Orthodontic Treatment Need in an Accredited Graduate Orthodontic Center in North America: A Pilot Study

Abstract Aim To assess the objective orthodontic treatment needs of patients treated in an accredited Orthodontic Clinic in North America using a retrospective cross sectional study and the Dental Aesthetic Index (DAI). Methods and Materials One hundred pre- and post-treatment study models were randomly selected from the model storage facility of the clinic. Only the pre-treatment study models were assessed using the DAI. The pre - treatment and post- treatment ages for each case were noted as recorded on the corresponding study models. The range for the pre-treatment age was 10 to 52 years, and from 12 years, 4 months to 54 years for the post-treatment age range. The duration of treatment for each case was calculated by subtracting the pre-treatment age from the post-treatment age (range of treatment duration, 9 to 125 months). Descriptive statistics as well as chi-square statistics were employed to analyze the data. Results Fifteen cases had normal or minor malocclusions with no treatment or slight need for treatment. Definite malocclusions with treatment highly desirable accounted for 22 cases and severe malocclusion with treatment highly desirable was found in 16 cases. Forty-seven cases had handicapping malocclusion with treatment considered mandatory. While a statistically significant association (p<0.05) was found between duration of treatment and severity levels of malocclusion (DAI scores), the association between pre-treatment age and DAI scores was not significant (p > 0.05). Conclusions Eighty-five percent of the study sample needed orthodontic treatment with different malocclusion severity levels, while 47% of the cases qualified for publicly subsidized treatment due to handicapping malocclusions. The association between duration of treatment and DAI score groups (malocclusion severity levels) was found statistically significant. It could be helpful for more clinic-based (demand populations) studies on treatment needs be undertaken across the globe using the DAI, at least for the purposes of comparison. Abstract Aim The aim of this retrospective analytical study was to assess the relationship between the Dental Aesthetic Index (DAI) and the Index of Complexity, Outcome, and Need (ICON) on the orthodontic treatment need and complexity in a group of Nigerian patients. Methods and Materials Fifty-six cases were randomly selected from the model storage of the orthodontic unit of the University College Hospital in Ibadan, Nigeria. The ICON was used to assess the pre-treatment study models for orthodontic treatment need and complexity of the cases while the DAI was also used to assess the same cases for treatment need. Descriptive statistics as well as the non-parametric (Spearman Rank-Order and Pearson's Product Moment) correlations were used to analyze the data. Results The mean ICON and DAI scores were 67.38±19.63 (SD) and 42.27±12 .66 (SD), respectively. Both the ICON and DAI agreed that one (1.8%) case did not need treatment while 46 (82.1%) needed treatment. All the difficult and very difficult cases according to the ICON were assessed as needing treatment by the DAI with 18 (13.1%) out of 22 (39.3%) in the very difficult category belonging to the handicapping group where treatment was mandatory. In all positive significant correlations were recorded for the ICON and DAI scores. Conclusion The ICON and DAI showed favorable agreement when used to assess treatment needs through the use of study casts. The ICON could be a good substitute for the DAI in assessing pre-treatment needs of Nigerian patients. Citation Onyeaso CO. Abstract: Orthodontic treatment need increases with the number of cases of malocclusion as one of the major problems in Indonesia and an oral health problems are sufficiently large as dental caries and periodontal disease. Orthodontic treatment need in a population is described by one measure, namely Index of Orthodontic Treatment Need (IOTN). The purpose of this study was to determine the needs of orthodontic treatment in the second-grade students of Junior High School 2 Bitung based IOTN consisting of Aesthetic Component and Dental Health Component. It was a cross sectional study of descriptive obsevational, conducted in Junior High School 2 Bitung at December 2013. It research was used the total population of the second grade students of Junior High School 2 Bitung, amounting to 460 students. The results showed, orthodontic care needs based on the AC is 304 people 77 % do not require nursing care or only mild, 74 people 19% of borderline and need care 18 4.5 really need treatment. While orthodontic treatment needs based DHC is 76% of 303 people do not require treatment or light treatment only, 63 people took care borderline 16%, 7.6% and 30 people really need treatment. Orthodontic treatment need by Index of Orthodontic Treatment Needs in second grade at Junior High School 2 Bitung mostly do not require or need minor maintenance in AC 77% and DHC 76%.Keywords: treatment need, IOTN, Student junior High School.  Abstrak: Kebutuhan perawatan ortodonsi meningkat seiring bertambahnya jumlah kasus maloklusi sebagai salah satu permasalahan utama di Indonesia dan merupakan masalah kesehatan gigi dan mulut yang cukup besar setelah karies gigi dan penyakit periodontal. Kebutuhan perawatan ortodonsi pada suatu populasi digambarkan dengan salah satu alat ukur yaitu Index of Orthodontic Treatment Need (IOTN). Tujuan penelitian ini ialah untuk mengetahui kebutuhan perawatan ortodonsi pada siswa-siswi kelas II SMP Negeri 2 Bitung berdasarkan IOTN yang terdiri dari Aesthetic Component (AC) dan Dental Health Component (DHC). Penelitian ini merupakan suatu penelitian Cross sectional yang bersifat deskriptif observasional, yang dilakukan di SMP Negeri 2 Bitung pada bulan Desember 2013. Pengambilan sampel pada penelitian ini menggunakan teknik total populasi pada siswa-siswi kelas II di SMP Negeri 2 Bitung yang berjumlah 460 orang. Hasil penelitian didapatkan, kebutuhan perawatan ortodonsi berdasarkan AC yaitu 304 orang 77 % tidak membutuhkan perawatan atau hanya perawatan ringan, 74 orang 18,7% butuh perawatan borderline dan 18 orang 4,5 sangat butuh perawatan. Sedangkan kebutuhan perawatan ortodonsi berdasarkan DHC yaitu 303 orang 76,5% tidak membutuhkan perawatan atau hanya perawatan ringan, 63 orang 15,9% butuh perawatan borderline, dan 30 orang 7,6% sangat butuh perawatan. Kebutuhan perawatan ortodonsi berdasarkan IOTN pada siswa kelas II di SMP Negeri 2 Bitung sebagian besar tidak membutuhkan atau butuh perawatan ringan pada AC 77% dan DHC 76%. Abstract Introduction Objective of this study was to investigate the incidence and severity of apical root resorptions (ARR) during orthodontic treatment with aligners. Materials and methods The sample comprised 100 patients (17–75 years of age) with a class I occlusion and anterior crowding before treatment, treated exclusively with aligners (Invisalign®, Align Technologies, Santa Clara, CA, USA). The following teeth were assessed: upper and lower anterior teeth and first molars. Root and crown lengths of a total of 1600 teeth were measured twice in pre- and post-treatment panoramic radiographs. Afterwards, relative changes of the root length during treatment were calculated by a root-crown-ratio taking pre- and post-treatment root and crown lengths into consideration. A reduction of this ratio was considered as a shortening of the initial root length. Additionally, tooth movements of the front teeth were assessed by lateral cephalograms and the 3-dimensonal set up of each patient. Results All patients had a reduction of the pre-treatment root length with a minimum of two teeth. On average 7.36 teeth per patient were affected. 54% of 1600 measured teeth showed no measurable root reduction. A reduction of >0%-10% of the pre-treatment root length was found in 27.75%, a distinct reduction of >10%-20% in 11.94%. 6.31% of all teeth were affected with a considerable reduction of >20%. We found no statistically significant correlation between relative root length changes and the individual tooth, gender, age or sagittal and vertical orthodontic tooth movement; except for extrusion of upper front teeth, which was considered as not clinical relevant due to the small amount of mean 4% ARR. Conclusions The present study is the first analyzing ARR in patients with a fully implemented orthodontic treatment with aligners (i.e. resolving anterior crowding). The variety was high and no clinical relevant influence factor could be detected. A minimum of two teeth with a root length reduction was found in every patient. On average, 7.36 teeth per patient were affected.

