Aim. Analyze epizootologic manifestations of natural-focal infections (NFI) in the south of Russia in 2015. Materials and methods. Reports provided by administrations of Federal Service of Surveillance for Protection of Consumers Rights and Human Welfare, Centers of Hygiene and Epidemiology, Research Institutes for Plague Control and Station for Plague Control were used. Data processing was carried out using Excel program. Results. A total of 37 738 samples of field material were studied, most of those - for tularemia (8268), Crimea hemorrhagic fever (6894) and West Nile fever (4875). Causative agents of 15 nosologic forms of NFI were detected during epizootologic examination of the territory of south of Russia, predominating amount of positive samples was obtained during study of material for tick-borne borreliosis (612), Crimea hemorrhagic fever (279) and tularemia (237). Conclusion. Analysis of epizootologic situation on natural-focal infections in the south of Russia gives evidence on the increase of activity of natural foci of bacterial (tularemia, tick-borne borreliosis), rickettsia (tick-borne spotted fevers) and viral infections (Crimea hemorrhagic fever, West Nile fever, tick-borne viral encephalitis), and thus problems of prophylaxis of natural-focal infectious diseases and constant monitoring of natural foci in subjects of Southern, North-Caucasian and Crimea federal districts are of particular importance.

Problematic issues on creation and practical introduction of specific immune biologic preparations for therapy and prophylaxis of natural-foci arbovirus infections - West Nile fever (WNF) and Crimean hemorrhagic fever (CHF), that are not available until now, are examined. Persistent natural foci of WNF and CHF with epidemic manifestations have formed in Southern and North Caucasian Federal Districts of Russia. Markers of Wfest Nile virus are being detected in central regions of Russia and Siberia, and the presence of fraction of population immune to this infection is also detected. Analysis of literature sources that has been carried out forms a theoretical basis for creation of novel specific preparations for etiotropic therapy and prophylaxis ofWNF and CHF. Use of blood from healthy donors with sufficiently high titers of class G immunoglobulins residing in certain subjects of Russian Federation in natural foci territories with the most intensive epidemic process is possible as raw material.

Aim. Analysis of epidemic manifestations of natural-foci infections (NFI), clarification of spectrum of their causative agents, determination of epizootic activity of natural foci in the Crimea Federal District (KFD). Materials and methods. Epizootologic examination of 10 administrative districts of KDF was carried out. 291 pools (2705 specimens) of ixodes ticks and 283 samples of organs of small mammals were studied by PCR method for the presence of DNA/RNA of causative agents of a number of NFI. Results. Morbidity by NFI in KFD was registered by 6 nosologies: Lyme borreliosis, Marseilles fever, leptospirosis, tularemia, intestine yersiniosis and tick-borne viral encephalitis, wherein, transmissive infections made up 91.6%. Circulation of causative agents of Crimea hemorrhagic fever, Q fever, group of tick-borne spotted fever, Lyme borreliosis, human granulocytic anaplasmosis, human monocytic ehrlichiosis, hemorrhagic fever with renal syndrome, West Nile fever, tularemia and leptospirosis was established. Conclusion. Due to activity of natural foci of NFI further monitoring of epidemiologic and epizootologic manifestations of these infections in the Crimea, including using genetic methods of analysis, is necessary for ensuring sanitary-epidemiologic welfare of KFD population.

Purpose: analysis of the arbovirus infections incidence in the south of the European part of the Russian Federation in 2015–2019.Materials and methods. Data from statistical documentation (epidemiological examination of the infectious disease cases, reports on the arbovirus infections incidence) reported by Departments of Rospotrebnadzor to Scientific and Methodological Center for monitoring pathogens of infectious and parasitic diseases of 1–3 risk groups for subjects of the North Caucasian and Southern Federal Districts were analyzed. The obtained data were processed using Microsoft Excel 2010 program.Results. In the south of the European part of the Russian Federation Crimean-Congo hemorrhagic fever (CCHF), West Nile fever (WNF), tick-borne viral encephalitis and dengue fever cases are registered annually.An expansion of the territory with registered epidemic manifestations of CCHF and WNF was noted, reflecting an increase in the area of circulation of their pathogens. An expansion of CCHF and WNF epidemic season with the involvement into the epidemic process of people from all age groups including young children was observed. Significant increase in number of imported cases of dengue fever was documented. Markers of tick-borne viral encephalitis, Batai, Inko, Sindbis, and Tyaginya fevers were detected in residents of a number of territories. A high levels of population humoral immunity to West Nile, Batai, Inko, Sindbis Tyaginya, Ukuniemi, Bhanja, Dkhori viruses were identified in the Astrakhan region.Conclusion. The data obtained indicate the need for epidemiological surveillance both for arbovirus infections with a pronounced epidemiological and clinical manifestations, and for infections, the proportion of which in the structure of infectious pathology in southern Russia has not been sufficiently studied, but carrying a potential risk of spreading.

The results of epizootological monitoring of the Southern European part of Russia (Southern and North- Caucasian Federal Districts), which has been tested for 18 nosological forms of natural focal infections are presented. There have been tested the laboratory studies of 45 615 samples of field material, the markers of pathogens of 15 nosological forms are identified. The circulation of Crimean-Congo hemorrhagic fever virus is identified in 10 subjects, the pathogen of tularemia – in eight regions, Lyme borreliosis – in seven, Q fever – in six; pathogens of hemorrhagic fever with renal syndrome, West Nile fever, leptospirosis and human granulocytic anaplasmosis – in five subjects. The markers of pathogens of human monocytic ehrlichiosis and tick-borne viral encephalitis were detected in four subjects, pseudotuberculosis and intestinal yersiniosis – in three and Arboviruses (Sindbis, Inkoо, Tahyna) – in one region.

