The growing population of COVID-19 survivors in second wave represents a clear challenge to clinicians and researchers to look beyond the search for a cure and to address the multifaceted needs of those living with and beyond diagnosis. Common complications that disrupt the psychosocial, physical aspects of life and multi systemic complication for adult. COVID-19 survivors after primary treatment include: fatigue, neurological, cardiovascular, hepatic impairment,ocular,dermatological manifestation,Oral manifestation,acute pancreatitis,musculoskeletal,renal and reproductory system complications. Psychosocial interventions,particularly group-based interventions,physical activity programs, continuous monitoring and prompt treatment have shown great promise in improving these outcomes.Future research will identify even bettertargeted, more efficacious, and more cost effective medication and vaccination policy to counter COVID-19 outcomes. Healthcare providers must realize that they serve as vital gatekeepers to services that will help optimize outcomes. Addressing these issues in the post-treatment period represents the new challenge to supportive care.

SummaryProblems with unfractionated heparin as an antithrombotic have led to the development of new therapeutic agents. Of these, low molecular weight heparin shows great promise and has led to out-patient therapy of DVT/PE in selected patients. Oral anticoagulants remain the choice for long-term therapy. More cost-effective ways to give oral anticoagulants are needed.

The wastewater from the city of Thessaloniki is discharged without treatment to the nearby inner part of the Thessaloniki Gulf. The existing, since 1989, treatment plant offers only primary treatment and did not operate since the expected effluent quality is not suitable for safe disposal to the available recipients. Upgrading of the plant for advanced biological treatment, including seasonal nitrogen removal, is due from 1995. In the mean time, after minor modifications completed in February 1992, the existing plant was put into operation as a two-stage chemical-biological treatment plant for 40 000 m3 d−1, which corresponds to about 35% of the total sewage flow. The operational results obtained during the two years operation period are presented and evaluated. All sewage and sludge treatment units of the plant perform better than expected, with the exception of the poor sludge settling characteristics, due to severe and persistent bulking caused by excessive growth of filamentous microorganisms, particularly M. Parvicella. Effective control of the bulking problem could lead to more cost-effective operation and increased influent flows.

Background: A simple, fast and economic analytical method for the determination of ethanol is important for clinical, biological, forensic and physico-legal purposes. Methods: Ni2+-NTA resin was used as an immobilization matrix for the simple one-step purification/ immobilization of his6-tagged ADH. Different alcohols with a concentration range of 0.5-50% V/V, namely methanol, ethanol and propanol were measured using prepared ADH enzyme thermistor. The ethanol content of Tsingtao beer was tested as a real sample containing alcohol. Reproducibility and stability of prepared ADH enzyme thermistor were also investigated by repeated measurements. Results: In comparison to the controlled pore glass (a common used support for the immobilization of enzyme) used in thermal biosensor, the use of Ni2+-NTA resin not only led to simple one-step purification/ immobilization by his6-tagged ADH binding to Ni2+-NTA resin, but also made the immobilizing supports reusable. The prepared biosensor can be used to determine ethanol and methanol by the calorimetric measurement. A linear range of 1 -32% (V/V) and 2-20% (V/V) was observed for ethanol and methanol, respectively. The detection limits were 0.3% (V/V) and 1% (V/V) for ethanol and methanol, respectively. The tested ethanol concentration of Tsingtao beer was 4.5% V/V, which is comparable with the labeled alcohol by volume (ABV) 4.80%. Conclusion: Ni2+-NTA resin, as an immobilization matrix in ET sensor, provides a simple one-step purification/immobilization for His6-tagged recombinase and a reusable immobilization matrix. The prepared biosensor exhibits good repeatability and stability. Such a new biosensor shows great promise for rapid, simple, and cost-effective analysis of ethanol and methanol, both in qualitative and in quantitative tests.

