Different Perceptions of Factors Affecting the Financial Sustainability in Public Administrations

With the public finance crisis, the analysis of financial sustainability in governments, its influential factors, and the examination of politicians' perceptions about them have become relevant. In fact, it could be necessary to know whether politicians perceive as drivers or risk factors of financial sustainability those variables that prior research has identified with empirical findings. This chapter seeks to identify whether politicians are aware of influential factors previously identified by prior research for financial sustainability and whether personal characteristics of politicians (age, gender, or educational background) could influence this perception. So, an e-survey targeting politicians was conducted in sample of Spanish municipalities to capture their perceptions on potential risk factors and drivers, comparing this result with the findings obtained by prior research. Findings of this chapter indicate that there are different perceptions of politicians about drivers and risk factors among respondents according to their age, gender, and educational background.

Abstract Objective To investigate the incidence and potential risk factors for development of fenofibrate-associated nephrotoxicity in gout patients. Methods A total of 983 gout patients on fenofibrate treatment who visited the dedicated Gout Clinic at the Affiliated Hospital of Qingdao University between September 2016 and June 2020 were retrospectively enrolled from the electronic records system. Fenofibrate-associated nephrotoxicity was defined as an increase in serum creatinine (SCr) ≥0.3 mg/dl within 6 months of fenofibrate initiation. The change trend of SCr and uric acid levels during the treatment period were assessed by a generalised additive mixed model (GAMM). Multivariate analysis was performed for risk factors affecting elevated SCr. Results A total of 100 (10.2%) patients experienced an increase in SCr ≥0.3 mg/dl within 6 months after fenofibrate initiation. The median change of SCr in the whole cohort was 0.11 mg/dl [interquartile range (IQR) 0.03–0.20], whereas it was 0.36 (0.33–0.45) in the fenofibrate-associated nephrotoxicity group. In a multivariable regression model, chronic kidney disease (CKD) [odds ratio (OR) 2.39 (95% CI 1.48, 3.86)] and tophus [OR 2.29 (95% CI 1.39, 3.78)] were identified to be risk predictors, independent of measured covariates, of fenofibrate-associated nephrotoxicity. During the treatment period, although SCr temporarily increased, serum urate and triglyceride concentrations decreased using the interaction analysis of GAMM. Of those with fenofibrate withdrawal records, the SCr increase in 65% of patients was reversed after an average of 49 days off the drug. Conclusions This observational study implied that fenofibrate-associated nephrotoxicity occurs frequently in gout patients, especially in patients with tophi or CKD. The potential renal risks of fenofibrate usage in gout needs additional research.

Abstract Objectives The increasing number of hip arthroplasties (HA), due to the growing elderly population, is associated with the risk of femoral periprosthetic fractures (FPFs). The purpose of this study was to identify potential risk factors for the development of FPFs after HA. Methods A systematic review was conducted in five data bases (Medline, Embase, Cochrane, Cinahl, ICTRP) according to the Preferred Reporting Items for Systematic reviews and Meta-analysis (PRISMA) guidelines up to May 2019, using the key words “risk factor,” “periprosthetic fracture,” and “hip replacement or arthroplasty.” Meta-analysis of the clinical outcomes of HA and subgroup analysis based on the factors that were implicated in FPFs was performed. Results Sixteen studies were included (sample size: 599,551 HA patients, 4253 FPFs, incidence 0.71%). Risk factors statistically associated with increased incidence of FPFs were female gender (+ 40%), previous revision arthroplasty surgery (× 3 times), and the presence of rheumatoid arthritis (× 2.1 times), while osteoarthritis (− 57%), cement application (− 59%), and insertion of Biomet (− 68%) or Thompson’s prosthesis (− 75%) were correlated with low prevalence of FPFs. Obesity, cardiac diseases, advanced age, bad general health (ASA grade ≥ 3), and use of Exeter or Lubinus prosthesis were not linked to the appearance of FPFs. Conclusion This meta-analysis suggested that female gender, rheumatoid arthritis, and revision arthroplasty are major risk factors for the development of FPFs after a HA. In those patients, frequent follow-ups should be planned. Further prospective studies are necessary to clarify all the risk factors contributing to the appearance of FPFs after HA.

