Maternal vitamin D deficiency is not uncommon. The lack of vitamin D during pregnancy may result in poor fetal growth and altered neonatal development that may persist into later life. Recognition of risk factors and early detection of vitamin D deficiency during pregnancy is important in order to prevent neonatal vitamin D deficiency and related complications. The aim of the current study is to assess the effect of maternal vitamin D status on the neonatal vitamin D stores. A total of 92 pregnant women at the end of the 3rd trimester and their newborns were recruited from Al Khafji Joint Operation Hospital, Saudi Arabia, during the year 2011. Maternal and cord blood samples were taken for determination of serum levels of circulating 25-hydroxyvitamin D3 [25(OH)D3] concentration, serum calcium (Ca++), phosphorus (PO4) and alkaline phosphatase (ALP). Compared with pregnant women with adequate vitamin D levels, women deficient in vitamin D had infants with vitamin D deficiency (X±SD 33.44±18.33 nmol/L <em>vs</em> 55.39±17.37 nmol/L, P=0.01). Maternal and neonatal serum 25(OH)D3 levels showed a positive correlation with serum Ca++ and negative correlation with serum PO4 and ALP. Neonatal 25(OH)D was related to maternal 3rd trimester levels (r=0.89, P=0.01). The newborn serum 25(OH)D3 concentrations rely on maternal vitamin D status. Poor maternal vitamin D status may adversely affect neonatal vitamin D status and, consequently, calcium homeostasis.

Controversy remains surrounding vitamin D routine supplementation in healthy pregnancy, and the doses are unclear. The aim of this study was to describe maternal vitamin D status throughout pregnancy in a group of Mexican women and evaluate the effect of frequently prescribed doses of vitamin D3 on longitudinal 25-OH-D concentrations, adjusting for obesity, season, and other factors. We conducted a cohort study (Instituto Nacional de Perinatología-INPer) (2017–2020)) of healthy pregnant women without complications. Pregestational overweight/obesity (body mass index ≥ 25), vitamin D3 supplementation (prescribed by physician; 0–250, 250–400, and >400 IU/day), and serum 25-OH-D concentrations (ELISA) were evaluated in each trimester of pregnancy. Vitamin D deficiency or insufficiency was computed (<20 and <30 ng/mL, respectively). We studied 141 adult women; 58.5% had pregestational obesity or overweight. In the first trimester, 45.8% of the women were supplemented with vitamin D3; 51.4% had vitamin D insufficiency and 37.3%, deficiency. In the third trimester, 75.4% of the women were supplemented, and 20% of them still had deficiency. The final general mixed linear model showed that 25-OH-D significantly increased throughout pregnancy (p < 0.001); the highest increase was observed in the third trimester in women with doses >400 IU/day of vitamin D3 (+4 ng/mL, 95% CI: 1.72–8.11 ng/mL). In winter/autumn, 25-OH-D concentrations were also lower (p ≤ 0.05). In this group of pregnant Mexican women, the prevalence of vitamin D deficiency and insufficiency was high. A higher increase in 25-OH-D concentrations during pregnancy was observed when the women were supplemented with >400 IU/day. Common supplementation doses of 250–400 IU/day were insufficient for achieving an adequate maternal vitamin D status.

Background: In the recent years there has been an increased understanding of the role that vitamin D plays in regulation of cell growth, calcium absorption and immunity and its impact on the developing fetus and maternal health is of significant concern. This study aims at evaluating the Vitamin D status in pregnant women and their newborns.Methods: A cross sectional study was done on 100 pregnant females according to inclusion and exclusion criteria. At the time of delivery, maternal blood was collected, and newborn samples were taken from newborn side of umbilical cord and sent for analysis.Results: The prevalence of Vitamin D deficiency has been found to be 85% of pregnant females and 91% of the newborns. Only 5% of pregnant females and 1% of the newborns showed Vitamin D sufficiency. Maternal and newborn vitamin D levels show a positive correlation. Mean maternal and newborn Vitamin D levels were found to be 16.78±7.04 ng/mL and 11.29±5.75 ng/ml.Conclusions: Vitamin D deficiency is highly prevalent among pregnant women in north India. Low maternal vitamin D levels lead to vitamin D deficiency in the newborns also.

