The aim of the present research was to improve the neglect behavior of pubertal children in an intervention program. Within the framework of the program, the main function of the spinal column and the strength and extensibility of the muscles responsible for posture were investigated. The participants included in the study are 7th grade children of the Neumann János Elementary School in Szombathely, Hungary, (nb=7); (ng=10). The Idiag M360 Spinal Mouse was used to investigate the physiological curvatures and main function of the spinal column before and after the program. The device was used to examine the children's spines in standing, bending forward and again in a standing position after the Matthiass test was performed. Pre-intervention (1) and post-intervention (2) data showed significant decreases in all assessed characteristics and similar increases in sacrum to hip ratio (Sac/Hip) (9.83±4.36 - 21.63±4.39); p<0.000.  The greatest change was found in the Th11/12 dorsal vertebral body angles (4.50±2.05 - -0.30±0.25); p<0.000. As a result of the postural improvement program, the degree of pelvic tilt and, in parallel, the physiological curvature of lumbar lordosis was reduced to within the age-specific reference values.

This study aimed to investigate the influence of a structured movement activity program on the motor development of children aged three to five years attending preschool. Participants were 136 preschool students with normative development at three to four years old who lived in the Region of Murcia (Spain). The McCarthy Children’s Psychomotricity and Aptitude Scales (MSCA) battery of psychomotor tests was used to evaluate the motor development profiles of preschoolers before and after the intervention. The sample was divided into two groups: an intervention group (28 students) and a comparison group (108 students). A structured 24 week physical education program was used in the intervention group. An experiential program based on free play was used in the comparison group during the same period. Preschoolers in both groups got a significant improvement in the contrast of pre-intervention with post-intervention in limb coordination. Statistically significant differences in the post-intervention measurements between the comparison group and the intervention group on arm and leg coordination were observed, whereby the intervention group presented higher arm coordination values (F1,134 = 14,389, p = 0.000, η2 = 0.097) and higher leg coordination values (F1,134 = 19,281, p = 0.000, η2 = 0.126) than the comparison group. It was pointed out that structured physical activity education is better educational methodology than free play to achieve adequate motor development in preschool children.

Abstract Objective :to evaluate the presence of depressive symptoms and cognitive disorders before and after an intervention program with a digital therapeutic game among elderly persons undergoing hemodialysis. Method: a quasi-experimental study was carried out with 26 elderly patients on hemodialysis. For the data collection, a questionnaire relating to sociodemographic and health conditions, the Geriatric Depression Scale - 15 items and Addenbrooke’s Cognitive Examination Revised were used. The intervention with the digital therapeutic game was performed over 5 sessions. Results: of the participants, 80.8% were male, with a mean age of 66.7 (± 5.8) years. The mean pre-intervention depressive symptom score was 3.9 (± 3.0) while post-intervention it was 2.8 (± 2.9), representing a statistically significant difference (p = 0.005). Regarding cognitive function, there was no statistically significant difference before and after the intervention. There was a statistically significant difference in the mean of the depressive symptom scores, which were lower after the intervention. In addition, there was no statistically significant difference in the mean of the cognitive assessments. Conclusion: intervention studies with patients undergoing hemodialysis treatment are still scarce and this study describes the positive results of an intervention with a digital therapeutic game, demonstrating improvement in the depressive symptoms of the participants.

PURPOSE: To evaluate physician-reported assessments of an established faculty orientation program for new radiation oncology physicians at a large academic center and to prospectively analyze the effects of an onboarding improvement program based on those assessments. MATERIALS AND METHODS: An anonymous survey was designed and distributed to physicians new to the department who received onboarding orientation between 2013 and 2017. Survey questions addressed the comprehensiveness, effectiveness, and utility of various orientation activities. On the basis of the survey results, an improved onboarding program was designed and implemented for nine new faculty members between May 2018 and November 2018. A post-intervention survey querying topics similar to those in the pre-intervention survey was distributed to the new faculty members. Descriptive statistics were generated to compare the pre-intervention and post-intervention groups. RESULTS: The overall rate of survey completion was 85% (17 of 20). The intervention program markedly improved physician assessment of comprehensiveness and effectiveness of the onboarding process. Physicians strongly and consistently identified mentor shadowing, on-the-job training, and other faculty mentorship activities as the most important components of an effective onboarding experience. CONCLUSION: An enhanced, tailored, person-oriented, formal onboarding improvement program significantly increased physician assessment scores of comprehensiveness and effectiveness of the faculty onboarding process. This model can serve as a framework for increasing physician preparedness, encouraging early physician mentorship, and ensuring a universal standard of quality across large practices.