Abstract: This study aims to develop and test the integrative model of the factors affecting customers’ acceptance intention of the adoption of blockchain technology. Data were collected from a sample of 195 customers who have been conducting transactions at Dong A Bank – Hue branch. Samples are selected by using the systematic random sampling method. Structural equation modelling (SEM) is used to test the hypothesized relationships. The findings indicate that six out of eight tested relationships are supported. Perceived usefulness (PU) and Perceived ease of use (PEU) are the most critical factors affecting customers’ Attitude (AT). Attitude also has a direct and positive correlation to customers’ acceptance Intention (IN). Notably, Personal characteristics (PC) and Risk perception (RP) are the two most influential factors affecting Perceived usefulness. And, the Perceived ease of use factor is only affected by customers’ Self-command (SC). In general, this study contributes to enriching the existing knowledge of blockchain adoption in banks and helps banks figure out an efficient way to adopt blockchain technology.Keywords: customers’ acceptance, blockchain adoption, Dong A Bank, Hue branch

Background: In the setting of acute coronary syndrome (ACS), ventricular fibrillation (VF) was serious and fatal complication. However, even though ischemia was well known as the risk for occurrence of VF, what influential factors of ACS patients related to VF were still unclear. We performed extracorporeal cardiopulmonary resuscitation for VF with ACS patients taken to our institute and performed coronary angiography to make definite diagnosis. Accordingly, We sought to the risk factors for VF with ACS. Methods: Consecutive 246 ACS patients hospitalized and underwent emergent coronary revascularization from September 2010 to September 2014 were enrolled. In these, patients with VF were categorized into VF group, and patients without VF were into non-VF group. We reviewed patients’ characteristics, including medications, past histories and laboratory findings, and CAG findings (lesion characteristics with ACC/AHA classification, number of disease vessels, existence of chronic total occlusion (CTO) and left-main coronary artery (LMCA) lesion) retrospectively. Results: Fifty-seven patients developed ACS with VF (VF group), and 189 patients were without VF (non-VF group). Comparing between the groups, the morbidity of previous angina pectoris or coronary artery bypass grafting were significantly higher in VF groups (32.1%-16.0%, p=0.01, 14.3%-1.1%, p<0.001, respectively). The prevalence of Complex lesion characteristics, such as multi vessel disease, CTO and LMCA lesion, was also higher in VF group (71.9%-42.8%, 43.9%-13.9%, 15.8%-6.3%, all p < 0.025, retrospectively). Moreover, multiple logistic regression analysis revealed multi vessel disease and existence of CTO lesion were independent factors for VF. Conclusion: VF group had more complex characteristics of coronary artery disease. Complex lesion characteristics, such as multi vessel disease, CTO and LMCA lesion, would be potential risk factors of VF with ACS patients.

The idea of consumer satisfaction is gaining momentum across all business sectors around the world and a satisfaction survey is utilized as an instrument to recognize deficiencies with various facets of services and offers a valuable opportunity for customers to evaluate their experience with healthcare services. A first research performed at a public hospital's paediatric clinic, which is also the first hospital to adopt the Public-Private-Partnership (PPP) model under the Ministry of Health Malaysia (MoH), with the aim of discovering prevalence and factors affecting the satisfaction of caregivers at the national referral centre. A cross-sectional research using the standard self-administered SERVQUAL questionnaire was conducted amongst caregivers accompanying their child to the clinic. It consists of 16 paired statements to evaluate their expectations and experiences with the clinic services. 459 caregivers were involved with a majority being satisfied with the clinic services. The most significant dimensions were tangibles, assurance and outcome. Caregivers from the Indian community, lower household income, and lower educational background demonstrated a higher level of satisfaction. This article suggests that although most caregivers are very satisfied with the services, greater emphasis must be placed on delivering reliable service in response to MoH's mission to provide quality and integrated people-centred health services in Malaysia.

