The aim: Was to evaluate clinical data after the use of pimecrolimus (1% cream “Elidel”) in patients with mild and moderate severity of atopic cheilitis, according to modern therapeutic requirements. There was an algorithm of treatment the patients with cheilitis proposed it based on the data from literary sources and personal clinical experience. Materials and methods: 22 patients with atopic cheilitis aged from 19 to 40 agreed on a clinical examination in accordance with the protocol of the study. Patients were prescribed “Elidel” in the form of 1% cream for lubrication of the affected areas of the skin and lips, and antihistamine “Erius”, for the normalization of the general condition used sedatives and vitamins after consultation of specialists in the general profile. Results: Patients of both groups during the re-examination after 3, 7, 10 days recorded a positive dynamics of the red border of the lips and skin: a significant reduction in the inflammatory process, normalization of indicators of general blood analysis, improvement in the overall quality of life of patients. Conclusions: The results of treatment allow to consider 1% cream “Elidel” (pimecrolimus) as a preparation of choice in the complex treatment of patients with atopic cheilitis of mild and moderate severity.

Annotation. Diabetes is one of the most common metabolic diseases in the world with a large number of complications. Threatening conditions that significantly reduce the quality of life and even lead to the death include a diabetic foot syndrome. The aim of the study was to investigate the effectiveness of combined ozone therapy in the treatment of patients with type 2 diabetes with chronic lower extremity trophic ulcers. A prospective study of the effectiveness of using the combined method of ozone therapy in the treatment of trophic ulcers of the lower extremities was conducted in the surgical and rehabilitation departments of the University Clinic of the National Pirogov Memorial Medical University. The study involved 52 patients with diadetes mellitus 2 type, who were randomly assigned to the main (n=26) and control (n=26) groups. Subjective and objective changes in the condition of patients were evaluated: an area of trophic ulcers (by A.V. Kuleshov's method), a rate of ulcer’s defect epithelization (by L. Popova's formula) and the quality of life parameters of patients (by the questionnaire SF-36) before and after the treatment. The endpoint of the study was a complete epithelization of the diabetic trophic ulcer. The treatment was carried out according to the generally accepted methodology in both observation groups, and the combination ozone therapy was additionally used for patients of the main group. The latter provided for the intravenous administration of an oxygen-ozonized 0.9% NaCl solution and local exposure of the trophic ulcer defect to the ozonized mixture by chamber aeration. The course of complex treatment was carried out after removal of necrotic tissue and wound cleansing, consisted of 10 local and general ozone therapy procedures, which were carried out every other day. The data obtained were processed using the SPSS for Windows 10.0 and STATISTICA 6.1 application software package (license number BXXR901E245722FA). One month after, the observation changes of the trophic skin defects and Quality of life indicators showed the significantly (p<0.01) better results of treatment in the main group than in the control group. The endpoint of the study was achieved in 85% of patients after complex treatment versus 29% of patients in the control group after a conventional treatment (p<0.01). Using as modern rehabilitation method as ozone therapy is an appropriate complement to the existing treatment regimens, which can significantly accelerate the clinical recovery of patients and, as a result, improve the quality of life. The particular value of the method confirmed by an absence of contraindications and drug overloads of the patient. The study of the effects of ozone therapy on the metabolism of patients with diabetes mellitus type 2 can be a promising area for further research.

The analysis of the results of treatment of 15 patients with paracolostomy fistulas treated on an outpatient basis with the use of the sorbent "Celoform" was carried out. The use of modern drugs for the local treatment of wounds at all stages of complex treatment makes it possible to shorten the time of systemic antimicrobial therapy, avoid the development of side effects, and significantly reduce the cost of expensive antibacterial drugs. Abstract Funding Acknowledgements Type of funding sources: None. Introduction. Cardiovascular diseases (CVD) remain the leading cause of premature death worldwide. According to epidemiological studies CVD prevention is highly effective. A 50% reduction in mortality from coronary artery disease (CAD) is associated with exposure to risk factors and only 40% with improved treatment. Purpose.  To assess the quality of life (QOL) in patients with arterial hypertension (AH) and metabolic disorders, to establish its impact on therapy effectiveness. We examined 280 patients with AH and comorbidity - 171 women and 109 men aged 45-75 years. Along with AH, all patients had comorbidities: 72.6 % suffered from coronary artery disease (CAD), 10.5% after myocardial infarction (MI), 35 % had clinical signs of heart failure (HF), 22, 1% - type 2 diabetes mellitus (t2DM), 27.4% - chronic kidney disease (CKD). Methods. Anthropometry, blood pressure (BP) measurement, immunoassay (C-reactive protein), biochemical blood analysis (lipid, carbohydrate metabolism parameters, QOL determination (questionnaire Sf-36). Results. In patients with comorbidity of pathologies (presence of AH, t2DM, CKD, CAD, obesity), there was a decrease in assessments in almost all indicators of QOL, and especially significant limitations in the performance of daily activities due to both physical and mental state. Conclusion. The degree of AH in patients with t2DM decreased QOL of physical activity, role-physical functioning, pain and general health, reflecting physical health, as well as mental health, including vitality, social activity, role emotional functioning. T2DM in patients with AH significantly worsens QOL of this category of patients as in physical component summary and mental component summary. The indicators of QOL are significantly affected by the duration of t2DM, as well as the degree of compensation. Achievement of the target BP levels in patients with AH with t2DM shows an improvement in a number of QOL parameters and makes it possible to recommend the Sf-36 questionnaire as a criterion for the effectiveness of the therapy.