Within the period of 2000-2011, in the West of Kazakhstan, identified have been five, previously unknown in the territory, natural foci of dangerous infectious diseases such as hemorrhagic fever with renal syndrome, Astrakhan spotty fever, Crimean hemorrhagic fever, West Nile fever, and tick-borne viral encephalitis. The reason is that key ecological factors for the persistence of the infections in the local biocoenoses occurred. It is characteristic that circulation of the agents of new infectious diseases is registered in the territory of the long-established natural plague and tularemia foci. Since 2000 and on, hemorrhagic fever with renal syndrome morbidity is registered in the Western-Kazakhstan Region on a regular basis. There is some evidence to identification of West Nile fever patients in the territory of Russia in 2012, which came from Kazakhstan. Based on the spatial distribution of the natural foci of various infections, Western-Kazakhstan Region has been subdivided into four areas. In order to provide for the effective prophylaxis of emerging diseases, it is essential that healthcare facilities and services dealing with infectious diseases are consolidated and reinforced, and epidemiological surveillance is improved with the current conditions in mind.

This paper includes review of innovative methods of monitoring of activity of natural foci of epidemically important for Russian Federation such viral infections as hemorrhagic fever with renal syndrome (HFRS) and Crimean-Congo hemorrhagic fever (CCHF), and the analysis of probability to control such «exotic» infections, as Denge fever and severe fever with thrombocytopenia syndrome (SFTS).

Viral hemorrhagic fevers (VHFs) represent a group of similar clinical entities contagious constitutional diseases, caused by four different types of RNA viruses: Flaviviridae, Bunyaviridae, Arenaviridae i Filoviridae. These diseases proceed with high fever and damage of the circulatory system leading to homeostasis disorders, commonly accompanied by symptoms of hemorrhagic diathesis. VHFs are typically transmitted through infection vectors (mosquito) or through direct physical contact with infectious material. West Nile fever is the disease which is caused by West Nile virus from the Flaviviridae family. It begins flu-like symptoms, then it appears maculopapular rash and lymphadenopathy. At the most cases the symptoms retreat idiopathically. This disease can proceed as West Nile Neurological Disease in 1% of infected. The article presents three diseases, which can be present in tropical climate such as: Ebola hemorrhagic fever, dengue hemorrhagic fever, West Nile fever.

The epidemic rise in the incidence of West Nile fever (WNF) in the season of 2018 was observed in the countries of the European Union (EU) and bordering states and exceeded the values of all previously recorded epidemic rises of 2010–2012. An increase in the incidence rate was registered in the USA and Canada, however, it did not exceed the indicators of epidemic rises of 2007–2012. In the territory of the Russian Federation, the WNF epidemiological process became more intense mainly in the territory of the Southern and North Caucasian Federal Districts. In general, in Russia, the incidence rates were 2 times lower than the average annual rates, but significantly exceeded those of 2017. The epidemic process had a number of peculiarities in the seasonality, the structure of morbidity and the clinical manifestation of WNF. Genotyping of the isolated WNV RNA fragments from clinical and biological material showed that I, II and IV West Nile virus genotypes were circulating in the European part of Russia. Forecast of epidemic situation development in 2019 reveals further increase in the incidence and does not exclude the possibility of a significant localincrease of WNF incidence in certain regions of Russia.

The paper presents an analysis of West Nile Fever incidence in the Russian Federation in 2020, summarizes the results and identifies problematic issues of the pathogen monitoring. Manifestations of West Nile Fever in 2020 were characterized by a low incidence rate (10 times lower than the average long-term value) with sporadic cases registration in the endemic areas of the Southern (9 cases) and Central (1 case) Federal Districts. A discrepancy between the morbidity structure (distribution by age, sex, social status) and the trends that have developed in Russia in recent years is shown. The analysis of officially recorded cases doesn’t characterize the epidemic process of West Nile fever in Russia during 2020-season as a whole. The generalized results of monitoring of the West Nile virus circulation in environmental objects in 2020 indicate a decrease in the effectiveness of its implementation and a low detectability of pathogen markers. A decrease in the volume of diagnostic studies for the active detection of patients with West Nile fever in the epidemic season (5.7 times lower compared to 2019), as well as serological screening of healthy population samples (1.6 times) has been established. The results of a molecular-genetic study of the pathogen showed that lineage 2 of the pathogen was circulating in the European part of Russia. The circulation of the lineage 4 of the virus in the enzootic cycle in the Republic of Kalmykia was found out. The genome sequences of 11 West Nile virus isolates allocated in 2019 and 2020 were obtained. Phylogenetic analysis revealed that all isolates allocated in the Volgograd Region and isolates from the Rostov and Astrakhan regions belong to the Volgograd clade of the lineage 2 of the West Nile virus. Based on the assessment of abiotic and biotic factors, possible local increases in the incidence of West Nile fever in 2021 in the regions of the Southern and North Caucasian Federal Districts and in the south of Western Siberia have been substantiated.
