Aim: This study aimed to review the clinical findings and surgical intervention of the hip fracture at the Hue University Hospital in Vietnam. Methods:The data of proximal femoral fractures was collected retrospectively. All patients, in a period of 5 years, from Jan 2008 to December 2012, suffered either from intertrochanteric or femoral neck fractures. The numbers of patients were gathered separately for each year, by age groups (under 40, 40-49, 50-59, 60-69, 70-79, older) and by sex. We analyzed what kind of treatment options were used for the hip fracture. Results:Of 224 patients (93 men and 131 women) studied, 71% patients are over 70 years old, 103 women and 56 men (p<0.05). For patients under 40 years, there were 1 woman and 11 men (p<0.05). There were 88 intertrochanteric and 136 femoral neck fractures. There was no significant difference in the two fractures between men and women. The numbers of hip fracture increased by each year, 29/224 cases in 2010, 63/224 cases in 2011, 76/224 cases in 2012. Treatment of 88 intertrochanteric fractures: 49 cases (55.7%) of dynamic hip screw (DHS), 14 cases of hemiarthroplasty (15.9%), 2 cases of total hip replacement (2.3%). Treatment of 136 femoral neck fractures: 48 cases of total replacement (35.3%), 43 cases of hemiarthroplasty (31.6%), 15 cases of screwing (11%). In cases of 40 patients (17.9%) hip fracture was managed conservatively, 23 were femoral neck fractures and 17 were intertrochanteric fractures. Conclusions: Hip fracture is growing challenge in Hue medical university hospital. The conservative approach is still high in people who could not be operable due to severe medical conditions as well as for patients with economic difficulties. Over 70% of the hip fractures in people 70+ are caused by osteoporosis. The number of hip fracture is increasing in the following years, most likely due to the increase in the prevalence of osteoporosis. Early detection and prevention of osteoporosis should be addressed, particularly in high risk population. More aggressive surgical approach should be implemented in order to improve the quality of life in patients with hip fractures. Key words:Hip fracture.

Abstract Objective To investigate the prevalence status of the isolated systolic hypertension (ISH) among the elderly Chinese population and analyze risk factors of ISH. Methods The survey was conducted from September 2015 to September 2016 enrolling 1269 people aging above 80 in the urban community in Chengdu, China. The participants were recruited by using a stratified cluster sampling method. The average blood pressure of an individual was obtained by using a standardized mercury sphygmomanometer to measure the blood pressure twice after a 10-minute seated rest. Results The prevalence of ISH was 53.0% among the elderly population; the ISH was accounted for 82.2% of all hypertension cases. The prevalence of ISH of males and females was 54.7% and 51.5%, respectively, without significant differences (P = 0.25). The prevalence of ISH in the 80–84 group, 85–89 group, and > 90 group were 52.5%, 53.0%, and 60.0%, respectively without significant differences (P for trend = 0.36). Multivariate logistic regression analysis showed that drinking alcoholic, obesity, and heart rate (HR) > 75 beats/min were all positively correlated to the incident of ISH; however, physical exercise was negatively correlated to the occurrence of ISH. Conclusion 53.0% of the elderly Chinese population aged above 80 has ISH. The gender and physical exercise are not the independent risk factor of ISH. But drinking alcoholic, obesity and HR > 75 are significantly related to the occurrence of ISH.

Abstract Background The relationship between preoperative hip measurements and dislocation after bipolar hemiarthroplasty is presently unclear. In the current study, we investigated the morphological risk factors associated with dislocation after bipolar hemiarthroplasty of the hip in patients with femoral neck fractures. Methods Between January 2011 and June 2017, a nested case-control design study was used to analyze the risk factors for dislocation in 348 patients who had undergone bipolar hemiarthroplasty because of femoral neck fractures. Twelve patients underwent at least one dislocation postoperatively. Sixty patients without dislocation were selected as controls matched in terms of time of surgery, age, and sex, at a ratio of 1:5. Patient acetabular measurements were compared between the dislocation group and the control group, including the center-edge angle, abduction angle, acetabular width and depth, depth-to-width ratio, femoral neck offset, leg length discrepancy, and femoral head coverage ratio. A multivariate logistic regression model was used to evaluate the morphological risk factors of dislocation. Results Postoperatively, the incidence of dislocation was 3.4%. A smaller center-edge angle was found to be a risk factor associated with dislocation after bipolar hemiarthroplasty of the hip. Patients with small acetabular depth and a small acetabular depth–width ratio were prone to dislocation. Patients with a center-edge angle of ≤ 45.4° or an acetabular depth of ≤ 19.12 mm were more likely to suffer dislocation. Conclusions Careful preoperative measurements before bipolar hemiarthroplasty of the hip are important. Surgical intervention for femoral neck fracture patients with a shallow acetabulum should be carefully planned and total hip arthroplasty should be considered when necessary.
