This chapter focuses on health professional competence and consequent evaluation by patients who establish with them a therapeutic relationship. Based on a literature review and proposals of best practices to implement in health encounters, it is concluded that competent communication has to give attention to verbal and non-verbal dimensions and to the patient's expectations. Results reveal that the competences are empathy, respect, inclusion of the patient in choices and decisions, confirmation of understanding, use of plain language, and positivity. On the patients' side, some common expectations are associated with the humanization of relationships, such as the manifestation of patience by professionals to the patients' queries and the adoption of personalized communication. The communication lapses are more related to poor attitudes of health professionals, and the patients' expectations can vary depending on their sociodemographic attributes. Different communication styles of health professionals generate different evaluations and expectations of patients.

This chapter focuses on the health professionals' competences and inherent evaluation by patients during the therapeutic relationship. It is grounded on a literature review and proposals on best practices to adopt in health encounters. It can be concluded that the use of communication competences by health professionals influences the patients' evaluation and their health outcomes, demonstrating that “it takes two to tango.” Results have shown that, among the communication competences, are empathy, respect, inclusion of patient in health decisions, confirmation of understanding, use of a plain language, positivity. On the patients' side, the expectations tend to be related to the communication styles of health professionals, the humanization of relationship, and the use of a personalized communication; the communication lapses are more associated with poor attitudes of health professionals, and the patients' sociodemographic attributes influence their expectations.

Health influences general well-being, and well-being affects future health. Oral health professionals report a decreased well-being and a higher burnout. This chapter measures and evaluates the perception of the health professional and the patient about factors of stress and well-being. It evaluates the strategies used to overcome the anxiety and stress that involve the meeting. Two surveys applied by questionnaire, with face-to-face and online dissemination, the first to patients and second to dentists, were done. Of the 245 patients, 46% consider themselves to be anxious. The instruments used in a clinical environment cause discomfort, and their noise is the predominant cause for this fear. Of the 306 dentists, 80% show the ability to face difficult situations. Finally, 90% have an awareness that contributes to the well-being of others. Oral health professionals should prepare themselves with techniques to develop a therapeutic relationship that is more positive, calm, and less stressful.

INTRODUCTION: The idea of the article arose during a course in which students were organized in groups to discuss a bioethics subject and the subject chosen was "saviour siblings". “Saviour sibling” is a child conceived in the hope of being genetically compatible with an older brother who has some pathology whose treatment consists of the donation of tissues. OBJECTIVE: To debate the conception of saviour siblings and the delicacy and complexity of the theme from a bioethical perspective. METHODOLOGY: Literature review carried out in August 2019, in the PubMed database, using the keyword “saviour siblings”. The material obtained was used for discussion and elaboration of a clinical case. RESULTS: 40 articles were found, nine of which were excluded from the abstract for diverging from the topic. The clinical case was developed collectively and the authors delegated with putting themselves in the place of those involved in this decision and narrating the possible thoughts and feelings of each one of them. Then, bibliographic references were used to discuss the constructed clinical case, from the bioethical perspective. CONCLUSION: Saviour siblings related bioethical issues are delicate and complex. On the one hand, there's a benefit of saving a sick child and bringing relief to the parents. But, on the other hand, this technique can bear on eugenic practices such as the selection of individuals based on their genetics. The violation of the saviour sibling's autonomy needs to be the focus of future ethical discussions in order to protect his rights. Teaching bioethics is essential for the training of health professionals.

