Regional  information  system  is a term that is used to describe a system consisting of a combination of input such as human resources (brainware), software, information needs format (informware), information technology and communication technology, the process of sending an analysis report recording (entry). the feedback and output data information mechanism that can be accessed together and has adequate quality, regional information system in the Langsat Health Center Medical Record Unit only started in 2017, SIKDA in the medical records unit lacks Human Resources because it does not there is a Medical Record and IT graduate. Obstacles that occur, such as disruption of soft ware and hard ware networks when opened, experience delays, causing delays in the given services. For the Fund itself, there is no special budget provided for the implementation of theregional information system  application because it has been allocated by the Health Office. Budget allocation of funds is held only if needed. This type of qualitative research uses interview and observational sheets. The research was conducted in February-September, at Langsat Public Health Center, Pekanbaru City. The research subjects were the Head of Langsat Community Health Center, the Head of the Medical Records Unit, the person in charge of SIKDA, and Operators. Data analysis was used by systematically managing the interview guidelines, then processing the data, data from observations were identified to describe each variable, a summary will be presented in a narrative form. The results of this study indicate that the Generic regional information system  in the Medical Records Unit has been running at its maximum, even though there is a lack of calm in Human Resources there are no medical records and IT graduates. Infrastructure is lacking in terms of ginset, Fund Allocation is not a special budget. Must improve Human Resources, Facilities and Infrastructure.

The information system at Kampung Sawah Community Health Center has several features such as patient registration, patient medical records, patient services, queues, and medical reports. Aim of this information system development is to facilitate the performance of administrators and doctors of Kampung Sawah Community Health Center in terms of searching patients’ data, storing the patient data, adding and storing the medical records, making the reports, and speeding up the data entry and identity data of the patients through fingerprint feature. This system facilitated the officials and doctors to process anywhere and the patient data was safe from physical disturbances because this system was connected to the internet.This medical record information system was built through PHP programming language supported by MySQL database. The design of this system involved the data flow diagrams. The result of this system was that the web-based information system for medical record had successfully been created and used by Kampung Sawah Community Health Center.Keywords: Information Systems, Medical Record, Web, Fingerprint

