged as new risk groups. The rhino-cerebral form of mucormycosis is most commonly seen in patients with diabetes mellitus, whereas, pulmonary mucormycosis in patients with haematological malignancy and transplant recipients. In immunocompetent hosts, cutaneous mucormycosis is commonly seen following trauma. The intriguing clinical entity, isolated renal mucormycosis in immunocompetent patients is only reported from China and India. A new clinical entity, indolent mucormycosis in nasal sinuses, is recently recognized. The causative agents of mucormycosis vary across different geographic locations. Though Rhizopus arrhizus is the most common agent isolated worldwide, Apophysomyces variabilis is predominant in Asia and Lichtheimia species in Europe. The new causative agents, Rhizopus homothallicus, Mucor irregularis, and Thamnostylum lucknowense are reported from Asia. In conclusion, with the change in epidemiology of mucormycosis country-wise studies are warranted to estimate disease burden in different risk groups, analyse the clinical disease pattern and identify the new etiological agents.

From the beginning of corona virus disease 19(COVID-19) pandemic, there has been concern how to protect vulnerable group like pregnant women from severe acute respiratory syndrome corona virus 2(SARS-CoV-2). Historically, pregnant women experiences increased mortality during any pandemic situation. Pregnant women show almost the similar clinical features as that of non-pregnant adults with COVID-19 infection. Different systematic reviews have begun to focus light on pregnancy outcomes in COVID-19 patients, but knowledge is very limited and still the basis is case series and individual experiences. Apart from the scientifically proven therapeutic options used in COVID-19 such as steroid, low molecular weight heparin, the role of convalescent plasma therapy (CPT) has never been evaluated. We present a case of a pregnant woman of 32 weeks of conception, treated with CPT with favourable outcome in a private hospital of Dhaka, Bangladesh. BSMMU J 2021; 14 (COVID -19 Supplement): 64-66