Описан клинический случай пациент с нефротическим синдромом, стероид резистентный вариант, с артериальной гипертензией, осложненный полисерозитом (гидроторакс, асцит, плеврит). Пациент поступил в клинику в состоянии средней степени тяжести, с массивными отеками, полисерозитом, артериальной гипертензией и выраженным нефротическим синдромом. Диагноз: Гломерулярная болезнь. Нефротический синдром, стероид резистентный вариант. Функция почек снижена (СКФ- 84мл/мин по Шварцу). Двухсторонний экссудативный плеврит. Гидроторакс. Полисерозит (в рамках нефротического синдрома) был установлен на основании выраженного нефротического синдрома. Пациенту была проведена патогенетическая терапия. Отмечено улучшение состояния больного, в виде снижения отеков, нормализаций артериального давления, что в свою очередь поспособствовало сохранению и улучшению качества жизни пациента. A clinical case of a patient with nephrotic syndrome, steroidresistant variant, with arterial hypertension complicated by polyserositis (hydrothorax, ascites, pleurisy) is described. The patient was admitted to the clinic in a state of moderate severity, with massive edema, polyserositis, arterial hypertension, and severe nephrotic syndrome. Diagnosis: Glomerular disease. Nephrotic syndrome, steroidresistant variant. The kidney function is reduced (GFR - 84 ml/min according to Schwartz). Bilateral exudative pleurisy. Hydrothorax. Polyserositis (within the framework of nephrotic syndrome) was established based on the severe nephrotic syndrome. The patient underwent pathogenetic therapy. An improvement in the patient's condition was noted, in the form of a decrease in edema, normalization of blood pressure, which in turn contributed to the preservation and improvement of the patient's quality of life.

Современные подходы к организации диагностики и лечения больных пожилого и старческого возраста со злокачественными новообразованиями позволяют улучшать качество жизни и увеличивать продолжительность жизни. Оценка гериатрического статуса в онкологии позволяет прогнозировать осложнения в процессе комплексного лечения, в том числе лекарственного, модифицировать терапию для уменьшения факторов риска неблагоприятных исходов, осуществлять отбор пациентов на специализированное лечение с использованием стандартных схем. Таким образом, своевременная оценка гериатрических синдромов и их коррекция способна расширить показания к специализированному лечению больных пожилого и старческого возраста. Modern approaches to the organization of diagnosis and treatment of elderly and senile patients with malignant tumors allow to maintain the necessary level of health, improve the quality of life and increase life expectancy. Assessment of geriatric status in Oncology allows: to predict complications during the complex treatment, including drug treatment; to modify treatment to reduce the risk factors of adverse outcomes; to select patients for specialized treatment using standard schemes. So, timely assessment of geriatric syndromes and their correction can expand the indications for specialized treatment of elderly and senile patients.

Introduction. Urethral transposition remains the most popular operation for postcoital cystitis, however, traumatism and complications, especially such as pudendal neuropathies, dyspareunia and anorgasmia, make us continue to search for effective, but safer techniques.The study objective is to evaluate the efficacy and safety of the proposed treatment of postcoital cystitis, including removal of urethrogymenal adhesions and subsequent paraurethral filler implantation, in comparison with isolated hymenoplasty.Materials and methods. Since 2013, 75 patients with postcoital cystitis have been treated. Patients were divided into two groups: main group – hymenoplasty (removal of urethral adhesions) with paraurethral filler implantation (n = 45), control group – hymenoplasty (n = 30). The gel was injected paraurethrically, fan-shaped, in the volume of 1–2 ml, from a point on the 6-hour conditional dial, creating a gel cushion and thus raising the meautus and distal urethra. To assess the quality of treatment, profile questionnaires were used.Results. In the main group of patient, the quality of life improved in 35 (78 %); in 5 patients, due to the process of biodegradation of the gel during 1 year, the cystitis recidivated, which required the filler reimplantation. In 5 patients, the operation was not effective. In the control group the efficiency of isolated hymenoplasty was noted in 3 (10 %) patients, relapse of cystitis occurred in 27 (90 %) patients, which later required the implantation of a filler. No complications were observed.Conclusions. The suggested combined technique allows to improve the results of treatment of patients with postcoital cystitis. The operation does not carry the risk of damage to the sprigs of the genital nerve, can serve as an alternative to traditional urethral transposition. The main disadvantage is the natural biodegradation of the gel, which may create the need for its reintroduction.