The aim of this study was to determine the relationship of orthodontic malocclusion with periodontal status, dental caries, and sociodemographic status. Our study population consisted of a sample of 836 school children (384 male and 452 female, aged 11–14 years). Four experienced orthodontists and two experienced periodontists performed the clinical examinations. The Treatment Priority Index (TPI), Community Periodontal Index of Treatment Needs (CPITN), decayed, missing, filled teeth (DMFT) scores, and a questionnaire that surveyed socio-demographic status of students were used. Spearman’s rank correlation coefficients were used to measure the association between variables. TPI scores showed that 36.4% of the students had normal occlusion, while 41.2% had slight, 15.7% had definite, 4% had severe, and 2.7% had very severe malocclusion. TPI values did not show any significant differences between pupils in different age, gender, socioeconomic status groups, and CPITN scores, whereas there was a significant relationship between TPI and DMFT scores. The orthodontic treatment need was not significantly correlated with CPITN or socio-demographic status; however, the correlation coefficient showed a significant relationship between TPI and DMFT scores.

Introduction: Malocclusion has an impact on the quality of life, thus becoming the motivation for orthodontic treatment. Subjective and objective needs for orthodontic treatment vary, where subjective needs are sometimes higher or maybe lower than objective needs, vice versa. The Index of Orthodontic Treatment Need (IOTN) is an index used to see the level of treatment needs. This study was aimed to analyse the correlation of orthodontic treatment motivation and the level of treatment needs based on the index of orthodontic treatment need (IOTN) of orthodontic patients. Methods: The research was correlation analytical. The motivation for orthodontic treatment was assessed using a questionnaire consisted of aspects of oral health motivation, oral function, aesthetics, psychosocial, and influence from others. The level of orthodontic treatment needs was assessed using the IOTN dental health component. Results: It was found that the patients’ motivation related to aesthetics was high. The motivation to get orthodontic treatment due to oral health, oral function, and psychosocial conditions was moderate, while related to the influence of others was low. The level of need for grade 4 treatment shows the largest percentage (48.57%). Gamma correlation analysis between motivation variables related to the oral and psychosocial function with the level of treatment needs showed a significantly positive correlation with the p-values of 0.003 and 0.022, categorized as weak correlation. Conclusion: There is a weak correlation of orthodontic treatment motivation and the level of treatment needs based on the index of orthodontic treatment need (IOTN) of orthodontic patients.

