SPIRITUALITY AND ITS REFLECTIONS IN ONCOLOGICAL PATIENTS

Since the dawn of mankind, science and faith have always been given as antonyms and have fought for popular recognition. However, these days the association between these areas is discussed for a better result, since believing in one of these aspects does not mean disregarding the importance of the other, even in terminal patients. The objective was to analyze the experience of spirituality/religion in supporting cancer patients and to discuss the lack of preparation and use of these tools by health professionals. The literature review was based on the BVS Brazil (Virtual Health Library), Google Scholar and PubMed databases from 2017 to 2020. The keywords used were “spirituality”, “religion”, “neoplasms” and "health". As an inclusion criterion, free full texts in English were used. The scientific search initially showed 86 articles where after the evaluation of their titles and respective abstracts, duplicates and those that were incompatible with the scope of the study were excluded, totaling 31 articles. After attaching an article separately, 31 articles were fully read for the construction of this work. Concluding that spirituality and religion are important in the lives of terminally ill patients who seek tools to cope with the disease and to continue treatment, showing the importance of these in the way of dealing with the patient, thus improving the prognosis.

19587 Background: The importance of spirituality and religion in coping with a terminal illness is becoming increasingly recognised. We aimed to assess the relation between spiritual well-being, religiosity, depression, and end-of-life despair in terminally-ill cancer patients. Methods: One hundred forty three terminally ill cancer patients with a life expectancy of less than 6 months were interviewed with a series of standardised instruments, including the FACIT Spiritual Well-Being Scale, a religiosity index similar to those used in previous research, the Hamilton depression rating scale, the Memorial Symptom Assessment Scale and the Duke-UNC Functional Social Support Questionnaire. Results: A strong negative association was observed between the FACIT Spiritual Well-Being scale and the HDRS, but no such relationship was found for religiosity. Similar patterns were observed for the FACIT subscales, finding a strong negative association between the meaning and peace subscale (which corresponds to the more existential aspects of spirituality) and HDRS scores, whereas a positive, albeit nonsignificant, association was observed for the faith subscale (which corresponds more closely to religiosity). Conclusions: These results suggest that the beneficial aspects of religion may be primarily those that relate to spiritual well-being rather than to religious practices per se. Spiritual well-being offers some protection against end-of-life despair in those for whom death is imminent. The area of spiritual work is fertile ground for further investigation, especially interventions aimed at improving spiritual health and general quality of life among terminally ill patients with cancer. No significant financial relationships to disclose.

INTRODUCTION: The idea of the article arose during a course in which students were organized in groups to discuss a bioethics subject and the subject chosen was "saviour siblings". “Saviour sibling” is a child conceived in the hope of being genetically compatible with an older brother who has some pathology whose treatment consists of the donation of tissues. OBJECTIVE: To debate the conception of saviour siblings and the delicacy and complexity of the theme from a bioethical perspective. METHODOLOGY: Literature review carried out in August 2019, in the PubMed database, using the keyword “saviour siblings”. The material obtained was used for discussion and elaboration of a clinical case. RESULTS: 40 articles were found, nine of which were excluded from the abstract for diverging from the topic. The clinical case was developed collectively and the authors delegated with putting themselves in the place of those involved in this decision and narrating the possible thoughts and feelings of each one of them. Then, bibliographic references were used to discuss the constructed clinical case, from the bioethical perspective. CONCLUSION: Saviour siblings related bioethical issues are delicate and complex. On the one hand, there's a benefit of saving a sick child and bringing relief to the parents. But, on the other hand, this technique can bear on eugenic practices such as the selection of individuals based on their genetics. The violation of the saviour sibling's autonomy needs to be the focus of future ethical discussions in order to protect his rights. Teaching bioethics is essential for the training of health professionals.