Puskesmas merupakan fasilitas pelayanan kesehatan yang menyelenggarakan upaya kesehatan masyarakat dan upaya kesehatan perseorangan tingkat pertama. Dalam melaksanakan tugasnya puskesmas berwenang untuk untuk melaksanakan pencatatan dan pelaporan kesehatan pasien serta melakukan evaluasi terhadap mutu dan cakupan pelayanan kesehatan. Pencatatan dan pelaporan pasien dapat dilihat dari berkas rekam medis pasien. Rekam medis pada sarana kesehatan non rumah sakit wajib disimpan sekurang-kurangnya untuk jangka waktu dua tahun terhitung dari tanggal terakhir pasien berobat. Setelah batas waktu penyimpanan dilampaui maka rekam medis dapat dimusnahkan. Puskesmas Kuranji merupakan salah satu puskemas di kota padang yang berlamat dikecamatan Kuranji. Berdasarkan hasil wawancara dengan petugas rekam medis, hingga saat ini Puskesmas Kuranji belum melakukan retensi dan pemusnahan berkas rekam medis sedangkan ruangan penyimpanan hanya ada satu. Berdasarkan pemaparan masalah di atas, maka pengabdi bermaksud untuk melakukan sosialisasi dan praktik langsung mengenai retensi dan pemusnahan berkas rekam medis di Puskesmas Kuranji Kota Padang. Target dan luaran dari pelaksanaan kegiatan pengabdian kepada masyarakat adalah kegiatan ini dapat dijadikan sebagai bahan pertimbangan untuk pelaksanaan retensi dan penghancuran berkas rekam medis. Target kedepannya agar sosialisi ini dapat digunakan sebagai dasar kegiatan retensi dan penghancuran berkas rekam medis. Kata Kunci: Puskesmas, Retensi, Pemusnahan, Rekam Medis ABSTRACT Public health center (PHC) is a health service facility that organizes public health efforts and first-level individual health efforts. In carrying out its duties PHC is authorized to carry out the recording and reporting of patient health and to evaluate the quality and scope of health services. Patient recording and reporting can be seen from the patient's medical record file. Medical records in non-hospital healthcare facilities must be kept for at least two years from the date the patient was treated. After the storage time limit is exceeded, the medical record can be destroyed. The Kuranji Community Health Center is one of the public health centers in the city of Padang which is well-known in the Kuranji sub-district. Based on the results of interviews with medical record officers, up to now the Kuranji Community Health Center has not retained and destroyed medical record files while there is only one storage room. Based on the explanation of the problem above, the service intends to conduct socialization and direct practice regarding the retention and destruction of medical record files at the Kuranji Health Center in Padang City. The target and output of the implementation of community service activities is that this activity can be used as consideration for the implementation of retention and destruction of medical record files. Human resources are an important and vital component in an organization's operations. Quality health services can not be separated from the organization of a good medical record. In order to run well, sufficient medical records are needed. In the Adan-Adan Health Center, the number of patient visits from July 2018 to July 2019 was 16,828 people. Medical records officer only numbered 1 person. The research objective is to calculate the needs of human resources in the medical record unit using the ABK method. The design of this research is descriptive observational with case study approach. Respondents in this study were medical staff at the Adan-adan Health Center in Kediri Regency with a total of 1 person. The results of the study are that there is a shortage of human resources in the Adan-Adan Health Center medical record unit which should be 3, but totaling 1 person. Suggestions for Adan-Adan Health Center are employee recruitment needs to be done so that the number of medical records officers can be fulfilled. AbstrakSumber daya manusia merupakan komponen penting dan vital dalam operasional sebuah organisasi. Pelayanan kesehatan yang bermutu tidak terlepas dari penyelenggaraan rekam medis yang baik. Agar berjalan dengan baik, maka dibutuhkan sumber daya rekam medis yang cukup. Kunjungan pasien di Puskesmas Adan-adan pada periode Juli 2018 – Juli 2019 sebesar 16.828 orang. Petugas rekam medis hanya berjumlah 1 orang. Tujuan penelitian untuk menghitung kebutuhan sumber daya manusia di unit rekam medis menggunakan metode ABK. Desain penelitian ini yaitu observasional deskriptif dengan pendekatan case study. Responden dalam penelitian ini yaitu petugas rekam medis Puskesmas Adan-adan Kabupaten Kediri yang bejumlah 1 orang. Hasil penelitian yaitu terdapat kekurangan jumlah sumber daya manusia di unit rekam medis Puskesmas Adan-adan yang seharusnya berjumlah 3, namun saat ini berjumlah 1. Saran bagi Puskesmas Adan-adan yaitu perlu dilakukan rekrutmen pegawai agar jumlah petugas rekam medis dapat terpenuhi.

In implementing health services, puskesmas must document all actions and treatments that are given to patients in a document called Medical Records. According to Minister of Health Regulation No.269 / MENKES / PER / III / 2008 article 1 (1), medical records are files containing notes and documents about patient identities. Medical records are of good quality if the medical record is accurate, complete, trustworthy, valid and timely. One form of management in Medical Records is reporting. According to Minister of Health Regulation No.269 / MENKES / PER / III / 2008 article 1 (1), Medical Record is a file that contains notes and documents about patient identity, examinations, actions, and other services that have been given to patients. In the statement, all information about a patient has been reflected which will be made the basis for determining further actions in services and other medical actions given to a patient who comes to the community health center. The Medical Record is said to be of high quality if the Medical Record is accurate, complete, trustworthy, valid and timely. The Medical Record Installation has activities such as registration, data processing, and storage. One form of processing data in medical records is the existence of assembling activities. Assembling is an assembling activity compiling empty Medical Record forms and storing them into Medical Records, ready to use neatly arranged both in terms of quality and quality.

The administrative process at Kotaratu health center still uses manual means. So, it takes a long time in handling patient visits and making reports. The goal of the study was to build a desktop-based medical record information system using the Microsoft Visual Basic. Net Programming Language and MySQL as its database. Data collection techniques through interviews, observations and literature studies. Meanwhile, to develop software (system) with waterfall method, while testing uses blackbox-testing. Our findings are in the form of a desktop-based information system at Kotaratu health center. Furthermore, Blackbox-testing results show that all components in this system are running well, and all medical record data is stored in the database to provide a convenience in managing patient data, searching for medical records, managing medical records and making reports periodically.

