Background:Telemedicine counseling (TMC) has gained rapid development during the COVID-19 pandemic. The prospect of using this technology in rheumatology was based on the possibility of getting maximum information about the patient during the survey, examination and interpretation of laboratory and instrumental data, that is excepting direct contact with the patient. Several rheumatological clinics have reported on the success of using TMC. However, there is very little data of the difficulties that can be encountered when organizing this process. Objectives:To characterize the identified problems during TMC in rheumatology, to suggest potential directions for their elimination. Methods:Since June 2021, on the basis of the Medical Association “New Hospital”, Yekaterinburg, Russian Federation, 76 TMCs have been performed on the profile of rheumatology in patients aged 29 to 71 years. Of these, 13 applied to the primary TMC, the other patients were preliminarily examined in person. The consultation included the preliminary acquaintance with the examination results, a 20-minute video communication and writing of a conclusion. After each TMC, a survey was conducted between the doctor and the patient, including the identified deficiencies in counseling. The frequency of identified problems is presented as an absolute indicator and as a percentage of the total number of TMCs performed. Results:We noted a high degree of patient satisfaction: 74 (97.4%) responded that they received answers to all. However, according to the doctor, the following groups of problems were identified.[1]Technical problems in 29 (38.2%): most often there were various problems with the Internet, but there were also registered: the end of the charge on the patient’s tablet, the patient was not registered in the electronic queue. Elimination of these violations depends on the work of IT-specialists, but each consulting physician should be prepared for an immediate transition to an alternative form of communication (for example - telephone).[2]Lack of objective examination, leading to the impossibility of correct remote diagnosis - 8 (10.5%). This problem was identified due to the inability to establish the presence or absence of arthritis during the initial diagnosis (6 cases) and to clarify the nature of the rash (2 cases). All patients are invited for a face-to-face consultation.[3]The need to write prescriptions for psychotropic drugs - 12 (15.8%), which under the conditions of national legislation cannot be done in the TMC regime.[4]The time spent directly on remote communication with the patient was 17.2 minutes (from 8 to 31), however, taking into account the study data and writing the conclusion, the total time was 40.7 minutes (from 21 to 73). Thus, it turned out that the average time for remote and face-to-face consultations is the same, while TMC’s payment is only about 50% of the face-to-face consultation. This situation reduces the doctor’s interest in carrying out TMC. The solution to the problem is associated with reducing the time for the documentation process through technical improvements. In addition, of the 9 patients in whom the TMC process lasted 60 minutes or more, 5 were diagnosed with fibromyalgia. It is possible that with a previously established diagnosis of fibromyalgia, only face-to-face counseling should be recommended to patients. Conclusion:The TMC system is promising, however, there are a number of problems that need to be improved, since they can reduce the doctor’s interest in using this technology.Disclosure of Interests:None declared

Abstract Most of the oil fields in Sarawak has already producing more than 30 years. When the fields are this old, the team is most certainly facing a lot of problems with aging equipment and facilities. Furthermore, the initial stage of platform installation was not designed to accommodate a large space for an artificial lift system. Most of these fields were designed with gas lift compressors, but because of the space limitation, the platforms can only accommodate a limited gas lift compressor capacity due to space constraints. Furthermore, in recent years, some of the fields just started with their secondary recovery i.e. water, gas injection where the fluid gradient became heavier due to GOR drop or water cut increases. With these limitations and issues, the team needs to be creative in order to prolong the fields’ life with various artificial lift. In order to push the limits, the team begins to improve gas lift distribution among gas lifted wells in the field. This is the cheapest option. Network model recommends the best distribution for each gas lifted wells. Gas lifted wells performance highly dependent on fluid weight, compressor pressure, and reservoir pressure. The change of these parameters will impact the production of these wells. Rigorous and prudent data acquisitions are important to predict performance. Some fields are equipped with pressure downhole gauges, wellhead pressure transmitters, and compressor pressure transmitters. The data collected is continuous and good enough to be used for analysis. Instead of depending on compressor capacity, a high-pressure gas well is a good option for gas lift supply. The issues are to find gas well with enough pressure and sustainability. Usually, this was done by sacrificing several barrels of oil to extract the gas. Electrical Submersible Pump (ESP) is a more expensive option compared to a gas lift method. The reason is most of these fields are not designed to accommodate ESP electricity and space requirements. Some equipment needs to be improved before ESP installation. Because of this, the team were considering new technology such as Thru Tubing Electrical Submersible Pump (TTESP) for a cheaper option. With the study and implementation as per above, the fields able to prolong its production until the end of Production Sharing Contract (PSC). This proactive approach has maintained the fields’ production with The paper seeks to present on the challenges, root cause analysis and the lessons learned from the subsequent improvement activities. The lessons learned will be applicable to oil fields with similar situations to further improve the fields’ production.

Purpose – This paper aims to present the evolution of Chinese business ethics and CSR policy. China currently has business ethics and corporate social responsibility (CSR) concerns primarily because its economy is in evolution. As the Chinese economy is in transition from the late 1970s from a command–Communist economy to a more hybrid system Beijing model (a system based on a capitalist market controlled through bureaucracy), much has to be understood if one wants to participate in its vast economic potential. Design/methodology/approach – The study is based on the building of a model that depicts the evolution of business ethics in China as a base for further research. Findings – With the rapid development of the Chinese economy, opportunities are opening up to Chinese entrepreneurs and connected officials. This has led to money worship and, in some cases, thorough unethical behavior. The uncertain and ever-changing rules of the game generate opportunity for business ethics and CSR problems. This wealth creates temptation and incentives to cut corners to get rich fast. Much research has tried to explain business ethics in China through the understanding of Guanxi (Chinese social network of reciprocal business relations common in Confucian cultures), but in my view, the evolutionary aspect of it is missing to explain the present and future situation. Whereas considerable research has dealt with the growth of Chinese industries in recent years, the key relationship between changes in its economy and shifts in Chinese business ethics has been neglected. This paper presents the evolution of Chinese business ethics and CSR policy. Originality/value – The paper illustrates the evolution of Chinese business ethics and how one has to adapt if one wants to succeed in its ever evolving and maturing market. A model is presented that can assist in future research in the area.

The rapid development of the Chinese economy over the past several decades has stimulated new interest in the institutions, practices, and social formations that supported the development of business in China before the intensification of pressure from Western traders to conform to “modern” practices in the late nineteenth and early twentieth centuries. This article aims to provide a foundation for understanding merchant practice as it developed during the important years of market expansion during the last Chinese dynasty and to dispel some of the enduring myths about the Chinese merchant, his relationship to family, community, and the state, and the ideological constraints on his activities. To that end I examine several aspects of late imperial merchant culture, beginning with the everyday practices that allowed business to flourish in the Qing, turning next to the large social formations through which long-distance merchants in particular identified and pursued their interests, and ending with some preliminary thoughts on the impact of the laissez-faire policies of the last dynasty and their implications for post-Imperial China.