Promoting the best interests of children and protecting their safety and well-being in the context of a divorce or parentage case where domestic violence has been alleged has become highly politicized and highly gendered. There are claims by fathers’ rights groups that mothers often falsely accuse fathers of domestic violence to alienate the fathers from their children and to improve their financial position. They also claim that children do better when fathers are equally involved in their children’s lives, but that judges favor mothers over fathers in custody cases. As a consequence, fathers’ rights groups have engaged in a nationwide effort to reform the custody laws to create a presumption of equal parenting time, with no exception when one of the parents has engaged in domestic violence. Domestic violence survivors and their advocates, however, claim that the needs of survivors of domestic violence and their children to be safe and free from further abuse are not being met in custody cases, that their claims of abuse are not being believed, and that the harm when a parent commits domestic violence against the other parent is not being recognized and addressed by judges and the family law professional upon whom they rely. This Article first presents a literature review, with articulated scientific standards applied to each of the pieces of research cited in this review, on what is happening outside of court and in court relating to domestic violence and best practices for taking domestic violence into account in these child custody cases. Among the key findings from this literature review are: (1) when a parent commits domestic violence against the other parent, this can cause serious long-term harm to children, (2) custody judgments tend to favor fathers over mothers because greater weight is placed on claims of alienation than on domestic violence claims, (3) long-term harms can be mitigated by evidence-based best practices, most notably, supporting non-abusive parents in their efforts to protect themselves and their children from further domestic violence, (4) family law judges and professionals must be trained on domestic violence and its nuances, as well as how to screen for domestic violence, to adequately support them, and (5) a component of this training is learning how to distinguish mutual “situational couple violence” for which “parallel parenting” custody arrangements might be feasible, from a pattern of “coercive abuse,” where sole decision-making and primary parenting time should be ordered to the non-abusive parent, and protective restrictions on parenting time should be ordered to the abusive parent. The Article then reports on a fifty-state review of custody-related laws (laws determining which parent makes major decisions relating to the child, who is allocated primary parenting time, and whether protective restrictions shall be placed on the parenting time of a parent who has engaged in domestic violence). This review found serious gaps between what evidence-based best practices suggest, and what is currently required by law in many states. These gaps in the law, including the failure of the law to require domestic violence screening and training for judges and other family law professionals, contribute to poor custody decision-making by them that compromises the safety and welfare of domestic violence survivors and their children. The Article then proposes nuanced law reforms that would align custody-related laws with evidence-based best practices for taking domestic violence into account in custody cases, including creating rebuttable presumptions, burdens of proof, and definitions of domestic violence that conform with these evidence-based best practices.


Since the dawn of mankind, science and faith have always been given as antonyms and have fought for popular recognition. However, these days the association between these areas is discussed for a better result, since believing in one of these aspects does not mean disregarding the importance of the other, even in terminal patients. The objective was to analyze the experience of spirituality/religion in supporting cancer patients and to discuss the lack of preparation and use of these tools by health professionals. The literature review was based on the BVS Brazil (Virtual Health Library), Google Scholar and PubMed databases from 2017 to 2020. The keywords used were “spirituality”, “religion”, “neoplasms” and "health". As an inclusion criterion, free full texts in English were used. The scientific search initially showed 86 articles where after the evaluation of their titles and respective abstracts, duplicates and those that were incompatible with the scope of the study were excluded, totaling 31 articles. After attaching an article separately, 31 articles were fully read for the construction of this work. Concluding that spirituality and religion are important in the lives of terminally ill patients who seek tools to cope with the disease and to continue treatment, showing the importance of these in the way of dealing with the patient, thus improving the prognosis.

Purpose – Quality of care and health professional burnout are important issues in their own right, however, relatively few studies have examined both. The purpose of this paper is to explore quality of care and health professional burnout in hospital settings. Design/methodology/approach – The paper is a narrative literature review of quality of care and health professional burnout in hospital settings published in peer-reviewed journals between January 2000 and March 2013. Papers were identified via a search of PsychInfo, PubMed, Embase and CINNAHL electronic databases. In total, 30 papers which measured and/or discussed both quality of care and health professional burnout were identified. Findings – The paper provides insight into the key health workforce-planning issues, specifically staffing levels and workloads, which impact upon health professional burnout and quality of care. The evidence from the review literature suggests that health professionals face heavier and increasingly complex workloads, even when staffing levels and/or patient-staff ratios remain unchanged. Originality/value – The narrative literature review suggests that weak retention rates, high turnover, heavy workloads, low staffing levels and/or staffing shortages conspire to create a difficult working environment for health professionals, one in which they may struggle to provide high-quality care and which may also contribute to health professional burnout. The review demonstrates that health workforce planning concerns, such as these, impact on health professional burnout and on the ability of health professionals to deliver quality care. The review also demonstrates that most of the published papers published between 2000 and 2013 addressing health professional burnout and quality of care were nursing focused.

