scholarly journals Reflecting on the Concept of Compassion Fatigue

A review of the literature on the health of nurses leaves little doubt that their work may take a toll on their psychosocial and physical health and well being.Nurses working in several specialty practice areas, such as intensive care, mental health, paediatrics, and oncology have been found to be particularly vulnerable to work-related stress. Several types of occupational stress have been identified, including burnout, compassion fatigue, and vicarious traumatization. While the emphasis of this article is on compassion fatigue and its theoretical conceptualization, the concepts of burnout and vicarious traumatization are also discussed. Two questions are posed for discussion: 1) Does compassion fatigue exist on a continuum of occupational stress? If so, is burnout a pre-condition for compassion fatigue; 2) What are the relationships between the types of occupational stress? To what extent does non-resolution of compassion fatigue increase the risk for developing vicarious traumatization? Case examples are provided to support this discussion.

This article presents a review of the literature that pertains to the experiences of therapists who work directly with child sex offenders and/or people with pedophilia. We draw together results from studies that attempted to identify how therapists experience such work and how they were personally impacted by it. Usually, such studies are embedded within one of the following theoretical frameworks: Secondary traumatic stress, compassion fatigue, vicarious traumatization and burnout. Most literature on the topic has therefore sought to determine to what extent and why, work-related stress responses may occur among these therapists. The aim of this paper is therefore to provide insight into this, arguably, important line of research, while evaluating the current knowledge as well as providing recommendations for future research efforts.

‘Mad doctors’ specialized in treating the insane, but what about the doctors whose own mental health was jeopardized? Oppenheim found that doctors who attended the mad were presumed to be particularly vulnerable, but there has been no research investigating this claim, nor identifying practitioners’ experiences as patients. This article analyses medical admissions to asylums via both case notes and other sources such as newspaper reports, revealing the responses of medical superintendents to their former colleagues and, in some cases, the judgements of practitioners on their institutional surroundings. It indicates the impact of work-related stress, as medicine became self-consciously professional, and the evolution of public reactions to doctors who could not maintain an appropriately sane identity.

This article presents an empirical study that identified agency supervisees' perceptions of clinical supervision and its influence on work satisfaction and work-related stress in professional settings. Because there is a paucity of literature addressing supervision of professional counselors, there is a need to better understand what influence supervision has beyond academic settings. Participants were 71 members of the American Mental Health Counseling Association who were selected using a criterion-based random sample methodology. The methodology pulls together a unique combination of variables and instruments for exploration with professional mental health counselors. Results revealed relationships between work setting, supervisees' perceptions of the supervisory working alliance, work satisfaction, and work-related stress variables. Implications for practice, training, and research are discussed.

This study determined the impact and influence of published articles on the field of occupational stress. A transdisciplinary approach was used to identify the 50 work-related stress articles with the most lifetime citations and the 50 work-related stress articles with the highest annual citation rates. Studies were categorized based on their primary focus: (a) etiology, (b) predictor of outcome for which occupational stress is the outcome or predictor of outcome for which occupational stress is an independent variable, (c) management/intervention, (d) theory/model/framework, or (e) methodologies. The majority of studies with the highest number of lifetime citations as well as the highest annual citation rates used stress as a predictor or outcome of another factor. The proportion of studies that were categorized by etiology, intervention/management, theory/model/framework, or methodologies was relatively low for both lifetime and annual citations.

IntroductionPhysician Suicide is a potential health risks resulting from strains and burden associated with medical education and profession. Suicide is an occupational hazard. Each year in the United States, 300 to 400 physicians take their own lives.ObjectiveTo provide a summary about physician suicide and its risk factors and mental health issues associated.MethodsThe search was conducted using PubMed with terms: “suicide in physicians”, “physician suicide”, “suicide in doctors”, “physician depression”, by using a review of literature with documents in English.DiscussionSuicide is a major health problem. Suicide death is a self-inflicted with evidence that the person aims die. Mental disorders represent a large burden of disease worldwide and can also damage to physical health. The most common psychiatric diagnoses among physicians who complete suicide are affective disorders, alcoholism, and substance use disorders. In physicians, the female suicide rates are higher than that in males. The most common means of suicide by physicians are lethal medication overdoses and firearms. There are common risk factors, such as work-related stress, depression, negative life events, alcohol and isolation. In addition, there is a physicians’ tendency not to recognize depression in themselves and not to seek help.ConclusionsPrioritize to physician mental health, change professional attitudes and institutional policies, learn to recognize depression and suicidality, educate medical students, residents, routinely screen all primary care patients for depression that can help physicians recognize depression in themselves and to seek treatment for depression and suicidality because there is “no health without mental health”.Disclosure of interestThe author has not supplied his/her declaration of competing interest.

This study evaluated the effects of locus of control, occupational stress, and psychological symptom distress on reported job satisfaction in a sample of 34 practicing nurses. As predicted, greater work-related stress and higher psychological symptom distress were significantly negatively correlated with job satisfaction. External locus of control was also negatively associated with job satisfaction.

PURPOSE: The purpose of the study is to identify the factors predicting psychiatric nurses’ decision to use physical restraint in a clinical psychiatric setting in the Province of Jeddah, Saudi Arabia. METHODS: A descriptive explanatory design was used. 110 nurses working in a psychiatric hospital in Jeddah city were recruited during the period 27th April to 29th June 2017. The outcome was assessed on the following scales: level of knowledge, occupational stress, working environment, nurses’ attitude toward use of physical restraint and nurses’ practice toward use of physical restraint. RESULTS: The study participants had sufficient knowledge about the use of physical restraint, experienced high levels of occupational stress, suffered an unproductive working environment and accepted attitudes and practice toward physical restraint. In addition, these variables significantly predicted the nurses’ use of physical restraint. CONCLUSION: The findings revealed that the level of knowledge and occupational stress scales, the working environment, and nurses’ attitude and practice toward the use of physical restraint significantly predicted the nurses’ use of physical restraint. RECOMMENDATIONS: The study recommends the establishment of educational and awareness programmes for nurses to better understand the concept of restraining a patient and the consideration of alternative measures for controlling agitated and violent patients. It also recommends that providing adequate staffing and other resources, maintaining a therapeutic ward environment, and decreasing work-related stress could influence psychiatric nurses’ decisions to use physical restraint on their patients.