Abstract Background: Vitamin D deficiency is a global public health issue in women and children, is associated with adverse impacts on child growth, such as rickets. However, prior studies have mainly focused on measuring vitamin D levels in singleton pregnant women and their offspring, very limited studies have revealed the prevalence of vitamin D deficiency in twin pregnant women and their offspring. The aim of this study was to investigate vitamin D levels in twin pregnant women and their neonates. We also explored the correlation of maternal vitamin D levels with neonatal outcomes and infant growth. Methods: A prospective subcohort investigation was carried out among 72 dichorionic twin pregnant mothers and their twin offspring from the Longitudinal Twin Study. Peripheral blood was collected from the mothers in the third trimester and cord blood was collected form neonates at birth to identify 25[OH]D levels. Data on the characteristics of the mothers and neonates were collected. Infant growth data and food sensitivities were also collected. Results: The average maternal 25[OH]D level was 31.78 ng/mL, with 19.4% being deficiency and 20.8% insufficiency, while the average neonatal 25[OH]D level was 15.37 ng/mL, with 99.3% being deficiency or insufficiency. A positive correlation was found between maternal and neonatal 25[OH]D levels (beta-value: 0.43, 95% CI: 0.37, 0.49). Interestingly, the higher the maternal 25[OH]D level was, the smaller the co-twins birthweight discordance (beta-value: -2.67, 95% CI: -5.11, -0.23). In addition, the infants of mothers with vitamin D deficiency were more likely to be allergic to foods at six months than those of mothers with vitamin D sufficiency. Conclusions: Twin neonates were at extremely high risk of vitamin D deficiency although their mothers’ vitamin D deficiency partially improved. Higher maternal vitamin D level was associated with smaller discordance of co-twins birthweight.Trial registration: ChiCTR-OOC-16008203

Background and Objective: Impaired fetal growth and stunting remain immense public health problems involving maternal nutrition during pregnancy, as linear growth failure in children is the most common form of undernutrition across the world. Although both are preventable through adequate prenatal care and nutrition, impaired fetal growth and stunting continue to be implicated in multiple child health morbidities, physical, and psychological functioning. Recent knowledge and requirements for normal fetal and neonatal development are lacking. This systematic review investigates the effects of maternal vitamin D status on fetal growth and stunting. Methods: We reviewed three widely-used publications databases: the National Institutes of Health’s PubMed, Clarivate Analytics’ Web of Science, and Google Scholar using pre-established inclusion and exclusion criteria and keyword search strategy. Studies from 2010 to 2020 were included if they reported vitamin D levels on pregnant women, indicated growth outcomes and used quantitative measurements. We excluded non-English language studies, studies with ambiguous outcomes, studies that did not specify vitamin D intake, and studies that involved other maternal health complications. The search was implemented using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Results: Out of a total of 2,481 studies reviewed, we identified 8 studies on vitamin D and fetal growth and stunting. Published literature addressing maternal vitamin D status on fetal growth and stunting remains ambiguous. Five studies demonstrated improvements in fetal and humerus z-scores, which are known proxies for fetal growth, in groups with higher vitamin D status. Three studies found no statistical significance between vitamin D levels and fetal growth. Vitamin D status and ethnicity were correlated; vitamin D interacts with calcium levels in pregnant mothers to improve bone mineralization and fetal growth. Conclusion and Implications for Translation: Further studies are needed to understand the relationship between maternal vitamin D, ethnicity, and fetal growth and the long-term effects of maternal vitamin D levels on neonatal, early childhood, and adolescent growth.   Copyright © 2021 Karmali et al. Background: The possibility of vitamin D deficiency is high among mothers and their premature newborns. Objectives: Due to the high rate of vitamin D deficiency in Iranian women, this study aimed to determine the association between the serum levels of vitamin D in the blood of premature newborns and their mothers in the Iranian population. Methods: This cross-sectional study was conducted on 324 mothers and their 324 newborns with gestational age less than 37 weeks, who were referred to Ghaem Hospital affiliated to Mashhad University of Medical Sciences, Mashhad, Iran, during 2016-2020. After clamping, 1.5 cc blood samples were obtained through the umbilical cord to determine the neonates’ vitamin D levels. Finally, the serum levels of vitamin D were measured, and the correlation between the levels of vitamin D of mothers and their newborns was calculated. Results: The mean vitamin D levels of newborns and mothers were 15.75 ± 11.18 and 20.16 ± 13.41 ng/mL, respectively. Moreover, there was a high correlation between the vitamin D levels of mothers and their newborns (r = 0.672; P < 0.001). Furthermore, a significant correlation was observed between the vitamin D levels of mothers, who used vitamin D supplementation during pregnancy and their neonates’ levels (r = 0.773; P < 0.001). Conclusions: The findings suggest a positive correlation between neonatal and maternal vitamin D levels. Moreover, it seems that maternal vitamin D levels can predict neonatal vitamin D deficiency.