Carbon-based electrodes have demonstrated great promise as electrochemical transducers in the development of biosensors. More recently, laser-induced graphene (LIG), a graphene derivative, appears as a great candidate due to its superior electron transfer characteristics, high surface area and simplicity in its synthesis. The continuous interest in the development of cost-effective, more stable and reliable biosensors for glucose detection make them the most studied and explored within the academic and industry community. In this work, the electrochemistry of glucose oxidase (GOx) adsorbed on LIG electrodes is studied in detail. In addition to the well-known electroactivity of free flavin adenine dinucleotide (FAD), the cofactor of GOx, at the expected half-wave potential of −0.490 V vs. Ag/AgCl (1 M KCl), a new well-defined redox pair at 0.155 V is observed and shown to be related to LIG/GOx interaction. A systematic study was undertaken in order to understand the origin of this activity, including scan rate and pH dependence, along with glucose detection tests. Two protons and two electrons are involved in this reaction, which is shown to be sensitive to the concentration of glucose, restraining its origin to the electron transfer from FAD in the active site of GOx to the electrode via direct or mediated by quinone derivatives acting as mediators.

Background: An estimated 95% of all online pharmacies operate unlawfully. Illegal online pharmacies distribute substandard and falsified medical products that may result in patient harm and suboptimal treatment, leading to an overall mistrust of medications, healthcare providers, and health systems. As medication experts, pharmacists are trusted to guide patients in selection of safe and effective medication therapy. Objective: The objective of this study was to determine gaps in knowledge and recognition of the negative clinical and safety impacts associated with illegal Internet pharmacies by licensed pharmacists. Methods: A 37-question electronic survey was developed and distributed to pharmacists across the United States by email via a database from the American Pharmacists Association. Descriptive statistics was utilized to analyze data. Results: A total of 347 pharmacists from across the United States responded to at least one question in the survey. In all, 58% of pharmacists reported a lack of confidence in their ability to counsel patients on the identification of illegal pharmacy websites. Fewer than 60% of pharmacists were able to accurately identify the legitimacy of a webpage based on visual characteristics. In addition, 75% of pharmacists reported being unfamiliar with resources available to help consumers identify safe and legitimate online pharmacies. Conclusion: Integration of the topic into pharmacy education curricula, training on available resources, and additional research into the prevalence and impact of illegal pharmacy websites are necessary to ensure that pharmacists and other healthcare professionals are adequately prepared to protect their communities from the threat of illegal online pharmacies.

Additive manufacturing is becoming an increasingly important technique for the production of dental restorations and assistive devices. The most commonly used systems are based on vat polymerization, e.g., stereolithography (SLA) and digital light processing (DLP). In contrast, fused filament fabrication (FFF), also known under the brand name fused deposition modeling (FDM), is rarely applied in the dental field. This might be due to the reduced accuracy and resolution of FFF compared to vat polymerization. However, the use of FFF in the dental sector seems very promising for in-house production since it presents a cost-effective and straight forward method. The manufacturing of nearly ready-to-use parts with only minimal post-processing can be considered highly advantageous. Therefore, the objective was to implement FFF in a digital dental workflow. The present report demonstrates the production of surgical guides for implant insertion by FFF. Furthermore, a novel approach using a temperature-sensitive filament for bite registration plates holds great promise for a simplified workflow. In combination with a medical-grade filament, a multi-material impression tray was printed for optimized impression taking of edentulous patients. Compared to the conventional way, the printed thermoplastic material is pleasant to model and can allow clean and fast work on the patient.