Abstract. West Java Provincial Health Office still faces difficulties in managing information, especially in medical records. Recording and reporting of malnutrition are still done in some stages starting from collecting data from village midwives, puskesmas, Regency/City Health Office then Provincial Health Office and forwarded to the the central office. It is necessary to manage information through service system by utilizing Cloud Computing based on information technology. This research uses The Open Group Architecture Framework (TOGAF) approach in Architecture Development Method (ADM), from Architecture Capability Iteration to  Architecture Development Iteration. Monitoring and Evaluation (M & E) are two integrated activities in the context of controlling a program. The results of this research are planning a medical record information system architecture and monitoring malnutrition based on Cloud Computing with the name of M2Rec (Medical Record and Monitoring) in the form of integrated recommendation and development between current information system and proposed information system architecture.Keywords: togaf adm, medical record and monitoring, cloud computing Abstrak. Sehingga perlu diupayakan pengelolaan informasi melalui sistem pelayanan dengan memanfaatkan teknologi informasi berbasis Cloud Computing. Information system at community health center is an information system that has several activities, such as registration, medical record, health care, and reporting.  Day to day operation, community health service, is using process manually. It is cause the stack of service. Sometime, the patient has to wait within several times. For Further, the patient did not know that the queuing is full. In order to help the problem, this paper wants to show about E-Health as service software. The research is completed by conveying the model like UML diagram. The UML diagrams are consisting such as usecase, class, activity, and component. The sequence of system construct is using Prototype Paradigm. The result is the software which has ability to service patient start from registration, medical check, medical prescription, until reporting. As an impact for Community health service is the service more efficiency. ABSTRACTBackground : Medical record is one of the most important pillars that can not be considered trivial in a hospital, with the development of medical scienceCommon Purpose : To find in-depth information about the implementation of medical records at the hospital Widiyanti PalembangResearch Method : Qualitative research design with data collection techniques are conducted in triangulation, The data analysis is inductive, and the results of the study are emphasized more at the meaning than the generalization. The Research Results : the Implementation of medical records have been running but there is no medical record organization, the implementation of medical record activities done by rolling. Human Resources (HR) medical records should be placed specifically in the medical record along with clear tasks. Method of organizing medical record has been run although the result is not optimal, because Standard Operational Procedure (SOP) that made not socialized. Facilities and infrastructure that support the implementation of the medical record is good, marked by the existence of a special records archive medical records. Facilities and infrastructure such as chairs, desks, computers, patient registration books and outpatient registration and inpatient services are available, do not have budget funds for medical record implementation, especially by sending medical recruiter for trainingConclusion : Implementation of medical records have been running but not optimal.

Good health care is a community need and is often a measure of development success. The purpose of this study was to analyze inpatient health care service level I to the satisfaction of BPJS patients in the Sikumana Health Center in Kupang City. The study was conducted at the Sikumana Community Health Center in Kupang City from 15 October to 10 November 2019. The research design used was a qualitative study using a cross sectional approach. The independent variable is health service and the dependent variable is patient satisfaction. The population of all BPJS patients in the Sikumana Kupang health center in the January-August period was 230 respondents. The sampling technique used was Simple Random Sampling with a sample of 146 respondents data analysis techniques using the Logistic regression test. The results of the study of 146 respondents the majority (56.2%) of respondents were very satisfied with the service of nurses, there were 82 research subjects, the majority (58.9%) of respondents were very satisfied with the service of doctors namely there were 86 research subjects and the majority (56, 2%) respondents are very Satisfied with Nurse services, there are 82 research subjects, almost half (45.9%) respondents get good service based on Tangibels, almost half (43.2%) respondents get good service based on reliability, most ( 54.1%) respondents get good service based on Responsiveness, almost half (46.6%) respondents get good service based on Assurance and almost half (44.5%) respondents get good service based on Empathy. Logistic regression data analysis concluded that BPJS patient satisfaction based on health services at the Community Health Center in Sikumana, Kupang, obtained a p value of 0.002 because p value <α (0.05) It is expected that research sites will improve the cleanliness of the ward each morning before the examination and in the afternoon, especially the bathroom of the patient and the staff must be diligent in controlling hygiene
