<p><strong>Resumen:</strong></p><p>La apoplejía hipofisiaria es una complicación poco frecuente, con escasos casos descritos en la literatura mundial. Su asociación con el embarazo genera dificultades diagnósticas, con secuelas a corto y largo plazo que ponen en peligro la vida de la paciente de no darse el seguimiento apropiado. Presentamos un caso de apoplejía hipofisiaria de presentación durante el embarazo y revisión de la literatura.</p><p>Palabras claves: Apoplejía hipofisiaria, hipófisis, embarazo.</p><p><strong>Abstract:</strong></p><p>Pituitary apoplexy is a rare complication, with few cases described in the world literature. Its association with pregnancy creates diagnostic difficulties, with short- and long-term sequelae that are life threatening if not properly followed up. The economy of Southeast Asia has been in relatively good shape in spite of the instability of the world monetary system, trade deficits, and the worldwide oil crisis. There are promising factors for economic growth, opportunities for employment, and possibilities of rising income. But Asian development presents short-and long-term problems of a very complicated nature. One of the most serious problems is inflation and its impact on the social and political programs of individual countries. At the same time, there are severe shortages of basic commodities, such as oil and food. My own country, Japan, is among those affected.

Abstract Background Pregnancy is a known risk factor for pituitary apoplexy, which is life threatening for both mother and child. However, very few clinical interventions have been proposed for managing pituitary apoplexy in pregnancy. Case presentation We describe the management of three cases of pituitary apoplexy during pregnancy and review available literature. Presenting symptoms in our case series were headache and/or visual disturbances, and the etiology in all cases was hemorrhage. Conservative therapy was followed until 34 weeks of gestation, after which babies were delivered by cesarean section with prophylactic bolus hydrocortisone supplementation. Tumor removal was only electively performed after delivery using the transsphenoidal approach. All three patients and their babies had a good clinical course, and postoperative pathological evaluation revealed that all tumors were functional and that they secreted prolactin. Conclusions Although the mechanism of pituitary apoplexy occurrence remains unknown, the most important treatment strategy for pituitary apoplexy in pregnancy remains adequate hydrocortisone supplementation and frequent hormonal investigation. Radiological follow-up should be performed only if clinical symptoms deteriorate, and optimal timing for surgical resection should be discussed by a multidisciplinary team that includes obstetricians and neonatologists.

A case is presented of dermatofibrosarcoma protuberans of the gluteal region with metastasis to the lung appearing 7 years after wide excision of the primary lesion. The world literature is reviewed. The clinical and pathologic features of dermatofibrosarcoma are reviewed and treatment is discussed, with the aim of emphasizing the need for long-term follow-up examination of lymph nodes and for metastases following wide excision of these lesions.

The COVID-19 pandemic continues to ravage the world, with the United States being highly affected. A vaccine provides the best hope for a permanent solution to controlling the pandemic. However, to be effective, a vaccine must be accepted and used by a large majority of the population. The aim of this study was to understand the attitudes towards and obstacles facing vaccination with a potential COVID-19 vaccine. To measure these attitudes a survey was administered to 316 respondents across the United States by a survey corporation. Structural equation modeling was used to analyze the relationships of several factors with attitudes toward potential COVID-19 vaccination. Prior vaccine usage and attitudes predicted attitudes towards COVID-19 vaccination. Assessment of the severity of COVID-19 for the United States was also predictive. Approximately 68% of all respondents were supportive of being vaccinated for COVID-19, but side effects, efficacy and length of testing remained concerns. Longer testing, increased efficacy and development in the United States were significantly associated with increased vaccine acceptance. Messages promoting COVID-19 vaccination should seek to alleviate the concerns of those who are already vaccine-hesitant. Messaging directed at the benefits of vaccination for the United States as a country would address the second predictive factor. Enough time should be taken to allay concerns about both short- and long-term side effects before a vaccine is released.

Hypertensive disorders in pregnancy are common and can occur as a result of pre-existing hypertension or as new onset hypertension usually in the second half of pregnancy. In either situation there is potential for considerable perinatal and maternal morbidity and mortality. This review article aims to compare therapeutic options outlined in a selection of national guidelines and to look in more detail at the most commonly prescribed drugs – labetalol, methyldopa and nifedipine – with respect to their pharmacology and the evidence for their use in pregnancy. We will also consider the rationale for identifying and treating hypertension in pregnancy and the effect this can have on short- and long-term maternal and neonatal outcomes.

Student mock elections are conducted in schools around the world in an effort to increase political interest and efficacy among students. There is, however, a lack of research on whether mock elections in schools enhance voter turnout in real elections. In this article, we examine whether the propensity to vote in Swedish elections is higher among young people who have previously experienced a student mock election. The analysis is based on unique administrative population-wide data on turnout in the Swedish 2010 parliamentary election and the 2009 European Parliament election. Our results show that having experienced a mock election as a student does not increase the likelihood of voting in subsequent real elections. This result holds when we study both short- and long-term effects, and when we divide our sample into different parts depending on their socio-economic status and study each part separately.

Abstract Primary tumors of the gasserian ganglion are usually benign, and secondary tumors are malignant. We report two cases of a malignant primary tumor of the gasserian ganglion, bringing the total in the world literature to five. The presentation, etiological features, and treatment of these cases are reviewed, together with a review of the literature. Preoperative differentiation from a benign tumor is not possible with certainty. A combination of operation and radiation therapy seems to be the best treatment at present.

Hypertension is the most common medical disorder encountered during pregnancy. Hypertensive disorders are one of the major causes of pregnancy-related maternal deaths in the United States. We will present a comprehensive update of the literature pertinent to hypertension in pregnancy. The paper begins by defining and classifying hypertensive disorders in pregnancy. The normal vascular and renal physiological changes which occur during pregnancy are detailed. We will summarize the intriguing aspects of pathophysiology of preeclampsia, emphasizing on recent advances in this field. The existing diagnostic tools and the tests which have been proposed for screening preeclampsia are comprehensively described. We also highlight the short- and long-term implications of preeclampsia. Finally, we review the current management guidelines, goals of treatment and describe the potential risks and benefits associated with various antihypertensive drug classes. Preeclampsia still remains an enigma, and the present management focuses on monitoring and treatment of its manifestations. We are hopeful that this in depth critique will stimulate the blossoming research in the field and assist practitioners to identify women at risk and more effectively treat affected individuals.

Abstract A lumbar spinal extradural ganglion cyst is reported, and the world literature on spinal ganglion cysts is reviewed. The clinical presentation, findings, etiology, and pathogenesis of these lesions are discussed.