Concerns exist about adequacy of vitamin D in pregnant women relative to both maternal and fetal adverse health outcomes. Further contributing to these concerns is the prevalence of inadequate and deficient vitamin D status in pregnant women, which ranges from 5 to 84% globally. Although maternal vitamin D metabolism changes during pregnancy, the mechanisms underlying these changes and the role of vitamin D during development are not well understood. Observational evidence links low maternal vitamin D status with an increased risk of non-bone health outcome in the mother (pre-eclampsia, gestational diabetes, obstructed labour and infectious disease), the fetus (gestational duration) and the older offspring (developmental programming of type 1 diabetes, inflammatory and atopic disorders and schizophrenia); but the totality of the evidence is contradictory (except for maternal infectious disease and offspring inflammatory and atopic disorders), lacking causality and, thus, inconclusive. In addition, recent evidence links not only low but also high maternal vitamin D status with increased risk of small-for-gestational age and schizophrenia in the offspring. Rigorous and well-designed randomised clinical trials need to determine whether vitamin D has a causal role in non-bone health outcomes in pregnancy.

Vitamin D insufficiency is a global health issue. Although classically associated with rickets, low vitamin D levels have also been linked to aberrant immune function and associated health problems such as inflammatory bowel disease (IBD). To test the hypothesis that impaired vitamin D status predisposes to IBD, 8-wk-old C57BL/6 mice were raised from weaning on vitamin D-deficient or vitamin D-sufficient diets and then treated with dextran sodium sulphate (DSS) to induce colitis. Vitamin D-deficient mice showed decreased serum levels of precursor 25-hydroxyvitamin D3 (2.5 ± 0.1 vs. 24.4 ± 1.8 ng/ml) and active 1,25-dihydroxyvitamin D3 (28.8 ± 3.1 vs. 45.6 ± 4.2 pg/ml), greater DSS-induced weight loss (9 vs. 5%), increased colitis (4.71 ± 0.85 vs. 1.57 ± 0.18), and splenomegaly relative to mice on vitamin D-sufficient chow. DNA array analysis of colon tissue (n = 4 mice) identified 27 genes consistently (P &lt; 0.05) up-regulated or down-regulated more than 2-fold in vitamin D-deficient vs. vitamin D-sufficient mice, in the absence of DSS-induced colitis. This included angiogenin-4, an antimicrobial protein involved in host containment of enteric bacteria. Immunohistochemistry confirmed that colonic angiogenin-4 protein was significantly decreased in vitamin D-deficient mice even in the absence of colitis. Moreover, the same animals showed elevated levels (50-fold) of bacteria in colonic tissue. These data show for the first time that simple vitamin D deficiency predisposes mice to colitis via dysregulated colonic antimicrobial activity and impaired homeostasis of enteric bacteria. This may be a pivotal mechanism linking vitamin D status with IBD in humans.