OBJETIVO: O objetivo do trabalho foi analisar os efeitos de um novo programa de intervenção na coordenação motora grossa (CMG) de crianças entre os 05 e 10 anos, avaliadas pela bateria de testes KTK. MÉTODOS: O programa foi realizado em 73 estudantes, selecionados de maneira não probabilística objetiva, com idades entre 05 e 10 anos, de uma rede municipal de ensino da cidade de Curitiba/PR, Brasil, divididos em dois grupos: Grupo Controle (GC=35) e Grupo Intervenção (GI=38). A CMG foi avaliada antes e depois da intervenção por meio da bateria de testes KTK. O programa de intervenção consistiu em 12 semanas de atividades alicerçadas nas estruturas da Técnica de Motivação Orientada Para Maestria (TARGET), e no livro “Escola da Bola – Um ABC para Iniciantes nos Jogos Esportivos”. Os efeitos foram testados utilizando o Teste de Cohen e teste t (dependente e independente).RESULTADOS: Na testagem t dependente, diferenças estatisticamente significativas foram encontradas no GI, do primeiro para o segundo momento, em todos os testes (d≥0,80). O GI obteve uma melhora significativa ao longo das 12 semanas (p0,05), enquanto no GC não houve diferenças estatisticamente significativas. Na testagem t independente, no momento pré-intervenção (1º Momento), foram encontradas diferenças significativas (p0,05) nas quatro tarefas, favoravelmente ao GC. No momento pós-intervenção, diferenças estatisticamente expressivas não foram observadas entre o GI e GC. CONCLUSÃO: Conclui-se que um programa interventivo, com duração de 12 semanas, utilizando os jogos da “Escola da Bola” em conjunto com o “TARGET”, trazem melhorias significativas no desempenho coordenativo das crianças, entre 5 e 10 anos de idade.Analyses of the effects of an intervention program based on TARGET structure and “Escola da Bola” on the gross motor coordinationABSTRACT OBJECTIVE: The objectives of the work were to analyze the effects of a new intervention program on gross motor coordination (GMC) of 5 and 10 year old children, diagnosed by the KTK battery test. METHODS: The program was carried out in 73 students, selected in an objective non-probabilistic way, aged between 5 and 10 years, from a municipal education network in the city of Curitiba, Brazil, divided into two groups: Control Group (CG=35) and Intervention Group (IG=38). CMG was assessed before and after the intervention using the KTK test battery. The intervention program consisted in 12 weeks of activities based on the structures of the Mastery-Oriented Motivation Technique (TARGET), and the book “Escola da Bola – Um ABC para Iniciantes nos Jogos Esportivos”. The effects were tested using the Cohen test and t-test (dependent and independent). RESULTS: In the dependent t-test, statistically significant differences were found on the IG, from the first to the second moment, in all tests (d≥0.80). The IG achieved a significant improvement over the 12 weeks (p0,05), while in the CG there were no statistically significant differences. In the independent t-test, in the pre-intervention moment (1st Moment), significant differences (p0,05) were found on the four tasks, in favor of the CG. In the post-intervention moment, statistically significant differences were not observed between the IG and CG. CONCLUSION: It follows that an intervention program, lasting 12 weeks, using the games of “Escola da Bola” together with “TARGET”, brings significant improvements in children’s coordinative performance, between 5 and 10 years old.