Aim. Assessment of the quality of life and physical activity level in students with and without symptoms of functional dyspepsia (FD) and irritable bowel syndrome (IBS) according to questionnaire “7×7” (7 symptoms per 7 days).Materials and methods. Symptoms of FD and IBS were surveyed using the “7×7” questionnaire. Level of physical activity was evaluated according to the short IPAQ, and quality of life — to WAM questionnaires.Results. The study surveyed 92 students aged 20.7 ± 0.2 years (56 men and 36 women). We report borderline manifestations of functional disorders of the gastrointestinal tract (GIT) in 51.1 %, and FD and/or IBS symptoms of mild to moderate severity in 23.9 % of respondents. Symptoms of FD and/or IBS were equally common in men and women. Severity of FD and/or IBS symptoms was rated 4 [[3; 7] in men and 4 [[4; 11] in women (p = 0.25). Physical activity of 57.6 % in students corresponded to a moderate level. Healthy students were more likely to exhibit higher physical activity than individuals with FD and/or IBS symptoms, 56.5 and 31.9 %, respectively (p = 0.04). The level of wellbeing and severity of FD and/or IBS symptoms correlated negatively (r = –0.28, p = 0.01). Wellbeing and mood correlated directly with physical activity, r = 0.33, p = 0.001 and r = 0.27, respectively (p = 0.01).Conclusions. 1. Symptoms of FD and/or IBS occur widely among students and equally in men and women. Functional disorders of the gastrointestinal tract of mild to moderate severity occur in 23.9 % of students, with borderline symptoms registered with every second individual. 2. Healthy students significantly more often exhibited higher physical activity compared to individuals with FD and/or IBS symptoms of varying severity in the ratios of 56.5 and 31.9 % (p = 0.04). 3. Severity of FD and/or IBS in students negatively correlates with the wellbeing component of quality of life (r = –0.28, p = 0.01).

Objective: to study the effectiveness of art therapy methods in the complex treatment of neuropsychological disorders and quality of life improvement in patients with relapsing-remitting and secondary-progressive multiple sclerosis (MS).Patients and methods. A group of 42 patients (5 men and 37 women, mean age 32.9 years, mean EDSS severity 3.8 points) with MS underwent outpatient continuous active art therapy cycle. The mean group training cycle duration was six months. The comparison group included 15 age- and sex-adjusted individuals without neurological disorders. Tests on various drawing topics were carried out before and after a three-month therapy course. We used Hospital Anxiety and Depression Scale (HADS) to assess the severity of anxiety and depression and the SF-36 scale to evaluate the quality of life (QoL).Results and discussion. Before the art therapy start, light blue, yellow, pink, blue, and green colors prevailed in the drawings. There was no predominance of any color depending on MS severity or course. The green color predominated in this test in the control group, and the light blue color was significantly less represented. The art therapy course did not lead to a significant change in the severity of MS according to the Expanded Disability Status Scale (EDSS). After the art therapy course, the color scheme of the drawings was mainly represented by red, light blue, green, yellow, and blue colors. In addition, a red-orange color appeared in the drawings, which was completely absent before the course. Positive trends were noted according to HADS: a decrease in anxiety by 23% and depression by 19%. The physical component of QoL assessed by the SF-36 scale did not change substantially, but the psychological component significantly improved by 11%.Conclusion. The predominance of light blue color observed in MS is associated with damage to the nervous and musculoskeletal systems, and blue color – with psychological problems and depression. However, after a course of art therapy for MS, red, red-orange, yellow, and green colors representation increased significantly, which indicates an increase in positive attitude, a decrease in the level of depression, and an improvement in QoL.

The aim of the study, analyze and evaluate the quality control and safety of medical activities carried out in medical organizations. Results: the essence of the concept of quality control in the field of medical care was determined, criteria for assessing the quality of medical activities were identified, the main aspects of developing a program for internal quality control and safety of medical activities were characterized, key risks existing in medical activities were identified, directions for improving the quality and safety of professional activities of medical workers. Conclusion: modern approaches to the organization of quality control and safety of medical activities make it possible to ensure a high level of medical care provided to the population and improve the quality of life of the population as a whole.