This chapter focuses on health professional competence and consequent evaluation by patients who establish with them a therapeutic relationship. Based on a literature review and proposals of best practices to implement in health encounters, it is concluded that competent communication has to give attention to verbal and non-verbal dimensions and to the patient's expectations. Results reveal that the competences are empathy, respect, inclusion of the patient in choices and decisions, confirmation of understanding, use of plain language, and positivity. On the patients' side, some common expectations are associated with the humanization of relationships, such as the manifestation of patience by professionals to the patients' queries and the adoption of personalized communication. The communication lapses are more related to poor attitudes of health professionals, and the patients' expectations can vary depending on their sociodemographic attributes. Different communication styles of health professionals generate different evaluations and expectations of patients.

Abstract: In Brazilian mental health services, namely in the Psychosocial Care Centers (CAPSs), several intervention tools have been used. The therapeutic workshops stand out as one of the main ones. The present study aimed to establish, through a literature review, an overview of the scientific production carried out in the format of article about the therapeutic workshops in the mental health field in Brazil. The following databases were consulted: Periódicos Eletrônicos de Psicologia (PEPsic), Base de Dados de Enfermagem (BDEnf) and Scientific Electronic Library Online-Brasil (SciELO-Brasil). The localized references were sorted to enable the discarding of possible duplications and then selected according to the theme. The corpus of analysis consisted of 11 references. It was found that the objectives of such references were relatively diverse, but most of them did not provide information on the frame of the therapeutic workshops they addressed. In any case, the references pointed out that such intervention tools, specially from the artistic expression and the work, fomented the interaction between the users, as well as the development of different abilities and, consequently, they provided subsidies for the psychosocial care. On the other hand, in some references, there were indications that the therapeutic workshops researched, at least in part, appeared as mere tasks to be performed mechanically by the users, because were conceived or ended up functioning as a kind of hobby. The present study, therefore, can be considered potentially useful for researchers and health professionals interested in the subject. Keywords: Mental health. Therapeutic workshops. e24128 Background: In recent years, an ever greater importance is given to the needs of cancer patients which could impact medical treatments, adherence and compliance, and patients’ quality of life (Teo et al. 2019; NCCN, 2015). However, recognizing and addressing the needs of patients may not be enough. Indeed, on one hand, needs that clinicians might consider as 'important' could play a marginal role, and – on the other hand – needs that clinicians may perceive as 'unimportant' may be central for patients. An innovative approach – psychometric network analysis (PNA; Epskamp, 2017) – was used to assess the network among needs ( nodes). Notably, the more a central node (need) is modified (addressed), the more a cascade change will occur in all the other nodes (needs). This study aimed to evaluate: (A) the structure of relationships among needs ( edges) of cancer patients, (B) which needs ( nodes) are the most relevant ( central), and (C) play a key role in the network. Methods: Patients ( n = 511; mean age = 65.95, SD = 12.72; 280 males) were enrolled at the Oncology Day Hospital at the “Presidio Ospedaliero” of Saronno, ASST Valle Olona, Italy. Patients were tested with the Need Evaluation Questionnaire (NEQ) which is composed of 23 items that investigate as many needs – divided into 4 areas: (1) information about diagnosis/prognosis, (2) information about exams and treatment, (3) communicative, and (4) relational needs. Results: Preliminary analysis revealed that all of the items were informative (SDitem < 2.5SDall_itemsSD) and there was no redundancy between items (redundancy index < 0.25). An Ising model (5,000 nonparametric bootstraps) with LASSO regularized nodewise logistic regression was performed. PNA showed a high accuracy: CS-coefficient = 0.56. On one hand, PNA showed that item#2 ( “I need more information about my future condition”; z = 2.039), item#17 ( “I need to speak with a psychologist”; z = 1.209), and item#13 ( “I need to be reassured more by the doctors”; z = 1.201) were the strongest (central) nodes. On the other hand, item#14 ( “ I need the hospital to provide better services”; z = -2.261) was the weakest node in the network. Conclusions: These findings show useful implications for clinical practice. Clinical interventions should address the needs showing the strongest connections in the network. These central nodes can influence all the other connected needs, thus representing important needs to be targeted by clinicians – allowing to tailoring more targeted and efficient therapeutic approaches to meet patients’ needs, with beneficial effects for medical treatments and quality of life.