Background Outdoor fitness training has become popular as a tool for improving the health, especially middle-aged and older adults. For this purpose, outdoor fitness equipment (OFE) have been installed in public areas. However, their safety and effectiveness are still unknown. The aim of the present research was to analyze the sagittal disposition of the spine and pelvic tilt during the use of OFE, and to determine the influence of anthropometric variables on these factors in middle-aged and older adults. Methods Seventy healthy volunteers, 56 women and 14 men (age: 63.14 ± 8.19 years) participated in the study. Sagittal spine disposition and pelvic tilt were measured using a Spinal Mouse®, in the relaxed standing position, and during the use of the OFE. In addition, kinanthropometry variables were also measured according to the guidelines of the International Society for the Advancement of Kinanthropometry. Results Regarding thoracic kyphosis, a significant decrease was found in thoracic kyphosis in the initial position (IP) in single bonny rider (SBR) (p = 0.006) and row (p = 0.046), and a significant increase in the final position (FP) in the row (p = 0.011), surfboard (p < 0.001) and air walker (p = 0.027) machines. In relation to the lumbar curvature and pelvic tilt, a significant decrease in lumbar lordosis and a decrease in pelvic anteversion were observed in the IP and FP in SBR and row; and in the bike (p < 0.001) machine. In the surfboard machine, a significant decrease in lumbar lordosis was found (p = 0.002), with no changes in pelvic tilt. According to the multiple linear regression analysis, the subjects with a higher cormic index and height were more at risk of increasing their thoracic kyphosis, decreasing lumbar lordosis and/or decreasing pelvic anteversion towards pelvic retroversion. Conclusions Middle-aged and older adults show spinal misalignments when using the OFE with respect to the standing position, showing a decrease in the thoracic kyphosis in IP of SBR and ROW, and a significant increase in the surfboard and air walker, and in the FP of Row, in the lumbar lordosis in all the OFE in sitting and some in standing, and in the pelvic anteversion in all the OFE in sitting. The variables height and the cormic index explained most of the changes in sagittal spine disposition.

Introduction: Postural alignment is commonly assessed, and the abnormal postures are associated with musculoskeletal disorders. There are different methods for screening spinal column anomalies. The flexible ruler provides a safe, portable, inexpensive and non-invasive way to measure the spinal curvature in clinical and larger population. Aim: To determine the intra-rater and inter-rater reliability of flexible ruler in measuring lumbar lordosis among children. Materials and Methods: In this cross-sectional study 81 school children that fulfilled the criteria were recruited with age ranging from 9 to 15 years. Youdas method in standing position was adopted. By using flexible ruler, measurement was taken and without altering the shape of the lumbar curve it was traced in a graph sheet. By using straight ruler, line L and H were drawn by following deep point method. Lumbar lordosis angle was calculated by using the formula, θ=4Arctan 2H/L. Intra-rater reliability: Four measurements were taken by evaluator 1 with a gap of one minute between first two measurements and 3 minutes rest then other two measurements were taken with a gap of one minute between each. The average of first two and the last two measurements were considered as measurements 1 and 2. Inter rater reliability: Two measurements were taken by evaluator 2 by adapting the same procedure. The averages of two measurements were considered as measurement for evaluator 2. This value was correlated with the value of measurement 1 of evaluator 1. The data was analysed using Intra Class Correlation coefficient (ICC) test. Results: The results showed high intra-rater reliability (ICC-0.991; p<0.001) for lumbar lordosis angle by flexible ruler and poor inter-rater reliability (ICC-0.241; p=0.11) for lumbar lordosis angle by flexible ruler. Conclusion: The flexible ruler has shown only intra-rater reliability and additional studies are required to prove inter-rater reliability in measuring lumbar lordosis among children.

Unnecessary pathology tests performed in intensive care units (ICU) might lead to increased costs of care and potential patient harm due to unnecessary phlebotomy. We hypothesised that a multimodal intervention program could result in a safe and effective reduction in the pathology tests ordered in our ICU. We conducted a single-centre pre- and post-study using multimodal interventions to address commonly ordered routine tests. The study was performed during the same six month period (August to February) over three years: 2012 to 2013 (pre-intervention), 2013 to 2014 (intervention) and 2014 to 2015 (post-intervention). Interventions consisted of staff education, designing new pathology forms, consultant-led pathology test ordering and intensive monitoring for a six-month period. The results of the study showed that there was a net savings of over A$213,000 in the intervention period and A$175,000 in the post-intervention period compared to the pre-intervention period. There was a 28% reduction in the tests performed in the intervention period (P <0.0001 compared to pre-intervention period) and 26% in the post-intervention period (P <0.0001 compared to pre-intervention period). There were no ICU or hospital mortality differences between the groups. There were no significant haemoglobin differences between the groups. A multimodal intervention safely reduced pathology test ordering in the ICU, resulting in substantial cost savings.