Tratou-se de uma revisão integrativa de literatura de artigospublicados nos últimos dez anos na base de dados Literatura Latino-Americana do Caribe em Ciências da Saúde(LILACS) e nas bibliotecas virtuais Biblioteca Virtual em Saúde (BVS) e Scientific Eletronic Library Online (SciELO).Foram combinados os descritores em português "Cateteres" AND "Infecções relacionadas a cateter" AND "InfecçãoHospitalar AND Pediatria”. Foram selecionados 11 artigos após aplicação dos critérios de inclusão e exclusão. Asevidências apontam para a prevalência de IPCS, contudo, estas podem ser prevenidas quando ocorre a adoção deboas práticas pelos serviços de saúde. Concluí-se que a pesar das medidas de prevenção e controle da IPCS estarembem estabelecidas, as evidências apontam níveis de desempenho insatisfatórios pelos profissionais de saúde naimplementação das mesmas.Palavras-chave: Enfermagem Pediátrica; Infecção Hospitalar; Cateteres. Resumo: Nos serviços de saúde mental brasileiros, nomeadamente nos Centros de Atenção Psicossocial (CAPSs), diversas ferramentas de intervenção têm sido utilizadas. As oficinas terapêuticas se destacam como uma das principais. O presente estudo teve como objetivo, por meio do empreendimento de uma revisão da literatura, estabelecer um panorama da produção científica veiculada no formato de artigo sobre as oficinas terapêuticas no campo da saúde mental no país. Para tanto, foram consultadas as seguintes bases de dados: Periódicos Eletrônicos de Psicologia (PEPsic), Base de Dados de Enfermagem (BDEnf) e Scientific Electronic Library Online-Brasil (SciELO-Brasil). As referências localizadas foram triadas a fim de viabilizar o descarte de eventuais duplicidades e então selecionadas em função do tema. O corpus de análise foi constituído por 11 referências. Observou-se que os objetivos de tais referências foram relativamente diversificados, mas a maioria delas não apresentava informações sobre o enquadre das oficinas terapêuticas que abordaram. De qualquer forma, as referências apontaram que tais ferramentas de intervenção, em especial a partir da expressão artística e do trabalho, fomentavam a interação entre os usuários, bem como o desenvolvimento de diferentes habilidades e, como consequência, forneciam subsídios para o cuidado psicossocial. Em contrapartida, em algumas referências foram veiculados indícios de que as oficinas terapêuticas pesquisadas, ao menos em parte, se afiguravam como meras “tarefas” a serem cumpridas mecanicamente pelos usuários, pois foram concebidas ou acabaram funcionando como uma espécie de passatempo. O presente estudo, assim, pode ser considerado potencialmente útil para pesquisadores e profissionais de saúde interessados no assunto.Palavras-chave: Saúde mental. Oficinas terapêuticas. Reforma Psiquiátrica.Abstract: In Brazilian mental health services, namely in the Psychosocial Care Centers (CAPSs), several intervention tools have been used. The therapeutic workshops stand out as one of the main ones. The present study aimed to establish, through a literature review, an overview of the scientific production carried out in the format of article about the therapeutic workshops in the mental health field in Brazil. The following databases were consulted: Periódicos Eletrônicos de Psicologia (PEPsic), Base de Dados de Enfermagem (BDEnf) and Scientific Electronic Library Online-Brasil (SciELO-Brasil). The localized references were sorted to enable the discarding of possible duplications and then selected according to the theme. The corpus of analysis consisted of 11 references. It was found that the objectives of such references were relatively diverse, but most of them did not provide information on the frame of the therapeutic workshops they addressed. In any case, the references pointed out that such intervention tools, specially from the artistic expression and the work, fomented the interaction between the users, as well as the development of different abilities and, consequently, they provided subsidies for the psychosocial care. On the other hand, in some references, there were indications that the therapeutic workshops researched, at least in part, appeared as mere tasks to be performed mechanically by the users, because were conceived or ended up functioning as a kind of hobby. The present study, therefore, can be considered potentially useful for researchers and health professionals interested in the subject.Keywords: Mental health. Therapeutic workshops. Abstract Background The nature of a new health professional’s transition from student to health professional is a significant determinant of the ease or difficulty of the journey to professional competence. The integrative review will explore the extent of literature on the factors that impact the transition of new health professionals into practice, identify possible gaps and synthesise findings which will inform further research. The aim was to identify research conducted in the last two decades on the barriers, facilitators and coping strategies employed by new health professionals during their transition into practice. Methods Whittemore and Knafl’s methodological framework for conducting integrative reviews was used to guide this review. Sources between 1999 and 2019 were gathered using EBSCOhost (including CINAHL, Medline, Academic Search Premier, Health Science: Nursing and Academic Edition), PubMed, Scopus, Cochrane and Web of Science, as well as hand searching and follow-up of bibliographies followed. The Covidence platform was used to manage the project. All studies were screened against a predetermined selection criteria. Relevant data was extracted from included sources and analysed using thematic analysis approach. Results Of the 562 studies identified, relevant data was extracted from 24 studies that met the inclusion criteria, and analysed to form this review. Thematic analysis approach was used to categorise the findings into theme areas. Four overarching themes emerged namely: systems and structures, personal capacities, professional competence and mediating processes. Each theme revealed the barriers, facilitators and coping strategies of transition into practice among new health graduates. Conclusion The transition into practice for new health practitioners has been described as complex and a period of great stress. Increasing clinical and practical experiences during education are required to support new health professionals in the process of closing the gap between learning and practice. Continued professional development activities should be readily available and attendance of these encouraged.