Aim: To validate and determine the applicability of OHIP-14 in assessing the impact of unmet dental treatment needs on the quality of life of adolescents in a rural community. Methods: The OHIP-14 questionnaire and the Aesthetic component (AC) of Index of Orthodontic Treatment Need (IOTN) were data collection instruments in a cross sectional survey among students in a rural community. The reliability and validity of the OHIP-14 as well as the association between it and dental treatment needs including malocclusion was assessed. Data obtained was analyzed using Mann Whitney U Test. Results: The mean age of participants was 14.9 (±1.6) years. The OHIP-14 had acceptable Cronbach alpha value of 0.8. It could discriminate between respondents with or without dental treatment needs due to caries and dental trauma (p <0.001). The OHIP-14 did not differentiate between respondents with or without orthodontic treatment need (p= 0.808). However, significant association existed between being irritable with people and unmet orthodontic treatment needs (p= 0.032). Conclusion: The OHIP-14 is a valid and reliable quality of life assessment tool in young adolescents in this rural community. However, only the social disability domain componente discriminated significantly between those with or without orthodontic treatment needs.

AIM: To measure the incidence and severity of root resorption after orthodontic treatment with Invisalign. MATERIAL AND METHODS: This retrospective study was conducted at Riyadh, Saudi Arabia from June 2017 to January 2018. Pre- treatment and post-treatment Orthopantographs were obtained from orthodontic records of 29 patients managed with aligners (Invisalign&reg;, Align Technologies, Santa Clara, CA, USA) at different dental clinics in Riyadh City. The selected sample was fulfilled the following criteria: (1) Class I malocclusion, (2) Mild to moderate crowding, (3) Non-extraction orthodontic treatment, (3) No evidence of root resorption before orthodontic treatment, (4) No root abnormalities or dilaceration, and (5) Good quality of pre- and post-treatment Orthopantographs. One examiner performed the measurements directly on the Orthopantographs using electronic digital caliper (Mitutoyo Manufacturing Co. Ltd., Tokyo, Japan) with an accuracy of 0.01mm. The measurements were performed on maxillary and mandibular central incisors, lateral incisors, and canines pre- and post-operatively, resulting in a total of 696 measurements. The crown length was measured from incisal edge to cemento- enamel-junction, while the root length from cemento-enamel-junction to root apex. RESULTS: In our study, 72% of the teeth demonstrated root resorption, in regard to the severity of root resorption, we found that mild root resorption &gt; 0% up to 2% in all the affected teeth. Upper Anterior teeth have more significant resorption rate than lower anterior teeth P&lt;0.05. CONCLUSION: The present study showed that incidence of root resorption was high after orthodontic treatment with Invisalign&reg;, however the severity is very low and it is limited to the surface resorption only.

ABSTRACT Objective: To evaluate the prevalence and severity of malocclusion in children suffering from β-thalassemia and to assess orthodontic treatment need using Grainger’s Treatment Priority Index (TPI) and index of orthodontic treatment need (IOTN)-dental health component (DHC). Methods: A cross-sectional study was conducted on 200 transfusion-dependent children diagnosed with homozygous β-thalassemia and 200 healthy school children aged 11-17 years. The TPI and IOTN-DHC data was recorded for both groups. Total TPI score for each subject was calculated and graded according to malocclusion severity estimate (MSE). Independent sample t-test was used to compare mean TPI scores, overjet and overbite between thalassemic and healthy children. Chi-square test was used to compare the frequency of IOTN-DHC grades, Angle’s classification, and MSE grades between thalassemic and healthy children. Results: The most prevalent malocclusion was Class I in normal children (67.5%) and Class II in thalassemic children (59%). The mean overjet and overbite were significantly (p<0.001) greater in thalassemic children than in healthy children. Severe tooth displacements were 3.5 times greater in thalassemic children, compared to controls. A greater proportion of thalassemic children were in IOTN grades 3 and 4, compared to the controls (p<0.001). MSE grades 4 and 5 were significantly (p<0.001) more prevalent in thalassemic children, compared to the controls. Conclusion: There is a high prevalence of Angle’s Class II malocclusion in thalassemic children. Majority of these children are categorized in higher grades of IOTN-DHC and TPI-MSE, showing a great severity of malocclusion and high orthodontic treatment needs.