Abstract Background Many studies conducted in healthcare settings with limited resources reported healthcare associated infections (HAIs) rates higher than in developed countries. Hand hygiene is considered as the most effective procedure for reducing infection in a wide variety of settings. The aim of this study was to assess the effectiveness of an intervention program to improve knowledge and perception of hand hygiene among healthcare workers (HCWs). Methods We conducted a prospective before-and-after study design in university hospital Sahloul, Sousse, Tunisia, from January 2015 to December 2016. The same questionnaire, constructed by the WHO was distributed and collected at HCWs’ workplace. Results A total of 149 and 146 HCWs were enrolled, respectively at pre and post intervention. The majority was nursed women aged between 20 and 35 in pre intervention, whereas in post intervention the majority where physicians. Globally, our results showed tendencies toward improvement, but only two items were significant: the HW is the appropriate technique after injury (73.5% vs 86.8%; p = 0.006) and the hands of HCWs are the lead vehicle of HAIs (65.1% vs 83.6%; p = 0.01). On the other hand, judging by the HCWs’ responses, the probably most effective intervention to enhance hand hygiene compliance was that an alcohol-based hand-rub is available at each point of patient care. In contrast, empowering patients to remind HCWs to perform hand hygiene was mostly rated to be the least effective intervention both in pre and post intervention. Conclusions Our intervention to positively influence hand hygiene knowledge was unfortunately less effective than we had hoped it would be, which suggests a HCWs’ reluctance to attend the training program. Key messages Hand hygiene is considered as the most effective procedure for reducing nosocomial infection. the intervention to positively influence hand hygiene knowledge was effective.

ABSTRACT Objective: This study aims to evaluate the reliability and equivalency of using the Cobbmeter application for iPhone compared to the manual measurement method in the analysis of the sagittal spinal alignment. Methods: Cross-sectional, prospective, single-center study that had 20 panoramic radiographs of the spine in lateral view, in a neutral standing position, analyzed blindly and randomly by three independent examiners in three different times. The parameters were pelvic incidence (PI), pelvic tilt (PT) and lumbar lordosis (LL). The statistical analysis was performed to measure the intraclass correlation coefficient (ICC) between the two measurement methods, in addition to measuring the intra and inter-evaluators reliability. Results: For reproducibility analysis, the intra-evaluators ICC using the application resulted in a Kappa (K) of 0.975 for the evaluation of pelvic incidence (PI) evaluation. For pelvic tilt (PT), the K value obtained was 0.981 and the K measured for lumbar lordosis (LL) analysis was 0.987. The inter-evaluators evaluation of reproducibility using the application resulted in a K value of 0.917 for PI, 0.930 for PT and 0.951 for LL. For the assessment of equivalency of methods, comparing the application to the standard method, with a goniometer and dermographic pencil, the K value found for PI was 0.873, for PV was 0.939 and for LL was 0.914. All values were significant (p<0.001) against the null hypothesis. Conclusion: This smartphone application is a valid and reliable instrument for measuring the angle involved in the sagittal balance of the spine. Furthermore, the results show that its applicability is not inferior to the manual method with goniometer and dermographic pencil.

This study reports a pre-experimental research on the impact of metacognitive instruction on EFL learners’ metacognitive awareness and their listening performance. To obtain the goal of the study, a group of 30 Iranian intermediate EFL learners, including 14 males and 16 females, were selected randomly. Their ages range from 20 to 24. The participants took part in 16 weeks’ intervention program based on metacognitive pedagogical sequence consisted of five stages. The metacognitive awareness listening questionnaire (MALQ), and a listening test were also used to find changes in metacognitive awareness and listening performance before and after the treatment. The results of comparing pre and posttests scores revealed that metacognitive instruction raised the learners’ metacognitive awareness and helped them improve their listening comprehension ability.