“Non-delivery” home oxygen technologies that allow self-filling of ambulatory oxygen cylinders are emerging. They can offer a relatively unlimited supply of ambulatory oxygen in suitably assessed people who require long-term oxygen therapy (LTOT), providing they can use these systems safely and effectively. This allows users to be self-sufficient and facilitates longer periods of time away from home. The evolution and evidence base of this technology is reported with the experience of a national service review in Scotland (UK). Given that domiciliary oxygen services represent a significant cost to healthcare providers globally, these systems offer potential cost savings, are appealing to remote and rural regions due to the avoidance of cylinder delivery and have additional lower environmental impact due to reduced fossil fuel consumption and subsequently reduced carbon emissions. Evidence is emerging that self-fill/non-delivery oxygen systems can meet the ambulatory oxygen needs of many patients using LTOT and can have a positive impact on quality of life, increase time spent away from home and offer significant financial savings to healthcare providers.Educational aimsProvide update for oxygen prescribers on options for home oxygen provision.Provide update on the evidence base for available self-fill oxygen technologies.Provide and update for healthcare commissioners on the potential cost-effective and environmental benefits of increased utilisation of self-fill oxygen systems.

Jeffrey Stiles pondered these seemingly straightforward questions. As IT Director of Jagged Peak, Inc., a developer of e-commerce solutions located in the Tampa Bay region of Florida, it would be his responsibility to oversee the implementation of security measures that went beyond the existing user name and password currently required for each user. Recent events suggested that a move towards increased security might be inevitable. In just the past year, highly publicized security failures at the U.S. Department of Defense, major healthcare providers and large companies, such as Sony and JP Morgan Chase, had made executives acutely aware of the adverse consequences of IT system vulnerabilities. In fact, a study of business risk managers conducted in 2014 found that 69% of all businesses had experienced some level of hacking in the previous year. The nature of Jagged Peak’s business made the security of its systems a particular concern. The company, which had grown rapidly over the years, reporting over $61 million in revenue in 2014, provided its customers with software that supported web-based ordering, fulfillment and logistics activities, built around a philosophy of “buy anywhere, fulfill anywhere, return anywhere”. To support these activities, the company’s Edge platform needed to handle a variety of payment types, including gift cards (a recent target of hackers), as well as sensitive personal identifying information (PII). Compounding the security challenge: each customer ran its own instance of the Edge platform, and managed its own users. When only a single customer was being considered, the addition of further layers of security to authenticate uses was an eminently solvable problem. A variety of alternative approaches existed, including the use of various biometrics, key fobs that provided codes the user could enter, personalized security questions, and many others. The problem was that where multiple customers were involved, it was much more difficult to form a consensus. One customer might object to biometrics because it users lacked the necessary hardware. Another might object to security keys as being too costly, easily stolen or lost. Personalized questions might be considered too failure-prone by some customers. Furthermore, it was not clear that adding additional layers of authentication would necessarily be the most cost-effective way to reduce vulnerability. Other approaches, such as user training might provide greater value. Even if Stiles decided to proceed with additional authentication, questions remained. Mandatory or a free/added-cost option? Developed in house or by a third party? Used for internal systems only, customer platforms only, or both? Implementation could not begin until these broad questions were answered.

ABSTRACTIntroductionThe impact of school opening on the SARS-CoV-2 pandemic is still unknown. This study aims to provide preliminary information about the number of SARS-CoV-2 cases among students attending Italian schools.MethodsData are extracted and analysed from an open access, online dataset that monitor, on a daily basis, media news about SARS-CoV-2 infections of students attending Italian schoolsResultsAs of 5 October 2020, a total of 1350 cases of SARS-CoV-2 infections have been registered in the Italian territory schools (involving 1059 students, 145 teachers and 146 other school members), for a total of 1212 out of 65104 (1.8%) Italian schools involved. National schools reported only 1 case of SARS-CoV-2 infection in more than 90% of cases, and only in one high school a cluster of more than 10 cases have been described (P 0.015). The detection of one or more SARS-CoV-2 infections leaded to the closure of 192 (14.2%) entire schools, more frequently nursery/kindergartens (P<0.0005).DiscussionOur preliminary data support low transmission of SARS-CoV-2 within schools, at least among younger students. However, entire schools are frequently closed in the fear of larger outbreaks. Continuous monitoring of school settings, hopefully through daily updated open access datasets, are needed to better understand the impact of schools on the pandemic, and provide guidelines that better consider different risks within different age groups.
