Urticaria is an allergic reaction of the skin to a variety of exogenous and endogenous antigens. It is a common disease nowadays, characterized by pale red rashes and severe itching caused by an allergic reaction. Vascular dilatation, the resultant dermal oedema and pruritis are due to the releases of histamine and other mediators from mast cells. Ayurvedic classics mentioned this similar condition as shitpitta, among twakroga which is vaatpradhan tridosha in nature. Symptoms mentioned by aacharyas are shotha, toda, kandu and daha. Even after the availability of newer medicine, there is a lack of promising results against this ailment. Moreover, the use of drugs like antihistamines and corticosteroids lead to various side effects. The study aims to evaluate the effect of ayurvedic treatment in the management of shitpitta with special reference to urticaria. Criteria’s like shotha, toda, kandu and daha were assessed before and after the treatment & significant result was found. The conclusion was drawn that given ayurvedic therapy was found to be significantly effective in the management of urticaria. Keywords: Shitpitta, Twakroga, Urticaria, Shodhan, Shamana

One of the basic goals of palliative care is to reduce the suffering of the patient, which includes relieving the pain. Some facts show that sometimes a doctor is afraid to give opioid analgesic mono-therapy to his patients because she is worried on one hand about the effect of addiction. In addition, in some cases, NSAID analgesics mono-therapy can also cause a series of side effects or even the patient may be contraindicated against the use of NSAID or paracetamol class drugs. Actually, this can be prevented by giving a combination of opioids and non-opioids analgesic. By providing this combination of therapy, it is hoped that this can accelerate the onset of work, extend the working period which can reduce the dose of drugs consumed, and more importantly minimize the side effects of each drug. The fixed dose combination of Tramadol-Paracetamol (Tramadol 37.5 mg–Paracetamol 325 mg) is an example of a drug combination preparation that has been shown to be effective in the treatment of pain. In this case report, a patient who is currently undergoing palliative therapy for cancer is described. On the way, the patient felt spinal pain as a result of the spread of the disease. The combination of Tramadol-paracetamol was administered twice a day with a dose that adjusted to the VAS Score observations before and after drug administration. And in this case, there was a decrease in the VAS score after being given Tramadol-Paracetamol combination.

This case report is about a 64-year-old woman who presented at the Emergency Walk-in Center with palpebral edema as well as various erythematous plaques in supraciliary and malar areas that have been gradually worsening a couple of days prior to presentation. While talking about history, the patient mentioned she was attending, for about four months, an Esthetic Private Clinic, where she was injected in various sessions with Metacrill®, as a facial lift, for beauty purpose. Due to suspecting an allergic reaction to the Metacrill and the failure of the initial treatments, she was referred to the dermatologist. After failed attempts to treat the patient with corticosteroids and antibiotics, the patient was sent for autoimmunity consultation at the hospital where she received an immunosuppressive treatment with Tacrolimus and was not presenting new symptoms ever since.