Background: Musculoskeletal disorders (MSDs) have long been recognized as the most common risks that operation of agricultural machineries poses, thus, undermining the ability to labor and quality of life. The purpose of this investigation was to thoroughly review the recent scholarly literature on ergonomics in agricultural mechanized operations; Methods: Electronic database research over the last ten years was conducted based on specific inclusion criteria. Furthermore, an assessment of the methodological quality and strength of evidence of potential risk factors causing MSDs was performed; Results: The results demonstrated that ergonomics in agriculture is an interdisciplinary topic and concerns both developed and developing countries. The machines with driving seats seem to be associated with painful disorders of the low back, while handheld machines with disorders of the upper extremities. The main roots of these disorders are the whole-body vibration (WBV) and hand-arm transmitted vibration (HATV). However, personal characteristics, awkward postures, mechanical shocks and seat discomfort were also recognized to cause MSDs; Conclusions: The present ergonomic interventions aim mainly at damping of vibrations and improving the comfort of operator. Nevertheless, more collaborative efforts among physicians, ergonomists, engineers and manufacturers are required in terms of both creating new ergonomic technologies and increasing the awareness of workers for the involved risk factors.

AbstractThe implementation of residential photovoltaics must increase more rapidly to combat climate change and its impacts. This challenge is addressed in this study by introducing a segmentation model in order to develop a theoretical and empirical foundation for understanding the heterogeneity of potential adopters. Data were collected by means of a survey among Dutch adopters (n = 1395) and the data is analysed with statistical descriptive analyses and nonparametric tests. The five segmentation groups are divided by the homeowners’ educational background or profession (technical, financial-economic or other) and level of environmental concern. The results demonstrate that the groups are substantial in size and that there are significant differences between these groups on personal characteristics such as homeowners’ level of environmental concern and the level of influence of their social network on their decision to adopt. In addition, significant differences are found between the groups on the perceived characteristics of the residential photovoltaics such as perceived complexity and aesthetics, and the amount of previous practice with other energy measures in their home. Accordingly, these insights can be used by policymakers and the public and private sectors to promote residential photovoltaics more effectively by targeting the segmentation groups more adequately. The different groups will be drawn to different aspects and therefore, a broader pallet of benefits must be presented; a mix of different communication channels must be used; objective and non-technical assistance in the decision-making must be offered; and different kind of products must be provided.

This study employed a subject-based framework to explore the factors that affect auditor independence in financial statement auditing in Vietnam. The data collected using one-on-one interview and group discussion techniques were qualitatively analyzed through open coding, axial coding, and selective coding. Thereafter, 100 experienced practicing auditors were interviewed to confirm the results of the research. The research findings showed that the main factors affecting auditor independence are personal characteristics of the auditor, the audit firm's environment, and legal enviroment in audit. At the same time, the research uncovered several new factors affecting auditors’ independence in Vietnam, namely, ethical awareness of auditors, learning and training, industry-specific skills, corporate governance, economic development of the country and international integration. These findings can serve to guide the management of the process of auditing financial statements while adhering to the ethical framework of auditing in Vietnam.

To identify the factors predicting intrauterine balloon tamponade (IUBT) failure for severe postpartum hemorrhage (PPH) after delivery, we conducted a retrospective cohort study of women who underwent IUBT for severe PPH after delivery from October 1, 2016 until September 30, 2017. The failure of IUBT was defined as the need of additional surgical procedures or uterine embolization. A total of 99,650 deliveries occurred during the study period. Among the patients, 106 cases of severe PPH were managed with IUBT, and the global success rate was 70.8% (75/106). Least absolute shrinkage and selection operator (LASSO) regression was performed to select the potential risk factors predicting IUBT failure. The associated risk factors—obesity, multiple gestation, cesarean delivery, estimated blood loss (EBL), and placenta accreta spectrum (PAS)—were included in multivariate logistic models. Ultimately, these models identified multiple gestation, EBL, and PAS as independent risk factors for IUBT failure. In conclusion, IUBT is an effective method for severe PPH. The presence of factors affecting IUBT failure should be recognized early, and other modalities of management should be anticipated.
